Development of a Decision Aid for Family Surrogate Decision Makers of Critically Ill Patients Requiring Renal Replacement Therapy in ICU:A User-Centered Design for Rapid Prototyping

Objectives Renal replacement therapy(RRT)is increasingly adopted for critically ill patients diagnosed with acute kidney injury,but the optimal time for initiation remains unclear and prognosis is uncertain,leading to medical complexity,ethical conflicts,and decision dilemmas in intensive care unit(ICU)*** study aimed to develop a decision aid(DA)for the family surrogate of critically ill patients to support their engagement in shared decision-making process with *** Development of DA employed a systematic process with user-centered design(UCD)principle,which included:(i)competitive analysis:searched,screened,and assessed the existing DAs to gather insights for design strategies,developmental techniques,and functionalities;(ii)user needs assessment:interviewed family surrogates in our hospital to explore target user group's decision-making experience and identify their unmet needs;(iii)evidence syntheses:integrate latest clinical evidence and pertinent information to inform the content development of *** The competitive analysis included 16 relevant DAs,from which we derived valuable insights using existing *** decision needs were explored among a cohort of 15 family surrogates,revealing four thematic issues in decision-making,including stuck into dilemmas,sense of uncertainty,limited capacity,and delayed decision confirmation.A total of 27 articles were included for evidence *** decision making knowledge on disease and treatment,as delineated in the literature sourced from decision support system or clinical guidelines,were formatted as the foundational knowledge ***-one items of evidence were extracted and integrated into the content panels of benefits and risks of RRT,possible outcomes,and reasons to *** DA was drafted into a web-based phototype using the elements of *** platform could guide users in their preparation of decision-making through a sequential four-step process:identifying

Ultrahigh lithiation dynamics of Li_(4)Ti_(5)O_(12)as an anode material with open diffusion channels induced by chemical presodiation

Spinel lithium titanate(Li_(4)Ti_(5)O_(12),LTO),with the merits of safety operation voltage,stable crystal structure,and minor lattice volume changes,becomes an optimal anode material for high-power Li-ion ***,the inherent wide bandgap and low lithiation reactivity of Li_(4)Ti_(5)O_(12)bring about poor conductivity and lithiation dynamics,limiting its further ***,we design and prepare unique Li_(4)Ti_(5)O_(12)anode materials with extremely low dopant content of Na^(+)utilizing the amorphous *** resultant Li_(4)Na_(0.008-)Ti_(5)O_(12.004)sample(denoted as NLTO-0.008)presents superior rate performances and cycle ability,with a reversible capacity of 149.4 mAh·g^(-1)at the current rate ***-0.008 retains the charge capacity of151.3 mAh·g^(-1)with a capacity loss of 0.5%after 1000cycles at the current rate of 1.0C(charge)/10.0C(discharge).The kinetic studies furtherly demonstrate that the lithiation reaction energy and diffusion energy barrier decrease by 28.8%and 30%,*** structure analysis indicates that Na^(+)occupies the 16d Li site and forms distorted LiO_(4)tetrahedron and TiO_(6)*** lattice distortion forms open diffusion channels,thus enhancing the Li^(+)diffusion dynamics and decreasing the lithiation reaction energy barrier for Li_(4)Ti_(5)O_(12).Therefore,the pre-sodiation strategy may arouse great interest in understanding and developing intercalation-type transitionmetal-based electrode materials in high-power lithium-ion batteries.

Efficacy and Safety of Bufei Jiedu Granules in Treating Multidrug-Resistant Pulmonary Tuberculosis:A Multi-center,Double-Blinded and Randomized Controlled Trial

Objective: To assess the efficacy and safety of Bufei Jiedu(BFJD) Granules as adjuvant therapy for patients with multidrug-resistant pulmonary tuberculosis(MDR-PTB). Methods: A large-scale, multi-center, double-blinded, and randomized controlled trial was conducted in 18 sentinel hospitals in China from December 2012 to December 2016. A total of 312 MDR-PTB patients were randomly assigned to BFJD Granules or placebo groups(1:1) using a stratified randomization method, which both received the long-course chemotherapy regimen for 18 months(6 Am-Lfx-P-Z-Pto, 12 Lfx-P-Z-Pto). Meanwhile, patients in both groups also received BFJD Granules or placebo twice a day for a total of 18 months, respectively. The primary outcome was cure rate. The secondary outcomes included time to sputum-culture conversion, changes in lung cavities and quality of life(QoL) of patients. Adverse reactions were monitored during and after the trial. Results: A total of 216 cases completed the trial, 111 in the BFJD Granules group and 105 in the placebo group. BFJD Granules, as adjuvant treatment, increased the cure rate by 13.6% at the end of treatment, compared with the placebo(58.4% vs. 44.8%, P=0.02), and accelerated the median time to sputum-culture conversion(5 months vs. 11 months). The cavity closure rate of the BFJD Granules group(50.6%, 43/85) was higher than that of the placebo group(32.1%, 26/81;P=0.02) in patients who completed the treatment. At the end of the intensive treatment, according to the 36-item Short Form, the BFJD Granules significantly improved physical functioning, general health, and vitality of patients relative to the placebo group(all P<0.01). Overall, the death rates in the two groups were not significantly different;5.1%(8/156) in the BFJD Granules group and 2.6%(4/156) in the placebo group. Aqueous zinc-ion batteries(ZIBs) combine the benefits of metallic Zn anodes with those of aqueous electrolytes and are well suited for large-scale energy storage because of their inherent high safety, cost-effectiveness, and eco-friendliness. Currently, the practical application of such batteries is hindered by the poor cycling performance of Zn anodes due to uncontrolled dendrite formation and severe side reactions, although recent reports suggest that these problems can be mitigated through the modification of Zn anodes with metal-based *** that the mechanisms of improving Zn deposition and the structural evolution of metal-based materials have not been systematically reviewed, we herein systematically overview the metal-based materials used to stabilize Zn anodes, starting with a brief summary of the anode working mechanism and the challenges faced by stabilized Zn anodes. Subsequently, the design principles of Zn anodes stabilized by metal-based materials and the related recent progress are reviewed, and the key challenges and perspectives for the future development of such Zn anodes are proposed.

