A comparative study of sequential *** type Ⅰ tympanoplasty in patients with bilateral chronic otitis media-Mucosal type

Background:Patients with bilateral chronic otitis media-mucosal type have been conventionally treated with sequential *** simultaneous tympanoplasty is usually not preferred because of the theoretical risk of iatrogenic sensorineural hearing *** the advent of modern surgical instruments and surgical techniques,the risk is expected to be *** study compares the clinical outcomes in typeⅠtympanoplasty performed simultaneously and ***&methods:This randomized prospective study was carried out in a tertiary care hospital between August 2015 and July 2017.A total of 30 patients were divided into two groups of 15 *** study analyzed the graft uptake,pure tone audiogram findings pre-and post-operatively,duration of surgery and number of hospital visit for each patient and the outco mes were co mpared between both the ***:Patients undergoing bilateral simultaneous tympanoplasty had significantly lesser mean duration of surgery and number of hospital visits than the patients undergoing sequential *** uptake and postoperative wound infections were similar in both the *** hearing improvement was significantly better in the bilateral simultaneous tympanoplasty ***,further studies are needed to authenticate this *** of the patients had a postoperative deterioration of hearing or sensorineural hearing ***:Bilateral simultaneous tympanoplasty is not only feasible but also better than sequential tympanoplasty,especially in terms of operating time,follow-up and overall financial implications on the patient.

Chronic hepatitis B(CHB) is a major global health problem affecting an estimated 350 million people with more than 786000 individuals dying annually due to complications, such as cirrhosis, liver failure and hepatocellular carcinoma(HCC). Liver transplantation(LT) is considered gold standard for treatment of hepatitis B virus(HBV)-related liver failure and HCC. However, post-transplant viral reactivation can be detrimental to allograft function, leading to poor survival. Prophylaxis with high-dose hepatitis B immunoglobulin(HBIG) and anti-viral drugs have achieved remarkable progress in LT by suppressingviral replication and improving long-term survival. The combination of lamivudine(LAM) plus HBIG has been for many years the most widely used. However, life-long HBIG use is both cumbersome and costly, whereas long-term use of LAM results in resistant virus. Recently, in an effort to develop HBIG-free protocols, high potency nucleos(t)ide analogues, such as Entecavir or Tenofovir, have been tried either as monotherapy or in combination with low-dose HBIG with excellent results. Current focus is on novel antiviral targets, especially for covalently closed circular DNA(ccc DNA), in an effort to eradicate HBV infection instead of viral suppression. However, there are several other molecular mechanisms through which HBV may reactivate and need equal attention. The purpose of this review is to address post-LT HBV reactivation, its risk factors, underlying molecular mechanisms, and recent advancements and future of anti-viral therapy.

Preparation and Characterization of Electrically and Thermally Conductive Polymeric Nanocomposites

The dielectric properties of composites and nanocomposites composed of epoxy resin as base matrix and AlN (Aluminum Nitride) as micro and nanofiller has been studied at variable loading of AlN. To improve the dispersion of the filler within the polymer matrix, AlN was surface modified with silane coupling agent (SCA). The thermal conductivity behavior of epoxy/AlN composites and nanocomposites has been studied at variable percentage of filler and temperatures. Test result indicated an increase of thermal conductivity of the composites at 20 wt% of AlN. Also, silane treated composites exhibited improved electrical conductivity properties wherein the electrical insulation property decreased in terms of di-electric strength and resistivity.

Evaluating role of bone marrow-derived stem cells in dry age-related macular degeneration using multifocal electroretinogram and fundus autofluorescence imaging

