Optimization of nursing interventions for postoperative mental status recovery in patients with cerebral hemorrhage

BACKGROUND Hypertensive cerebral hemorrhage(HCH),the most common chronic diseases,has become a topic of global public health discussions.AIM To investigate the role of rehabilitative nursing interventions in optimizing the postoperative mental status recovery phase and to provide clinical value for future rehabilitation of patients with HCH.METHODS This randomized controlled study included 120 patients with cerebral HCH who were contained to our neurosurgery department between May 2021–May 2023 as the participants.The participants have randomly sampled and grouped into the observation and control groups.The observation group received the rehabilitation nursing model,whereas the control group have given conventional nursing.The conscious state of the patients was assessed at 7,14,21,and 30 d postoperatively.After one month of care,sleep quality,anxiety,and depression were compared between the two groups.Patient and family satisfaction were assessed using a nursing care model.RESULTS The results showed that the state of consciousness scores of the patients in both groups significantly increased(P<0.05)after surgical treatment.From the 14th day onwards,differences in the state of consciousness scores between the two groups of patients began to appear(P<0.05).After one month of care,the sleep quality,anxiety state,and depression state of patients were significantly better in the observation group than in the control group(P<0.05).Satisfaction with nursing care was higher in the observation group than in the control group(P<0.05).CONCLUSION The rehabilitation nursing model has a more complete system compared to conventional nursing,which can effectively improve the postoperative quality of life of patients with cerebral hemorrhage and improve the efficiency of mental state recovery;however,further analysis and research are needed to provide more scientific evidence.

污水处理厂是抗生素抗性基因(antibiotic resistance genes,ARGs)和抗生素抗性细菌(antibiotic resistant bacteria,ARB)重要的源和汇,生物气溶胶是ARGs和ARB自污水处理厂向周边环境释放的关键载体。目前缺乏对污水处理厂生物气溶胶抗生素抗性污染特征、来源及潜在风险的系统性总结。本文从采样方法、检测方法、逸散特征、来源、潜在危害和风险评估等方面对污水处理厂抗生素抗性污染研究现状进行综述。惯性采样法和过滤法是常用的污水处理厂抗生素抗性生物气溶胶主要采集方法,而宏基因组测序、组装和分箱为其ARGs组成、可移动性和宿主提供了有效的检测方法,抗多药类、抗杆菌肽类、抗氨基糖苷类、抗四环素类、抗β-内酰胺类、抗磺胺类、抗大环内酯类和抗糖肽类等抗性基因在污水处理厂PM10、PM_(2.5)和PM_(1.0)颗粒物中广泛检出。格栅间、生化反应池和污泥处理单元是污水处理厂PM10、PM_(2.5)和PM_(1.0)负载ARGs和ARB的主要释放单元。污水处理厂不同粒径生物气溶胶中致病性ARB的存在增加了抗生素治疗的难度,而污水和污泥对ARGs和ARB的释放起到了重要的源的贡献。本文在研究内容、研究技术和控制策略等方面也提出了相关展望,以期为污水厂生物气溶胶抗生素抗性污染的监测和防护提供参考和借鉴。

EDGE non-invasive radiosurgery for gastric neuroendocrine hepatic portal lymph node metastases:a case report

Treating metastatic gastric neuroendocrine neoplasms(gNENs) is challenging, especially for those with progressive disease during somatostatin analog processing. In this report, we present a case of a welldifferentiated grade 2, type 3 g NEN with metastatic hepatic portal lymph nodes. EDGE non-invasive radiosurgery(800 cGy × 5 F) was performed to radiate the metastatic hepatic portal lymph nodes. Three months after the hyperfractionated radiotherapy, no signs of metastatic hepatic portal lymph nodes were observed using 8 Ga-dotatate positron emission tomography-computed tomography or magnetic resonance imaging. Therefore, EDGE non-invasive radiosurgery could be a potential option for treating local metastatic nodes.

