Comparison of semen quality and outcome of assisted reproductive techniques in Chinese men with and without hepatitis B

In this study, we aimed to determine the effects of hepatitis B virus （HBV） infection on sperm quality and the outcome of assisted reproductive technology （ART）. A total of 916 men （457 HBV-positive and 459 HBV-negative） seeking fertility assistance from January 2008 to December 2009 at the Women＇s Hospital in the School of Medicine at Zhejiang University were analysed for semen parameters. Couples in which the men were hepatitis B surface antigen （HBsAg）-seropositive were categorized as HBV-positive and included 587 in vitrofertilisation （IVF） and 325 intracytoplasmic sperm injection （ICSI） cycles from January 2004 to December 2009; negative controls were matched for female age, date of ova retrieval, ART approach used （IVF or ICSI） and randomized in a ratio of 1：1 according to the ART treatment cycles （587 for IVF and 325 for ICSI）. HBV-infected men exhibited lower semen volume, lower total sperm count as well as poor sperm motility and morphology （P〈O.05） when compared to control individuals. Rates of two-pronuclear （2PN） fertilisation, high-grade embryo acquisition, implantation and clinical pregnancy were also lower among HBV-positive patients compared to those of HBV-negative patients after ICSI and embryo transfer （P〈0.05）; IVF outcomes were similar between the two groups （P〉0.05）. Logistic regression analysis showed that HBV infection independently contributed to increased rates of asthenozoospermia and oligozoospermia/azoospermia （P〈0.05） as well as decreased rates of implantation and clinical pregnancy in ICSI cycles （P〈0.05）. Our results suggest that HBV infection in men is associated with poor sperm quality and worse ICSI and embryo transfer outcomes but does not affect the outcome of IVF and embryo transfer.

Background:With the unequal distribution of medical resources in developing countries,critially ill children need to be transferred to tertiary hospitals from primary hospitals with limited *** a large number of critically ill children are transferred each day in China,the standard process of inter-hospital transport is not ***:We retrospectively analyzed the data collected during transport.A total number of 9231 patients(≤14 years)who had been transferred to the Hunan Children's Hospital by a specialized team from primary hospitals from January 1,2009 to June 30,2012 were included in the ***:Nearly half of the critically ill children were neonates(48.72%)and two thirds of the children were suffering from respiratory,neurological and cardiac *** adjusted odds ratios(OR)and 95%confidence intervals(CI)were calculated using unconditional logistic *** time in non-working hours was longer than the working hours(OR=1.186,95%CI=1.059-1.329).Our study showed that mobilization time for neonates was shorter than that for older children(OR=0.801,95%CI=0.692-0.928).The mobilization time of referral cases was shorter in areas within a radius of 50 km than in those within a radius of over 250 km(OR=0.427,95%CI=0.350-0.521).Referred patients in summer needed a significantly shorter mobilization time than in winter(OR=0.705,95%CI=0.616-0.806).Conclusion:Standardized processes and guidelines for inter-hospital transport would be essential to ensure effective transport of patients and reduce the mobilization activation time.

Objective： Aberrant expression of ten-eleven translocation 1 （TET1） plays a critical role in tumor development and progression. We systematically summarized the latest research progress on the role and mechanisms of TET1 in cancer biology. Data Sources： Relevant articles published in English from 1980 to April 2016 were selected from the PubMed database. Tile terms ＂＇ten-eleven translocation 1,＂＂＇SmC,＂＇5hmC,＂＇microRNA,＇＂＂hypoxia,＂ and ＂＇embryonic stem cell＇＂ were used tbr the search. Study Selection： Articles focusing on the role and mechanism ofTETI in tumor were reviewed, including clinical and basic research articles. Results： TET proteins, the key enzymes converting 5-methylcytosine to 5-hydroxymethylcytosine, play vital roles in DNA demethylation regulation. Recent studies have shown that loss of TETI is associated with tumorigenesis and can be used as a potential biomarker for cancer therapy, which indicates that TETI serves as tumor suppressor gene. Moreover, besides its dioxygenase activity, TET1 could induce epithelial-mesenchymal transition and act as a coactivator to regulate gene transcription, such as developmental regulator in embryonic stem cells （ESCs） and hypoxia-responsive gene in cancer. The regulation of TETl is also correlated with microRNA in a posttranscriptional modification process. Hence, it is complex but critical to coaprehend the mechanisms of TETI in the biology of ESCs and cancer. Conclusions： TET I not only serves as a demethylation enzyme but also plays multiple roles during tumorigenesis and progression. More studies should be carried out to elucidate the exact mechanisms of TETI and its associations with cancer betbre considering it as a therapeutic tool.

