Flexible ureteroscopic treatment of kidney stones: How do the new laser systems change our concepts?

Objective: Flexible ureteroscopy (fURS) has become a widely accepted and effective technique for treating kidney stones. With the development of new laser systems, the fURS approach has evolved significantly. This literature review aims to examine the current state of knowledge on fURS treatment of kidney stones, with a particular focus on the impact of the latest laser technologies on clinical outcomes and patient safety.Methods: We conducted a search of the PubMed/PMC, Web of Science Core Collection, Scopus, Embase (Ovid), and Cochrane Databases for all randomized controlled trial articles on laser lithotripsy in September 2023 without time restriction.Results: We found a total of 22 relevant pieces of literature. Holmium laser has been used for intracavitary laser lithotripsy for nearly 30 years and has become the golden standard for the treatment of urinary stones. However, the existing holmium laser cannot completely powder the stone, and the retropulsion of the stone after the laser emission and the thermal damage to the tissue have caused many problems for clinicians. The introduction of thulium fiber laser and Moses technology brings highly efficient dusting lithotripsy effect through laser innovation, limiting pulse energy and broadening pulse frequency.Conclusion: While the holmium:yttrium-aluminum-garnet laser remains the primary choice for endoscopic laser lithotripsy, recent technological advancements hint at a potential new gold standard. Parameter range, retropulsion effect, laser fiber adaptability, and overall system performance demand comprehensive attention. The ablation efficacy of high-pulse-frequency devices relies on precise targeting, which may pose practical challenges.

BACKGROUND Rectal mucinous adenocarcinoma(MAC)is a rare pathological type of rectal can-cer with unique pathological features and a poor prognosis.It is difficult to diag-nose and treat early because of the lack of specific manifestations in some aspects of the disease.The common metastatic organs of rectal cancer are the liver and lung;however,rectal carcinoma with metastasis to subcutaneous soft tissue is a rare finding.CASE SUMMARY In this report,the clinical data,diagnosis and treatment process,and postope-rative pathological features of a patient with left waist subcutaneous soft tissue masses were retrospectively analyzed.The patient underwent surgical treatment after admission and recovered well after surgery.The final pathological diagnosis was rectal MAC with left waist subcutaneous soft tissue metastasis.CONCLUSION Subcutaneous soft tissue metastasis of rectal MAC is rare,and it can suggest that the tumor is disseminated,and it can appear even earlier than the primary ma-lignant tumor,which is occult and leads to a missed diagnosis and misdiagnosis clinically.When a subcutaneous soft tissue mass of unknown origin appears in a patient with rectal cancer,a ma-lignant tumor should be considered.

A universal performance-enhancing method for li-S batteries:the cathode material of li_(2)S@li_(2)S_(2)@li_(2)S_(6) double-shell structure

lithium sulfide(li_(2)S)as a cathode material for lithium-sulfur(li-S)batteries,one of the most promising advanced batteries in the future,has received tremendous attention in the past decades.However,developing the practical li_(2)S cathode confronts challenges of low conductivity for li-ions and electrons,high sensitivity to environmental moisture,big overpotential barrier to electrochemical activation,and poor cyclability due to the shuttle effect of intermediate species.This article herein reports a simple and effective strategy for making li_(2)S@li_(2)S_(2)@li_(2)S_(6) double-shelled microparticles,which can significantly mitigate these problems.They are synthesized by dissolving li2S together with S in dimethoxyethane,then drying off the solvent,and finally calcining the collected solid.Compared with pure li_(2)S,such a double-shell material presents a 26.7% improvement in cycling capacity,0.5 V lower in activation overpotential,and prolonged tolerance in the ambient environment.The density functional theory calculation shows that the performance enhancement lies in the higher stability of li2S6in contact with moisture and some autocatalytic effect of li_(2)S_(2)@li_(2)S_(6).Such a double-shell structure becomes a universal performanceenhancing approach when being combined with other means,such as cathodes composited with catalytic MoS_(2),separators modified with selenium-doped sulfurized-polyacrylonitrile/montmorillonite,electrolytes containing fluorenone additive,and li anodes coated with a layer of li_(3)N.The corresponding capacity retention shows up to 80%improvement compared with pure li_(2)S.

