Characteristics and quality of clinical practice guidelines for diabetic kidney disease: a systematic review

OBJECTIVE: To assess the quality of Clinical practice guidelines(CPGs) in the context of diabetic kidney disease(DKD) and determine whether any factors affect the quality. METHODS: We searched eight databases along with five international and national organizations to develop or archive guidelines from their inception to July 2023, with an additional search of ***. And the authoritative organizations related to nephrology. CPGs and consensus statements created using direct differential diagnosis or therapy for DKD were included without language restrictions. Their quality was evaluated by four reviewers using the Appraisal of Guidelines for Research and Evaluation Ⅱ(AGREE Ⅱ) instrument. Along with the item and domain scores, the guideline was also allocated an overall quality score, which ranged from 1(lowest possible quality) to 7(highest possible quality). Moreover, an overall recommendation for use was also assigned(“recommended”, “recommended with modifications” or “not recommended”). RESULTS: A total of 16 CPGs were included, of which 14 were from Asia and the remaining two from Europe. These two CPGs were updated in the third version. Six CPGs were recommended for use because their primary domains scored in the medium or high category. Furthermore, five CPGs were recommended with modifications as the stakeholder involvement, applicability, and editorial independence domains were evaluated as low categories. In all domains, the lowest average score was for rigour of development(33%), followed by application(36%), and stakeholder involvement(51%). The highest average score was for scope and purpose(79%), followed by clarity of presentation(75%). None of the CPGs considered the patient's viewpoint, and six of 16 CPGs did not use any grading system to translate the evidence into recommendations. Additionally, only three of 16 CPGs shared search strategy, and eight of 16 CPGs did not declare a funding source. CONCLUSIONS: According to the AGREE II evaluation

Further study on the genesis of lamellar calcite veins in lacustrine black shale--A case study of Paleogene in Dongying Depression, China

In 2022,the Pakistan witnessed the hottest spring and wettest summer in *** devastating floods inundated a large portion of Pakistan and caused enormous ***,the primary water source and its contributions to these unprecedented floods remain *** on the reservoir inflow measurements,Multi-Source Weighted-Ensemble Precipitation(MSWEP),the fifth generation ECMWF atmospheric reanalysis(ERA5)products,this study quantified the contributions of monsoon precipitation,antecedent snow-melts,and orographic precipitation enhancement to floods in *** found that the Indus experienced at least four inflow up-rushes,which was mainly supplied by precipitation and snowmelt;In upper Indus,abnormally high temperature continued to influence the whole summer and lead to large amounts of snowmelts which not only was a key water supply to the flood but also provided favorable soil moisture conditions for the latter *** July,the snowmelt has higher contributions than the precipitation to the streamflow of Indus River,with contribution value of more than 60%.Moreover,the snowmelt could still supply 20%-40%water to the lower Indus in July and August;The leading driver of 2022 mega-floods over the southern Pakistan in July and August was dominated by the precipitation,where terrain disturbance induced precipitation account to approximately 33%over the southern *** results help to understand the mechanisms of flood formation,and to better predict future flood risks over complex terrain regions.

目的:观察超声引导下针刀疗法辅助治疗神经根型颈椎病低温等离子髓核射频消融术(PCDN)术后残留症状的疗效及安全性。方法:将70例神经根型颈椎病患者随机分为试验组和对照组,每组35例。对照组患者接受PCDN治疗。试验组患者在对照组基础上在PCDN术后第2天行超声引导下针刀治疗,每隔5~7 d进行1次,共行4~6次。分别于PCDN术前和术后1 d(针刀治疗前)及1、3、6个月时采用疼痛视觉模拟量尺(VAS)评分评价患者疼痛程度,采用颈部功能障碍指数(NDI)评分、日本骨科学会颈椎病(JOA)评分、田中靖久20分法量表评估患者颈椎病的严重程度,评估两组患者的临床疗效并计算治疗有效率和优良率,记录术后不良反应及并发症。结果:与PCDN术前比较,术后1 d及1、3、6个月时两组患者的VAS评分和NDI评分均降低(P<0.05),JOA评分和田中靖久20分法评分均升高(P<0.05)。与术后1 d比较,试验组患者术后1、3、6个月时VAS评分和NDI评分均降低(P<0.05),JOA评分和田中靖久20分法评分均升高(P<0.05)。与同时点对照组比较,试验组术后1、3、6个月时VAS评分和NDI评分均降低(P<0.05),JOA评分和田中靖久20分法评分均升高(P<0.05)。试验组患者PCDN术后1、3、6个月时有效率和优良率均高于同时间点对照组(P<0.05)。随访至术后1年,两组患者均无明显术后不良反应及并发症。结论:超声引导下针刀疗法可显著改善神经根型颈椎病PCDN后的残留症状,疗效明显优于单纯行PCDN手术,且安全可靠。

