Development of heart failure risk prediction models based on a multi-marker approach using random forest algorithms

Background:The early identification of heart failure(HF)risk may favorably affect outcomes,and the combination of multiple biomarkers may provide a more comprehensive and valuable means for improving the risk of stratification.This study was conducted to assess the importance of individual cardiac biomarkers creatine kinase MB isoenzyme(CK-MB),B-type natriuretic peptide(BNP),galectin-3(Gal-3)and soluble suppression of tumorigenicity-2(sST2)for HF diagnosis,and the predictive performance of the combination of these four biomarkers was analyzed using random forest algorithms.Methods:A total of 193 participants(80 patients with HF and 113 age-and gender-matched healthy controls)were included from June 2017 to December 2017.The correlation and regression analysis were conducted between cardiac biomarkers and echocardiographic parameters.The accuracy and importance of these predictor variables were assessed using random forest algorithms.Results:Patients with HF exhibited significantly higher levels of CK-MB,BNP,Gal-3,and sST2.BNP exhibited a good independent predictive capacity for HF(AUC 0.956).However,CK-MB,sST2,and Gal-3 exhibited a modest diagnostic performance for HF,with an AUC of 0.709,0.711,and 0.777,respectively.BNP was the most important variable,with a remarkably higher mean decrease accuracy and Gini.Furthermore,there was a general increase in predictive performance using the multi-marker model,and the sensitivity,specificity was 91.5%and 96.7%,respectively.Conclusion:The random forest algorithm provides a robust method to assess the accuracy and importance of predictor variables.The combination of CK-MB,BNP,Gal-3,and sST2 achieves improvement in prediction accuracy for HF.

Surgical outcome after complete repair of tetralogy of Fallot with absent pulmonary valve: comparison between bovine jugular vein-valved conduit and monocusp-valve patch

Background The prognosis of tetralogy of Fallot with absent pulmonary valve (TOF/APV) without operation is poor. We evaluated the surgical outcome of TOF/APV in a single center. Methods Twenty-two TOF/APV patients underwent complete surgical correction in our hospital. Right ventricular outflow tract reconstruction was performed using bovine jugular vein (BJV)-valved conduit implantation (n=10), homograft-valved conduit implantation (n=2), or monocusp-valve patch (n=10). Health-related quality of life (QOL) was evaluated during follow-up. Results The overall survival at 5 and 10 years was 86.4±7.3% (confidence interval 69.4–97.2%). The survival rates were significantly different between patients with and without bronchial stenosis (40 and 100%, P=0.0003, log-rank test). The survival of patients aged>6 months was higher than those≤6 months (100 vs. 40%, P=0.0003, log-rank test). Patients with BJV-valved conduits had higher systolic gradients from the right ventricle to the pulmonary artery (RV–PA) compared to those with monocusp-valve patches. BJV-valved conduit implantation was a risk factor for post-operative pulmonary-valve stenosis. The QOL score for patients with BJV-valved conduits was lower than those with monocusp-valve patches (P<0.05). No reoperation was performed during follow-up. Conclusions Bronchial stenosis and lower age (≤6 months) were the main factors influencing post-operative survival. The use of a BJV-valved conduit was a main reason for RV–PA restenosis;thus, the use of a BJV-valved conduit may increase the need for repeat intervention and decrease the post-operative quality of life.

Mutations in the SARS-CoV-2 spike receptor binding domain and their delicate balance between ACE2 affinity and antibody evasion

Intensive selection pressure constrains the evolutionary trajectory of SARS-CoV-2 genomes and results in various novel variants with distinct mutation profiles.Point mutations,particularly those within the receptor binding domain(RBD)of SARS-CoV-2 spike(S)protein,lead to the functional alteration in both receptor engagement and monoclonal antibody(mAb)recognition.Here,we review the data of the RBD point mutations possessed by major SARS-CoV-2 variants and discuss their individual effects on ACE2 affinity and immune evasion.Many single amino acid substitutions within RBD epitopes crucial for the antibody evasion capacity may conversely weaken ACE2 binding affinity.However,this weakened effect could be largely compensated by specific epistatic mutations,such as N501Y,thus maintaining the overall ACE2 affinity for the spike protein of all major variants.The predominant direction of SARS-CoV-2 evolution lies neither in promoting ACE2 affinity nor evading mAb neutralization but in maintaining a delicate balance between these two dimensions.Together,this review interprets how RBD mutations efficiently resist antibody neutralization and meanwhile how the affinity between ACE2 and spike protein is maintained,emphasizing the significance of comprehensive assessment of spike mutations.

