Ivabradine in the treatment of systolic heart failure- A systematic review and meta-analysis

AIM To perform a systematic-review and meta-analysis to compare outcomes of ivabradine combined with betablocker to beta-blocker alone in heart failure with reduced ejection fraction(HFr EF).METHODS We searched PubM ed, Cochrane, EMBASE, CINAHL and Web of Science for trials comparing ivabradine + betablocker to beta-blocker alone in HFr EF. We performed a systematic-review and meta-analysis of published literature. Primary end-point was combined end point of cardiac death and hospitalization for heart *** Six studies with 17671 patients were included. Mean follow-up was 8.7 ± 7.9 mo. Combined end-point of heart failure readmission and cardiovascular death was better in ivabradine + beta-blocker group compared to beta-blocker alone(RR: 0.93, 95%CI: 0.79-1.09, P = 0.354). Mean difference(MD) in heart rate was higher in the ivabradine + beta-blocker group(MD: 6.14, 95%CI: 3.80-8.48, P < 0.001). There was no difference in all cause mortality(RR: 0.98, 95%CI: 0.89-1.07, P = 0.609), cardiovascular mortality(RR: 0.99, 95%CI: 0.86-1.15, P = 0.908) or heart failure hospitalization(RR: 0.87, 95%CI: 0.68-1.11, P = 0.271). CONCLUSION From the available clinical trials, ivabradine + betablocker resulted in a significantly greater reduction in HRcoupled with improvement in combined end-point of heart failure readmission and cardiovascular death but with no improvement in all cause or cardiovascular mortality. Given the limited evidence, further randomized controlled trials are essential before widespread clinical application of ivabradine + beta-blocker is advocated for HFrEF.

Multiferroic Consequence of Porous (BiFeO_3)_x–(BiCrO_3)_(1-x) Composite Thin Films by Novel Sol–Gel Method

In this work,we have presented a spin-coating method to produce thin films started with pure BiCrO3（BCO）and ended up with BiFeO3（BFO）by increasing x values in the（BiFeO3）x-（BiCrO3）1-x *** the produced thin films have been crystallized at the annealing temperatures of 400 ℃ for 0.5 *** XRD and EDAX spectrums give insight that the two crystal phases related to BCO and BFO stayed together within the thin film *** analysis showed that the prepared composite had macroporous morphology with interconnected pores and its width（size）decreased with increasing x *** strong correlations are observed among the microstructure,dielectric,ferroelectric,ferromagnetic properties and Fe *** all composites,the composition of 0.75 shows an attractive magnetization,polarization,switching and improved dielectric behaviors at room *** increase in the multiferroic characteristics of 0.75 composition is due to arise of lower leakage current by causing reduction in oxygen vacancy density,and enhancement of super-exchange magnetic interaction between Fe3＋ and Cr3＋ at BFO/BCO interface *** result shows that the thin layer on Pt（111）/Ti/SiO2/Si substrate possesses simultaneously improved ferroelectric and ferromagnetic properties which make an inaccessible potential application for nonvolatile ferroelectric memories.

Microstructure evolution and mechanical property of Cu-15Ni-8Sn-0.2Nb alloy during aging treatment

Aging treatment of Cu-based alloys is essential to enhance their strength that is desirable for their extensive engineering applications in electrical industry,whereas the underlying mechanism of strengthening is essential for massive manufacturing of these ***,the microstructure evolution of a supersaturated solid solution Cu-15Ni-8Sn-0.2Nb alloy aged at 400℃for different time was characterized at atomic scale using state-of-the-art transmission electron microscopy(TEM)and the corresponding mechanical property was also *** results reveal that the modulated structure,DO_(22)/L1_(2)ordering,and discontinuous precipitation(DP)appeared in the advances of aging *** the early stage of aging treatment,component modulation waves and satellite spots appeared from spinodal decomposition and the modulation wavelength was identified in the range of 1-7 *** the modulated structures formed-poor-rich solute regions,of which DO22ordering was present in the Ni-poor region while L1_(2)ordering appeared in the Ni-rich *** sequence of ordering precipitates was further verified by density functional theory(DFT)***,orientation relationships and interfacial structures between DO_(22),L1_(2)phases and the parent matrix were *** measured hardness of alloy reached a maximum value of 335 HV after aging for 120 min due to the coherence between the two ordering phases and *** results illustrated the importance of aging on structural evolution and mechanical property of Cu-15Ni-8Sn alloy at various heat treatment stages,which could potentially help in manufacturing promising alloys for their extensive engineering applications.

BACKGROUND Itraconazole is a broad-spectrum triazole antifungal inhibiting fungal growth by inhibiting ergosterol synthesis and exhibits a nonlinear pharmacokinetic *** absorption pattern with wide fluctuations in blood levels causes inconsistent and unpredictable clinical behaviour of this drug despite its low minimum inhibitory concentration(MIC)as compared to other antifungal *** To compare the oral bioavailability and bioequivalence of Fixtral SB(supra bioavailable itraconazole)with reference product R2(supra bioavailable 2×50 mg itraconazole).METHODS The study population consisted of 54 healthy volunteers,aged between 18-45 years and randomized to receive a single oral dose of either test[T;Fixtral SB(supra bioavailable itraconazole)100 mg]or reference product(R1;Sporanox 100 mg×2 capsules and R2;Lozanoc capsules 50 mg×2 capsules).Blood samples were taken pre-dose and post-dose up to 96 *** study evaluated bioequivalence by comparing the oral bioavailability of the test product with reference product *** pharmacodynamic characteristics of the drug were evaluated by comparing the test product with reference product ***(PK)-PD comparative analysis[area under the concentration-time curve(AUC)/minimum inhibitory concentration(MIC)>25]was performed for conventional itraconazole 100 mg and supra bioavailable itraconazole 50 *** events(AEs)assessments were performed in each study period and post-study *** Statistical analysis of primary PK variables revealed bioequivalence,with confidence intervals being completely inside the acceptance criteria of 80%-125%.The peak concentration levels of itraconazole were achieved at 10 h(T)and 8.5 h(R2),*** parameter assessment showed that AUC/MIC for R1 are comparable to Fixtral SB 100mg for MIC levels up to 16mcg/mL(P>0.05 and observed P=0.3196).Six AEs were observed that were mild to moderate in severity and *** severe AE was reported.
