Obesity is a well-known modifiable risk factor for breast cancer and is considered a poor prognostic factor in pre-and post-menopausal women.While the systemic effects of obesity have been extensively studied,less is known about the mechanisms underlying obesityassociated cancer risk and the local consequences of obesity.Thus,obesity-induced inflammation has become the focus of research interest.Biologically,the development of cancer involves a complex interaction with numerous components.As the tumor immune microenvironment changes due to obesity-triggered inflammation,an increase in infiltration occurs for proinflammatory cytokines and adipokines,as well as adipocytes,immune cells,and tumor cells in the expanded adipose tissue.Complicated cellular-molecular crosstalk networks change critical pathways,mediate metabolic and immune function reprogramming,and have a significant role in tumor metastasis,proliferation,resistance,angiogenesis,and tumorigenesis.This review summarizes recent research findings on how inflammatory mediators in the in situ tumor microenvironment regulate the occurrence and development of breast cancer in the context of obesity.We analyzed the heterogeneity and potential mechanisms of the breast cancer immune microenvironment from the perspective of inflammation to provide a reference for the clinical transformation of precision targeted cancer therapy.

Anxiety and depression in older adult patients undergoing elective liver surgery in allopatry medical treatment

Background:Allopatry medical treatment is common in China.However,allopatry medical therapy can result in many problems,including a negative psychological impact on patients.Patients undergoing liver surgery often experience anxiety and depression.To understand the psychological status of older adult patients undergoing surgery better,this study was designed to investigate the incidence of anxiety and depression in older adult patients undergoing elective liver surgery and to analyze factors associated with it in allopatry medical therapy.Methods:A total of 173 older adult patients undergoing elective liver surgery were included in the study.Patients were evaluated using the hospital anxiety and depression scale(HADS).The contributing factors affecting the psychological state of older adult patients undergoing elective surgery were analyzed using a linear regression method.Results:The HADS-A(hospital anxiety and depression scale-anxiety).The correlation(scale)score of the older adult patients undergoing elective liver surgery was(8.692.38),including 53 asymptomatic patients,86 suspicious patients,and 34 symptomatic patients.The HADS-D(hospital anxiety and depression scale-depression)score was(8.312.90),including 83 asymptomatic patients,56 suspicious patients,and 34 symptomatic patients.Multivariate analysis showed that residence and complication grade(Clavien–Dindo Classification of Surgical Complications or Accordion Severity Grading System)significantly correlated with the anxiety level of patients undergoing elective surgery.Residence,a requirement of blood transfusion,total transfusion volume,and Accordion complication grade showed a significant correlation with depression in patients undergoing elective surgery.Conclusions:Anxiety and depression in older adult patients undergoing elective liver surgery were common.Regional differences(local patients vs non local patients)and the severity of complications were the risk factors for anxiety and depression in older adult patients undergoing elective liver sur

Are activated astrocytes pro-excitatory in epilepsy?The answer tends to be"yes"with cumulative evidence proposing that astrocytic activation leads to hyperexcitability through excitatory transmitter release or neuroinflammatory pathways[1-3].But a very recent study published in Nature Communications by Zhao J et al.has refreshed this viewpoint by showing that optogenetic activation of astrocytes can alleviate neocortical seizures(a type of epileptic symptom that originates in the neocortex which is part of the external surface layer of the brain)through astrocytic Na^(+)-K^(+)-ATPase in animal models[4](Fig.1).

Effective dental care can reduce the incidence of oral diseases(dental caries,dentin sensitivity,tooth discoloration,etc.).However,delayed or inappropriate usage of care tools not only fails to eliminate external adverse stimuli,but sometimes even causes dental injury.Inspired by the traditional culture of“lacquer teeth”,a sprayable superhydrophobic protectant(ZFP)composed of ZnO,fluorine modified nano-silica(FSNs),and polydimethylsiloxane(PDMS)is constructed similar to automobile wax,for routine dental protection.With its superhydrophobic properties,this protective membrane can effectively resist the adhesion of bacteria,proteins,and food residues to the teeth surface.Meanwhile,ZFP exerts stable superhydrophobic selfcleaning properties under external mechanical stimulation,temperature alternation,and acidic environment.In addition,ZnO in ZFP can significantly restrain the growth of dental caries-related bacteria(Streptococcus mutans),with an enhanced antibacterial capacity when combined with yellow light irradiation.Both in vivo and in vitro experiments demonstrate that ZFP is a safe,convenient,and quadruplex-effective dental protectant,which is expected to serve as a promising toothpaste companion for joint maintenance of dental health.