钢结构工程高强度螺栓微小松动视觉检测方法研究

基于计算机视觉的螺栓松动检测方法中,一类是通过对连接节点图像进行透视矫正并分析螺栓轮廓边缘直线角度的变化以及时发现微小的松动缺陷。然而,目前这类方法在图像矫正和螺栓轮廓边缘检测的可靠性方面还有较大的改进空间。为此,针对钢结构螺栓连接节点,提出了基于深度学习的螺栓松动视觉检测方法。首先,基于YOLOv5检测螺栓目标。针对螺栓感兴趣区域(RoI),基于鲁棒性较高的Attention U-Net提取螺栓轮廓边缘直线。为了提高螺栓目标检测精度,目标检测模型应设定较低置信度阈值以保证目标无漏检,再通过螺栓RoI提取的边缘直线数量筛除伪螺栓。使用透视变换法对节点图像进行矫正,变换所需的参考点是根据螺栓检测框之间空间移动后的相交关系自动定位的。最后,根据矫正图像中的螺栓轮廓边缘直线计算螺栓角度,通过检测螺栓和基准螺栓之间的角度差异判断松动情况。研究结果表明:螺栓目标检测的AP值为0.97;螺栓轮廓边缘检测的准确率、召回率和F1的均值分别为0.846、0.807与0.825,且在多种复杂背景干扰下具有较高的鲁棒性;伪螺栓筛除法可过滤掉99.82%的伪螺栓目标;提出的图像矫正法适用于常见的多种螺栓排列形式的连接节点;当松动判别阈值仅为2.8°时,螺栓松动检测的准确率可达99.7%。该方法在大型钢结构螺栓连接节点自动化运维方面具有较好的应用前景。

数字经济时代,数据要素成为企业实现高质量发展的重要支撑。本文以2007—2021年中国上市公司数据为样本,利用Python爬取上市公司历年年报,实证研究数据要素对企业全要素生产率的影响及机制。研究发现:数据要素提升了企业全要素生产率;数据要素通过增强包括企业适应能力、吸收能力、创新能力在内的动态能力,提升企业全要素生产率;数据要素在非国有企业、中小规模企业、高科技行业企业、中西部地区企业、成熟期企业中对全要素生产率的提升作用更大;组织赋能、环境支撑、技术驱动、战略引领与数据要素形成互补效应,提高企业全要素生产率,这在一定程度上解释是否存在“数据生产率悖论”。本文对激活数据活力,释放数据价值,触发动态能力,发挥数据要素互补效应,实现企业高质量发展提供了理论解释和经验支撑。

氨基酸表面活性剂（AAS）具有低毒性、抗菌性、温和性和抗腐蚀性等优良性能，近年来在日化、食品和医药等行业受到了广泛关注。研究表明，AAS分子级尺度的结构特点与其物理化学性质密切相关。本研究采用多种高分辨固体核磁共振（SSNMR）技术开展了固体状态下N-月桂酰基-L-丙氨酸（NLLA）氢键结构、局部动态学和分子组装结构的研究。通过2D1H-1H DQ-SQ和2D1H-1H DQ sideband pattern等技术并结合分子模拟，发现两个NLLA分子的羧基之间存在分子间氢键作用；结合13C-1H FSLG-HETCOR谱和13C T1的研究发现，相邻NLLA分子的酰胺基团之间存在氢键作用，继而推断NLLA分子通过分子间氢键作用形成具有反式和顺式两种构型的组装结构；此外，依据13C{1H}CP/MAS谱化学位移信息，发现NLLA烷烃链端呈反式（trans）和旁式（gauche）两种构象，其中，trans构象表现为两个13C共振谱峰，分别对应于NLLA分子通过氢键作用所组装的两种构型结构；最后，依据SSNMR实验数据，绘制了NLLA分子在固体状态下的分子组装结构示意图。本研究为NLLA等AAS样品或同类型材料的分子间氢键作用、组装结构的表征提供了可选择的SSNMR实验技术方案，从而为相关材料的构效关系研究提供数据支持。
