本文是《PLoS Medicine》关于卫生系统知证政策指导三篇系列文章的第三篇,阐述了应如何将证据转化为指导,以形成卫生系统政策,并促进临床和公共卫生干预的实施。其要点如下:①评估不同类型研究证据是决定解决卫生系统问题的政策方案的关键;②鉴于许多卫生系统干预的复杂性,系统透明的评价方法尤为重要;③当前有不少好的工具可用于评价对支持政策制定各阶段不同类型的研究证据的信心,其中评价关于效果的证据最多;④我们需要制定工具来帮助评价其他关键因素的系统评价证据,如政策方案对于利益相关者的可接受性、实施可行性及公平性;⑤还需要进一步研究全球卫生系统指导政策方案的制定、构建及呈现方法。

目的在16p13-12．1位点20 cm的区域内明确致病基因,了解基因型和临床表型的关系。方法采用双向DNA直接测序法筛选候选基因;运用免疫组化和蛋白免疫印迹法等了解突变基因的表达情况。运用质粒重组及体外表达方法获得野生型和突变型蛋白,使用免疫共沉淀法了解这两者的结合程度。结果在两个家系中发现编码平滑肌肌球蛋白的MYH11基因突变,所有TAAD和PDA患者均为突变基因携带者。在法国家系中发现两个突变。前者为剪切点供体杂合子突变,导致编码71个氨基酸的第32外显子完全缺失:第2个突变为第37外显子上的1个点突变,导致精氨酸变为谷氨酸。在美国家系中发现1个杂合子突变,为第28外显子上编码24个氨基酸的72个碱基缺失。在法国患者的病变动脉标本上运用蛋白免疫印迹法,发现野生型和突变型肌球蛋白蛋白共存,但有异常免疫组化表现。体外实验显示重组野生型和突变型蛋白不能正常结合。结论MYH11基因突变导致家族性TAAD合并PDA,野生型和突变型平滑肌肌球蛋白不能正常结合,推论杂合子突变导致“Dominant-negative”作用。

Adequate antiplatelet regimen in patients on chronic anti-vitamin K treatment undergoing percutaneous coronary intervention

AIM: To investigate the impact of dual antiplatelet therapy (DAT) in patients on antivitamin K (AVK) regimen requiring percutaneous coronary intervention (PCI).METHODS: Between February 2006 and February 2008, 138 consecutive patients under chronic AVK treatment were enrolled in this registry. Of them, 122 received bare metal stent implantation and 16 received drug elutingstent implantation. The duration of DAT, on top of AVK treatment, was decided at the discretion of the clinician. Adequate duration of DAT was def ined according to type of stent implanted and to its clinical indication. RESULTS: The baseline clinical characteristics of patients reflect their high risk, with high incidence of comorbid conditions (Charlson score ≥ 3 in 89% of the patients). At a mean follow-up of 17 ± 11 mo, 22.9% of patients developed a major adverse cardiac event (MACE): 12.6% died from cardiovascular disease and almost 6% had an acute myocardial infarction. Major hemorrhagic events were observed in 7.4%. Adequate DAT was obtained in only 44% of patients. In the multivariate analysis, no adequate DAT and Charlson score were the only independent predictors of MACE (both P = 0.02). CONCLUSION: Patients on chronic AVK therapy represent a high risk population and suffer from a high MACE rate after PCI. An adequate DAT regimen and absence of comorbid conditions are strongly associated with better clinical outcomes.

