Sexual and Reproductive Health Knowledge and Behaviour among Adolescents in Rural Burkina Faso

Background: Adolescents in developing countries have limited knowledge about the prevention of STIs and unwanted pregnancies. In several African countries, risky sexual behaviour persists, including early sexual debut, multiple sexual partners, economic and sexual exchange, and low condom and contraceptive use. The purpose of this study is to assess the sexual and reproductive health knowledge and behaviour of adolescents in rural Burkina Faso in order to improve their sexual and reproductive health. Method: This was a descriptive cross-sectional study with prospective data collection conducted from 07 March to 04 April 2022 in the area of the Demographic and Health Surveillance System of the Nouna Research Centre. A questionnaire was administered to a randomly selected sample of 1202 adolescents. Results: The mean age of the adolescents was 14.1 years and 56.6% were male. Only 58.2% of the adolescents had ever heard of HIV/AIDS. Of these, 95.7% did not know their HIV status. In addition, 15.8% had ever had sexual intercourse and 2.4% had been sexually active at an early age. Almost 45% had not used any contraceptive method the last time they had sex. Condoms were the most frequently used contraceptive method (47.4%). Among teenagers, 27.6% had been pregnant at least once, 7.3% were married and the average age at marriage was 18.45 years. Almost 7% (6.9%) of teenagers had more than one sexual partner. Conclusion: Adolescents have limited knowledge about reproductive health. Risky sexual behaviour persists. Awareness campaigns need to be intensified to improve their sexual and reproductive health.

Intestinal Parasitosis in Children among Internally Displaced Persons in the Hauts-Bassins Region of Burkina Faso

Introduction: The setting up of camps for internally displaced persons in Burkina Faso as a result of terrorism creates favorable conditions for the emergence of digestive parasitosis in these displaced populations. The objective of this study was to evaluate the prevalence of digestive parasitosis in children of this population in the Hauts-Bassins region. Material and methods: This was a prospective cross-sectional study from November to December 2020, in which stool samples were collected from internally displaced children from the Sinfra reception site in Banzon and from Poya in Karangasso Vigué. The stool samples were stored at room temperature, protected from dust and insects, in a cooler and then sent to the laboratory of the CHUSS in Bobo-Dioulasso where the parasitological analyses were performed. Results: The mean age was 5.8 ± 2.4 years. The most represented age group was 5 to 10 years (75.31%). The sex ratio is 1.61. The overall prevalence of intestinal parasite infection was 53.09%. The prevalence was statistically higher among those who consumed well water than those consuming tap water (p = 0.01). Monoparasitism by protozoa and helminths represented 74.42% and 6.68% respectively. The polyparasitism rate was 9.31%. The species identified in monoparasite patients were protozoa with Entamoeba coli (37.21%), Giardia lamblia (25.58%), Endolimax nanus (6.98%) and Entamoeba histolytica/dispar (4.65%). Biparasitism was identified with combinations, such as E. coli + E. histolytica/dispar (11.63%). Conclusion: There is a high prevalence of digestive parasitosis among IDPs in the Sinfra and Poya reception site. Educational sessions on hand washing before meals, use of drinking water, etc. should emphasize preventive measures in these IDP camps.

Spatio-Temporal Trend and Geographic Disparity of Infertility Prevalence in Burkina Faso, 2011 to 2020

Background: Infertility affected 10% to 25% of couples globally, and about half of the infertility cases were reported in sub-Saharan Africa. Infertility poses significant social, cultural, and health challenges, particularly for women who often face stigmatization. However, comprehensive and nationally representative data, including prevalence, temporal trends, and risk factors, are lacking, prompting a study in Burkina Faso to address the need for informed policies and programs in infertility care and management. Objectives: This study aims to better understand the spatiotemporal trend of infertility prevalence in Burkina Faso. Methodology: This is a retrospective population-based study of women infertility from healthcare facilities in Burkina Faso, during January 2011 to December 2020. We calculated the prevalence rates of infertility and two disparity measures, and examined the spatiotemporal trend of infertility. Results: Over the 10-year period (2011 to 2020), 143,421 infertility cases were recorded in Burkina Faso healthcare facilities, resulting of a mean prevalence rate of 3.61‰ among childbearing age women and 17.87‰ among women who consulted healthcare facilities for reproductive issues (except contraception). The findings revealed a significant increase of infertility, with the prevalence rate varied from 2.75‰ in 2011 to 4.62‰ in 2020 among childbearing age women and from 13.38‰ in 2011 to 26.28‰ in 2020 among women who consulted healthcare facilities for reproductive issues, corresponding to an estimate annual percentage change of 8.31% and 9.80% respectively. There were significant temporal and geographic variations in the prevalence of infertility. While relative geographic disparity decreased, absolute geographic disparity showed an increasing trend over time. Bilateral Anterior Shoulder Fracture-Dislocation: About a Case

