Intense venous reflux,quantified by a new software to analyze presurgical ultrasound,is associated with unfavorable outcomes of microsurgical varicocelectomy

The hemodynamic characteristics of venous reflux are associated with infertility in patients with varicocele;however,an effective method for quantifying the structural distribution of the reflux is lacking.This study aimed to predict surgical outcomes using a new software for venous reflux quantification.This was a retrospective cohort study of a consecutive series of 105 patients(age range:22–44 years)between July 2017 and September 2019.Venous reflux of the varicocele was obtained using the Valsalva maneuver during scrotal Doppler ultrasonography before microsurgical varicocelectomy.Using this software,the colored reflux signals were segmented,and the gray scale of the color pixels representing the reflux velocity was comprehensively quantified into the mean reflux velocity of the green layer(MRVG)and the reflux velocity standard deviation of the green layer(RVSDG).Spontaneous pregnancy and changes from baseline in the semen parameters were assessed during a 12-month follow-up period.Data were analyzed using logistic regression analysis.An association of the high MRVG group with impaired progressive motility(odds ratio[OR]=2.868,95%confidence interval[CI]:1.133–7.265)and impaired sperm concentration(OR=2.943,95%CI:1.196–7.239)was found during multivariate analysis.High MRVG(OR=2.680,95%CI:1.086–6.614)and high RVSDG(OR=2.508,95%CI:1.030–6.111)were found to be independent predictors of failure to achieve pregnancy following microsurgical repair.In summary,intense venous reflux is an independent predictor of impaired progressive motility,sperm concentration,and pregnancy outcomes after microsurgical varicocelectomy.

To the Editor:Keloids are a specific type of scar that exceeds the original wound boundary and unlikely to shrink spontaneously.Numerous methods for treating keloids have been reported,including surgical resection,radiation,injection,laser therapy,and cryotherapy.However,no single method can ensure a complete cure.Among the reported methods,the intralesional injection of steroids is widely used.A large number of studies have shown that the injection of glucocorticoids combined with 5-fluorouracil(5-FU)into the keloid can soften the texture,reduce the volume,and relieve the associated symptoms.[1]The conventional intralesional injection was routinely performed by directly injecting the drug into the parenchyma through the surface of the keloid in a multipoint manner.

Surgical treatment of malignant biliary papillomatosis invading adjacent organs: A case report

BACKGROUND Biliary papillomatosis(BP) is a rare disease characterized by multiple papillary adenomas in the intrahepatic and extrahepatic biliary tree and has a high risk of malignant transformation. Early diagnosis and treatment for BP are challenges, as it spreads along the biliary tract. A radical resection with an adequate resection margin is advocated in patients with malignant BP.CASE SUMMARY We report a case of BP with malignant transformation in a 52-year-old female patient who presented with emaciation, severe malnutrition, and a T-tube for biliary drainage that was placed for almost 2 years at the time she visited us. The enhanced magnetic resonance imaging of the upper abdomen revealed a neoplasm with a diameter of 15 cm located in the gallbladder area surrounding the common bile duct where a drainage tube was placed. It was the first case with malignant BP involving multiple organs and the abdominal wall and receiving a radical resection. Plastic surgeons helped close the incision by transferring the rectus muscle and external oblique muscle flap. A diagnosis of papillary carcinoma with diffuse malignant papillomatosis of the biliary duct was given.The postoperative course was uneventful and she was asymptomatic and in good physical condition at 35 mo postoperatively with appropriate chemotherapy.CONCLUSION Aggressive surgical treatment with appropriate chemotherapy is advocated for patients with malignant BP even if multiple organ invasion is present.

Corporate Credit Ratings Based on Hierarchical Heterogeneous Graph Neural Networks

order to help investors understand the credit status of target corporations and reduce investment risks,the corporate credit rating model has become an important evaluation tool in the financial market.These models are based on statistical learning,machine learning and deep learning especially graph neural networks(GNNs).However,we found that only few models take the hierarchy,heterogeneity or unlabeled data into account in the actual corporate credit rating process.Therefore,we propose a novel framework named hierarchical heterogeneous graph neural networks(HHGNN),which can fully model the hierarchy of corporate features and the heterogeneity of relationships between corporations.In addition,we design an adversarial learning block to make full use of the rich unlabeled samples in the financial data.Extensive experiments conducted on the public-listed corporate rating dataset prove that HHGNN achieves SOTA compared to the baseline methods.