Background:Infections still represent the main factors influencing morbidity and mortality following liver *** study aimed to evaluate the incidence and risk factors for infection and survival after liver ***:We retrospectively examined medical records in 210 liver recipients who underwent liver transplantation between April 2015 and October 2017 in our *** manifestations and results of pathogen detection test were used to define *** analyzed the prevalence,risk factors and prognosis of patients with ***:The median follow-up was 214 days;the incidence of infection after liver transplantation was 46.7%(n=98)which included pneumonia(43.4%),biliary tract infection(21.9%),peritonitis(21.4%)and bloodstream infection(7.6%).Among the pathogens in pneumonia,the most frequently isolated was Acinetobacter baumanii(23.5%)and Klebsiella pneumoniae(21.2%).Model for end-stage liver disease(MELD)score(OR=1.083,95%CI:1.045–1.123;P<0.001),biliary complication(OR=4.725,95%CI:1.119–19.947;P=0.035)and duration of drainage tube(OR=1.040,95%CI:1.007–1.074;P=0.017)were independent risk factors for posttransplant ***-cause mortality was 11.0%(n=23).The prognostic factors for postoperative infection in liver recipients were prior-transplant infection,especially pneumonia within 2 weeks before ***-Meier curves of survival showed that recipients within 2 weeks prior infection had a significantly lower cumulative survival rate compared with those without infection(65.2%vs.90.0%;hazard ratio:4.480;P<0.001).Conclusions:Infection,especially pneumonia within 2 weeks before transplantation,complication with impaired renal function and MELD score after 7 days of transplantation was an independent prognostic factor for postoperative infection in liver transplant recipients.

AIM:To confirm the hypothesis that polymorphisms of the uncoupling protein 3(UCP3)gene are associated with the occurrence of nonalcoholic fatty liver disease(NAFLD).METHODS:A total of 250 NAFLD patients(147 malesand 103 females)and 200 healthy individuals who served as controls(control,109 males and 91 females),aged between 6 and 16 years were enrolled in this *** four non-synonymous single nucleotide polymorphisms(SNPs)in the UCP3 gene polymorphisms of rs1726745,rs3781907,rs11235972 and rs1800849,were genotyped using *** mass index(BMI),waist and hip circumference,blood pressure(BP),fasting blood glucose(FBG),insulin and lipid profiles were measured and B-ultrasound examination was performed in all ***:NAFLD patients showed risk factors for metabolic syndrome:elevated BMI,waist-to-hip ratio,BP,FBG,homeostasis model assessment-estimated insulin resistance,total triglyceride,total cholesterol and low-density lipoprotein-cholesterol,while decreased high-density lipoprotein-cholesterol level compared with the control *** GG genotype distributions of rs11235972 in the NAFLD group differed significantly from that in the control *** found that waist circumference between CC(58.76±6.45 cm)and CT+TT(57.00±5.59 cm),and hip circumference between CC(71.28±7.84 cm)and CT+TT genotypes(69.06±7.75 cm)were significantly different with and without rs1800849 variation(P<0.05).CONCLUSION:A higher prevalence of rs11235972 GG genotype was observed in the NAFLD group compared with the control *** differences were observed for the other ***,there was a significant difference in body height in addition to waist and hip circumference between the CC(mutant type group)and CT+TT group with and without rs1800849 variation.