Constrained docking control for shipborne UAV SideArm recovery

In this work,we sought to investigate constrained docking control during shipborne SideArm recovery of an Unmanned Aerial Vehicle(UAV)under preassigned safe docking constraints,rough ocean environments,and different initial positions.The aim was to solve the UAV tracking-lag problem that manifests when attempting to dock with a rapidly moving SideArm and to improve the accuracy and rapidity of docking.First,together with the formulations of the shipborne SideArm system and environmental airflows,the affine nonlinear dynamics of the hook was established to reduce tracking lag.Then,echo state network approximators with good approximation capacity and low computational consumption were designed to accurately approximate the UAV’s unknown nonlinear dynamics.With feedforward compensation provided by these approximators,a nonlinear-mapping-based constrained docking control law was developed for shipborne SideArm recovery of UAVs.This approach to controlling the docking trajectory and the forward docking speed of the UAV can achieve rapid and exact docking with a moving SideArm,without violating the preassigned safe docking-constraint envelopes.Simulations under different docking scenarios were used to validate the effectiveness and advantages of the proposed docking-control algorithm.

An Efficient Multifunctional Soluble Catalyst for li-O2 Batteries

Aprotic lithium-oxygen(li-O_(2))batteries have a high theoretical energy density,but they face challenges such as cathode blockage,high charge overpotential,and poor cycling stability.These are caused by sluggish reaction kinetics and severe parasitic reactions.Enhancing the performance of li-O_(2) batteries necessitates the development of efficient catalysts.These catalysts not only augment both the oxygen reduction reaction(ORR)and the oxygen evolution reaction(OER)but also inhibit undesirable parasitic reactions.In this work,we demonstrated for the first time a multifunctional soluble catalyst of iridium(III)acetylacetonate(Ir(acac)_(3))that could speed up oxygen electrochemistry.Ir(acac)_(3) regulated the ORR pathway and the reactivity of superoxide radical species by forming a reversible intermediate complex(Ir(acac)_(3)-O_(2)^(−)).During charging,Ir(acac)_(3) acted as a redox mediator and aided in li_(2)O_(2) decomposition by reacting with superoxide intermediates.Moreover,as demonstrated by operando UV-visible spectroscopy,the lower charge potential significantly reduced the generation of highly reactive singlet oxygen(^(1)O_(2))intermediates.As a result,the Ir(acac)_(3)-mediated li-O_(2) battery showed low overpotential,large capacity,and stable cyclability.This study offers a new approach to achieving efficient li-O_(2) batteries and provides an opportunity to suppress parasitic reactions.

Succinylation is a highly conserved post-translational modication that is processed via enzymatic and non-enzymatic mechanisms.Succinylation exhibits strong effects on protein stability,enzyme activity,and transcriptional regulation.Protein succinylation is extensively present in the liver,and increasing evidence has demonstrated that succinylation is closely related to hepatic metabolism.For instance,histone acetyltransferase 1 promotes liver glycolysis,and the sirtuin 5-induced desuccinylation is involved in the regulation of the hepatic urea cycle and lipid metabolism.Therefore,the effects of succinylation on hepatic glucose,amino acid,and lipid metabolism under the action of various enzymes will be discussed in this work.In addition,how succinylases regulate the progression of different liver diseases will be reviewed,including the desuccinylation activity of sirtuin 7,which is closely associated with fatty liver disease and hepatitis,and the actions of lysine acetyltransferase 2A and histone acetyltransferase 1 that act as succinyltransferases to regulate the succinylation of target genes that influence the development of hepatocellular carcinoma.In view of the diversity and significance of protein succinylation,targeting the succinylation pathway may serve as an attractive direction for the treatment of liver diseases.