Lithium metal batteries are regarded as prominent contenders to address the pressing needs owing to the high theoretical capacity.Toward the broader implementation,the primary obstacle lies in the intricate multi-electron,multi-step redox reaction associated with sluggish conversion kinetics,subsequently giving rise to a cascade of parasitic issues.In order to smooth reaction kinetics,catalysts are widely introduced to accelerate reaction rate via modulating the energy barrier.Over past decades,a large amount of research has been devoted to the catalyst design and catalytic mechanism exploration,and thus the great progress in electrochemical performance has been realized.Therefore,it is necessary to make a comprehensive review toward key progress in catalyst design and future development pathway.In this review,the basic mechanism of lithium metal batteries is provided along with corresponding advantages and existing challenges detailly described.The main catalysts employed to accelerate cathode reaction with emphasis on their catalytic mechanism are summarized as well.Finally,the rational design and innovative direction toward efficient catalysts are suggested for future application in metal-sulfur/gas battery and beyond.This review is expected to drive and benefit future research on rational catalyst design with multi-parameter synergistic impacts on the activity and stability of next-generation metal battery,thus opening new avenue for sustainable solution to climate change,energy and environmental issues,and the potential industrial economy.

Linear Quadratic Optimal Control for Systems Governed by First-Order Hyperbolic Partial Differential Equations

This paper focuses on linear-quadratic(LQ)optimal control for a class of systems governed by first-order hyperbolic partial differential equations(PDEs).Different from most of the previous works,an approach of discretization-then-continuousization is proposed in this paper to cope with the infinite-dimensional nature of PDE systems.The contributions of this paper consist of the following aspects:(1)The differential Riccati equations and the solvability condition of the LQ optimal control problems are obtained via the discretization-then-continuousization method.(2)A numerical calculation way of the differential Riccati equations and a practical design way of the optimal controller are proposed.Meanwhile,the relationship between the optimal costate and the optimal state is established by solving a set of forward and backward partial difference equations(FBPDEs).(3)The correctness of the method used in this paper is verified by a complementary continuous method and the comparative analysis with the existing operator results is presented.It is shown that the proposed results not only contain the classic results of the standard LQ control problem of systems governed by ordinary differential equations as a special case,but also support the existing operator results and give a more convenient form of computation.

Association between the cumulative triglyceride-glucose index and the recurrence of atrial fibrillation after radiofrequency catheter ablation

BACKGROUND Triglyceride-glucose(TyG)index values are a new surrogate marker for insulin resistance.This study aimed to explore the relationship between cumulative TyG index values and atrial fibrillation(AF)recurrence after radiofrequency catheter ablation(RFCA).METHODS A total of 576 patients with AF who underwent RFCA at the Second Affiliated Hospital of Xi'an Jiaotong University were included in this study.The participants were grouped based on cumulative TyG index values tertiles within 3 months after ablation.Cox regression and restricted cubic spline analyses were used to determine the relationship between cumulative TyG index values and AF recurrence.The predictive value of all risk factors was assessed by receiver operating curve analysis.RESULTS There were 375 patients completed the study(age:63.23±10.73 years,64.27%male).The risk of AF recurrence increased with increasing cumulative TyG index values tertiles.After adjusting for potential confounders,patients in the medium cumulative TyG index group[hazard ratio(HR)=4.949,95%CI:1.778–13.778,P=0.002]and the high cumulative TyG index group(HR=8.716,95%CI:3.371–22.536,P<0.001)had a higher risk of AF recurrence than those in the low cumulative TyG index group.The restricted cubic spline regression model also showed an increased risk of AF recurrence with increasing cumulative TyG index values.When considering cumulative TyG index values,left atrial diameter,and lactate dehydrogenase levels as a comprehensive factor,the model could effectively predict AF recurrence after RFCA[area under the curve(AUC)=0.847,95%CI:0.797–0.897,P<0.001].CONCLUSIONS Cumulative TyG index values were a risk factor for AF recurrence after RFCA.Monitoring longitudinal TyG index values may assist with optimized for risk stratification and outcome prediction for AF recurrence.

