儿童急性淋巴细胞白血病约占整体儿童白血病的75%,其中,急性B淋巴细胞白血病占儿童急性淋巴细胞白血病的80%以上。半个世纪以来,利用新技术不断发现新的分子生物靶标并用于精准的疾病预后分层,儿童急性淋巴细胞白血病的5年总生存率逐年提高。随着对远期生活质量的关注日益增强,儿童急性B淋巴细胞白血病的治疗从诱导治疗到维持治疗强度在不断优化,包括髓外白血病治疗去除放疗也有尝试,并获得成功。优化治疗的实现也得益于免疫学、分子生物学相关新技术的发展、规范化临床队列及与之相应的生物样本库建立。该文对近年来精准分层的实施及急性B淋巴细胞白血病降低强度优化治疗的相关研究进行梳理总结,为临床医生提供参考。

Oxidative stress is involved in the pathogenesis of vascular dementia. Studies have shown that lycopene can significantly inhibit oxidative stress;therefore, we hypothesized that lycopene can reduce the level of oxidative stress in vascular dementia. A vascular dementia model was established by permanent bilateral ligation of common carotid arteries. The dosage groups were treated with lycopene(50, 100 and 200 mg/kg) every other day for 2 months. Rats without bilateral carotid artery ligation were prepared as a sham group. To test the ability of learning and memory, the Morris water maze was used to detect the average escape latency and the change of search strategy. Hematoxylin-eosin staining was used to observe changes of hippocampal neurons. The levels of oxidative stress factors, superoxide dismutase and malondialdehyde, were measured in the hippocampus by biochemical detection. The levels of reactive oxygen species in the hippocampus were observed by dihydroethidium staining. The distribution and expression of oxidative stress related protein, neuron-restrictive silencer factor, in hippocampal neurons were detected by immunofluorescence histochemistry and western blot assays. After 2 months of drug administration,(1) in the model group, the average escape latency was longer than that of the sham group, and the proportion of straight and tend tactics was lower than that of the sham group, and the hippocampal neurons were irregularly arranged and the cytoplasm was hyperchromatic.(2) The levels of reactive oxygen species and malondialdehyde in the hippocampus of the model group rats were increased, and the activity of superoxide dismutase was decreased.(3) Lycopene(50, 100 and 200 mg/kg) intervention improved the above changes, and the lycopene 100 mg/kg group showed the most significant improvement effect.(4) Neuron-restrictive silencer factor expression in the hippocampus was lower in the sham group and the lycopene 100 mg/kg group than in the model group.(5) The above data indicate that lycopene 100 mg/kg co

Background Pediatric liver transplantation is an important modality for treating biliary atresia.The overall survival(OS)rate of pediatric liver transplantation has significantly improved compared with that of 20 years ago,but it is still unsatisfactory.The anesthesia strategy of maintaining low central venous pressure(CVP)has shown a positive effect on prognosis in adult liver transplantation.However,this relationship remains unclear in pediatric liver transplantation.Thus,this study was conducted to review the data of pediatric living-donor liver transplantation to analyze the associations of different CVP levels with the prognosis of recipients.Methods This was a retrospective study and the patients were divided into two groups according to CVP levels after abdominal closure:low CVP(LCVP)(≤10 cmH2O,n=470)and high CVP(HCVP)(>10 cmH2O,n=242).The primary outcome measured in the study was the overall survival rate.The secondary outcomes included the duration of mechanical ventilation in the intensive care unit(ICU),length of stay in the ICU,and postoperative stay in the hospital.Patient demographic and perioperative data were collected and compared between the two groups.Kaplan-Meier curves were constructed to determine the associations of different CVP levels with the survival rate.Results In the study,712 patients,including 470 in the LCVP group and 242 in the HCVP group,were enrolled.After propensity score matching,212 pairs remained in the group.The LCVP group showed a higher overall survival rate than the HCVP group in the Kaplan-Meier curves and multivariate Cox regression analyses(P=0.018),and the HCVP group had a hazard ratio of 2.445(95%confidence interval,1.163–5.140).Conclusion This study confirmed that a low-CVP level at the end of surgery is associated with improved overall survival and a shorter length of hospital stay.

Objective To determine the impact of smoking on disease-specific health care utilization and medical costs in patients with chronic non-communicable diseases(NCDs).Methods Participants were middle-aged and elderly adults with chronic NCDs from a prospective cohort in China.Logistic regressions and linear models were used to assess the relationship between tobacco smoking,health care utilization and medical costs.Results Totally,1020 patients with chronic obstructive pulmonary disease(COPD),3144 patients with coronary heart disease(CHD),and 1405 patients with diabetes were included in the analysis.Among patients with COPD,current smokers(β:0.030,95%CI:−0.032-0.092)and former smokers(β:0.072,95%CI:0.014-0.131)had 3.0%and 7.2%higher total medical costs than never smokers.Medical costs of patients who had smoked for 21-40 years(β:0.028,95%CI:−0.038-0.094)and≥41 years(β:0.053,95%CI:−0.004β0.110)were higher than those of never smokers.Patients who smoked≥21 cigarettes(β:0.145,95%CI:0.051-0.239)per day had more inpatient visits than never smokers.The association between smoking and health care utilization and medical costs in people with CHD group was similar to that in people with COPD;however,there were no significant associations in people with diabetes.Conclusion This study reveals that the impact of smoking on health care utilization and medical costs varies among patients with COPD,CHD,and diabetes.Tobacco control might be more effective at reducing the burden of disease for patients with COPD and CHD than for patients with diabetes.

