BACKGROUND Rapidly progressive pneumoconiosis(RPP) occasionally occurs in coal workers, particularly those with high exposure to silica. Here, we report the case of a 64-year-old male miller with RPP. CASE SUMMARY The patient had a persistent cough for one month and had been clinically diagnosed with pulmonary tuberculosis in 2011. He worked in a stone processing factory from the ages of 20 through 37 and has owned his own mill for the past 25 years. His chest radiograph showed significant increases in the size and number of lung nodules since his last follow-up in 2013. By percutaneous needle lung biopsy, the nodular lesions showed diffuse infiltration of phagocytic macrophages and birefringent crystals by polarizing microscopy. He was finally diagnosed with RPP of mixed dust pneumoconiosis combined with silicosis. CONCLUSION In this case, mixed dust pneumoconiosis with silicosis might be accelerated by persistent exposure to graindust from working in a mill environment.

Rational urban design helps to build sustainable cities with high ventilation capacity and pollutant removal ca-pacity,but the effect of building height on ventilation and pollutant dispersion inside asymmetric canyons has not been fully *** this paper,we studied the effect of increasing the degree of building height asym-metry(DBHA)on canyon ventilation and pollutant diffusion in shallow and deep asymmetric street canyons by considering six different building height ratios(BHR=3/4,1/2,1/3,4/3,2/1 and 3/1).The results show that increasing the DBHA in asymmetric canyons can improve the ventilation and pollutant removal *** step-up canyons,increasing the downwind building height is very useful to improve ventilation and pollutant *** shallow/deep step-up canyons with BHR=1/3,the air exchange rate(ACH)increased to 211.2%and 380.1%of the flat canyons,*** spatially-average pollutant concentration in the pedestrian zones(leeward Kavg∗ang windward Kavg∗)decreases significantly with the increase of DBHA,especially for the deep step-up canyon with BHR=1/3,the leeward Kavg∗and windward Kavg∗decrease to 15.3%and 3%,***,increasing the upwind building height can also improve the ventilation capacity in the step-down *** the deep step-down canyon with BHR=3/1,the leeward Kavg∗and windward Kavg∗decreased to 40.6%and 24.1%of the deep flat canyon,***,the ventilation capacity is very low for step-down canyons with BHR=4/3,and for step-down canyons with BHR≥2/1,the ventilation capacity and pollutant removal capacity increase significantly with the increase of ***,in urban planning,step-down canyons with BHR=4/3 should be avoided and designed to satisfy the condition of BHR≥2/*** findings will be a valuable reference for urban designers to build sustainable cities with high ventilation capacity.

Introduction Antibiotic use for acute otitis media(AOM)is one of the major sources of antimicrobial ***,the effective minimal antibiotic duration for AOM remains ***,guidelines often recommend broad ranges(5-10 days)of antibiotic use,yet the clinical impact of such a wide window has not been *** We systematically searched PubMed/MEDLINE,Embase,Scopus,Web of Science,and Cochrane Library from database inception to 6 October *** meta-analysis was conducted on randomized controlled trials that assessed antibiotic treatment for AOM in children(PROSPERO CRD42020196107).Results For amoxicillin and amoxicillin-clavulanate,7-day regimens were noninferior to 10-day regimens in clinical responses[amoxicillin:risk ratio(RR)0.919(95%CI 0.820-1.031),amoxicillin-clavulanate:RR 1.108(0.957-1.282)],except for≤2 *** the third-generation cephalosporins,7-day and 10-day regimens had similar clinical responses compared to placebo[7-day:RR 1.420(1.190-1.694),10-day:RR 1.238(1.125-1.362)compared to placebo].However,5-day regimens of amoxicillin-clavulanate and third-generation cephalosporins were inferior to 10-day *** to amoxicillin,a shorter treatment duration was tolerable with *** Our findings indicated that 1O days of antibiotic use may be unnecessarily long,while the treatment duration should be longer than 5 ***,5-day regimens would be sufficient for a modest treatment *** findings revealed that the current wide range of recommended antibiotic durations may have influenced the clinical outcome of AOM,and a narrower antibiotic duration window should be re-established.

Efficacy of early treatment with infliximab in pediatric Crohn’s disease

AIM: To investigate the effectiveness of early infliximab use for induction and maintenance therapy in pediatric Crohn’s disease. METHODS: We performed a retrospective chart review of 36 patients with Crohn’s disease. Ten patients (group A) were treated with mesalamine after induction therapy with oral prednisolone, and 13 patients (group B) were treated with azathioprine after induction therapy with oral prednisolone. Thirteen patients (group C) received infliximab and azathioprine for induction and maintenance therapy for the first year, and were treated with azathioprine after 1 year. All patients were followed for at least 24 mo. Efficacy was determined by the relapse rate using the pediatric Crohn’s disease activity index score in each group at 12 and 24 mo. RESULTS: At the 1 year follow-up, the relapse ratee, Mi Jin Kim, Hae Jeong Lee, yon Ho Choe (23.1%, 3 of 13 patients) in group C was lower than that (61.5%, 8 of 13 patients) in group B (P = 0.047). At the 2 years follow-up, the relapse rate (38.5%, 5 of 13 patients) in group C was lower than that (76.9%, 10 of 13 patients) in group B (P = 0.047). Adverse events in group C were fewer than in groups A and B. CONCLUSION: Early induction with infliximab at diagnosis, known as "top-down" therapy, was effective for reducing the relapse rate compared to conventional therapies for at least 2 years.

本文探討方石英高低温型轉变温度的变化規律及其原因。就形成方石英的原料颗粒大小、燒成温度、原料类型和矿化剂种类等因素对轉变温度的影响进行研究。結果表明: 1.原料的顆粒大小对1600°以下温度形成的方石英轉变温度有影响,顆粒愈小,轉变温度愈向高温移动,这种傾向对67.5—90μ范圍内的顆粒特別明显。 2.燒成温度以直綫关系影响轉变温度向高温移动,以方程式ti=a+btf（ti—轉变温度;tf—燒成温度）表示此关系时,a,b的值依原料类型不同,非晶質原料的—a值小,b值大;結晶質則相反。 3.原料类型依結晶度的高低影响轉变温度移动,結晶度愈高的原料形成的方石英轉变温度愈高,即按:脉石英>石英岩>燧石岩>非晶質硅酸的次序温度升高。 4.矿化剂作用力的强弱对方石英的高低温型轉变温度亦有影响,矿化作用愈强的矿化剂愈影响所形成的方石英轉变温度向高温移动,其次序为:Li2CO3>Na2CO3>FeO>CaO。
