Development and validation of a nomogram to predict allograft survival after pediatric liver transplantation

Background Liver transplantation is the main treatment for cholestatic liver disease and some metabolic liver diseases in children.However,no accurate prediction model to determine the survival probability of grafts prior to surgery exists.This study aimed to develop an effective prognostic model for allograft survival after pediatric liver transplantation.Methods This retrospective cohort study included 2032 patients who underwent pediatric liver transplantation between January 1,2006,and January 1,2020.A nomogram was developed using Cox regression and validated based on bootstrap sampling.Predictive and discriminatory accuracies were determined using the concordance index and visualized using calibration curves;net benefits were calculated for model comparison.An online Shiny application was developed for easy access to the model.Results Multivariable analysis demonstrated that preoperative diagnosis,recipient age,body weight,graft type,preoperative total bilirubin,interleukin-1β,portal venous blood flow direction,spleen thickness,and the presence of heart disease and cholangitis were independent factors for survival,all of which were selected in the nomogram.Calibration of the nomogram indicated that the 1-,3-,and 5-year predicted survival rates agreed with the actual survival rate.The concordance indices for graft survival at 1,3,and 5 years were 0.776,0.757,and 0.753,respectively,which were significantly higher than those of the Pediatric End-Stage Liver Disease and Child-Pugh scoring systems.The allograft dysfunction risk of a recipient could be easily predicted using the following URL:https://aspelt.***.io/ASPELT/l Conclusion The allograft survival after pediatric liver transplantation(ASPELT)score model can effectively predict the graft survival rate after liver transplantation in children,providing a simple and convenient evaluation method for clinicians and patients.

To deepen the understanding in the effect of potassium lactate on the sensory quality and safety of Rugao ham,sensory attributes,physicochemical parameters,total volatile basic nitrogen(TVBN),microorganism community and biogenic amines of Rugao ham manufactured with different potassium lactate levels(0%,0.5%,1%,2%)were investigated;the relationship between microbial community and the formation of TVBN and biogenic amines was further evaluated.With the increase of potassium lactate from 0%to 2%,the increased sensory scores and the decreased total aerobic bacterial count and TVBN were observed;the abundance of Staphylococcus increased,while the content of Halomonas decreased.LDA effect size(LEf Se)and correlations analysis showed that Staphylococcus equorum and Lactobacillus fermentum could be the key species to improve sensory scores and decrease biogenic amines and TVBN.Metabolic pathway analysis further showed that amino acids metabolism and nitrogen metabolism were mainly involved in decreasing TVBN and biogenic amines in the treatment of 2%potassium lactate.

Effects of dietary nickel on apoptosis of hemocytes of Spodoptera litura (Fabricius) larvae

Effects of dietary nickel on apoptosis of hemocytes of the 6th instar larvae of Spodoptera litura Fabricius were evaluated using flow cytometry and Laser Scanning Confocal Microscopy (LSCM) by feeding S. litura larvae with artificial diets amended with different concentrations of nickel (1, 5, 10, 20 and 40 mg/kg) for 3 generations. Results showed that the hemocyte apoptosis rate of the 1st generation larvae increased with the increasing nickel concentrations in diet, and the apoptosis rate in the treatment with 40 mg/kg nickel was significantly higher than that in other treatments. In the 2nd generation, the highest hemocytes apoptosis rate was recorded in the treatment of the larvae exposed to 5 mg/kg nickel. The apoptosis rates in the larvae stressed by higher concentrations of nickel were lower than that in control. The apoptosis rates in the larvae exposed to nickel for 3 generations showed similar concentration-responses with that in the 2nd generation. However, the apoptosis rates in the larvae stressed with higher levels of nickel were all significantly inhibited in comparison with that in control. Therefore, effects of nickel stress on apoptosis of S. litura larvae were related with the nickel concentration in diets and the larval generations that were stressed by nickel.