Clinical efficacy and pathological outcomes of transanal endoscopic intersphincteric resection for low rectal cancer

BACKGROUND Transanal endoscopic intersphincteric resection(ISR)surgery currently lacks sufficient clinical research and reporting.AIM To investigate the clinical effectiveness of transanal endoscopic ISR,in order to promote the clinical application and development of this technique.METHODS This study utilized a retrospective case series design.Clinical and pathological data of patients with lower rectal cancer who underwent transanal endoscopic ISR at the First Affiliated Hospital of Xiamen University between May 2018 and May 2023 were included.All patients underwent transanal endoscopic ISR as the surgical approach.We conducted this study to determine the perioperative recovery status,postoperative complications,and pathological specimen charac-teristics of this group of patients.RESULTS This study included 45 eligible patients,with no perioperative mortalities.The overall incidence of early complications was 22.22%,with a rate of 4.44%for Clavien-Dindo grade≥III events.Two patients(4.4%)developed anastomotic leakage after surgery,including one case of grade A and one case of grade B.Postoperative pathological examination confirmed negative circumferential resection margins and distal resection margins in all patients.The mean distance between the tumor lower margin and distal resection margin was found to be 2.30±0.62 cm.The transanal endoscopic ISR procedure consistently yielded high quality pathological specimens.CONCLUSION Transanal endoscopic ISR is safe,feasible,and provides a clear anatomical view.It is associated with a low incidence of postoperative complications and favorable pathological outcomes,making it worth further research and application.

Design and Cryogenic Performance of a Hexapod Platform for a Large Ground-based Wide Field Survey Telescope

The thermal gradient is an important factor that causes degradation to the image quality of telescopes. In order to ensure the accurate alignment of the primary focus unit and the primary mirror, the hexapod platform(as a corrector) is investigated in this paper. First, a ground-based telescope with 2.5 m aperture and 3.5 deg field of view is described. The telescope is under construction, and it is expected to be finished in 2023. Second, the hexapod platform with flexure hinges utilized to adjust the primary focus unit is proposed, which is applied as a corrector.Then, the inverse kinematics of the platform is established and an open-loop control system is built based on it.Finally, the cryogenic performance test for the hexapod platform is performed. The experimental results show that the resolution and repeatability of the translation for the hexapod platform can be achieved at the micrometer level.The resolution and repeatability of the rotation can be achieved at the arc-second level. Therefore, the cryogenic performance of the hexapod platform can meet the optical imaging requirements of the wide-field ground-based telescope. The kinematic analysis and cryogenic performance tests in the paper provide a technical reference for the precise alignment of the primary focus unit and the primary mirror, which can improve the imaging quality of the telescope.

An Improved Golden Jackal Optimization Algorithm Based on Multi-strategy Mixing for Solving Engineering Optimization Problems

Nowadays,optimization techniques are required in various engineering domains to find optimal solutions for complex problems.As a result,there is a growing tendency among scientists to enhance existing nature-inspired algorithms using various evolutionary strategies and to develop new nature-inspired optimization methods that can properly explore the feature space.The recently designed nature-inspired meta-heuristic,named the Golden Jackal Optimization(GJO),was inspired by the collaborative hunting actions of the golden jackal in nature to solve various challenging problems.However,like other approaches,the GJO has the limitations of poor exploitation ability,the ease of getting stuck in a local optimal region,and an improper balancing of exploration and exploitation.To overcome these limitations,this paper proposes an improved GJO algorithm based on multi-strategy mixing(LGJO).First,using a chaotic mapping strategy to initialize the population instead of using random parameters,this algorithm can generate initial solutions with good diversity in the search space.Second,a dynamic inertia weight based on cosine variation is proposed to make the search process more realistic and effectively balance the algorithm's global and local search capabilities.Finally,a position update strategy based on Gaussian mutation was introduced,fully utilizing the guidance role of the optimal individual to improve population diversity,effectively exploring unknown regions,and avoiding the algorithm falling into local optima.To evaluate the proposed algorithm,23 mathematical benchmark functions,CEC-2019 and CEC2021 tests are employed.The results are compared to high-quality,well-known optimization methods.The results of the proposed method are compared from different points of view,including the quality of the results,convergence behavior,and robustness.The superiority and high-quality performance of the proposed method are demonstrated by comparing the results.Furthermore,to demonstrate its applicability,it is employed to solve four c