Objective:Neoadjuvant therapy(NAT)has been widely implemented as an essential treatment to improve therapeutic efficacy in patients with locally-advanced cancer to reduce tumor burden and prolong survival,particularly for human epidermal growth receptor 2-positive and triple-negative breast cancer.The role of peripheral immune components in predicting therapeutic responses has received limited attention.Herein we determined the relationship between dynamic changes in peripheral immune indices and therapeutic responses during NAT administration.Methods:Peripheral immune index data were collected from 134 patients before and after NAT.Logistic regression and machine learning algorithms were applied to the feature selection and model construction processes,respectively.Results:Peripheral immune status with a greater number of CD3^(+)T cells before and after NAT,and a greater number of CD8^(+)T cells,fewer CD4^(+)T cells,and fewer NK cells after NAT was significantly related to a pathological complete response(P<0.05).The post-NAT NK cell-to-pre-NAT NK cell ratio was negatively correlated with the response to NAT(HR=0.13,P=0.008).Based on the results of logistic regression,14 reliable features(P<0.05)were selected to construct the machine learning model.The random forest model exhibited the best power to predict efficacy of NAT among 10 machine learning model approaches(AUC=0.733).Conclusions:Statistically significant relationships between several specific immune indices and the efficacy of NAT were revealed.A random forest model based on dynamic changes in peripheral immune indices showed robust performance in predicting NAT efficacy.

Localized delivery,comparing to systemic drug administration,offers a unique alternative to enhance efficacy,lower dosage,and minimize systemic tissue toxicity by releasing therapeutics locally and specifically to the site of interests.Herein,a localized drug delivery platform ("plum?pudding"structure) with controlled release and long-acting features is developed through an injectable hydrogel ("pudding") crosslinked via self-assembled triblock polymeric micelles ("plum") to help reduce renal interstitial fibrosis.This strategy achieves controlled and prolonged release of model therapeutics in the kidney for up to three weeks in mice.Following a single injection,local treatments containing either anti-inflammatory small molecule celastrol or anti-TGFb antibody effectively minimize inflammation while alleviating fibrosis via inhibiting NF-k B signaling pathway or neutralizing TGF-b1 locally.Importantly,the micelle-hydrogel hybrid based localized therapy shows enhanced efficacy without local or systemic toxicity,which may represent a clinically relevant delivery platform in the management of renal interstitial fibrosis.

Renal tubular epithelial cells(RTECs)are important target cells for the development of kidney-targeted drug delivery systems.Under physiological conditions,RTECs are under constant fluid shear stress(FSS)from original urine in the renal tubule and respond to changes of FSS by altering their morphology and receptor expression patterns,which may affect reabsorption and cellular uptake.Using a microfluidic system,controlled shear stress was applied to proximal tubule epithelial cell line HK-2.Next,2-glucosamine,bovine serum albumin,and albumin nanoparticles were selected as representative carriers to perform cell uptake studies in HK-2 cells using the microfluidic platform system with controlled FSS.FSS is proven to impact the morphology of HK-2 cells and upregulate the levels of megalin and clathrin,which then led to enhanced cellular uptake efficiencies of energy-driven carrier systems such as macromolecular and albumin nanoparticles in HK-2 cells.To further investigate the effects of FSS on endocytic behavior mediated by related receptors,a mice model of acute kidney injury with reduced fluid shear stress was established.Consistent with in vitro findings,in vivo studies have also shown reduced fluid shear stress down-regulated the levels of megalin receptors,thereby reducing the renal distribution of albumin nanoparticles.

Caries is considered to be the most prevalent non-communicable disease in humans, mainly deriving from acidogenic bacterial biofilm and resulting in the demineralization and decomposition of hard dental tissue. Herein, a composite responsive foam brace loaded with magnesium organic framework (MPC) is designed for caries prevention and tooth remineralization. MPC can intelligently release organic antibacterial molecules (gallic acid) and mineralized ions (Mg2+, Ca2+ and PO43􀀀 ) under acidic conditions (pH < 5.5) of biofilm infection, regulating pH and killing bacteria. Additionally, due to the excellent photothermal conversion efficiency, MPC can further enhance the destruction of bacterial biofilm by inhibiting virulence genes and destroying bacterial adhesion under near-infrared light irradiation (808 nm). More importantly, MPC can not only reverse the cariogenic environment at both pH and microbial levels, but also promote self-healing of demineralized teeth in terms of both the micro-structure and mechanical properties.
