Relationship between Lingual Papilla Surface Roughness and Oral Condition in Elderly Individuals

Objectives: Recently, an increasing number of elderly patients have complained of tongue pain and fissured tongue, while atrophy of the lingual papillae with low nutrition has also become a commonly encountered condition. The viewpoint that lingual papilla atrophy reflects systemic and pathological conditions, such as diabetes and in the circulatory system abnormalities, is supported by many clinicians. The present study was conducted to clarify the relationship of degree of atrophy of tongue papillae with oral symptoms, with an aim to evaluate the usefulness of clinical diagnostic criteria for tongue papillae atrophy. Subjects and Methods: The subjects were asked to protrude the tongue forward. The tongue was held at rest and impressions were taken of the tongue dorsum 15 mm posterior to the apex of the tongue. Surface impressions were measured using a Surfcoder SE300 and the obtained values were used to define surface roughness (Ra). Multivariate analyses of the relationships between the Ra measurements and the living environment, subjective symptoms of oral health, and survey results of measurements were performed to examine factors associated with Ra. The tongue was photographed with a digital camera, then RGB color value of four random points 15 mm from the tongue apex was determined. Using those findings, redness was calculated. Results and Conclusions: One hundred and six subjects with a mean age of 79.5 ± 9.2 years were analyzed. In the smooth group, there was more redness in the tongue dorsum as compared to the rough group. Total and Sagittal Ra values of subjects with dysphagia were lower than those without dysphagia, suggesting that the degree of oral mucosal atrophy is related to subjective symptoms of dysphagi

Fistula between digestive tract and airway is one of the complications after esophagectomy with lymph node dissection. A case of esophagotracheal fistula secondary to esophagitis 9 years after esophagectomy and gastric pull-up for treatment of esophageal carcinoma is described. It was successfully treated with transposition of a pedicled pectoralis major muscle flap.

Endoscopic submucosal dissection of a rectal carcinoid tumor using grasping type scissors forceps

Endoscopic submucosal dissection （ESD） with a knife is a technically demanding procedure a ssociated with ahigh complication rate. The shortcomings of this meth-od are the inability to fix the knife to the target lesion,and compression of the lesion. These can lead to major complications such as perforation and bleeding. To reduce the risk of complications related to ESD, we developed a new grasping type scissors forceps （GSF）,which can grasp and incise the targeted tissue using electrosurgical current. Colonoscopy on a 55-year-old woman revealed a 10-ram rectal submucosal *** histological diagnosis of the specimen obtained by biopsy was carcinoid tumor. Endoscopic ultrasonog-raphy demonstrated a hypoechoic solid tumor limitedto the submucosa without lymph node involvement. Itwas safely and accurately resected without unexpectedincision by ESD using a GSF. No delayed hemorrhage or perforation occurred. Histological examination confirmed the carcinoid tumor was completely excisedwith negative resection margin.

Long-term effects of lamivudine treatment in Japanese chronic hepatitis B patients

AIM： To analyze the association between the emergence of tyrosine-methionine-asparatate-asparatate （YMDD） mutants （reverse transcription; rtM204I/V） and deterioration of liver function during long-term lamivudine treatment of Japanese patients with chronic hepatitis B virus （HBV） infection. METHODS： The data of 61 consecutive Japanese pa- tients with chronic hepatitis B who underwent continu- ous lamivudine treatment for more than 24 mo and had a virological response were analyzed. Analysis of YMDD mutants was done by real-time polymerase chain reaction with LightCycler probe hybridization assay for up to 90 mo （mean, 50.8 too; range, 24-90 too）.RESULTS： A mixed mutant-type （YMDD ＋ tyrosine-iso- leucine-asparatate-asparatate： YIDD or tyrosine-valineasparatate-asparatate： YVDD） or a mutant-type （YIDD or YVDD） were found in 57.4% of 61 patients at i year, 78.7% of 61 patients at 2 years, 79.6% of 49 patients at 3 years, 70.5% of 34 patients at 4 years, 68.4% of 19 patients at 5 years, 57.1% of 14 patients at 6 years, and 33.3% of 6 patients at 7 years. Of the 61 patients, 56 （92%） had mixed mutant- or a mutant-type. Only 5 （8%） had no mutants at each observation point. Vi- rological breakthrough was found in 26 （46.4%） of 56 patients with YMDD mutants, 20 of whom had a hepa- titis flare-up： the remaining 30 （53.6%） had neither a virological breakthrough nor a flare-up. All 20 patients who developed a hepatitis flare-up had a biochemical and virological response after adefovir was added to the lamivudine treatment. CONCLUSION： Our results suggest that it is possible to continue lamivudine treatment, even after the emergence of YMDD mutants, up to the time that the patients develop a hepatitis flare-up.