Diabetes and cancer: Associations, mechanisms, and implications for medical practice

Both diabetes mellitus and cancer are prevalent diseases worldwide. It is evident that there is a substantial increase in cancer incidence in diabetic patients. Epidemiologic studies have indicated that diabetic patients are at significantly higher risk of common cancers including pancreatic, liver, breast, colorectal, urinary tract, gastric and female reproductive cancers. Mortality due to cancer is moderately increased among patients with diabetes compared with those without. There is increasing evidence that some cancers are associated with diabetes, but the underlying mechanisms of this potential association have not been fully elucidated. Insulin is a potent growth factor that promotes cell proliferation and carcinogenesis directly and/or through insulin-like growth factor 1(IGF-1). Hyperinsulinemia leads to an increase in the bioactivity of IGF-1 by inhibiting IGF binding protein-1. Hyperglycemia serves as a subordinate plausible explanation of carcinogenesis. High glucose may exert direct and indirect effects upon cancer cells to promote proliferation. Also chronic inflammation is considered as a hallmark of carcinogenesis. The multiple drugs involved in the treatment of diabetes seem to modify the risk of cancer. Screening to detect cancer at an early stage and appropriate treatment of diabetic patients with cancer are important to improve their prognosis. This paper summarizes the associations between diabetes and common cancers, interprets possible mechanisms involved, and addresses implications for medical practice.

Intracytoplasmic sperm injection (ICSI) is commonly used to solve male infertility problems. Previous studies showed that early environmental exposure of an embryo may influence postnatal development. To detect whether ICSI operations affect the reproductive health of a male or his offspring, we established assisted reproductive technologies (ART) conceived mouse models, and analyzed gene expression profiles in the testes of both ICSI and naturally conceived (NC) newborn F1 mice using micro-array analysis. Among the differentially expressed genes, we focused on the expression of eight male reproduction-related genes. Quantitative real-time reverse transcription-polymerase chain reaction (qRT-PCR) was used to analyze the expression of these genes in the testes of both adult and old F1 generation mice and adult F2 generation mice. Our results showed that down-regulated and somatic cell-expressed genes in newborn mice retained their differential expression patterns in adult and old F1 generation individuals, implying the persistence and fetal origin of the alteration in the expression of these genes. The intergenerational transmission of differential gene expression was observed, but most changes tended to be reduced in adult F2 generations. Controlled ovarian hyperstimulation (COH) and in vitro fertilization (IVF) mice models were added to explore the precise factors contributing to the differences in ICSI offspring. The data demonstrated that superovulation, in vitro culture, and mechanical stimulation involved in ICSI had a cumulative effect on the differential expression of these male reproductive genes.

Ectopic pregnancy is identified with the widely-applied assisted reproductive technology （ART）. Bilateral ectopic pregnancy is a rare form of ectopic pregnancy which is difficult to be diagnosed at the pre-operation stage. In this paper, we presented an unusual case of heterochronic bilateral ectopic pregnancy after stimulated intrauterine insemination （IUI）, where there has been a delay of 22 d between the diagnoses of the two ectopic pregnancies. Literature was reviewed on the occurrence of bilateral ectopic pregnancy during the past four years in the MEDLINE database. We found 16 cases of bilateral ectopic pregnancy reported since 2008, and analyzed the characteristics of those cases of bilateral ectopic pregnancy. We emphasize that ovulation induction and other ARTs may increase the risk of bilateral ectopic pregnancy. Because of the difficulty in identification of bilateral ectopic pregnancy by ultraso- nography, the clinician should be aware that the treatment of one ectopic pregnancy does not preclude the occurrence of a second ectopic pregnancy in the same patient and should pay attention to the intra-operation inspection of both side fallopian tubes in any ectopic pregnancy case.