Experimental Study on Wire Melting Control Ability of Twin-Body Plasma Arc

The twin-body plasma arc has the decoupling control ability of heat transfer and mass transfer,which is beneficial to shape and property control in wire arc additive manufacturing.In this paper,with the wire feeding speed as a characteristic quantity,the wire melting control ability of twin-body plasma arc was studied by adjusting the current separation ratio(under the condition of a constant total current),the wire current/main current and the position of the wire in the arc axial direction.The results showed that under the premise that the total current remains unchanged(100 A),as the current separation ratio increased,the middle and minimum melting amounts increased approximately synchronously under the effect of anode effect power,the first melting mass range remained constant;the maximum melting amount increased twice as fast as the middle melting amount under the effect of the wire feeding speed,and the second melting mass range was expanded.When the wire current increased,the anode effect power and the plasma arc power were both factors causing the increase in the wire melting amount;however,when the main current increased,the plasma arc power was the only factor causing the increase in the wire melting amount.The average wire melting increment caused by the anode effect power was approximately 2.7 times that caused by the plasma arc power.The minimum melting amount was not affected by the wire-torch distance under any current separation ratio tested.When the current separation ratio increased and reached a threshold,the middle melting amount remained constant with increasing wire-torch distance.When the current separation ratio continued to increase and reached the next threshold,the maximum melting amount remained constant with the increasing wire-torch distance.The effect of the wire-torch distance on the wire melting amount reduced with the increase in the current separation ratio.Through this study,the decoupling mechanism and ability of this innovative arc heat source is more clearly.

Optimization of the co-closing law of guide vanes and blades for bulb turbines based on CFD

The load rejection transient process of bulb turbine units is critical to safety of hydropower stations,and determining appropriate closing laws of guide vanes(GVs)and runner blades(RBs)for this process is of significance.In this study,we proposed a procedure to optimize the co-closing law of GVs and RBs by using computational fluid dynamics(CFD),combined with the design of experiment(DOE)method,approximation model,and genetic optimization algorithm.The sensitivity of closing law parameters on the histories of head,speed,and thrust was analyzed,and a two-stage GVs’closing law associating with a linear RBs’closing law was proposed.The results show that GVs dominate the transient characteristics by controlling the change of discharge.Speeding GVs’first-stage closing speed while shortening first-stage closing time can not only significantly reduce the maximum rotational speed but also suppress the water hammer pressure;slowing GVs’second-stage closing speed is conducive to controlling the maximum reverse axial force.RBs directly affect the runner force.Slowing RBs’closing speed can further reduce the rotational speed and the maximum reverse axial force.The safety margin of each control parameter,flow patterns,and pressure pulsations of a practical hydropower station were all improved after optimization,demonstrating the effectiveness of this method.

Testing Linearity in Functional Partially Linear Models

For the functional partially linear models including flexible nonparametric part and functional linear part,the estimators of the nonlinear function and the slope function have been studied in existing literature.How to test the correlation between response and explanatory variables,however,still seems to be missing.Therefore,a test procedure for testing the linearity in the functional partially linear models will be proposed in this paper.A test statistic is constructed based on the existing estimators of the nonlinear and the slope functions.Further,we prove that the approximately asymptotic distribution of the proposed statistic is a chi-squared distribution under some regularity conditions.Finally,some simulation studies and a real data application are presented to demonstrate the performance of the proposed test statistic.

Maintaining thermal comfort within the human body is crucial for optimal health and overall well-being.By merely broadening the setpoint of indoor temperatures,we could significantly slash energy usage in building heating,ventilation,and air-conditioning systems.In recent years,there has been a surge in advancements in personal thermal management(PTM),aiming to regulate heat and moisture transfer within our immediate surroundings,clothing,and skin.The advent of PTM is driven by the rapid development in nano/micro-materials and energy science and engineering.An emerging research area in PTM is personal radiative thermal management(PRTM),which demonstrates immense potential with its high radiative heat transfer efficiency and ease of regulation.However,it is less taken into account in traditional textiles,and there currently lies a gap in our knowledge and understanding of PRTM.In this review,we aim to present a thorough analysis of advanced textile materials and technologies for PRTM.Specifically,we will introduce and discuss the underlying radiation heat transfer mechanisms,fabrication methods of textiles,and various indoor/outdoor applications in light of their different regulation functionalities,including radiative cooling,radiative heating,and dual-mode thermoregulation.Furthermore,we will shine a light on the current hurdles,propose potential strategies,and delve into future technology trends for PRTM with an emphasis on functionalities and applications.