Effects of herbivore-induced rice volatiles on the host selection behavior of brown planthopper,Nilaparvata lugens

It has been suggested that herbivore would react to volatiles produced by herbivore infested plant due to potential change, either positive or negative, in the acceptability of the host plant. This hypothesis was tested for the brown planthopper (BPH) in the laboratory. Sixteen components of the headspace volatiles from rice seedlings with different treatments were collected with SPME and Tenax-TA trap and analyzed with GC and GC-MS. Significant differences in volatile emissions were observed for rice plants with different treatments. Undamaged control plants, mechanically damaged plants and the plants infested by BPH for 1 or 2 d emitted much lower amounts of volatiles compared to the plants infested by BPH for 3 or 5 d. The plants infested by BPH for 3 or 5 d emitted several volatiles that were not detected in undamaged control plants, mechanically damaged plants or the plants infested by BPH for 1 or 2 d. 机器人辅助椎弓根外穿刺经皮后凸成形术

[目的]探讨骨科机器人辅助下经椎弓根外入路穿刺后凸成形术(percutaneous kyphoplasty, PKP)治疗骨质疏松性椎体压缩性骨折(osteoporotic vertebral compression fracture, OVCF)的疗效。[方法]回顾性分析2019年3月—2022年3月单侧椎弓根外入路穿刺PKP治疗OVCF的43例患者的临床资料。根据医患沟通结果,18例采用机器人辅助导航穿刺(机器人组),25例采用传统透视引导穿刺(透视组)。对比分析两组围手术期、随访及影像学资料。[结果]机器人组手术时间显著长于透视组[(43.5±5.2) min vs (26.2±4.9) min, P0.05)。术后平均随访时间(19.8±5.7)个月,两组恢复完全负重活动时间的差异无统计学意义(P>0.05)。术后随时间推移,两组患者VAS、ODI评分均显著减少(P0.05)。影像方面,两组骨水泥分布优秀率比较差异无统计学意义(P>0.05),两组术后局部后凸Cobb角、椎体前缘相对高度均显著改善(P0.05)。[结论]机器人导航可以减少透视次数,但是增加了手术时间,可以作为目前手术方法的一种有益补充。

3D-QSAR Analysis of a Series of 1,2,3-Triazole-chromenone Derivatives as an Acetylcholinesterase Inhibitor against Alzheimer’s Disease

Chromenones have attracted much attention since they are excellent acetylcholinesterase inhibitor(AChEi).The 1,2,3-triazoles are multifunctional anti-acetylcholinesterase(AChE)agents.In this paper,we report the three-dimensional quantitative structure-activity relationship(3D-QSAR)study of 251,2,3-triazolechromenone derivatives based comparative molecular field analysis(CoMFA)and comparative molecular similarity indices analysis(CoMSIA).To construct CoMFA and CoMSIA models,the 25 active molecules were randomly divided into the training and test sets.The obtained cross-validation Q2 of the CoMFA model,the coefficient of non-cross-validation R2,and the test value F are 0.597,0.994,and 396.726,respectively.The cross-validation Q2 of the CoMSIA model,the coefficient of the non-cross-validation R2,and the test value F are 0.721,0.979,and 131.107,respectively.The predictive correlation coefficient(rpred2)is 0.728 for CoMFA and 0.805 for CoMSIA,which verifies that the model is predictable.Based on the potential maps of CoMFA and CoMSIA,a library containing a set of potent AChEi was designed.The inhibitory potential of the compounds in this library was found to be greater than the inhibitory potential of the most active compounds in the data set.The results obtained from this study laid the foundation for the development of effective drugs for AChEi.

Primary Aspergillus spondylodiscitis in a liver transplant recipient

Solid organ transplant recipients are at increased risk for Aspergillus infections. However, the cases of Aspergillus spondylodiscitis are rare and mostly resulted from the hematogenous spread of invasive pulmonary Aspergillosis. Here, we report a case of primary spondylodiscitis in a liver transplant recipient. Six months after transplantation, a chronic and progressive lumbar back pain was presented. The patient had no fever and the white blood cell count was normal. High plasma （l^3）-beta-d-glucan （BDG） level was detected at the time of back pain. The pathogen was Aspergillus flavus. Clinical and radiological healing was achieved through posterior only debridement and voriconazole therapy.
