We shall introduce a new geometric constant A（X） of a Banach space X, which is closely related to the modulus of smoothness px （T）, and investigate it in relation with the constant As （X） by Baronti et al., the von Neumann-Jordan constant CNj（X） and the James constant J（X）. A sequence of recent results on these constants as well as some other geometric constants will be strengthened and improved.

Since extrahepatic bile duct cancer is difficult to diagnose and to cure, a safe and radical surgical strategy is needed. In this review, the modes of infiltration and spread of extrahepatic bile duct cancer and surgical strategy are discussed. Extended hemihepatectomy, with or without pancreatoduodenectomy （PD）, plus extrahepatic bile duct resection and regional lyrnphadenectomy has recently been recognized as the standard curative treatment for hilar bile duct cancer. On the other hand, PD is the choice of treatment for middle and distal bile duct cancer. Major hepatectomy concomitant with PD （hepatopancreatoduodenectomy） has been applied to selected patients with widespread tumors. Preoperative biliary drainage （BD） followed by portal vein embolization （PVE） enables major hepatectomy in patients with hilar bile duct cancer without mortality. BD should be performed considering the surgical procedure, especially, in patients with separated intrahepatic bile ducts caused by hilar bile duct cancer. Right or left trisectoriectomy are indicated according to the tumor spread and biliary anatomy. As a result, extended radical resection offers a chance for cure of hilar bile duct cancer with improved resectability, curability, and a 5-year survival rate of 40%. A 5-year survival rate has ranged from 24% to 39% after PD for middle and distal bile duct cancer.

To evaluate the stability of N-alkylated pyrrolidone derivatives （NRPs）, which are supposed to be used as precipitants for U（VI） and Pu（IV, VI） species in HNO3 media, under irradiation environment, some candidate NRPs were irradiated by γ-ray. Irradia- tion to HNO3 solutions up to 6 mol dm-3 （= M） containing 2 M N-n-butyl-2-pyrrolidone （NBP）, one of NRPs with lower hy- drophobicity, has revealed that the residual ratios of NBP in the samples of HNO3 up to 3 M decreased identically and linearly. Approximately 20% of NBP was found to be degraded after the irradiation at 1 MGy. It was also found that the decrease in the precipitation ratio of UOf＋（P.R., %） was gentle and that the P.R. values were relatively in accordance with the residual ratios of NBP. On the other hand, the degradation of the samples irradiated in 6 M HNO3 was found more distinguished. It was pro- posed from the analyses of degraded compounds that the degradation of NBP in HNO3 by γ-ray irradiation started from the cleavage of the pyrrolidone ring by the addition of oxygen atom originating from HNO3, followed by the formation of chain compounds by the successive addition of oxygen, leading to the generation of oxalic acid and acetic acid. The stability of other NRPs in 3 M HNO3 was evaluated to be nearly identical with that of NBP except lower P.R. values of the samples containing NRPs with higher hydrophobicity irradiated at more than 0.5 MGy.

Spontaneous rupture of splenic hamartoma in a patient with hepatitis C virus-related cirrhosis and portal hypertension:A case report and review of the literature

Spontaneous rupture is a rare complication of splenic hamartoma. A review of the literature revealed only four such cases. To the best of our knowledge, this is the first report of spontaneous rupture of splenic hamartoma associated with liver cirrhosis and portal hypertension. A 53-year-old woman, who was followed up for aortic dissection and hepatitis C virus （HCV）-related liver cirrhosis, was referred with sudden left chest and shoulder pain. An abdominal ultrasound showed intraabdominal bleeding, and computed tomography indicated rupture of a splenic tumor. Emergent splenectomy was carried out. The postoperative course was uneventful, and the patient was discharged on the 13th postoperative day. Pathology revealed the tumor to be a ruptured splenic hamartoma. The non-tumorous splenic parenchyma revealed congestive changes. We consider that the presence of liver cirrhosis and portal hypertension are risk factors for spontaneous rupture of the splenic hamartoma.

KL-6 mucin expression in carcinoma of the ampulla of Vater: Association with cancer progression

AIM： To assess histochemical expression of KL-6 and its clinicopathological significance in carcinoma of the ampulla of Vater. METHODS： Ampullary carcinoma tissues were collected from 38 patients who underwent pancreatoduodenectomy or local resection. Tissues were subjected to immunohistochemical analysis using KL-6 antibody. RESULTS： Positive staining of ampullary carcinoma cells was observed in 26 （68.4%） cases. Staining was not found in the surrounding non-cancer regions of the ampullary tissues. Remarkable KL-6 expression was observed in invasive carcinoma cells in pancreatic and duodenal tissues and in metastatic carcinoma cells in lymph nodes. Positive KL-6 expression was related to lymph node metastasis （P = 0.020）, pancreatic invasion （P = 0.016）, duodenal invasion （P = 0.034）, and advanced stage of TNM clinical classification （P = 0.010）. Survival analysis showed that positive expression of KL-6 was related to a poorer prognosis （P = 0.029）. CONCLUSION： The aberrant expression of KL-6 mucin is significantly related to unfavorable behaviors of cardnoma of the ampulla of Vater.

