Multicenter Survey of the Isometric Lower Extremity Strength in Patients with Type 2 Diabetes (MUSCLE-std): Design and Study Protocol

Background: The lower extremity muscle strength has been shown to be weaker in patients with diabetes mellitus, primarily due to the effects of diabetic polyneuropathy, a common diabetic complication. With respect to assessments of the lower extremity muscle strength, reference values for the knee extension force in healthy Japanese subjects have been reported. The clinical significance of applying knee extension force in the assessment of efficacy of exercise therapy and patient education has also been demonstrated, and the use of an individualized approach in physical exercise based on the quantitative assessment of the lower extremity muscle strength is becoming one of the most important strategies for promoting effective rehabilitation. Methods: The study is a multicenter, cross-sectional investigation. We will measure the knee extension force and evaluate the diabetic polyneuropathy in patients with type 2 diabetes using uniform equipment and methodology at all centers. We will compare the knee extension force between the subjects with and without diabetic polyneuropathy and will investigate the associations between the knee extension force and both glycemic control and exercise habits. The trial registration is UMIN CTR (UMIN000002810). Discussion: The lower extremity muscle strength has been insufficiently investigated in Japanese patients with diabetes. This study was designed to establish reference values for the knee extension force in Japanese diabetic patients with and without diabetic polyneuropathy.

Multi-Factorial Functional Analysis of the Patients with Reconstruction for Oral Carcinoma

This paper is to evaluate postoperative function which has undergone reconstruction with vascularized or pedicled flaps for the treatment of oral carcinomas. The subjects consisted of 22 cases of 17 male and 5 female patients, and their mean age was 55 years. The sites of reconstruction were the tongue in 12 cases, mandible in 10 cases. The reconstruction was performed using deltopectoral (DP) flaps in 6 cases, pectoralis major musculo cutaneous (PMMC) flaps in 3 cases, DP flap + PMMC flap in one case, forearm (FA) flaps in 8 cases and peroneal (P) flaps with fibula in 4 cases. Postoperative masticatory, speech, swallowing functions and facial appearance were evaluated by Multi-factorial analysis. All functional results seem to be related to the type of resection. By multi-factorial analysis, the type of mandibular resection, flap area, and anterior resection were related to the masticatory or speech function in tongue resection. In mandibular resection, masticatory function was subjected to age;speech function was subjected to type of mandibular resection and plate reconstruction. The facial appearance was subjected to flap area. These results suggested that postoperative function was not related to only one factor, but using the classification of each defect site as one of the factor of multi-factorial analysis was useful.

Expression of Neuropeptide Y gene in mouse testes during testicular development

AIM： To determine the efficacy of long-term lamivudine treatment of a large number of Japanese patients with chronic hepatitis B. METHODS： In this retrospective, multi-center trial, 318 Japanese patients with chronic hepatitis B received 100 mg of lamivudine daily for up to 36 （median 21） mo. Virological response was a decline to a serum HBV DNA level less than 3.7 log copies/mL. Virological breakthrough was defined as the reappearance of a serum HBV DNA level to more than 10-fold the minimum during treatment. RESULTS： Lamivudine produced virological response in 86.8% of the 318 patients at 6 mo, in 80.2% of 252 patients at 12 mo, in 69.2% of 133 patients at 24 mo, and in 53.6% of 28 patients at 36 mo. Forward stepwise logistic regression analysis showed an HBV DNA level less than 6.8 log copies/mL （P〈 0.0001）, HBeAg negativity （P〈 0.0001）, a platelet count of 100×10^9/L or more （P= 0.0162） at baseline, and a decline of the HBV DNA level of more than 3.2 log copies/mL as compared with the baseline level at 3 mo after the start of treatment （P= 0.0003） to be significantly associated with virological response. Among patients with a virological response, virological breakthrough was seen in 5.3% of 19 patients who responded virologically at 1 mo, in 20.7% of 203 patients at 3 mo, in 27.5% of 51 patients at 6 mo, in 33.3% of 12 patients at 9 mo, and in 100% of 3 patients at ≥15 mo. A virological breakthrough was found significantly more often in patients with delayed virological response. CONCLUSION： Lamivudine treatment could suppress serum HBV DNA in most of the tested Japanese patients. Long-term efficacy might be seen in patients without HBeAg at baseline, in the absence of cirrhosis, and in patients with a decline in HBV DNA level soon after the start of treatment.

