目的探讨老年维持性血液透析(MHD)患者老年营养危险指数(GNRI)与不良预后的关系。方法前瞻性选取2017年4月至6月北京市11家医院血液透析中心年龄≥60岁的MHD患者337例,收集患者基线资料,根据GNRI分为3组,无营养不良组226例(GNRI≥98)、... 详细信息

目的探讨老年维持性血液透析(MHD)患者老年营养危险指数(GNRI)与不良预后的关系。方法前瞻性选取2017年4月至6月北京市11家医院血液透析中心年龄≥60岁的MHD患者337例,收集患者基线资料,根据GNRI分为3组,无营养不良组226例(GNRI≥98)、轻度营养不良组81例(92≤GNRI<98)和重度营养不良组30例(GNRI<92)。随访至2018年6月,终点事件为全因死亡和心血管死亡。Kaplan-Meier生存曲线比较3组累积生存率的差异,多因素Cox回归模型分析GNRI与全因病死率和心血管病死率的关系。结果轻度营养不良组和重度营养不良组体质量指数和血清白蛋白水平及GNRI明显低于无营养不良组,差异有统计学意义(P<0.01)。中位随访时间52周(4.4~52.0周),期间共有56例(16.6%)死亡,心血管死亡25例(44.6%)。Kaplan-Meier生存曲线显示,3组患者的全因病死率(χ^(2)=30.484,P<0.01)和心血管病死率(χ^(2)=22.398,P<0.01)均有显著差异。多因素Cox回归分析显示,作为连续变量,GNRI升高是全因死亡(HR=0.910,95%CI:0.870~0.952,P=0.000)和心血管死亡(HR=0.895,95%CI:0.852~0.940,P=0.000)的保护因素;作为分类变量,轻度和重度营养不良与全因死亡和心血管死亡独立相关(P<0.05)。结论GNRI是老年MHD患者发生全因死亡和心血管死亡的独立危险因素,使用GNRI监测老年MHD患者营养状况可以预测不良预后的发生风险。

目的评价八段锦锻炼6个月对社区老年人平衡功能的影响。方法使用两平行组整群随机对照试验的方法对我国5个省(市)40个社区的1028名60~80岁社区居住老年人进行干预研究。干预组研究对象(20个社区、523名)进行每天1 h、每周5 d、持续6个月的八段锦锻炼和3次预防老年人跌倒健康教育;对照组研究对象(20个社区、505名)仅接受3次预防跌倒健康教育。研究主要指标为Berg平衡量表(BBS)得分,次要指标包括单脚站立时间、双脚前后站立时间、闭目原地踏步时间、计时起立行走时间。结果1028名研究对象进入分析,包括731名女性(71.11%)和297名男性(28.89%),年龄(69.87±5.67)岁。与基线调查相比,干预3个月后干预组BBS得分较对照组增加3.05(95%CI:2.23~3.88)分(P<0.001);干预6个月后干预组BBS得分较对照组增加4.70(95%CI:4.03~5.37)分(P<0.001)。与对照组相比,干预6个月后干预组研究对象的各项次要指标均有改善(P<0.05)。结论八段锦运动可以提高60~80岁社区老年人的平衡功能,运动时间越长,改善效果越好。

目的探讨我国头部外伤的基本特征,为其防控提供依据。方法收集2016年1月1日至2019年12月31日全国伤害监测系统(NISS)中的头部外伤病例数据,分析头部外伤患者的社会人口学特征、伤害事件的基本特征及临床特征。结果2016—2019年,NISS急诊或门诊首诊病例中,27.14%(1503196/5537821)为头部外伤病例;<1岁的患儿中,头部外伤者占该年龄段伤害病例的53.93%(9668/17927)。急诊或门诊头部外伤病例中的90.42%(1359210/1503196)因非故意伤害导致,主要致伤原因为跌倒或坠落(589830例,39.24%)、钝器伤(320483例,21.32%)和道路交通伤(273349例,18.18%)。急诊或门诊头部外伤中,10.67%(160436/1503196)的患者表现为脑震荡或脑挫裂伤。2016—2019年期间,NISS每年急诊或门诊伤害病例中,头部外伤病例的占比自27.81%(349978/1258658)降至25.92%(414262/1598413);所有头部外伤病例中,1~17岁[由24.88%(87065/349978)升至28.90%(119707/414262)]和60岁及以上病例[由11.84%(41437/349978)升至13.26%(54947/414262)]占比有所升高。结论儿童和老年人是目前我国头部外伤病例中的重点人群,跌倒、坠落和道路交通伤害是我国人群头部外伤的主要致伤原因,儿童坠落、老年人跌倒和道路交通伤害预防应成为头部外伤防控的重点。