Safety advantage of endocut mode over endoscopic sphincterotomy for choledocholithiasis

AIM： To evaluate whether an automatically controlled cut system （endocut mode） could reduce the complication rate of endoscopic sphincterotomy （EST） and serum hyperamylasemia after EST compared to the conventional blended cut mode. METHODS： From January 2001 to October 2003, 134 patients with choledocholithiasis were assigned to either endocut mode group or conventional blended cut mode group at the time of sphincterotomy. The two groups were retrospectively compared for the complications after EST and serum amylase level before and 24 h after the procedure. RESULTS： Of the 134 patients treated, 79 were assigned to conventional blended cut mode group and 55 to endocut mode group. There was no significant difference in age, sex, and serum amylase level before EST between the two groups. Complications were found in 5 patients of the endocut mode group （9%）： hyperamylasemia （5 times higher than normal） in 4 and moderate pancreatitis in 1. Complications were found in 13 patients of the conventional blended cut mode group （16%）： hyperamylasemia in 12 and moderate pancreatitis in 1. Serum amylase levels were elevated in both groups 24 h after EST （P〈0.02）. The average serum amylase level 24 h after EST in the conventional blended cut mode group was significantly higher than that in the endocut mode group （P〈 0.05）. CONCLUSION： Endocut mode offers a safety advantage over conventional blended cut mode for pancreatitis after EST by reducing hyperamylasemia.

Hepatocellular carcinoma metastasizing to the skull base involving multiple cranial nerves

We describe a rare case of HCV-related recurrent multiple hepatocellular carcinoma （HCC） metastasizing to the skull base involving multiple cranial nerves in a 50-year-old woman. The patient presented with symptoms of ptosis, fixation of the right eyeball, and left abducens palsy, indicating disturbances of the right oculomotor and trochlear nerves and bilateral abducens nerves. Brain contrast-enhanced computed tomography （CT） revealed an ill-defined mass with abnormal enhancement around the sella turcica. Brain magnetic resonance imaging （HR/） disclosed that the mass involved the clivus, cavernous sinus, and petrous apex. On contrast-enhanced HRI with gadolinium-chelated contrast medium, the mass showed inhomogeneous intermediate enhancement. The diagnosis of metastatic HCC to the skull base was made on the basis of neurological findings and imaging studies including CT and MRI, without histological examinations, Further studies may provide insights into various methods for diagnosing HCC metastasizing to the craniospinal area.

A Hybrid Particle-Grid Scheme for Computing Hydroelastic Behaviors Caused by Slamming

Capable and accurate predictions of some effects of strongly nonlinear interaction wave-ship associated with hydroelastic behaviors are very required for simulation tool in naval architect and ocean engineering. It can guarantee ship safety at the sea state by producing proper design. Therefore, we have developed a hybrid scheme based on both grid and particle method. In order to clarify hydroelastic behaviors of a ship, a dropping test of a ship with elastic motion has been performed firstly. The developed scheme has been then validated on ship dropping case under the same conditions with experiment. The comparisons showed consistently in good agreement. Furthermore, evaluation on hydroelastic behaviors of ship motion under slamming, the impact pressure tends to increase in increasing Froude number. （Fn） The bending moment and torque defined at the centre gravity due to hogging and sagging events can be predicted well, and their effects on the ship increase in increasing wave length even though the impact pressure decreases in increasing wave length after wave length λ/L, where L is ship length, is equal to 1.0. Moreover, hydroelastie behaviors affect the large heave and pitch amplitudes. Finally, the developed scheme can predict simultaneously hydrodynamic and hydroelastic with a strongly nonlinear interaction between wave and ship.