Bilateral anterior dislocations of the shoulder are rare lesions. They are even rarer when combined with a bilateral fracture of major tubercles. We report the case of a 28-year-old patient admitted to trauma emergencies after trauma by falling from the top of a stool. Clinical and radiographic findings revealed bilateral anterior shoulder dislocation associated with bilateral fracture of major tubercles. The patient was treated orthopedically by a reduction of dislocations under General Anesthesia, immobilization of the shoulders for 6 weeks followed by reeducation. The functional result after 12 months was satisfactory with a UCLA score of 30 points.

Background: In sub Saharan Africa, small size surveys have demonstrated early high mortality among infected patients on antiretroviral therapies (ART). Few studies have been conducted in large cohorts of HIV-patients in public health care system in West Africa. Objectives: Our study aims to determine mortality rate and its predictors in a cohort of patients on ART in a public daycare hospital in Burkina Faso. Methods: We have carried out a retrospective cohort study. All HIV-infected patients on ART between January 1st 2008 and December 31st 2011 were included in the study. Survival probability was estimated by the Kaplan-Meier method. Cox regression analysis was used to identify associated factors to mortality. Results: A total of 2243 HIV-infected patients were included in the study. During the follow-up, 218 patients representing 9.7% were lost. About 104 patients representing 4.6% were transferred and 1691 representing 75.4% were still in the therapeutic cohort. There were 230 death cases for a total of 4282 persons-years, (5.4 deaths for 100 persons-years;95% CI: 4.8 -6.3). The survival probabilities after 6 months, 1 year and 2 years were 92.6%, 91% and 88.9% respectively. For the multivariate analysis, the following factors were independently associated to death: male gender, BMI .5 kg/m2, WHO stage 3 and 4, HIV-2, T-CD4 lymphocytes < 200/μl, haemoglobin rate g/dl and creatinine clearance 2. Conclusions: Our study provides for the first time mortality rates and its predictors among HIV-patients on antiretroviral treatment in a large cohort in public health sector in Burkina Faso. It highlights the importance of early HIV screening to limit ART initiation at advanced HIV infection stages.

Objective: To study emergency cesarean section in teenagers with the goal of reducing morbidity and maternal-fetal mortality. Methods: This was a descriptive cross-sectional study with prospective collection of data on 248 cases collected from July 1st to December 31st, 2016. Results: The incidence of cesarean section in teenagers was 56.4%. The average age was 18 ± 0.4 years old. The vast majority were primiparous (92.7%). The main indications for emergency Caesarean section were: pre-eclampsia and its complications (20.2%), acute fetal distress (18.5), pre-rupture syndrome (14.1%) and bone dystocia (11.7%). The maternal mortality rate was 1.6% and perinatal mortality was 134 per 1000 live births. Conclusion: The rate of caesarean section is high at Yalgado OUéDRAOGO Teaching Hospital of Ouagadougou. Postoperative complications are sometimes serious and compromise the maternal and fetal outcome. There is a need for increased surveillance of all pregnant and recently delivered women to reduce maternal and perinatal mortality in teenagers.

This paper reports the case of K.Z, a 15 year-old child received in the Dental Surgery Unit of the Yalgado Ouédraogo Teaching Hospital for consultation for some aesthetic reasons due to dental retentions. On his X-ray, the premolars 25, 34, 35, 44, 45 and the molar 47 were deeply impacted, retained by supernumerary teeth. The treatment consisted in the removal of persistent temporary teeth and supernumerary teeth with poor radicular set, except one premolar. The removal of this premolar located between the roots of tooth 36, was source of major risk;therefore, it was put under clinical supervision. After these extractions, a fixed orthodontic appliance enabled bringing impacted teeth to a functional position. Orthodontic treatment lasted 3 years with a monthly check-up. Ours results are stable after one year of decline.