Plants affect the horizontal transmission of a new densovirus infecting the green peach aphid Myzus persicae by modulating honeydew production

In a tritrophic context of plant-insect-entomopathogen,plants play important roles in modulating the interaction of insects and their pathogenic viruses.Currently,the influence of plants on the transmission of insect viruses has been mainly studied on baculoviruses and some RNA viruses,whereas the impact of plants on other insect viruses is largely unknown.Here,we identified a new densovirus infecting the green peach aphid Myzus persicae and tested whether and how host plants influence the transmission of the aphid densovirus.The complete single-stranded DNA genome of the virus,M.persicae densovirus 2,is 5727 nt and contains inverted terminal repeats.Transcription and phylogenetic analysis indicated that the virus was distinct from other a few identified aphid densoviruses.The virus abundance was detected highly in the intestinal tract of aphids,compared with the lower level of it in other tissues including head,embryo,and epidermis.Cabbage and pepper plants had no obvious effect on the vertical transmission and saliva-mediated horizontal transmission of the virus.However,the honeydew-mediated horizontal transmission among aphids highly depended on host plants(65%on cabbages versus 17%on peppers).Although the virus concentration in the honeydew produced by aphids between 2 plants was similar,the honeydew production of the infected aphids reared on peppers was dramatically reduced.Taken together,our results provide evidence that plants influence the horizontal transmission of a new densovirus in an aphid population by modulating honeydew secretion of aphids,suggesting plants may manipulate the spread of an aphid-pathogenic densovirus in nature.

Low-molecular-weight heparin and preeclampsia—does the sword cut both ways?Three case reports and review of literature

BACKGROUND Low-molecular-weight heparins(LMWH)are the most commonly used anticoagulants during pregnancy.It is considered to be the drug of choice due to its safety in not crossing placenta.Considering the beneficial effect in the improvement of microcirculation,prophylactic application of LMWH in patients with preeclampsia became a trend.However,the bleeding risk related with LMWH in preeclampsia patients has seldomly been evaluated.This current study aimed to identify the potential risks regarding LMWH application in patients with preeclampsia.CASE SUMMARY Herein we present a case series of three pregnant women diagnosed with preeclampsia on LMWH therapy during pregnancy.All the cases experienced catastrophic hemorrhagic events.After reviewing the twenty-one meta-analyses,the bleeding risk related with LMWH seems ignorable.Only one study analyzed the bleeding risk of LMWH and found a significantly higher risk of developing PPH in women receiving LMWH.Other studies reported minor bleeding risks,none of these were serious enough to stop LMWH treatment.Possibilities of bleeding either from uterus or from intrabdominal organs in preeclampsia patients on LMWH therapy should not be ignored.Intensive management of blood pressure even after delivery and homeostasis suture in surgery are crucial.CONCLUSION Consideration should be given to the balance between benefits and risks of LMWH in patients with preeclampsia.

Owing to the distinctive structural characteristics,vanadium nitride(VN)is highly regarded as a catalyst for oxygen reduction reaction(ORR)in zinc-air batteries(ZABs).However,VN exhibits limited intrinsic ORR activity due to the weak adsorption ability to O-containing species.Here,the S-doped VN anchored on N,S-doped multi-dimensional carbon(S-VN/Co/NS-MC)was constructed using the solvothermal and in-situ doping methods.Incorporating sulfur atoms into VN species alters the electron spin state of vanadium in the S-VN/Co/NS-MC for regulating the adsorption energy of vanadium sites to oxygen molecules.The introduced sulfur atoms polarize the V 3d_(z)^(2) electrons,shifting spin-down electrons closer to the Fermi level in the S-VN/Co/NS-MC.Consequently,the introduction of sulfur atoms into VN species enhances the adsorption energy of vanadium sites for oxygen molecules.The*OOH dissociation transitions from being unspontaneous on the VN surface to a spontaneous state on the S-doped VN surface.Then,the ORR barrier on the S-VN/Co/NS-MC surface is reduced.The S-VN/Co/NS-MC demonstrates a higher half-wave potential and limiting current density compared to the VN/Co/N-MC.The S-VN/Co/NS-MC-based liquid ZABs display a power density of 195.7 m W cm^(-2),a specific capacity of 815.7 m A h g^(-1),and a cycling stability exceeding 250 h.The S-VN/Co/NS-MC-based flexible ZABs are successfully employed to charge both a smart watch and a mobile phone.This approach holds promise for advancing the commercial utilization of VN-based catalysts in ZABs.
