AIM:To review safety,efficacy,and proper nursing care of double-balloon enteroscopy (DBE) in pediatric patients with small intestinal ***:Our study included 37 patients with abdominal pain,diarrhea,passage of blood in the stools,and other symptoms,who underwent DBE from December 2006 to July *** was retrograde in 36 procedures,antegrade in six,and from both ends in *** diagnostic significance and salient points in nursing care are discussed in this ***:At least one lesion was discovered in 28 out of 37 patients,which yielded a positive diagnosis in 75.7% of *** bowel preparation and skilled nursing care not only shortened the procedure time,but could also alleviate patient discomfort and enhance the quality of *** serious procedure-related complications were observed in any ***:DBE is a new modality of endoscopic procedure that improves the standard of diagnosis and treatment of small bowel diseases in *** nursing care is essential to the successful execution of the procedure.

AIM To evaluate the efficacy of thalidomide for treating troublesome cases of pediatric Crohn's disease(CD) with tuberculosis *** A retrospective study of clinical outcome among children treated with thalidomide was conducted. All patients had evidence of tuberculosis infection with a failure of anti-tuberculosis treatment for more than one year,and were subsequently diagnosed with CD. All the patients received thalidomide treatment with a starting dose of 1.2-2.5 mg/kg per day. Remission was defined as pediatric CD activity index less than or equal to *** Ten patients with CD were treated with thalidomide at an average age of 7.2 years and followed up for a median of 22.2 mo. Clinical remission rate was 60% after 9-12 mo of thalidomide treatment. One patient with no response had an interleukin-10 receptor alpha gene mutation. Erythrocyte sedimentation rate,C-reactive protein and platelet count showed a dramatic decrease; hemoglobin level and weight improved significantly after thalidomide treatment when compared with the baseline *** Thalidomide is an effective and safe drug for remission of CD in pediatric patients who have been treated for tuberculosis.

The pathogenesis of Crohn’s disease is complex and involves various factors that interplay in the initial and persistent damage to the intestinal tissue which begins at the mucosal layer with eventual spread to the other layers of the intestine. It probably involves defects in immunoregulation and maintenance of the mucosal barrier. But exactly how such functional changes are translated to morphological deformities is unknown and the earliest morphological changes, believed to be an aphthoid ulcer, has not been well characterized, especially in the small intestine.

This study explored the clinical value of endoscopic ligation for the treatment of upper gastrointestinal(GI)protuberant lesions in *** to the appearance and size of lesions,we used different ligation techniques for the treatment of the *** ultrasonography was used for preliminary characterization of the *** case diagnosed with PeutzJeghers syndrome was successfully treated by a detachable *** cases with semi-pedunculated or broad-base lesions originating from the submucosal layer of the upper GI were treated with endoscopic variceal ligation;endoscopic examination showed that one case had complete healing 11 wk after ligation,while an ulcer scar was observed at the ligation site after 6 wk in the other *** lesions were successfully ligated at the first *** significant complications occurred either during or after the *** endoscopic ligation of upper GI lesions is an effective and safe treatment for upper GI protuberant lesions in children.

Background Good quality of care for inflammatory bowel disease(IBD)depends on high-standard management and facility in the IBD ***,there are no clear measures or criteria for evaluating pediatric IBD(PIBD)center in *** aim of this study was to develop a comprehensive set of quality indicators(QIs)for evaluating PIBD center in *** A modified Delphi consensus-based approach was used to identify a set of QIs of structure,process,and outcomes for defining the *** process included an exhaustive search using complementary approaches to identify potential QIs,and two web-based voting rounds to select the QIs defining the criteria for PIBD *** A total of 101 QIs(35 structures,48 processes and 18 outcomes)were included in this *** QIs focused on the composition of multidisciplinary team,facilities and services that PIBD center should *** QIs highlight core requirements in diagnosing,evaluating,treating PIBD,and disease *** QIs mainly included criteria evaluating effectiveness of various interventions in PIBD *** The present Delphi consensus developed a set of main QIs that may be useful for managing a PIBD center.

Solar-powered interfacial evaporation is an energy-efficient solution for water *** requires solar absorbers to facilitate upward water transport and limit the heat to the surface for efficient ***,downward salt ion transport is also desired to prevent salt ***,achieving simultaneously fast water uptake,downward salt transport,and heat localization is challenging due to highly coupled water,mass,and thermal ***,we develop a structurally graded aerogel inspired by tree transport systems to collectively optimize water,salt,and thermal *** arched aerogel features root-like,fan-shaped microchannels for rapid water uptake and downward salt diffusion,and horizontally aligned pores near the surface for heat localization through maximizing solar absorption and minimizing conductive heat *** structural characteristics gave rise to consistent evaporation rates of 2.09 kg m^(-2) h^(-1) under one-sun illumination in a 3.5 wt%NaCl solution for 7 days without *** in a high-salinity solution of 20 wt%NaCl,the evaporation rates maintained stable at 1.94 kg m^(-2) h^(-1) for 8 h without salt crystal *** work offers a novel microstructural design to address the complex interplay of water,salt,and thermal transport.

We aim to evaluate prostate health index as an additional risk-stratification tool in patients with Prostate Imaging Reporting and Data System score 3 lesions on multiparametric magnetic resonance *** with biochemical or clinical suspicion of having prostate cancer who underwent multiparametric magnetic resonance imaging in two tertiary centers(Queen Mary Hospital and Princess Margaret Hospital,Hong Kong,China)between January 2017 and June 2022 were ***-magnetic resonance imaging fusion biopsies were performed after prostate health index *** who only had Prostate Imaging Reporting and Data System score 3 lesions were further stratified into four prostate health index risk groups and the cancer detection rates were *** of the 747 patients,47.3%had Prostate Imaging Reporting and Data System score 3 lesions *** detection rate of clinically significant prostate cancer in this group was 15.0%.The cancer detection rates of clinically significant prostate cancer had statistically significant differences:5.3%in prostate health index<25.0,7.4%in prostate health index 25.0-34.9,17.9%in prostate health index 35.0-54.9,and 52.6%in prostate health index≥55.0(P<0.01).Among the patients,26.9%could have avoided a biopsy with a prostate health index<25.0,at the expense of a 5.3%risk of missing clinically significant prostate *** health index could be used as an additional risk stratification tool for patients with Prostate Imaging Reporting and Data System score 3 *** could be avoided in patients with low prostate health index,with a small risk of missing clinically significant prostate cancer.
