Risk factors of biliary intervention by imaging after livingdonor liver transplantation

AIM:To determine the risk factors of biliary intervention using magnetic resonance cholangiopancreatography(MRCP) after living donor liver transplantation(LDLT).METHODS: We retrospectively enrolled 196 patients who underwent right lobe LDLT between 2006 and 2010 at a single liver transplantation center. Direct duct-to-duct biliary anastomosis was performed in all 196 patients. MRCP images routinely taken 1 mo after LDLT were analyzed to identify risk factors for biliary intervention during follow-up, such as retrograde cholangiopancreatography or percutaneous transhepatic biliary drainage. Two experienced radiologists evaluated the MRCP findings, including the anastomosis site angle on three-dimensional images, the length of the filling defect on maximum intensity projection, bile duct dilatation, biliary stricture, and ***: Eighty-nine patients underwent biliary intervention during follow-up. The anastomosis site angle [hazard ratio(HR) = 0.48; 95% confidence interval(CI), 0.30-0.75, P par.ng biliary intervention-free survival according to the anastomosis site angle revealed that lower survival was associated with a narrower anastomosis site angle(36.3% vs 62.0%, P < 0.001).CONCLUSION: The biliary anastomosis site angle in MRCP after LDLT may be associated with the need for biliary intervention.

Long-distance migratory birds travel more rapidly in spring than in autumn,as they face temporal breeding ***,several species travel slower in spring owing to environmental influences,such as food availability and wind *** trackers were attached to 17 Whooper Swans(Cygnus cygnus) inhabiting northeastern Mongolia,to determine their migration routes and stopover sites in spring and *** between spring and autumn migrations,migration-influencing par.meters,and the effect of spring stopover site temperatures were *** swans completed perfect tours between their wintering and breeding sites,and these data were used for *** migration lasted 57 days,with 49.2 days spent at 3.7 stopover *** migration lasted 21.5 days,with 17.5 days spent at 1.0 stopover ***,the swans traveled more rapidly in autumn than in *** distance,number of stopovers,migration speed,and straightness were important migration determinants in both spring and *** distance,stopover duration,number of stopovers,daily travel speed,travel duration,and migration speed differed significantly between spring and *** spring migration,the temperature at the current stopover sites and that at the future stopover sites displayed significant variations(t=1585.8,df=631.6,p par.icularly significant contribution toward developing detailed management plans for the conservation of their key habitats.

AIM: To evaluate the prognostic factors and efficacy of hepatic arterial infusion chemotherapy in hepatocellular carcinoma with portal vein tumor thrombosis. METHODS: Fifty hepatocellular carcinoma (HCC) patients with portal vein tumor thrombosis (PVTT) were treated using hepatic arterial infusion chemotherapy (HAIC) via a subcutaneously implanted port. The epirubicin-cisplatin-5-fluorouracil (ECF) chemotherapeutic regimen consisted of 35 mg/m 2 epirubicin on day 1, 60 mg/m 2 cisplatin for 2 h on day 2, and 500 mg/m 2 5-fluorouracil for 5 h on days 1-3. The treatments were repeated every 3 or 4 wk. RESULTS: Three (6%) of the 50 patients achieved a complete response (CR), 13 (26%) showed par.ial responses (PR), and 22 (44%) had stable disease (SD).The median survival and time to progression were 7 and 2 mo, respectively. After 2 cycles of HAIC, CR was achieved in 1 patient (2%), PR in 10 patients (20%) and SD in 26 patients (52%). Significant pre-treatment prognostic factors were a tumor volume of < 400 cm 3 (P = 0.01) and normal levels of protein induced by vitamin K absence or antagonist (PIVKA)-Ⅱ (P = 0.022). After 2 cycles of treatment, disease control (CR + PR + SD) (P = 0.001), PVTT response (P = 0.003) and α-fetoprotein reduction of over 50% (P = 0.02) were independent factors for survival. Objective response (CR + PR), disease control, PVTT response, and combination therapy during the HAIC were also significant prognostic factors. Adverse events were tolerable and successfully managed. CONCLUSION: HAIC may be an effective treatment modality for advanced HCC with PVTT in patients with tumors < 400 cm 3 and good prognostic factors.