Comparison of the hygrothermal performance of two light-framed timber structure buildings under different operation modes

Light-framed timber structure(LTS)buildings have been highly valued in recent years due to their low-carbon characteristics.However,the applicability of the building envelope is closely related to indoor and outdoor conditions.The hot summer and cold winter(HSCW)climate zone in China has high humidity and great temperature variation throughout the year,resulting in distinct outdoor environments in different seasons.The indoor environment is greatly affected by energy-consumption patterns and window-opening habits,which largely depend upon the regulation operations of occupants.All these interrelated factors lead to extremely complex boundary conditions on each side of the building envelope.Whether the structures of LTS buildings are applicable in this climate zone,therefore,needs to be carefully considered.In this study,two LTS buildings with different envelopes were established in Haining,China,situated in the HSCW climate zone,and selected as the study objects.Different operation modes were adopted to create a variety of indoor environments.Under each condition,the processes of heat and moisture transfer within the building envelopes and the indoor environment were monitored and compared.The comparison indicated that the building envelope with high moisture storage and insulation ability maintained a relatively stable indoor environment,especially when the environment changed abruptly.Conversely,if the outdoor environment was equable(e.g.,relative humidity within the range of 30%–60%)or intermittent energy consumption modes were adopted,the building envelope with a low thermal inertia index and weak moisture-buffering ability performed better because it enabled a faster response of the indoor environment to air conditioning.Moreover,a high risk of moisture accumulation between the thermal insulation layer and other materials with a large water vapour transfer resistance factor was also identified,suggesting a higher requirement for the vapour insulation of the envelopes of LTS buildings.

Significant contribution of carbonyls to atmospheric oxidation capacity(AOC)during the winter haze pollution over North China Plain

Atmospheric carbonyl compounds play significant roles in the cycling of radicals and have exhibited surprisingly high levels in winter that were well correlated to particulate matter,for which the reason have not been clearly elucidated.Here we measured carbonyl compounds and other trace gasses together with PM_(2.5)over urban Jinan in North China Plain during the winter.Markedly higher carbonyl concentrations(average:14.63±4.21 ppbv)were found during wintertime haze pollution,about one to three-times relative to those on nonhaze days,with slight difference in chemical composition except formaldehyde(HCHO).HCHO(3.68 ppbv),acetone(3.17 ppbv),and acetaldehyde(CH_3CHO)(2.83 ppbv)were the three most abundant species,accounting for~75% of the total carbonylson both haze and non-haze days.Results from observational-based model(OBM)with atmospheric oxidation capacity(AOC)indicated that AOC significantly increased with the increasing carbonyls during the winter haze events.Carbonyl photolysis have supplied key oxidants such as RO_(2) and HO_(2),and thereby enhancing the formation of fine particles and secondary organic aerosols,elucidating the observed haze-carbonyls inter-correlation.Diurnal variation with carbonyls exhibiting peak values at early-noon and night highlighted the combined contribution of both secondary formation and primary diesel-fuel sources.1-butene was further confirmed to be the major precursor for HCHO.This study confirms the great contribution of carbonyls to AOC,and also suggests that reducing the emissions of carbonyls would be an effective way to mitigate haze pollution in urban area of the NCP region.

Fast prediction method of full-space bistatic scattering sound field of underwater stiffened double cylindrical shell

A fast prediction method is presented for the full-space bistatic scattering sound field of the underwater stiffened double cylindrical shell. The scattering sound field is represented as the product of the acoustic scattering transfer function and the sound source density function. The target surface grid information and a small amount of simulation or experimentation multistatic scattering sound pressure data are used as known information to forecast other bistatic scattering sound fields with the help of numerical integration, matrix theory, and least squares method. The scattered sound pressure data from simulation of the finite element method(FEM) and experimental tests are used as input to calculate the multistatic scattered sound field of the underwater stiffened double cylindrical shell, respectively. The calculated results are compared with those calculated exclusively by the FEM, and the effects of different input data and calculation frequencies on the calculation results are discussed. The results show that the proposed method can predict the full-space bistatic scattered sound field of the target when the surface structure and part of the scattered sound field data are known. The greater amount of known scattered sound pressure data and lower computational frequency lead to higher predicted accuracy.