Effect of Bendiocarb (Ficam<sup>®</sup>80% WP) on Entomological Indices of Malaria Transmission by Indoor Residual Spraying in Burkina Faso, West Africa

Context: The vector control is essential in malaria prevention strategies in several endemic countries in Africa including Burkina Faso. The high transmission of malaria occurs during the period of high vector abundance (August to October) in Burkina Faso. Therefore, a vector control strategy based on the use of indoor residual spraying targeting this period should provide effective protection against malaria. This study aimed to evaluate the effect of bendiocarb applied in indoor residual spraying on entomological parameters of malaria transmission in a pyrethroid resistance area in southwestern, Burkina Faso. Methods: CDC light trap and early morning collections by pyrethrum spray catches were performed monthly to determine the change in entomological parameter within malaria vector in sprayed (Diebougou) and unsprayed sites (Dano). The female’s malaria vectors collected by both methods were used to determine their blood feeding pattern, biting and sporozoites rates as well as the malaria transmission risk estimated by entomological inoculation rate. Results: A total of 26,276 mosquitoes (13,555 anopheline and 12,721 other culicines) were collected using both CDC light trap (9158 mosquitoes) and PSC collection methods (17,118 mosquitoes) from June to December 2012. An. gambiae complex was the predominant species collected. An. gambiae was the predominant species collected (P = 0.0005), comprising 88% of the total collected and the most infected species. Malaria vectors densities were significantly lower in sprayed villages (n = 4303) compared with unsprayed villages (n = 12,569) during post-spraying period (P = 0.0012). Evaluation of the Purification Performance of the Kossodo Wastewater Treatment Plant in Ouagadougou

The efficiency of a wastewater treatment system is classically measured by the quality of its discharge into the aquatic environment. This quality can only be achieved if certain aspects are taken into account, such as the design and operation of the system in place. The objective of the present work is to evaluate the purification performance of this system after 17 years of operation. It should be noted that the lagoon plant (WWTP) has a total of eight basins, three of which are anaerobic (ANA). The evaluation of the physicochemical and microbiological parameters in the anaerobic basins and in the maturation basins allowed us to note slightly basic pH values in the maturation basins (8.7 to 9.2) against values from 6.8 to 7.4 in the anaerobic basins. Also, the values in terms of organic and bacteriological pollution have also evolved. For COD and BOD5, we obtained respectively abatements of more than 73% for filtered COD, 55% for gross COD and 81% for BOD5 even if the residual concentrations do not respect the standards of wastewater discharge in our country. Also, the residual concentrations of ammonia nitrogen in the output effluents (on average 12.43 mg/L), do not meet the standard of discharge into surface waters of Burkina Faso (1.29 mg/L). Thus, nutrients such as nitrogen and phosphorus pollution have average abatement rates of 31% and 27% respectively. In sum, we can say that the lagoon system set up does not meet the requirements in terms of quality of discharges, which remains an environmental problem for the receiving environment.

Human Immunodeficiency Virus Prevention among HIV-Serodiscordant Couples in Burkina Faso: Biomedical Issues, Bioethical and Cultural Challenges

Context: In Burkina Faso, there are young HIV-serodiscordant partners who want to get married and wish to procreate. Objectives: The purpose of this research was: 1) to assess the sexual behaviour of young people in Burkina Faso, 2) to estimate their knowledge about the modes of HIV transmission, 3) to appreciate the cultural reasons of the desire to procreate among HIV serodiscordant couples and 4) to draw some bioethical lessons. Methods: From April to Septemr 2009, the survey on HIV and reproductive health in Burkina Faso carried out with 815 young people. Among them, 407 were females and 408 males (average age: 23.59 ± 2.99). They have voluntarily agreed to answer a questionnaire which gave the following results. Results: This study shows that young people in Burkina Faso had their first sexual intercourse at 18.99 ± 2.76 years, 23.68% (193/815) were afraid to be tested for HIV, 30.92% (252/815) have not yet been tested for HIV-serostatus and 39.75% (324/815) of them have never heard of HIV vertical transmission. Despite the fact that 36.81% (300/815) are HIV negative, they would be willing to marry the beloved HIV positive person and amongst them, 28.34% (231/815) want a child even with an HIV positive person beloved. Conclusion: So far, there is no effective vaccine against HIV. However, it is clear that Highly Active Anti-Retroviral Therapy (HAART) associated with ethical and cultural good options can contribute to reduce the spread of HIV in Sub-Saharan Africa.
