Background:Dysfunction of cerebral autoregulation is one of the pathophysiological mechanisms that causes delayed cerebral ischemia(DCI)after subarachnoid hemorrhage(SAH).Pressure reactivity index(PRx)have been confirmed to reflect the level of cerebral autoregulation and used to derive optimal cerebral perfusion pressure(CPPopt).The goal of this study is to explore the associations between autoregulation,CPPopt,PRx,and DCI.Methods:Continuous intracranial pressure(ICP),arterial blood pressure(ABP),and cerebral perfusion pressure(CPP)signals acquired from 61 aSAH patients were retrospectively analyzed.PRx was calculated and collected by Pneumatic computer system.The CPP at the lowest PRx was determined as the CPPopt.The duration of a hypoperfusion event(dHP)was defined as the cumulative time that the PRx was>0.3 and the CPP was

Tumor recurrence rate after surgery or ablation of hepatocellular carcinoma(HCC)is as high as 70%.However,there are no widely accepted adjuvant therapies;therefore,no treatment has been recommended by guidelines from the American Association for the Study of Liver Disease or the European Association for the Study of the Liver.All the registered trials failed to find any treatment to prolong recurrence-free survival,which is the primary outcome in most studies,including sorafenib.Some investigator-initiated studies revealed that antihepatitis B virus agents,interferon-a,transcatheter chemoembolization,chemokine-induced killer cells,and other treatments prolonged patient recurrence-free survival or overall survival after curative therapies.In this review,we summarize the current status of adjuvant treatments for HCC and explain the challenges associated with designing a clinical trial for adjuvant therapy.Promising new treatments being used as adjuvant therapy,especially anti-PD-1 antibodies,are also discussed.

Liver cancer,mostly hepatocellular carcinoma(HCC),is the second leading cause of cancer mortality globally.Most patients were diagnosed at an advanced stage,and systemic therapy is the standard of care.All the approved systemic therapies for HCC are molecular targeted therapies with anti-angiogenic effects targeting the vascular endothelial growth factor signaling pathway.Sorafenib and lenvatinib are the first-line treatment,and regorafenib,ramucirumab,and cabozantinib are second-line treatment options.Although anti-PD-1 antibodies,including nivolumab and pembrolizumab,demonstrated promising anti-tumor effects as monotherapy for advanced HCC in phase II clinical trials,both failed in phase III studies.Anti-angiogenic treatment remains the backbone of systemic therapy for HCC.In this review,we summarized the approved anti-angiogenic medicines and discussed the potential strategies to improve the efficacy of anti-angiogenic therapy,including combination therapy with other treatments,and discussed the approaches to overcome the drawbacks of anti-angiogenic therapies.

As a highly evolutionary conserved long non-coding RNA,metastasis associated lung adenocarcinoma transcript 1(MALAT1)was first demonstrated to be related to lung tumor metastasis by promoting angiogenesis.To investigate the role of MALAT1 in traumatic brain injury,we established mouse models of controlled cortical impact and cell models of oxygen-glucose deprivation to mimic traumatic brain injury in vitro and in vivo.The results revealed that MALAT1 silencing in vitro inhibited endothelial cell viability and tube formation but increased migration.In MALAT1-deficient mice,endothelial cell proliferation in the injured cortex,functional vessel density and cerebral blood flow were reduced.Bioinformatic analyses and RNA pull-down assays validated enhancer of zeste homolog 2(EZH2)as a downstream factor of MALAT1 in endothelial cells.Jagged-1,the Notch homolog 1(NOTCH1)agonist,reversed the MALAT1 deficiency-mediated impairment of angiogenesis.Taken together,our results suggest that MALAT1 controls the key processes of angiogenesis following traumatic brain injury in an EZH2/NOTCH1-dependent manner.

Traumatic brain injury (TBI) is the lead- ing cause of mortality and disability among young individuals in our society, and globally the incidence of TBI is rising sharply. Mounting evidence has indicated that apolipoprotein E (apoE:protein; APOE:gene) genotype influences the outcome after TBI. The proposed mechanism by which APOE affects the clinicopathological consequences of TBI is multifactorial and includes amyloid deposition, disruption of lipid distribution, dysfunction of mitochondrial energy production, oxidative stress and increases intracellular calcium in response to injury. This paper reviews the current state of knowledge regarding the influence of apoE and its receptors on cerebral amyloid betaprotein precursor metabolism following TBI.

Online hierarchical recognition method for target tactical intention in beyond-visual-range air combat

Online accurate recognition of target tactical intention in beyond-visual-range (BVR) air combat is an important basis for deep situational awareness and autonomous air combat decision-making, which can create pre-emptive tactical opportunities for the fighter to gain air superiority. The existing methods to solve this problem have some defects such as dependence on empirical knowledge, difficulty in interpreting the recognition results, and inability to meet the requirements of actual air combat. So an online hierarchical recognition method for target tactical intention in BVR air combat based on cascaded support vector machine (CSVM) is proposed in this study. Through the mechanism analysis of BVR air combat, the instantaneous and cumulative feature information of target trajectory and relative situation information are introduced successively using online automatic decomposition of target trajectory and hierarchical progression. Then the hierarchical recognition model from target maneuver element, tactical maneuver to tactical intention is constructed. The CSVM algorithm is designed for solving this model, and the computational complexity is decomposed by the cascaded structure to overcome the problems of convergence and timeliness when the dimensions and number of training samples are large. Meanwhile, the recognition result of each layer can be used to support the composition analysis and interpretation of target tactical intention. The simulation results show that the proposed method can effectively realize multi-dimensional online accurate recognition of target tactical intention in BVR air combat.