Based on the Crank-Nicolson and the weighted and shifted Grunwald operators,we present an implicit difference scheme for the Riesz space fractional reaction-dispersion equations and also analyze the stability and the convergence of this implicit difference scheme.However,after estimating the condition number of the coefficient matrix of the discretized scheme,we find that this coefficient matrix is ill-conditioned when the spatial mesh-size is sufficiently small.To overcome this deficiency,we further develop an effective banded M-matrix splitting preconditioner for the coefficient matrix.Some properties of this preconditioner together with its preconditioning effect are discussed.Finally,Numerical examples are employed to test the robustness and the effectiveness of the proposed preconditioner.

Closed-Form Solution for a Circular Nanohole with Surface Effects Under Uniform Heat Flux

This paper investigates the steady-state thermoelastic problem of a circular nanohole embedded in an infinitely large elastic plane subjected to a uniform far-field heat flux.A lowly conductive surface model is used to account for the effects of surface phonon scattering,while the complete Gurtin–Murdoch model is utilized to characterize the effects of surface tension and surface elasticity.The closed-form solution to the temperature and stress field surrounding the hole is derived in the context of complex variable methods.Several numerical examples are presented to analyze the influence of surface effects on thermal stress fields.It is shown that surface effects induce notable increases in normal and shear stresses around the hole.Specifically,all three stress components(hoop,normal,and shear)in the vicinity of the hole exhibit substantial augmentation with increasing surface tension and surface modulus.In particular,it is found that the presence of surface effects amplifies the variation in stress gradients and intensifies stress concentration around the hole.

Motion Planning for Autonomous Driving with Real Traffic Data Validation

Accurate trajectory prediction of surrounding road users is the fundamental input for motion planning,which enables safe autonomous driving on public roads.In this paper,a safe motion planning approach is proposed based on the deep learning-based trajectory prediction method.To begin with,a trajectory prediction model is established based on the graph neural network(GNN)that is trained utilizing the INTERACTION dataset.Then,the validated trajectory prediction model is used to predict the future trajectories of surrounding road users,including pedestrians and vehicles.In addition,a GNN prediction model-enabled motion planner is developed based on the model predictive control technique.Furthermore,two driving scenarios are extracted from the INTERACTION dataset to validate and evaluate the effectiveness of the proposed motion planning approach,i.e.,merging and roundabout scenarios.The results demonstrate that the proposed method can lower the risk and improve driving safety compared with the baseline method.

目的评价并比较含整合酶抑制剂的单片制剂方案比克恩丙诺片(B/F/TAF)与拉米夫定多替拉韦片(3TC/DTG)用于初治艾滋病期患者的临床疗效及安全性。方法本研究为一项回顾性队列研究,通过收集2020年10月至2023年7月在上海市(复旦大学附属)公共卫生临床中心使用B/F/TAF或3TC/DTG作为初始抗反转录病毒治疗方案且持续用药时间≥24周的艾滋病期患者的临床资料,比较B/F/TAF组与3TC/DTG组基线人类免疫缺陷病毒(HIV)-1 RNA,基线和治疗12周的CD4+T淋巴细胞计数,治疗24周的病毒学抑制率和病毒学失败率,以及治疗前后总胆固醇、血肌酐、尿酸等指标变化。统计学分析采用独立样本t检验、校正t检验、曼-惠特尼U检验、威尔科克森符号秩检验、χ^(2)检验。结果189例初治艾滋病期患者中,B/F/TAF组141例,3TC/DTG组48例。B/F/TAF组患者基线HIV-1 RNA为1.77(0.78,4.52)×10^(5)拷贝/mL,3TC/DTG组为0.97(0.24,2.20)×10^(5)拷贝/mL,差异有统计学意义(U=2221.00,P=0.006)。B/F/TAF组治疗24周病毒学抑制率为77.3%(109/141),无病毒学失败的患者;3TC/DTG组治疗24周病毒学抑制率为85.4%(41/48),1例(2.1%)病毒学失败。治疗12周时,B/F/TAF组CD4+T淋巴细胞计数较基线升高30%以上的患者占92.2%(130/141),3TC/DTG组为85.4%(41/48);B/F/TAF组CD4+T淋巴细胞计数较基线升高>100.00/μL的比例为67.4%(95/141),3TC/DTG组为52.1%(25/48);两组间差异均无统计学意义(χ^(2)=1.91、3.61,P=0.167、0.733)。B/F/TAF组和3TC/DTG组患者治疗24周时总胆固醇(W=2036.00,t=-5.42)、血肌酐(W=1098.00、234.00)、尿酸(W=2188.00、299.00)水平均较治疗前升高,轻中度肾功能不全患者比例(χ^(2)=22.29、8.22)均较治疗前增加,差异均有统计学意义(均P<0.01)。结论B/F/TAF与3TC/DTG用于初治艾滋病期患者具有良好的病毒学、免疫学疗效和安全性。