AIM： To study a more accurate quantification of hepatic fibrosis which would provide dinically useful information for monitoring the progression of chronic liver disease. METHODS： Using a cDNA microarray containing over 22000 clones, we analyzed the gene-expression profiles of non-cancerous liver in 74 patients who underwent hepatic resection. We calculated the ratio of azanstained： total area, and determined the morphologic fibrosis index （MFI）, as a mean of 9 section-images. We used the MFI as a reference standard to evaluate our method for assessing liver fibrosis. RESULTS： We identified 39 genes that collectively showed a good correlation （r 〉 0.50） between geneexpression and the severity of liver fibrosis. Many of the identified genes were involved in immune responses and cell signaling. To quantify the extent of liver fibrosis, we developed a new genetic fibrosis index （GFI） based on gene-expression profiling of 4 clones using a linear support vector regression analysis. This technique, based on a supervised learning analysis, correctly quantified the various degrees of fibrosis in both 74 training samples （r = 0.76, 2.2% vs 2.8%, P 〈 0.0001） and 12 independent additional test samples （r = 0.75, 9.8% vs 8.6%, P 〈 0.005）. It was far better in assessing liver fibrosis than blood markers such as prothrombin time （r = -0.53）, type IV collagen 7s （r = 0.48）, hyaluronic acid （r = 0.41）, and aspartate aminotransferase to platelets ratio index （APRI）（r = 0.38）. CONCLUSION： Our cDNA microarray-based strategy may help clinicians to precisely and objectively monitor the severity of liver fibrosis.

AIM： To investigate the cumulative development incidence and predictive factors for idiopathic pulmonary fibrosis in hepatitis C virus （HCV） positive patients. METHODS： We studied 6150 HCV infected patients who were between 40-70 years old （HCV-group）. Another 2050 patients with hepatitis B virus （HBV） were selected as control （HBV-group）. The mean observation period was 8.0 ± 5.9 years in HCV-group and 6.3 ± 5.5 years in HBV-group. The primary goal is the development of idiopathic pulmonary fibrosis （IPF） in both groups. The cumulative appearance rate of IPF and independent factors associated with the incidence rate of IPF were calculated using the Kaplan- Meier method and the Cox proportional hazard model. All of the studies were performed retrospectively by collecting and analyzing data from the patient records in our hospital. RESULTS： Fifteen patients in HCV-group developed IPF. On the other hand, none of the patients developed IPF in HBV-group. In HCV-group, the cumulative rates of IPF development were 0.3% at 10th year and 0.9% at 20th year. The IPF development rate in HCV-group was higher than that in HBV-group （P = 0.021）. The IPF development rate in patients with HCV or HBV was high with statistical significance in the following cases：（1） patients ≥ 55 years （P 〈 0.001）; （2） patients who had smoking index （package per day x year） of ≥20 （P = 0.002）; （3） patients with liver cirrhosis （P = 0.042）. CONCLUSION： Our results indicate that age, smoking and liver cirrhosis enhance the development of IPF in HCV positive patients.

In this session, I will start with the brief historyand principle of xerography, followed by introducing the latest status of xerography and digital color printer as a typical application product. Since Carlson invented xerography in 1938, it has been adopted as a high speed-copying machine, because the technology has an advantage in the image quality. Now, we handle huge volume of documents that created digitally. Hardcopy, however,still plays an important role to create and exchange knowledge. Especially, color hardcopy is recognized as an effective means for such communication. Various marking processes are used for digital color xerographic printers and multi-function printers (MFP). The methods and features of these processes are described first, and then my talk is converged on subjects of marking technologies of tandem color xerographic engine. The progresses in the key technologies, such as precise reproduction of fine dots, improvements of color registration and image stabilization technology, have enabled the tandem engine to achieve high performance products. The performance being required for printers or MFPs is not simply printing speed or print quality. From such a standpoint, I will describe the technologies that make products more valuable, such as downsizing, cost reduction and improvements in energy saving. In the rest of my talk, I will introduce Xerox iGen3 digital production press as an example of high speed and high quality printing technology in the print-on-demand market together with the perspective of xerography.