Background:Studies about treatment of patients with chronic portal vein thrombosis(CPVT)are still limited,especially in different types of CPVT.This study aimed to evaluate the effect of transjugular intrahepatic portosystemic shunt(TIPS)in all types of CPVT with variceal bleeding.Methods:Patients with CPVT who received TIPS treatment between January 2011 and June 2019 were divided into four types according to the extent of thrombosis.All patients had a history of variceal bleeding.The characteristics and clinical parameters were collected and recorded.Data on procedure success rate,variation in portal vein pressure,rebleeding,hepatic encephalopathy(HE),stent stenosis,and overall mortality were analyzed.Results:A total of 189 patients were included in this study(39 in type 1,84 in type 2,48 in type 3,18 in type 4).The TIPS procedure success rate was 86.2%.The success rate was significantly different among the four types(89.7%vs.88.1%vs.83.3%vs.77.8%,P=0.001).In the TIPS success group,portal vein pressure was significantly reduced from 27.15±6.59 to 19.74±6.73 mm Hg after the procedure(P<0.001)and the rebleeding rate was significantly lower than that of the fail group(14.7%vs.30.8%,P=0.017).In addition,there were no significant differences in HE rate(30.7%vs.26.9%,P=0.912)or overall mortality(12.9%vs.19.2%,P=0.403)between the TIPS success group and the fail group.In the TIPS success group,we found that the occurrence of HE was significantly different(P=0.020)among the four types,while there were no significant differences in rebleeding rate(P=0.669),stent stenosis rate(P=0.056),or overall mortality(P=0.690).Conclusions:TIPS was safe and effective in decreasing portal vein pressure and rebleeding rate in patients with CPVT.

Micro RNA-491-5 p(miR-491-5 p) plays an important role in regulating cell proliferation and migration;however,the effect of miR-491-5 p on neovascularization after traumatic brain injury remains poorly understood.In this study,a controlled cortical injury model in C57 BL/6 mice and an oxygen-glucose deprivation model in microvascular endothelial cells derived from mouse brain were established to simulate traumatic brain injury in vivo and in vitro,respectively.In the in vivo model,quantitative real-time-polymerase chain reaction results showed that the expression of miR-491-5 p increased or decreased following the intracerebroventricular injection of an miR-491-5 p agomir or antagomir,respectively,and the expression of miR-491-5 p decreased slightly after traumatic brain injury.To detect the neuroprotective effects of miR-491-p,neurological severity scores,Morris water maze test,laser speckle techniques,and immunofluorescence staining were assessed,and the results revealed that miR-491-5 p downregulation alleviated neurological dysfunction,promoted the recovery of regional cerebral blood flow,increased the number of lectin-stained microvessels,and increased the survival of neurons after traumatic brain injury.During the in vitro experiments,the potential mechanism of miR-491-5 p on neovascularization was explored through quantitative real-time-polymerase chain reaction,which showed that miR-491-5 p expression increased or decreased in brain microvascular endothelial cells after transfection with an miR-491-5 p mimic or inhibitor,respectively.Dual-luciferase reporter and western blot assays verified that metallothionein-2 was a target gene for miR-491-5 p.Cell counting kit 8(CCK-8) assay,flow cytometry,and 2′,7′-dichlorofluorescein diacetate(DCFH-DA) assay results confirmed that the downregulation of miR-491-5 p increased brain microvascular endothelial cell viability,reduced cell apoptosis,and alleviated oxidative stress under oxygen-glucose deprivation conditions.Cell scratch assay,Transwell assay,tube formation

The hot compression behavior and tensile strength after compression of a new Ni-Co-based superalloy produced using electron beam smelting layered(EBSL)solidification technology were investigated.Isothermal compression tests were performed at temperatures of 1120℃and 1150℃,with strain rates of 1 s^(-1) and 0.01 s^(-1),reaching a true strain of 0.51.Tensile strength was evaluated at room temperature.The results revealed that this EBSL technology accelerates dynamic recrystallization(DRX),without compromising the strength of alloy.A significant correlation between the volume fraction of DRX and the strain rate was observed,with higher fractions at lower strain rates,leading to higher tensile strength.Additionally,at the same strain rate,the specimens compressed at 1120℃exhibited higher tensile strength due to undissolvedγ′precipitates.After solution and aging heat treatment,the alloy maintained high tensile strength.The results suggested that the EBSL Ni-Co-based superalloy offers excellent prospects for practical applications.