Inflammatory bowel disease of primary sclerosing cholangitis:A distinct entity?

This is a review of the characteristic findings of inflammatory bowel disease(IBD)associated with primary sclerosing cholangitis(PSC)and their usefulness in the diagnosis of sclerosing *** is a chronic inflammatory disease characterized by idiopathic fibrous obstruction and is frequently associated with *** with PSC(PSC-IBD)shows an increased incidence of pancolitis,mild symptoms,and colorectal *** an increased incidence of pancolitis is a characteristic finding,some cases are endoscopically diagnosed as right-sided ulcerative *** studies have revealed that inflammation occurs more frequently in the right colon than the left *** frequency of rectal sparing and backwash ileitis should be investigated in a future study based on the same *** cholangiographic findings of immunoglobulin G4-related sclerosing cholangitis(IgG4-SC)are similar to those of *** rare association between IBD and IgG4-SC and the unique characteristics of PSC-IBD are useful findings for distinguishing PSC from IgG4-SC.

Special alloy steel is widely used for the bearings and structural parts for the automobile, the office automation equipment and home electronics. NIPPON STEEL ENGINEERING CO.,LTD（NSEC）. started up first continuous caster for the special alloy steel in 1977. Since then, NSEC have been highly appreciated by special steel industry and have supplied the bloom continuous casters which applied a number of innovative technology to the special steel manufacturer and the integrated iron and steel manufacturer all over the world including Nippon Steel Corporation. In Japan before 1980, the special alloy steel was mainly produced by the vertical type bloom continuous caster in order to reduce the inclusions and the pinholes of bloom. However, the vertical type bloom continuous caster has many problems such as low productivity, expensive construction cost and difficult maintenance. In 1982, NSEC developed and supplied the large radius type large section bloom caster which can produce the special alloy steel for the automobile parts and the bearing, etc and can improve the productivity, the bloom internal and surface quality and the production yield. NSEC’s large radius type large section bloom caster has a number of advantages. Some of the advantages on bloom quality improvements are:（1）Reduction of the surface and internal cracks and internal inclusion thanks to Optimum Roll Profile Engineering.（2）Reduction of the surface cracks thanks to High Accuracy Mold Oscillation, Uniform Secondary Cooling and Bloom Surface Uniform Quenching.（3）Improvement of the center segregation thanks to Tundish Plasma Heater and Optimum Soft Reduction.（4）Decrease of the surface inclusions and pinholes thanks to Optimum M-EMS.（5）Decrease of the center porosity thanks to Innovative Equipment. All the above technology can be applied to new or existing bloom casters supplied not only by NSEC but also by other caster suppliers.

Coptidis Rhizoma Water Extract Stimulates 5'-AMP-Activated Protein Kinase in Rat Skeletal Muscle

AIM:Coptidis Rhizoma(CR),the dried rhizomes of Asian herbs(including Coptis chinensis Franch),has been used to treat diabetes mellitus for thousands of *** explored the possibility that CR acts directly on skeletal muscle,the major organ responsible for glucose homeostasis,and activates 5'-AMP-activated protein kinase(AMPK),a signaling intermediary leading to metabolic enhancement of skeletal ***:Isolated rat epitrochlearis and soleus muscles were incubated in a buffer containing a CR water extract(CE),and activation of AMPK and related events were ***:In response to CE treat-ment,phosphorylation of Thr172 at the catalyticαsubunit of AMPK,an essential step for full kinase activation,increased in both *** of Ser79 of acetyl CoA carboxylase(ACC),an endogenous substrate of AMPK,increased *** of isoform-specific AMPK activity revealed that CE activated both the α1 and α2 isoforms of the catalytic ***,the maximal effect of CE on AMPK phosphorylation was significantly greater than that of berberine(BBR),indicating that the action of CE is not totally ascribed to ***:We propose that CE is an acute activator of AMPK in both fast-and slow-twitch skeletal muscles.
