Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department *** syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in china, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, beijing Pediatric Society, beijing Medical Association(BMA). The present guideline includes the underlying diseases of syncope in children and adolescents, the diagnostic procedures, methodology and clinical significance of standing test and headup tilt test, the clinical diagnosis vasovagal syncope, postural orthostatic tachycardia syndrome, orthostatic hypotension and orthostatic hypertension, and the treatment of syncope as well as follow-up.

Colorectal cancer(CRC)is a common malignant tumor that affects people *** analyses have shown an enrichment of Fusobacterium nucleatum(***)in colorectal carcinoma tissue;many studies have indicated that *** is closely related to the colorectal *** this review,we provide the latest information to reveal the related molecular *** known virulence factors of *** promote adhesion to intestinal epithelial cells via FadA and ***,Fap2 also binds to immune cells causing ***,*** recruits tumor-infiltrating immune cells,thus yielding a pro-inflammatory microenvironment,which promotes colorectal neoplasia *** was also found to potentiate CRC development through toll-like receptor 2(TLR2)/toll-like receptor 4(TLR4)signaling and microRNA(miRNA)-21 *** addition,*** increases CRC recurrence along with chemoresistance by mediating a molecular network of miRNA-18a*,miRNA-4802,and autophagy ***,viable *** was detected in mouse xenografts of human primary colorectal adenocarcinomas through successive *** findings indicated that an increased number o f *** in the tissues is a biomarker for the diagnosis and prognosis of CRC,and the underlying molecular mechanism can probably provide a potential intervention treatment strategy for patients with ***-associaied CRC.

As china’s first X-ray astronomical satellite, the Hard X-ray Modulation Telescope (HXMT), which was dubbed as Insight-HXMT after the launch on June 15, 2017, is a wide-band(1-250 ke V) slat-collimator-based X-ray astronomy satellite with the capability of all-sky monitoring in 0.2-3 Me V. Azvudine(FNC)is a nucleoside analog that inhibits HIV-1 RNA-dependent RNA polymerase(RdRp).Recently,we discovered FNC an agent against SARS-CoV-2,and have taken it into Phase III trial for COVID-19 *** monophosphate analog inhibited SARS-CoV-2 and HCoV-OC43 coronavirus with an EC_(50) between 1.2 and 4.3 μM,depending on viruses or cells,and selective index(SI)in 15-83 *** administration of FNC in rats revealed a substantial thymus-homing feature,with FNC triphosphate(the active form)concentrated in the thymus and peripheral blood mononuclear cells(PBMC).Treating SARS-CoV-2 infected rhesus macaques with FNC(0.07 mg/kg, qd,orally)reduced viral load,recuperated the thymus,improved lymphocyte profiles,alleviated in flammation and orga n damage,and lessened grou nd・glass opacities in chest *** gle-cell seque ncing suggested the promotion of thymus function by FNC.A randomized,single-arm clinical trial of FNC on compassionate use(n=31)showed that oral FNC(5 mg,qd)cured all COVID-19 patients,with 100%viral ribonucleic acid negative conversion in 3.29±2.22 days(range:1-9 days)and 100%hospital discharge rate in 9.00±4.93 days(range:2-25 days).The side-effect of FNC is minor and transient dizziness and nausea in 16.12%(5/31)***,FNC might cure COVID-19 through its anti-SARS-CoV-2 activity concentrated in the thymus,followed by promoted immunity.

There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1(PD-[L]1)inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1 inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1 blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2018 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-11.0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,19.2[16.1-27.3]vs.15.7 months[13.0-20.2];adjusted HR,0.63,P=0.001;ORR,60.1%vs.32.0%;P1).Grade 3/4 adverse events were observed at a rate of 15.8%and 7.5%in combination and monotherapy groups,*** results suggest that TACE plus PD-(L)1 blockades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.