Umami: An Alternative Japanese Approach to Reducing Sodium While Enhancing Taste Desirability

COVID-19 has highlighted the underlying global burden from non-communicable diseases (NCDs) and the lack of resiliency in health systems to tackle highly preventable risk factors, which have left populations vulnerable to acute health crises. The persistent issue of unhealthy behaviors, particularly those related to a high-sodium diet, suggests inadequate policy attention and insufficient funding for public health and behavioral research. This year, Japan will host the Tokyo Nutrition for Growth (N4G) Summit 2021. The summit is being held in the era of COVID-19, offering a historic opportunity to make a difference on the global health agenda for food and nutrition and to reaffirm the global commitment to prevent future health crises by addressing the growing burden of NCDs and building better prepared and more resilient health systems. This perspective article suggests that we, global health researchers and decision-makers, should rethink the value of using “Umami” substances as candidates for a practical salt-reducing alternative that originated in Japan (while maintaining the affordability, accessibility, availability, and desirability of healthy food). Reduced salt intake will help address the global syndemic of COVID-19 pandemic and high rates of hypertension, heart diseases, diabetes, and other NCDs. By addressing these issues, it will make countries more resilient to future pandemic threats.

Effect of Verbal Instructions in Pain Assessment during a Passive Straight Leg Raise Test in People with Chronic Low Back Pain

The most prevalent issue in physical therapy is pain. Due to the subjective nature of pain, assessment tools are essential in understanding it as objective data. However, assessment of pain may result in distress for the patient. A physical therapist (PT) should conduct these tests as quickly and accurately as possible. Straightforward instructions are vital in such cases. This study aimed to clarify the effect of verbal instructions for pain assessment during a passive straight leg raise (PSLR) test for participants with chronic low back pain (CLBP). This study included 22 participants who provided informed consent and received three consecutive PSLR tests with measurement of the hip flexion range of motion (HFROM) and were instructed to cease the test at submaximal pain before the first test. Following the second and third tests, participants were given specific verbal instructions to remember pain intensity, quality, and location. After each test, participants were to circle the pain location on the body chart and rate their pain intensity on a numeric rating scale (NRS) and pain quality. All participants were then interviewed about the differences between having and not having specific verbal instructions. The results of HFROM, NRS, and pain extent were not significantly different between the first and second tests or between the second and third tests using a paired t-test. Eleven changes in pain location were found in the second test compared to those in the first test. In the third test, only three participants circled a different area than in the second test. Ten participants showed similar changes with pain location in pain quality in the three PSLR tests. This study revealed the effect of specific verbal instructions prior to PSLR tests. Particularly, participants could notice exact pain location. Our findings may help PT to understand pain cause and reduce patients’ stress during pain assessment in clinical settings.

Effects of Cross Linking on the Chromatographic Nitrogen Isotope Separation

The effects of cross linking with porous cation exchange resin were studied for nitrogen isotope separation. The displacement chromatography was conducted by the resin with cross linking rang from 20% to 40%. A sharp adsorbed ammonium band was maintained for each operation. Enriched 15N isotopes with 0.93% were obtained by 20% cross linking resin and two meters chromatographic operation which started from the natural abundance of ammonium molecule. The effect of cross linking percentage on the height equivalent to a theoretical plate (HETP) was evaluated in the present work and the HETP value is proportional to the cross linking percentage. HETP value of 0.036 cm was obtained at the present system by using 20% cross linking resin.

The effects of ultrasonic irradiation in combination with ozone on the reduction of residual ethion of tangerine (<i>Citrus reticulata</i>Blanco cv. Sai Nam Pung) fruit after harvest

Ethion residue removal efficiency using individual and combined effects of ultrasonication (US), ozonation (O3), ultrasonication and ozonation (US/O3) was investigated. A 1 mg·L-1 standard ethion was subjected to an ultrasonic irradiation (3W input power) device with different frequencies of 108, 400, 700, 1000 kHz, and to an ozone with a concentration of 200 mg· mL-1 with various exposure times (0, 15, 30, 45 and 60 minutes). Reduction of residual ethion was determined by gas chromatography with flame photometric detection (GC-FPD). It was found that 1000 kHz ultrasonic irradiation for 60 minutes was the most effective treatment for reducing ethion concentration. Additionally, ozone could reduce the concentration of standard ethion after 60 minutes of exposure. Ultrasonic irradiation in combination with ozone had a synergistic effect in reducing standard ethion concentration, with the highest rate occurring within the first 15 minutes. Ethion concentration was reduced to 75.43% after ultrasonic irradiation at 1000 kHz and ozone exposure for 60 minutes. The effectiveness of the combined scheme for reducing of ethion residues on tangerine was also determined by GC-FPD. The ethion residue on fresh tangerines after ultrasonic irradiation and ozone washing was reduced by 73.03% after 60 minutes. The present study demonstrates that ultrasonic irradiation and ozone exposure could be used to reduce ethion residues in treated tangerine in order to meet the Standards for Pesticide Residue Limits.
