Management of.Penetrating Cranioencephalic Trauma Caused by Sharp Metal Objects—Therapeutic and Evolutionary Aspects: 12 Cases at the Renaissance University Hospital in N’Djamena

Introduction: Cranioencephalic trauma caused by bladed weapons is rare, and that caused by sharp objects is exceptional. The aim of.our study was to describe the clinical, therapeutic and evolutionary aspects. Materials and method: This was a descriptive and analytical study over a 48-month period at CHU la Renaissance f.om January 1, 2018 to December 31, 2021, concerning patients admitted f.r penetrating cranioencephalic trauma by pointed object. Results: Twelve cases, all male, of.penetrating cranioencephalic sharp-f.rce trauma were identif.ed. The mean age was 34 ± 7 years, with extremes of.11 and 60 years. f.rmers and herders accounted f.r 31% and 25% of.cases respectively. The average admission time was 47 hours. Brawls were the circumstances of.occurrence in 81.2% of.cases. Knives (33%), arrows (25%) and iron bars (16.6%) were the objects used. Altered consciousness was present in 43.8% of.cases, and f.cal def.cit in 50%. Scannographic lesions were f.acture and/or embarrhment (12 cases), intra-parenchymal haematomas (6 cases) and presence of.object in place (4 cases). Surgery was perf.rmed in 11 patients. Postoperative outcome was f.vorable in 9 patients. Af.er 12 months, 2 patients were declared unf.t. Conclusion: Penetrating head injuries caused by sharp objects are common in Chad. Urgent surgery can prevent disabling af.er-ef.ects.

In this study,Mn_(3)O_(4) spherical particles(SPs)were synthesized by the sol-gel process,af.er which they were thermally annealed at 400℃,and comprehensively characterized.X-ray Dif.raction(XRD)revealed that Mn_(3)O_(4) exhibited a tetragonal spinel structure,and f.urier transf.rmed inf.ared(f.IR)spectroscopy identif.ed surf.ceadsorbed f.nctional *** electron microscopy(SEM)and the specif.c surf.ce area analyses by Brunauer−Emmett−Teller(BET)revealed a porous,homogeneous surf.ce composed of.strongly agglomerated spherical grains with an estimated average particle size of.35 nm,which corresponded to a large specif.c surf.ce area of.81.5 m^(2)/g.X-ray photoelectron spectroscopy(XPS)analysis indicated that Mn_(3)O_(4) was composed of.metallic cations(Mn^(4+),Mn_(3)+,and Mn^(2+))and oxygen species(O_(2)−,OH−and CO_(3)2−).The optical bandgap energy is∼2.55 *** of.the catalytic perf.rmance of.the Mn_(3)O_(4) SPs indicated T90 conversion of.CH4 to CO_(2) and H2O at 398℃ f.r gas hourly space velocity(GHSV)of.72000 mL^(3) g^(−1) h^(−1).This observed perf.rmance can be attributed to the cooperative ef.ects of.the smallest spherical grain size with a mesoporous structure,which is responsible f.r the larger specif.c surf.ce area and available surf.ce-active oxygenated *** cooperative ef.ect of.the good reducibility,higher ratio of.active species(OLat/OAds),and results of.density f.nctional theory(Df.)calculations suggested that the total oxidation of.CH4 over the mesoporous Mn_(3)O_(4) SPs might take place via a two-term process in which both the Langmuir−Hinshelwood and Mars−van Krevelen mechanisms are cooperatively involved.

Two novel Schif. bases, 3-[1-(2-(phthalazin-1-yl)hydrazono)ethyl)-1,3-oxa- zinane (PHEO) and 2-[(2-(phthalazin-1-yl)hydrazono)methyl]phenol (PHMP), derived f.om hydralazine hydrochloride, an ef.ective drug against hypertension, were synthesized and characterized by spectroscopic methods, Inf.ared (IR), Proton Nuclear Magnetic Resonance (1H NMR) and Carbon-13 Nuclear Magnetic Resonance (13C NMR). PHEO showed low antimicrobial activity against one bacterial strain with MIC value of.250 μg/ml while PHMP showed interesting activity against 4 bacterial strains with MIC of.31.25 - 250 μg/ml compared to the standard drug, amoxicillin. PHEO and PHMP showed higher reducing activity on f.rric ions compared to Vitamin C. On lipid peroxidation, PHEO showed higher inhibition while PHMP showed lower inhibition compared to Vitamin C. Both compounds presented lower stimulating ef.ect and lower catalase activity compared to the standard Vitamin C. PHEO and PHMP showed less than 80% inhibition in the preliminary antiplasmodial assay and so were not considered f.r the dose-response studies.

Dissemination of.Resistance Integrons and Genes Coding f.r Blse and Cabapenemases in the Urban Drainage Network in Cote d’Ivoire

Antibiotic resistance has become a major threat to human health worldwide. Environment, particularly the water environment, has long been overlooked as a player in the antibiotic resistance cycle, although its role remains unclear. These can provide an ideal setting f.r the acquisition and dissemination of.antibiotic resistance, as they are f.equently af.ected by anthropogenic activities. The objective of.this study was to establish a dif.usion map of.resistance integrons used as genetic markers of.resistance associated with antibiotic resistance conf.rring genes (ARGs). Total DNA extracts f.om non-cultivable bacterial communities were used f.r the analyses. These communities were obtained f.om wastewater samples f.om 14 sites upstream and downstream of.drainage channels or ef.luents in the cities of.Abidjan, Bouaké, and Yamoussoukro. The results obtained correspond to the number of.positives among the treated samples (n = 39). Among the genetic markers of.dissemination, class 1 integrons were the most evident in 94.8% of.samples in Abidjan (93.3%), Bouaké (100%) and Yamoussoukro (91.6%). Class 2 integrons and class 3 integrons were f.und respectively in 41% and 51% of.all samples. Genes coding f.r β-lactamases and blaTEM was identif.ed in almost all samples at a rate of.97.4%. A co-presence of.the three genes blaTEM, blaSHV and blaCTX-M is also remarkable in the sites of.the city of.Yamoussoukro. Among the genes coding f.r carbapenemases, only blaKPC 17.94%, blaNDM 30.76% and blaOXA48 38.46% were detected in the samples.