BACKGROUND We invented Endoscopic Ruler,a new endoscopic device to measure the size of varices in patients with cirrhosis and portal *** To assess the feasibility and safety of Endoscopic Ruler,and evaluate the agreement on identifying large oesophageal varices(OV)between Endoscopic Ruler and the endoscopists,as well as the interobserver agreement on diagnosing large OV using Endoscopic *** We prospectively and consecutively enrolled patients with cirrhosis from 11 hospitals,all of whom got esophagogastroduodenoscopy(EGD)with Endoscopic *** primary study outcome was a successful measurement of the size of varices using Endoscopic *** secondary outcomes included adverse events,operation time,the agreement of identifying large OV between the objective measurement of Endoscopic Ruler and the empirical reading of endoscopists,together with the interobserver agreement on diagnosing large OV by Endoscopic *** From November 2020 to April 2022,a total of 120 eligible patients with cirrhosis were recruited and all of them underwent EGD examinations with Endoscopic Ruler successfully without any adverse *** median operation time of Endoscopic Ruler was 3.00 min[interquartile range(IQR):3.00 min].The kappa value between Endoscopic Ruler and the endoscopists while detecting large OV was 0.52,demonstrating a moderate *** kappa value for diagnosing large OV using Endoscopic Ruler among the six independent observers was 0.77,demonstrating a substantial *** The data demonstrates that Endoscopic Ruler is feasible and safe for measuring the size of varices in patients with cirrhosis and portal *** Ruler is potential to promote the clinical practice of the two-grade classification system of OV.

背景:在巴西,对于潜伏结核感染(LTBI)的治疗只应用于那些接触痰菌涂阳患者、年龄小于15岁、结核菌素皮试(TST)结果≥10mm、没有接种过卡介苗(BCG)的人群以及那些无论是否接种过BCG但TST结果≥15mm的接触者。目的:评估2年发病率和不符合巴西LTBI治疗标准的结核病接触者中的一些预测指标。设计:人群回顾性研究。12～15岁接触者以及那些>15岁但不符合巴西LTBI治疗标准者进入研究范围。结果:结核病发病率为3.2%(22/667),估计结核病发病率为1649/10万;349例TST结果≥5mm接触者(5.4%)发病的危险性高于318例TST结果<5mm接触者(0.9%;RR6.04,95%CI1.7～20.6)结论:不符合巴西LTBI治疗标准的接触者中,如此高的发病率强烈建议这些标准应重新考虑。另外,即使接种过BCG的接触者,TST硬结≥5mm是唯一的可以预测在2年内发病的变量。

[背景]地址地理编码中的错误可能会影响空气污染对健康影响的估计。[目的]研究4种地理编码技术对采用小尺度(10m×10m)分散模型估计的城市空气污染与成人肺功能之间关联的影响。[方法]测量法国格勒诺布尔市354名成年居民的1秒用力呼气容积(FEV1)与用力肺活量(FVC)。这些居民参加了两个特征明确的研究:遗传和环境对哮喘影响的流行病学研究(EGEA)和欧洲共同体呼吸系统健康调查(ECRHS)。使用单个建筑物匹配作为参考方法以及三种空间插值方法编码家庭地址。使用扩散模型估计在进行肺功能测量前的12个月期间,每个参加者住址的PM10和二氧化氮的平均浓度。以个体混杂因素及同日空气污染物暴露校正暴露与肺功能参数之间的关联。对地理编码技术在地理坐标之间距离、暴露估值以及暴露估值与健康影响之间关联等方面进行比较。[结果]利用建筑物匹配和三种插值技术估计坐标之间的中位数距离分别为26.4、27.9、35.6m。相对于基于建筑匹配的暴露估值,基于三种插值技术的PM10浓度往往被高估。当使用建筑物匹配估计暴露估值时,PM10每增加一个四分位数间距(3.0μg/m3),FVC%预测值下降3.72点(95%CI:-0.56～-6.88)、FEV1%预测值下降3.86点(95%CI:-0.14～-3.24)。当使用其他地理编码方法时,关联的幅度下降[例如,使用NavTEQ预测FVC%-2.81(95%CI:-0.26,-5.35),或者使用谷歌地图预测FVC%-2.08(95%CI:-4.63～0.47,P=0.11)。[结论]研究结果表明,当使用小尺度暴露模型估算空气污染暴露时,地理编码技术的选择可能会对健康影响的评估产生影响。