Enhancing single-cell encapsulation in droplet microfluidics with fine-tunable on-chip sample enrichment

Single-cell encapsulation in droplet microfluidics is commonly hindered by the tradeoff between cell suspension density and on-chip focusing performance.In this study,we introduce a novel droplet microfluidic chip to overcome this challenge.The chip comprises a double spiral focusing unit,a flow resistance-based sample enrichment module with fine-tunable outlets,and a crossflow droplet generation unit.Utilizing a low-density cell/bead suspension(2×10^(6) objects/mL),cells/beads are focused into a near-equidistant linear arrangement within the double spiral microchannel.The excess water phase is diverted while cells/beads remain focused and sequentially encapsulated in individual droplets.Focusing performance was assessed through numerical simulations and experiments at three flow rates(40,60,80μL/min),demonstrating successful focusing at 40 and 80μL/min for beads and cells,respectively.In addition,both simulation and experimental results revealed that the flow resistance at the sample enrichment module is adjustable by punching different outlets,allowing over 50%of the aqueous phase to be removed.YOLOv8n-based droplet detection algorithms realized the counting of cells/beads in droplets,statistically demonstrating single-cell and bead encapsulation rates of 72.2%and 79.2%,respectively.All the results indicate that this on-chip sample enrichment approach can be further developed and employed as a critical component in single-cell encapsulation in water-in-oil droplets.

湖南沅江鼎城段河蚬的性别组成与精子形态特征

本研究以湖南沅江鼎城段河蚬（Corbicula fluminea）为研究对象，在进一步验证该群体性别组成的基础上，分析壳长组成与性别比例之间关系，探讨该群体雄性、雌性和雌雄同体之间关系；同时以单鞭毛精子为参照，分析比较双鞭毛精子的形态特征，以期为我国河蚬的性别发生及生殖适应策略研究提供基础资料。结果显示：沅江鼎城段河蚬（n=770）雄性、雌性和雌雄同体最小性成熟壳长分别为2.92mm、5.66mm和5.30mm。697只性成熟河蚬中雄性、雌性和雌雄同体的比例近似1︰1︰6。雌雄同体的平均壳长[（22.55±0.33）mm,n=517]显著大于雄性[（20.44±1.03）mm,n=95]和雌性[（19.79±0.99）mm,n=85](P 0.05）。河蚬可以产生单鞭毛和双鞭毛两种类型的精子，单鞭毛精子头长范围4.93～21.79μm，平均值（14.27±0.82）μm(n=30)，双鞭毛精子头长范围10.29～22.04μm，平均值（15.62±0.62）μm(n=26)。单、双鞭毛精子头长差异不显著（P> 0.05）。双鞭毛精子（n=26）长尾的平均长度[（38.07±1.44）μm]显著大于其短尾[（31.08±1.60）μm]和单鞭毛精子（n=30）尾部长度[（30.15±1.75）μm](P0.05）。结果表明：湖南沅江鼎城段河蚬为雄性先熟，且可能存在雄性和（或）雌性向雌雄同体转换现象。河蚬具有单鞭毛型和双鞭毛型2种类型的精子，且双鞭毛精子的2个尾部不等长。同域共存河蚬的单鞭毛与双鞭毛精子在运动及受精能力方面的差异值得深入研究。