Air filters with a low resistance,high filtration efficiency,and long lifetime are important to ensure good indoor air quality.In this study,the lattice Boltzmann method is applied on six types of fibrous filter media with lognormal-distribution models,which consider the influence of the solid fraction,number of fibers,and average fiber diameter.The influences of the filtration velocity and fiber layout on the resistance,efficiency,and quality factor are discussed.The resistance is found to be relatively low when the solid fraction inside the filter media is uniformly distributed.The filter media with a random lognormal-distribution model demonstrated the best filtration performance in terms of quality factor.However,when the solid fraction is uniform along the thickness of the filter media,the comprehensive filtration performance is the best when a small fiber is near the inlet and a large one is close to the outlet.This study provides a viable numerical method for performance optimization of air-filtration devices for the next-generation cleanroom industry.

Objective:Cytogenetic abnormalities have been proven to be the most valuable parameter for risk stratification of childhood acute lymphoblastic leukemia(ALL).However,studies on the prevalence of cytogenetic abnormalities and their correlation to clinical features in Chinese pediatric patients are limited,especially large-scale studies.Methods:We collected the cytogenetics and clinical data of 1541 children newly diagnosed with ALL between 2001 and 2014 in four Chinese hospitals,and retrospectively analyzed their clinical features,prognosis and risk factors associated with pediatric ALL.

Silicon carbide fiber reinforced silicon carbide matrix(SiC_(f)/SiC)composite is the key cladding material of nuclear fuel,which determines the safety and reliability of nuclear fuel storage and transportation.The replacement of its storage and transportation scenario needs to be completed by the manipulator,but the application of SiC_(f)/SiC wear,fracture,and nuclear leakage in the snatching process of brittle-flexible-rigid contact in the irradiation environment has been seriously restricted due to unclear understanding of the damage mechanism.Therefore,the effects of irradiation dose and clamping load on the friction characteristics of the contact interface between SiC_(f)/SiC clad tube are studied in this paper,and the effects of irradiation parameters and clamping force on the static friction coefficient of the contact interface between the clad tube and flexible nitrile are obtained.Based on the Greenwood-Williamson tribological model,a numerical model of the shape and structure of the contact micro-convex at the micro-scale of the clamping interface is constructed by introducing the multi-surface integral,and finally verified by experiments.The research results show that there is a unique“Irradiation suppression zone”under the clamping condition of SiC_(f)/SiC cladding tube under the nuclear irradiation environment,and the growth of static friction coefficient slows down until stagnates after irradiation reaches a certain extent(600 kGy),and there will be a decline when the irradiation dose continues to increase,among which the clamping force of 15.2 N within the irradiation interval of 1,000 kGy can meet the safety of nuclear environment operation.The results of this paper can provide an important theoretical basis and application guidance for the safe operation of SiC_(f)/SiC cladding tubes in the storage and transportation clamping process.

With the world talking about climate change, the United States （U.S.）, China and India have announced their carbon emission reduction targets. For these three countries to achieve their targets, significant questions arise, shch as what will be the annual emission reduction efforts to achieve those targets, how much it would cost and what would be the economic effects. This paper puts the carbon intensity reduction targets of China and India together with the absolute emission reduction target of the U.S. into the same non-linear model to quantitatively study the optimal emission control strategies and associated total cost for achieving those targets by the year 2020, and estimate and compare the minimized total costs of the three countries to reach their targets. Our results show that the total cost for the U.S. to achieve its emission reduction target is greater than those of China and India in terms of absolute amount. However, in terms of proportion of total cost to GDP, China and India＇s ratios are significantly greater than that of the U.S., indicating that for the developing countries such as China and India, the achievement of emission reduction targets needs relatively greater effort.

Objective: Rubicon is an inhibitory interacting protein of the autophagy-related protein Uvrag. We previously showed thatRubicon deficiency promotes autophagic fluxin vivo and that autophagy can degrade lipid droplets. This study aimed to investigate the effects of Rubicon deficiency on fasting-induced hepatic steatosis.Methods: Two-month-old wild-type (WT) andRubicon-deficient mice were subjected to feeding or fasting for 24 hours to induce hepatic steatosis. The distribution of liver lipid droplets was revealed by oil red O staining. Hepatic and plasma triglyceride, non-esterified fatty acid (NEFA), and cholesterol levels were detected using commercially available kits. Real-time reverse transcriptasepolymerase chain reaction was performed to analyze the mRNA expression of genes related to lipid metabolism in the liver. Western blot was conducted to assess autophagy-related protein levels in the liver. The animal experiments were approved by the Institutional Animal Care and Use Committee at Shanghai Jiao Tong University, China.Results: We showed that under fasting conditions,Rubicon-deficient mice had more lipid droplets in the liver than WT controls. Consistent with these results, the hepatic triglyceride, NEFA, and cholesterol levels in fastedRubicon-deficient mice were significantly higher than those of fasted WT controls. The levels ofSREBP-1, a key regulator of lipid synthesis, were significantly lower in livers from fasted WT mice than those of fed WT mice. However, the decrease inSREBP-1 in fasted mice was attenuated byRubicon deficiency. Western blot analysis demonstrated that the fasting-induced increase in autophagic flux was amplified byRubicon deficiency. Finally, we showed thatRubicon deficiency in mice led to elevated plasma triglyceride and NEFA acid levels under fasting conditions.Conclusion: Rubicon deficiency exacerbates fasting-induced hepatic steatosis in mice.