Epidemiological studies of nuclear industry workers have important role to examine the health effect of low dose and low dose-rate radiation *** this study we analyzed the cohort of Korean nuclear power plants *** cohort consists of 7,459 male workers exposed to ionizing radiation and 6,508 controls without exposure *** paper presents the results of analysis on their cancer incidence between 1993 and 2002.A total of 58 cancers were ascertained for radiation workers and 50 for non-radiation ***-standardized rate ratio for two groups was 1.09（95% CI:0.74,1.62）.Trend test showed that cancer incidence rate in individual dose group was not significantly increased with increasing cumulative dose（p=0.2506）.Excess relative risk for all cancers combined was 6.77（95% CI:-4.02,27.54）per sievert,suggesting no statistically significant association between radiation dose and cancer *** of this study are not conclusive because of considerable *** size of the cohort,small cases of cancer incidence,and young ages of subjects limit the power of the *** we need further follow-up and the expansion of cohort to increase the power and to obtain stable risk estimate.

背景与目的一项比较艾日布林联合吉西他滨(eribulin plus gemcitabine,EG)或紫杉醇联合吉西他滨(paclitaxel plus gemcitabine,PG)作为转移性乳腺癌(metastatic breast cancer,MBC)患者一线化疗方案的II期临床试验发现,EG方案的神经毒性较低,但疗效与PG方案相似。在本研究中,我们分析了该临床实验中的癌症患者生命质量测评量表–紫杉烷(functional assessment of cancer therapy-taxane,FACT-Taxane)问卷评分以确定他们的生活质量(quality of life,QoL)。方法使用韩国版的FACT-Taxane问卷评估QoL。在评估基线之后,前12个疗程中每2个疗程评估1次QoL,之后每3个疗程评估1次QoL。使用线性混合模型评估EG组和PG组QoL的差异。结果在118例入组患者中,117例在基线时回复了FACT-Taxane问卷,PG组中有1例患者没有回复。EG组和PG组之间的基线QoL评分没有差异。在治疗期间,除了第11个疗程外,在第2–13个化疗疗程后,PG组紫杉烷亚量表评分显著高于EG组(均P<0.05)。神经病变特异性分析显示PG组患者的神经病症状比EG组患者更早出现且更严重(P<0.001)。结论在我们的QoL分析中,与PG方案相比,EG方案延迟并减少神经病变的发生。因此,艾日布林是MBC一线化疗药物紫杉醇的合理替代品。

Background: Electroacupuncture(EA) may reduce the severity of acute pancreatitis(AP) and provide additional pain relief in patients with chronic pancreatitis. However, the ability of EA to relieve pain in patients with AP has not been well ***: This study was undertaken to compar. the pain-relieving effects of EA and conventional treatment in patients with ***, setting, par.icipants and interventions: This study was conducted using a randomized, controlled,three-arm, par.llel-group and multi-center design. Patients diagnosed with AP were randomly and equally assigned to EA1, EA2 or control groups. All par.icipants received conventional standard-of-care therapy for AP. Local EA alone was administered in EA1, and local plus distal EA was given in *** EA included two abdominal acupoints, while distal EA included twelve peripheral acupoints. EA groups underwent one session of EA daily for 4 days(days 1–4), or until pain was resolved or *** outcome measures: The primary outcome measure was the change in the visual analogue scale(VAS;0–100) pain score between baseline and day ***: Eighty-nine par.icipants were randomized into EA1, EA2 and control groups, and 88(EA1, 30;EA2, 29;control, 29) were included in the full-analysis set. VAS score change(median [interquartile range]) on day 5 was(12.3 ± 22.5) in the EA1 group,(10.3 ± 21.5) in the EA2 group, and(8.9 ± 15.2) in the control group. There were not significant differences in the change in VAS score among treatments(P = 0.983). However, time to food intake was significantly shorter in the EA group(EA1 + EA2) than in the control group(median 2.0 days vs 3.0 days), with a hazard ratio of 0.581(P = 0.022;95% CI,0.366–0.924). No significant adverse events ***: EA treatment did not significantly reduce pain after 4 days of treatment in patients with AP-associated abdominal pain but significantly reduced time to first food intake.