Remote sensing of air pollution incorporating integrated-path differential-absorption and coherent-Doppler lidar

Previous studies have shown that low platelet count combined with high plasma total homocysteine(tHcy)increased stroke risk and can be lowered by 73% with folic ***,the combined role of other platelet activation parameters and the methylenetetrahydrofolate reductase(MTHFR)C677T genotypes on stroke risk and folic acid treatment benefit remain to be *** study aimed to investigate if platelet activation parameters and MTHFR genotypes jointly impact folic acid treatment efficacy in first stroke *** were derived from the China Stroke Primary Prevention *** study includes a total of 11,185 adult hypertensive patients with relevant platelet activation parameters and MTHFR genotype *** simultaneously considering both platelet activation parameters(plateletcrit,platelet count,mean platelet volume,platelet distribution width)and MTHFR genotypes,patients with both low plateletcrit(Q1)and the TT genotype had the highest stroke incidence rate(5.6%)in the enalapril *** subgroup significantly benefited from folic acid treatment,with a 66% reduction in first stroke(HR:0.34;95%CI:0.14-0.82;p=0.016).Consistently,the subgroup with low plateletcrit(Q1)and the CC/CT genotype also benefited from folic acid treatment(HR:0.40;95%CI:0.23-0.70;p=0.001).In Chinese hypertensive adults,low plateletcrit can identify those who may greatly benefit from folic acid treatment,in particular,those with the TT genotype,a subpopulation known to have the highest stroke risk.

Significantly Enhanced Energy Storage Performances of PEI-based Composites Utilizing Surface Functionalized ZrO_(2) Nanoparticles for High-Temperature Application

Polymer dielectrics with a high energy density and an available energy storage capacity have been playing an important role in advanced electronics and power systems. Nevertheless, the use of polymer dielectrics in harsh environments is limited by their low energy density at high temperatures. Herein, zirconium dioxide(ZrO_(2)) nanoparticles were decorated with amino group utilizing 4,4-methylenebis(phenyl isocyanate)(AMEO) and successfully incorporated into polyetherimide(PEI) matrix. The dielectric properties, breakdown strength, and energy storage performances of PEI/ZrO_(2)-AMEO nanocomposites were investigated from 25 ℃ to 150 ℃. It is found that the combination of moderate bandgap ZrO_(2) with modest dielectric constant and polar groups at interface with deep trap can offer an available strategy to simultaneously increase the dielectric constant and breakdown strength of polymer dielectrics. As a result, the composites containing ZrO_(2)-AMEO exhibit excellent energy storage performance at elevated temperatures. Specially, the PEI-based composites with 3 vol% ZrO_(2)-AMEO display a maximum discharged energy density(U_(d)) of 3.1 J/cm^(3) at 150 ℃, presenting 90% higher than that of neat PEI. This study may help to better develop the polymer-based dielectric composite applied at elevated temperatures.

Identification of Motor Nuclei in the Medulla Oblongata of Carp for Biological Control

Brain-controlled technology is the key technology in biological control,and the corresponding relationship between animal brain nuclei and motor behavior is the *** purpose of this study was to explore the motor nuclei of the medulla oblongata in *** carps were subjected to electrical stimulation and chemical stimulation experiments,respectively,in the water-free state,and the effective chemical stimulation sites were injected with the pontamine sky blue *** brain tissue sections were obtained by paraffin tissue section technology and the neutral red staining *** comparing the positions of the brain nuclei shown in earlier studies,the motor nerve nuclei in the medulla oblongata were *** brain electrode was implanted into the motor nucleus of the medulla oblongata,and the underwater control experiment and behavioral tests were carried out with different electrical stimulation *** results showed that the abducens nucleus(NVI)was the motor nucleus that controls the ipsilateral steering,and the facial nucleus(NVII)was the motor nucleus that controls the forward *** adjusting the stimulation voltage and the stimulation pulse number,the carp can be stably controlled to achieve a left–right steering motion of 30°–180°and a forward motion of more than 80 cm/*** study indicated that the quantitative control of the steering and forward behavior of the carp can be achieved by electrical stimulation of the NVI and NVII,which provided a certain experimental basis for the accurate control of the carp robot.
