Clinical characteristics of postoperative necrotizing enterocolitis in patients with congenital jejunoileal atresia and its risk factors

Objective To review postoperative necrotizing enterocolitis(NEC)in patients with jejunoileal atresia(JIA)and to explore the potential risk factors related to the concurrence ofNEC.Methods Patients diagnosed with JIA who received surgical treatment from January 2016 to June 2021 were enrolled.Demographics,viral infection of the fetus,transfusion within 48 hours before NEC,sepsis before JIA repair,pathological and anatomical classification of JIA,combined malformation,occurrence time of NEC after the operation,treatment,and prognosis of patients were analyzed.Patients were divided into NEC group and non-NEC group,and all patients were followed up for 3-6 months to observe for complications.Results A total of 180 patients with JIA were included,of whom 12 were diagnosed with NEC after surgery and 1 patient with NEC died during follow-up.The average age,birth weight,gestational age,proportion of premature infants,proportion of preoperative infections,and pathological classification of JIA did not significantly differ between the two groups.The probability of patients with proximal jejunal atresia(PJA)in the NEC group(58.3%)was higher than that in the non-NEC group(22.6%)(p=0.011),and patients with PJA had longer parenteral nutrition time than patients without PJA(26.64±9.21 days vs 15.11±6.58 days,p<0.001).Conclusion PJA was more likely to be associated with concurrent NEC after surgery,which is a highly NEC-related risk factor inherent in JIA.

The existingmultipath routing in Software Defined Network (SDN) is relatively blind and inefficient, and there is alack of cooperation between the terminal and network sides, making it difficult to achieve dynamic adaptationof service requirements and network resources. To address these issues, we propose a multi-constraint pathoptimization scheme based on information fusion in SDN. The proposed scheme collects network topology andnetwork state information on the network side and computes disjoint paths between end hosts. It uses the FuzzyAnalytic Hierarchy Process (FAHP) to calculate the weight coefficients of multiple constrained parameters andconstructs a composite quality evaluation function for the paths to determine the priority of the disjoint paths. TheSDN controller extracts the service attributes by analyzing the packet header and selects the optimal path for flowrule forwarding. Furthermore, the service attributes are fed back to the path composite quality evaluation function,and the path priority is dynamically adjusted to achieve dynamic adaptation between service requirements andnetwork status. By continuously monitoring and analyzing the service attributes, the scheme can ensure optimalrouting decisions in response to varying network conditions and evolving service demands. The experimentalresults demonstrated that the proposed scheme can effectively improve average throughput and link utilizationwhile meeting the Quality of Service (QoS) requirements of various applications.

Transcriptional regulation plays a key role in the control of seed dormancy,and many transcription factors(TFs)have been documented.However,the mechanisms underlying the interactions between different TFs within a transcriptional complex regulating seed dormancy remain largely unknown.Here,we showed that TF PHYTOCHROME-INTERACTING FACTOR4(PIF4)physically interacted with the abscisic acid(ABA)signaling responsive TF ABSCISIC ACID INSENSITIVE4(ABI4)to act as a transcriptional complex to promote ABA biosynthesis and signaling,finally deepening primary seed dormancy.Both pif4 and abi4 single mutants exhibited a decreased primary seed dormancy phenotype,with a synergistic effect in the pif4/abi4 double mutant.PIF4 binds to ABI4 to form a heterodimer,and ABI4 stabilizes PIF4 at the protein level,whereas PIF4 does not affect the protein stabilization of ABI4.Subsequently,both TFs independently and synergistically promoted the expression of ABI4 and NCED6,a key gene for ABA anabolism.The genetic evidence is also consistent with the phenotypic,physiological and biochemical analysis results.Altogether,this study revealed a transcriptional regulatory cascade in which the PIF4–ABI4 transcriptional activator complex synergistically enhanced seed dormancy by facilitating ABA biosynthesis and signaling.

[目的]配方对防火涂料的性能有很大影响。[方法]以苯丙乳液和硅溶胶作为基料,聚磷酸铵、季戊四醇和三聚氰胺作为阻燃体系,二氧化钛作为颜填料,六偏磷酸钠作为分散剂,磷酸三丁酯作为消泡剂,羧甲基纤维素作为增稠剂,研制了一种适用于钢结构的水性饰面型复合膨胀防火涂料。利用模拟大板燃烧试验考察了各组分的质量分数对涂料性能的影响,并采用扫描电镜(SEM)对防火涂层燃烧后的微观结构进行表征。[结果]最佳配方为:苯丙乳液20.0%,硅溶胶4.0%,聚磷酸铵27.0%,季戊四醇22.0%,三聚氰胺21.0%,颜填料4.0%,助剂2.0%。此时涂料的耐燃时间长达57 min,燃烧后形成的炭层致密。[结论]该涂料具有良好的防火阻燃效果。

