Case Report of Combined Sixth and Twelfth Cranial Nerve Palsy: A Rare Case of Clival Syndrome Arising from Thymoma

Godtfredsen syndrome or clival syndrome is a rare syndrome of abducens and hypoglossal nerve palsies (cranial nerve 6th and 12th respectively) that localizes to a clival mass. There are few reported cases of this clinical presentation. The aim of this case report is to describe this rare manifestation observed in a woman with clival metastases arising from a thymoma. A previously well 34-year-old native lady presented to a district hospital in Sabah, Malaysia, with history of blurring of vision and headache for 1 month. Cranial nerve examination reveals right abducens nerve palsy (right 6th CN) and right hypoglossal nerve palsy (right 12th CN). Initial imaging with CT brain reveals a subtle extra-axial hyperdense mass adjacent to the clivus and a routine chest x-ray reveals a mediastinal mass. Further imaging of thorax showed right anterior mediastinal mass, which then proceeded with Video-Assisted Thoracoscopic Surgery (VATS) guided biopsy. The biopsy result was consistent with the finding of thymoma, type B2. She was then diagnosed with aggressive form of thymoma, which unfortunately has metastasized to the bone, lung, liver and brain. Despite prognosis at the time of diagnosis is guarded, she still opted to undergo chemotherapy. Despite the completion of 6 cycles of chemotherapy, her disease progressed, and she eventually succumbed to the illness. In short, the presence of combined 6th and 12th palsy should alert clinician to the possibility of clival mass or metastases and hence could earlier workup with appropriate imaging can lead to earlier diagnosis and better treatment outcome.

ASLOP:A field-access affinity-based structure data layout optimizer

By rearranging the data,data layout optimizations improve the utilization of a cache line between two of its successive refills,thus reducing the total number of cache line refills and improving the performance of a *** this paper,we show that to enable structure data layout optimizations to be effective,two parameters,namely intra-instance affinity and inter-instance affinity,need to be considered at the same time in order to model the cache line utilization more *** also propose a lightweight approach to measure intra-instance affinity and inter-instance affinity to avoid complex memory trace analyses.A prototype,called ASLOP,has been implemented in the Open64 compiler and evaluated using benchmarks from SPEC CPU 2000,SPEC CPU 2006 and Olden benchmark suites that have extensive structure *** approach can achieve up to 48.1% performance improvement over the original programs,and 11.9% over the optimized programs using maximal reshaping,an existing approach that is known to produce close to the best results,on the two platforms we tested.

Mercury: Combining Performance with Dependability Using Self-Virtualization

Virtualization has recently gained popularity largely due to its promise in increasing utilization, improving availability and enhancing security. Very often, the role of computer systems needs to change as the business environment changes. Initially, the system may only need to host one operating system and seek full execution speed. Later, it may be required to add other functionalities such as allowing easy software/hardware maintenance, surviving system failures and hosting multiple operating systems. Virtualization allows these functionalities to be supported easily and effectively. However, virtualization techniques generally incur non-negligible performance penalty. Fortunately, many virtualization- enabled features such as online software/hardware maintenance and fault tolerance do not require virtualization standby all the time. Based on this observation, this paper proposes a technique, called Self-virtualization, which provides the operating system with the capability to turn on and off virtualization on demand, without disturbing running applications. This technique enables computer systems to reap most benefits from virtualization without sacrificing performance. This paper presents the design and implementation of Mercury, a working prototype based on Linux and Xen virtual machine monitor. The performance measurement shows that Mercury incurs very little overhead： about 0.2 ms on 3 GHz Xeon CPU to complete a mode switch, and negligible performance degradation compared to Linux.

Trust is one of the core components of any ad hoc network security *** management(TM)has always been a challenging issue in a vehicular *** such developing network is the Internet of vehicles(IoV),which is expected to be an essential part of smart *** originated from the merger of Vehicular ad hoc networks(VANET)and the Internet of things(IoT).Security is one of the main barriers in the on-road IoV *** security standards are insufficient to meet the extremely dynamic and rapidly changing IoV *** plays a vital role in ensuring security,especially during vehicle to vehicle *** networks,having a unique nature among other wireless ad hoc networks,require dedicated efforts to develop trust *** TM schemes are inflexible and *** scenarios and limited parameters are the basis for existing TM models that are not suitable for vehicle *** vehicular network requires agile and adaptive solutions to ensure security,especially when it comes to critical *** vehicle network’s wireless nature increases its attack surface and exposes the network to numerous security ***,internet involvement makes it more vulnerable to *** proposed TM framework is based on context-based cognition and machine learning to be best suited to IoV *** learning is the best solution to utilize the big data produced by vehicle *** handle the uncertainty Bayesian machine learning statistical model is *** proposed framework can adapt scenarios dynamically and infer using the maximum possible parameter *** results indicated better performance than existing TM ***,for future work,a high-level machine learning model is proposed.

在各种不同氢气储存方式中,用金属氢化物储存氢气具有体积小,压力低,安全性高等优点,其中A2B型的Mg2Ni介金属化合物具有高储氢量,质量轻,价格便宜等优点,而被视为最具潜力储氢系统.本实验采用(Mg2Ni)100-xAgxx(x=0,1.0及5.0)合金系统,进行机械合金法球磨处理,以X光衍射分析(XRD)检测技术观察粉体在球磨过程中结构变化,并将15h球磨处理后合金粉末进行储氢动力学曲线,压力-成分-温度曲线(P-C-Tcurve)与热分析等检测,对合金粉末储氢特性进行评估.结果发现15h球磨处理后,银元素会与Mg2Ni,Mg,Ni形成不同比例纳米晶粒混合物.在储氢特性方面,银元素的添加对Mg2Ni有明显改善,在350℃下,添加1.0%银元素与Mg2Ni粉体相比较,最大吸氢量由3.14%(ω)增加至3.83%(ω),可逆吸氢量由2.4%(ω)增加至3.0%(ω).

A Retrospective Study of Directed Blood Donations in the Kasungu District of Malawi

Since the discovery of blood circulation and transfusion, there has been an insatiable demand for voluntary blood donations throughout the world. However, gathering blood donors has never been easy because eligible donors constitute only a fraction of the general population and are often reluctant to donate. This is especially challenging in underprivileged countries of sub-Saharan Africa such as Malawi whose nationally run blood transfusion service struggles to maintain hospital blood banks. As a result, hospitals turn to their local communities for directed donations. A retrospective analysis from January 2014 to June 2016 of directed blood donor data from two hospitals in the Kasungu District of Malawi was conducted. The analysis of 2134 donations was carried out with respect to sex, age, hemoglobin concentration, blood group, and presence of transfusion-transmissible infections. On average, donors were 30 years of age and predominately male. Blood group O+ constituted more than half of all directed blood donations. Ultimately, about one third of donations were unable to be utilized for transfusion.