Finding the electromagnetic （EM） counterpart of binary compact star merger, especially the binary neutron star （BNS） merger, is critically important for gravitational wave （GW） astronomy, cosmology and fundamental physics. On Aug. 17, 2017, Advanced LIGO and Fermi/GBM independently triggered the first BNS merger, GW170817, and its high energy EM counterpart, GRB 170817A, respectively, resulting in a global observation campaign covering gamma-ray, X-ray, UV, optical, IR, radio as well as neutrinos. The High Energy X-ray telescope （HE） onboard Insight-HXMT （Hard X-ray Modulation Telescope） is the unique high-energy gamma-ray telescope that monitored the entire GW localization area and especially the optical counterpart （SSS17a/AT2017gfo） with very large collection area （M000 cm2） and microsecond time resolution in 0.2-5 MeV. In addition, Insight-HXMT quickly implemented a Target of Opportunity （TOO） observation to scan the GW localization area for potential X-ray emission from the GW source. Although Insight-HXMT did not detect any significant high energy （0.2-5 MeV） radiation from GW170817, its observation helped to confirm the unexpected weak and soft nature of GRB 170817A. Meanwhile, Insight-HXMT/HE provides one of the most stringent constraints （-10-7 to 104 erg/cm2/s） for both GRB170817A and any other possible precursor or extended emissions in 0.2-5 MeV, which help us to better understand the properties of EM radiation from this BNS merger. Therefore the observation of Insight-HXMT constitutes an important chapter in the full context of multi-wavelength and multi-messenger observation of this historical GW event.

目的探讨造成胃浅表病变内镜黏膜下剥离术(ESD)手术时间较长(大于120 min)的独立危险因素。方法从北京友谊医院内镜中心病历库中,收集2015年1月至2017年12月因术前诊断早期胃癌而行ESD治疗的病例资料,包括患者基本信息(年龄、性别、体重指数、合并症等),病灶特征(大小、部位、形态等),操作者经验,手术时间及术后病理等信息,最终纳入193例(195处病灶)。对收集到的资料先行单因素分析以寻找ESD手术时间的影响因素,对于其中有统计学差异的因素再纳入logistic回归分析以寻找ESD手术时间大于120 min的独立危险因素。结果193例患者年龄为(63.34±9.11)岁,手术时间为120.00(95.00,165.00)min,病灶长轴为1.50(1.00,2.38)cm,术后病理诊断早期胃癌164例(84.10%),其中162例(98.78%)达到整块切除,148例(90.24%)达到治愈性切除。单因素分析中,患者性别(P=0.018)、病灶部位(P1)及病灶长轴大小(r=0.209,P=0.007)与手术时间相关,而年龄、体重指数、美国麻醉医师协会分级(ASA分级)、病灶表面是否粗糙、有无白苔、有无溃疡、病变浸润深度、手术时期、病灶大体形状、分化程度及操作者年资均不是手术时间的影响因素(P均>0.05)。患者性别、病灶部位、病灶长轴纳入多因素logistic回归分析发现,病灶位于贲门/胃底(OR=5.656,95%CI:2.291~13.964,P1)、胃体(OR=2.667,95%CI:1.048~6.785,P=0.040)及病灶长轴>2 cm(OR=2.761,95%CI:1.229~6.205,P=0.014)是手术时间超过120 min的独立危险因素。结论胃浅表病变行ESD治疗时,病灶位于贲门/胃底、胃体及病灶长轴>2 cm者易发生手术时间较长(大于120 min)的情况。

The LIGO detection of gravitational waves(GW) from merging black holes in 2015 marked the beginning of a new era in observational astronomy. The detection of an electromagnetic signal from a GW source is the critical next step to explore in detail the physics involved. The Antarctic Survey Telescopes(AST3),located at Dome A, Antarctica, is uniquely situated for rapid response time-domain astronomy with its continuous night-time coverage during the austral winter. We report optical observations of the GW source(GW 170817) in the nearby galaxy NGC 4993 using AST3. The data show a rapidly fading transient at around 1 day after the GW trigger, with the i-band magnitude declining from 17:23 ± 0:13 magnitude to 17:72 ± 0:09 magnitude in ~1:8 h. The brightness and time evolution of the optical transient associated with GW 170817 are broadly consistent with the predictions of models involving merging binary neutron stars. We infer from our data that the merging process ejected about ~10^(-2) solar mass of radioactive material at a speed of up to 30% the speed of light.