AIM：To evaluate the role of bone marrow-derived stem cells in the treatment of advanced dry age-related macular degeneration（AMD）using multifocal electroretinogram（mf-ERG）and fundus autofluorescence ***：Thirty patients（60 eyes）with bilateral central geographic atrophy（GA）were *** eye of each patient received autologous bone marrow-derived hematopoietic stem cells（BM-HSCs）（group 1）and the fellow eye with better visual acuity served as control（group2）.The effect of stem cell therapy was determined in terms of visual acuity,amplitude and implicit time in mf-ERG and size of GA on fundus autofluorescence *** tests were performed at presentation and first,third and sixth month follow *** events（if any）were also ***：At 6mo follow-up there was no statistically significant improvement in median log MAR best corrected visual acuity（BCVA）in either ***-ERG revealed significant improvement in amplitude and implicit time in the intervention group.A significant decrease was also noted in greatest linear dimension（GLD）of GA in the eyes receiving stem cells[6.78±2.60 mm at baseline to 6.56±2.59 mm at 6mo（P=0.021）].However,no such improvement was noted in the control ***：Electrophysiological and anatomical improvement in the intervention group sheds light on the therapeutic role of *** studies are required to determine the stage of disease at which the maximal benefit can be achieved and to standardize the dose andfrequency of stem cell injection.

高桩桥梁受地震影响较大,特别是在可液化场地,以唐山地震中倒塌的胜利桥为原型,进行1∶14.44缩尺模型振动台试验。通过连续的白噪声信号,获得桩自振频率的变化,从而研究可液化场地高桩桥梁的倒塌机制。结果表明,振动台试验可以很好地再现液化宏观现象,随着液化深度的加深,桩自振频率不断减小,从而导致桩顶位移增大,进而可能导致桥梁破坏甚至倒塌。

Distribution of geochemical fractions of phosphorus and its ecological risk in sediment cores of a largest brackish water lake, South Asia

The current study focuses on the vertical profile of different geochemical fractions of phosphorus-loosely bound(Lo–P), aluminium bound(Al–P), iron bound(Fe–P), calcium bound(Ca–P), and organic bound phosphorus(O–P) along with ecological risk assessment of sediment cores from Chilika Lake, eastcoast of India. The percentage contribution of the different fractions to the sedimentary phosphorus in the sediment column of the whole lake are on the order: O–P(33.2%) 4 Ca–P(20.3%) 4 Fe–P(18%) 4 Al–P(6.7%) 4 Lo–P(0.35%). The Phosphorus Pollution Index(PPI) revealed the contamination of lake sediment with phosphorus. The principal component and cluster analyses highlighted the anthropogenic contribution of phosphorus. The negative loading of Ca–P with Ca points towards its origin from marine shells. The discriminate analysis showed that the variables like Ca–P, bio-available phosphorus(BAP), and p H were able to effectively discriminate the sectors in a significant manner.

System Identification becomes very crucial in the field of nonlinear and dynamic systems or practical *** most practical systems don’t have prior information about the system behaviour thus,mathematical modelling is *** authors have proposed a stacked Bidirectional Long-Short Term Memory(Bi-LSTM)model to handle the problem of nonlinear dynamic system identification in this *** proposed model has the ability of faster learning and accurate modelling as it can be trained in both forward and backward *** main advantage of Bi-LSTM over other algorithms is that it processes inputs in two ways:one from the past to the future,and the other from the future to the *** this proposed model a backward-running Long-Short Term Memory(LSTM)can store information from the future along with application of two hidden states together allows for storing information from the past and future at any moment in *** proposed model is tested with a recorded speech signal to prove its superiority with the performance being evaluated through Mean Square Error(MSE)and Root Means Square Error(RMSE).The RMSE and MSE performances obtained by the proposed model are found to be 0.0218 and 0.0162 respectively for 500 *** comparison of results and further analysis illustrates that the proposed model achieves better performance over other models and can obtain higher prediction accuracy along with faster convergence speed.

ICRC Bacillus a Vaccine Candidate Strain (C-44) Is Coated with Human IgG

Indian Cancer Research Centre (ICRC) bacillus strain (C-44), a candidate vaccine against leprosy is cultured in vitro in Dubos medium enriched with amino acids and human serum. The study was conducted to find out whether ICRC bacilli obtained from these cultures are coated with anti mycobacterial antibody. Anti-ICRC antibody raised by intradermal inoculation of sonicated ICRC bacilli in rabbits reacted with both human immunoglobulin G (IgG) and antigens of ICRC. Further, ICRC bacilli could also be fluoresced directly with FITC labelled anti human IgG. Positive fluorescence of ICRC could be abolished by digestion of human IgG with trypsin and carbon tetrachloride (CCL4). It is concluded that ICRC bacilli present in the vaccine are coated with human IgG.