目的探讨改良牙根屏障技术(MSST)即刻种植即刻修复后1年内种植位点唇侧软组织的动态变化,为评估MSST对种植体周软组织的作用提供依据。方法前瞻性纳入2022年1月至2024年1月于重庆医科大学附属口腔医院种植科完成MSST美学区即刻种植即刻修复术后1年的患者,分别于术前和术后3、6、12个月使用口内扫描仪获取患者口内光学模型,导入Geomagic Studio 2013软件进行多时间点虚拟轮廓重叠配准,于多个层面分析种植位点唇侧软组织轮廓变化,精确计算术后各时间点唇侧龈缘顶点冠根向变化量、龈乳头冠根向变化量及唇侧软组织轮廓唇腭向厚度平均变化量,并通过粉色美学评分进行美学效果评价。结果共纳入患者22例(22枚种植体),患者年龄为(40±13)岁(21~75岁),包括9例男性和13例女性,12个月的随访观察期内,所有种植体均无不良事件发生。术后12个月种植位点唇侧龈缘顶点冠根向变化量为0.08(0.07)mm,近中龈乳头冠根向变化量为0.19(0.25)mm,远中龈乳头冠根向变化量为0.19(0.10)mm,唇侧软组织轮廓唇腭向厚度平均变化量为(0.39±0.09)mm,轮廓塌陷主要发生于龈缘根方2 mm内。种植位点唇侧牙槽骨高度变化量为(0.17±0.08)mm,种植体肩台根方1、3、5 mm处水平向唇侧牙槽骨厚度变化量分别为(0.13±0.08)、(0.12±0.10)、0.04(0.17)mm。粉色美学评分为13.00(2.25)分,所有种植位点均获得理想的美学效果。结论利用MSST于美学区行即刻种植即刻修复,12个月随访观察期内,种植位点唇侧软组织轮廓变化微量。

Mesenchymal stromal cells(MSCs)have been considered a promising alternative for treatment of acute respiratory distress syndrome(ARDS).However,there is significant heterogeneity in their therapeutic efficacy,largely owing to the incomplete understanding of the mechanisms underlying the therapeutic activities of MSCs.Here,we hypothesize that the cholinergic antiinflammatory pathway(CAP),which is recognized as a neuroimmunological pathway,may be involved in the therapeutic mechanisms by which MSCs mitigate ARDS.Using lipopolysaccharide(LPS)and bacterial lung inflammation models,we found that inflammatory cell infiltration and Evans blue leakage were reduced and that the expression levels of choline acetyltransferase(ChAT)and vesicular acetylcholine transporter(VAChT)in lung tissue were significantly increased 6 hours after MSC infusion.When the vagus nerve was blocked orα7 nicotinic acetylcholine(ACh)receptor(α7nAChR)-knockout mice were used,the therapeutic effects of MSCs were significantly reduced,suggesting that the CAP may play an important role in the effects of MSCs in ARDS treatment.Our results further showed that MSC-derived prostaglandin E2(PGE2)likely promoted ACh synthesis and release.Additionally,based on the efficacy of nAChR andα7nAChR agonists,we found that lobeline,the nicotinic cholinergic receptor excitation stimulant,may attenuate pulmonary inflammation and alleviate respiratory symptoms of ARDS patients in a clinical study(ChiCTR2100047403).In summary,we reveal a previously unrecognized MSC-mediated mechanism of CAP activation as the means by which MSCs alleviate ARDS-like syndrome,providing insight into the clinical translation of MSCs or CAP-related strategies for the treatment of patients with ARDS.

[Objectives]To investigate the antipyretic effect and action mechanism of Jinzhen Oral Liquid.[Methods]The yeast-induced fever model was used to determine the rectal temperature of rats at 6,8,and 10 h after yeast injection.The contents of IL-6 and TNF-αin the rat serum and the contents of 5-HT and NE in the hypothalamus were detected by radioimmunoassay;the model of yeast-induced fever in rat was established by intraperitoneal injection of lipopolysaccharide(LPS),rectal temperature of the rats was determined,the contents of TNF-αand IL-1βin serum,cyclic adenosine monophosphate(cAMP)and prostaglandin E2(PGE2)in hypothalamus were detected by radioimmunoassay.[Results]After the intervention of Jinzhen Oral Liquid,the body temperature of rats in the yeast-induced fever model was significantly reduced,the IL-6 and TNF-αcontents in serum were significantly reduced,the 5-HT content in the hypothalamus was significantly reduced,while the NE content was significantly increased;after 2 h of intraperitoneal injection of LPS,the body temperature of the model rats increased significantly;after the intervention of Jinzhen Oral Liquid,the increase in rectal temperature of LPS-induced fever model rats decreased,and the TNF-αand IL-1βcontents in serum and cAMP and PGE2 contents in hypothalamus of model rats were significantly decreased.[Conclusions]Jinzhen Oral Liquid has a significant antipyretic effect,and its action mechanism may be related to reducing the inflammatory factors in the serum,thereby inhibiting the generation of pyrogenic media in the hypothalamus.
