Prognostic value of postoperative complication for early recurrence after curative resection of hepatocellular carcinoma

Background: Postoperative complications may adversely affect oncological outcomes. The aim of this study was to evaluate the impact of postoperative complications on early-phase recurrence after curative resection for hepatocellular carcinoma(HCC).Methods: We included 145 HCC patients who underwent initial and curative resection between January2004 and December 2013. Postoperative complications of grade III or higher based on Clavien–Dindo classification were defined as clinically relevant postoperative complications. Recurrence within two years after hepatectomy was defined as early-phase ***: Thirty-eight patients(26%) developed postoperative complications. The only predictive factor for postoperative complication was longer operative duration(P = 0.037). The disease-specific survival rate of patients with complication was lower than that of patients without complications(P = 0.015). Earlyphase recurrence was observed in 20/38(53%) patients who suffered postoperative complications and36/107(34%) patients with no complications, which was statistically significant(P = 0.039). Multivariate analysis identified four factors contributing to early-phase recurrence: high serum AFP level(P = 0.042),multiple tumors(P y-phase recurrence after curative resection of HCC. Close observation of patients with postoperative complications may be a necessary treatment strategy for HCC.

Refueling for Steady-State Plasma by Repetitive Pellet Injection in Large Helical Device

A repetitive pellet injector has been developed for investigation ofrefueling issues towards the steady-state operation in Large Helical Device (LHD). Continuousoperation of more than 10000 pellet launching at 10 Hz has been demonstrated. The maximum repeatingrate is 11 Hz. No technical constraint for longer operation has been found. The reliability ofpellet launch has exceeded 99.9%. The initial application to the NBI-heated plasmas has beensuccessful in the last experimental campaign of LHD. Although the pulse length is limited by theoperational constraint of NBI, the plasma with a density of 8 X 10^(19) m^(-3) has been sustainedfor 2 s by the pellet injection at 10 Hz. A prospect for the future experiment is discussed on thebasis of the initial result.

During the coronavirus disease 2019(COVID-19)pandemic,the Japanese government implemented several infection control measures,such as states of emergency(SoE)and school closures,and encouraged universal masking and hand *** activity restrictions and improved hygiene were found to be associated with declines in the incidence of infuenza during the 2019–2020 winter season[1].Prior studies have found that the incidence of seven pediatric infections,such as pharyngoconjunctivitis and varicella,decreased during and after the school closures in Japan[2].As activity restrictions continue to be lifted,there is growing concern that pediatric infections may suddenly return given that the pool of susceptible children increased during the pandemic[3].In this study,we use national infectious disease sentinel data from January 2012 to October 2021 to assess changes in the reported number of pediatric infections in Japan during the COVID-19 *** hypothesize that the reporting of many infectious diseases decreased during the early stages of the pandemic but ultimately returned to normal when activity restrictions were lifted.

AIM To investigate non-ossifying fibromas(NOFs) common fibrous bone lesions in children that occur in bones of the lower *** We analyzed 44 cases of NOF including 47 lesions, which were referred with a working diagnosis of neoplastic lesions. Lesions were located in the upper extremities(1 proximal humerus, 1 distal radius) and the lower extremities(25 distal femurs, 12 proximal and 4 distal tibias, and 4 proximal fibulas).RESULTS Three cases had NOFs in multiple anatomical locations(femur and fibula in 1 case, femur and tibia in 2 cases). Overall, larger lesions > 4 cm and lesion expansion at the cortex were seen in 21% and 32% of cases, respectively. Multiple lesions with bilateral symmetry in the lower extremities suggest that these NOFs were developmental bone defects. Two patients suffered from fracture and were treated without surgery, one in the radius and one in the femur. Lesions in the upper extremities(i.e., humerus of a 4-year-old female and radius of a 9-year-old male) expanded at the cortex and lesion size increased with slow *** NOFs in the lower extremity had fewer clinical problems, regardless of their size and expansiveness. In these two upper extremity cases, the NOFs had aggressive biological features. It seems that there is a site specific difference, especially between the upper extremity and the lower extremity. Furthermore, NOFs in the radius are predisposed to fracture because of the slender structure of the radius and the susceptibility to stress.

Hatchery-roared juvenile black sea breams are characterized by a low level of highly unsaturated fatty acids in their bodies, as compared with wild fish. To assess the effect of docosahaxaenoic acid （DHA） on lipegenic and lipelysis enzymes, one-year fish were roared on a casein-based purified diet and a DHA fortified diet （ 1.5% DHA ethyl ester/kg diet） for 60 d, followed with a period of 55 d for starvation. Dietary DHA was effectively incorporated into the fish body. Fortification of DHA depressed activities of glucose-6-phosphate dehydrogenase and NADP-isocitrate dehydrogenase as lipogenic enzymes in the hepatopancroas and intraperitoneal fat body. Carnitine palmitoyltransferase as lipolysis enzyme in the hepatopancreas was active in the DHA fortified fish. Starvation after feeding experiment induced increased carnitine palmitoyltransferase activity in both control and DHA fortified fish and the activity remained higher in the DHA fortified fish, while the monoenes were selectively consumed prior to highly unsaturated fatty acids. These results indicated that dietary DHA depressed lipogenesis and activated lipolysis.
