Abnormal intrinsic brain activity in amnestic mild cognitive impairment revealed by amplitude of low-frequency fluctuation： a resting-state functional magnetic resonance imaging study

Background Previous studies have shown that brain functional activity in the resting state is impaired in Alzheimer＇s disease （AD） patients. However, alterations in intrinsic brain activity patterns in mild cognitive impairment （MCI） patients are poorly understood. This study aimed to explore the differences in regional intrinsic activities throughout the whole brain between aMCI patients and controls. Methods In the present study, resting-state functional magnetic resonance imaging （fMRI） was performed on 18 amnestic MCI （aMCI） patients, 18 mild AD patients and 20 healthy elderly subjects. And amplitude of low-frequency fluctuation （ALFF） method was used. Results Compared with healthy elderly subjects, aMCI patients showed decreased ALFF in the right hippocampus and parahippocampal cortex, left lateral temporal cortex, and right ventral medial prefrontal cortex （vMPFC） and increased ALFF in the left temporal-parietal junction （TPJ） and inferior parietal Iobule （IPL）. Mild AD patients showed decreased ALFF in the left TPJ, posterior IPL （plPL）, and dorsolateral prefrontal cortex compared with aMCI patients. Mild AD patients also had decreased ALFF in the right posterior cingulate cortex, right vMPFC and bilateral dorsal MPFC （dMPFC） compared with healthy elderly subjects. Conclusions Decreased intrinsic activities in brain regions closely related to episodic memory were found in aMCI and AD patients. Increased TPJ and IPL activity may indicate compensatory mechanisms for loss of memory function in aMCI patients. These findings suggest that the fMRI based on ALFF analysis may provide a useful tool in the study of aMCI patients.

However, most studies focused on the relationship between different brain areas, rather than the amplitude or strength of the regional brain activity. The purpose of this study was to explore the functional brain changes in AD patients by measuring the amplitude of the blood oxygenation level dependent （BOLD） functional MRI （fMRI） signals.

目的应用基于低频振幅(ALFF)的静息态功能磁共振成像(fMRI)技术,研究遗忘型轻度认知损害(aMCI)患者的静态脑活动特征及其可能的神经机制。方法选取22例aMCI患者(aMCI组)作为研究对象,以22例轻度阿尔茨海默病患者(AD组)和25名健康老年人(正常组)作为对照。所有受试者接受fMRI检查,利用ALFF算法,对三组人群全脑静息态数据进行对比分析。结果与正常组比较,aMCI组海马、海马旁回和侧颞叶皮质等脑区ALFF减弱,颞顶交界和顶下小叶ALFF增强;与AD组比较,aMCI组颞顶交界和后顶下小叶、前额叶背外侧等多个脑区ALFF增强;与正常组比较,AD组后扣带回、前额叶中内侧腹侧、前额叶中内侧背侧等脑区ALFF减弱。结论 aMCI患者与情景记忆密切相关的海马、海马旁回等脑区活动强度减低,颞顶交界和顶下小叶活动增强,提示aMCI患者脑内可能存在代偿机制。基于ALFF的静息态fMRI技术可为MCI的研究提供重要手段。

Interleukin-37b(hereafter called IL-37)was identified as fundamental inhibitor of natural and acquired *** molecular mechanism and function of IL-37 in colorectal cancer(CRC)has been ***,we found that IL-37 transgenic(IL-37tg)mice were highly susceptible to colitis-associated colorectal cancer(CAC)and suffered from dramatically increased tumor burdens in ***,IL-37 is dispensable for intestinal mutagenesis,and CRC cell proliferation,apoptosis,and ***,IL-37 dampened protective cytotoxic T cell-mediated immunity in CAC and B16-OVA ***8^(+)T cell dysfunction is defined by reduced retention and activation as well as failure to proliferate and produce cytotoxic cytokines in IL-37tg mice,enabling tumor evasion of immune *** dysfunction led by IL-37 antagonizes IL-18–induced proliferation and effector function of CD8+T cells,which was dependent on SIGIRR(single immunoglobulin interleukin-1 receptor-related protein).Finally,we observed that IL-37 levels were significantly increased in CRC patients,and positively correlated with serum CRC biomarker CEA levels,but negatively correlated with the CD8+T cell infiltration in CRC *** findings highlight the role of IL-37 in harnessing antitumor immunity by inactivation of cytotoxic T cells and establish a new defined inhibitory factor IL-37/SIGIRR in cancerimmunity cycle as therapeutic targets in CRC.

Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

目的利用fMRI技术,通过听觉呈现语言任务,探讨刺激呈现通道对负激活脑区的影响,进一步检验静息状态人脑默认活动假说。方法13名健康成年志愿者参加实验。进行2次fMRI实验。实验1(简单任务)任务期要求受试者听无意义假词;实验2(复杂任务)要求受试者听真词并作词语属性判断(具体或抽象)。静息期要求受试者闭眼、静卧,不要做任何主动思维活动。利用SPM2软件进行数据处理。先分析单个被试,然后行组间比较。采用反减法获得负激活图。并把本次实验结果与以往视觉呈现任务结果进行比较。结果初级视觉皮层与初级听觉皮层的负激活存在明显的通道依赖性,听觉呈现刺激引起视觉皮层负激活,视觉呈现刺激时听觉皮层表现为负激活。非任务依赖性负激活脑区包括扣带回后部/楔前叶(BA31/30)、扣带回前部(BA24/32)、两侧颞上回(BA8)、两颞下回前部(BA20)、两侧顶下小叶(BA39/40)。这些区域的负激活与刺激呈现通道方式及特定刺激任务无关。该负激活脑区模式与人类默认脑活动网络基本一致。结论刺激呈现通道是影响任务依赖性负激活的因素之一,探讨负激活问题时应该考虑到这一因素。同时,本研究进一步验证了静息状态时人脑默认活动假说。

Benign prostatic hyperplasia (BPH) is highly prevalent among older men, impacting on their quality of life, sexual function, and genitourinary health, and has become an important global burden of disease. Transurethral plasmakinetic resection of prostate (TUPKP) is one of the foremost surgical procedures for the treatment of BPH. It has become well established in clinical practice with good efficacy and safety. In 2018, we issued the guideline “2018 Standard Edition”. However much new direct evidence has now emerged and this may change some of previous recommendations. The time is ripe to develop new evidence-based guidelines, so we formed a working group of clinical experts and methodologists. The steering group members posed 31 questions relevant to the management of TUPKP for BPH covering the following areas: questions relevant to the perioperative period (preoperative, intraoperative, and postoperative) of TUPKP in the treatment of BPH, postoperative complications and the level of surgeons’ surgical skill. We searched the literature for direct evidence on the management of TUPKP for BPH, and assessed its certainty generated recommendations using the grade criteria by the European Association of Urology. Recommendations were either strong or weak, or in the form of an ungraded consensus-based statement. Finally, we issued 36 statements. Among them, 23 carried strong recommendations, and 13 carried weak recommendations for the stated procedure. They covered questions relevant to the aforementioned three areas. The preoperative period for TUPKP in the treatment of BPH included indications and contraindications for TUPKP, precautions for preoperative preparation in patients with renal impairment and urinary tract infection due to urinary retention, and preoperative prophylactic use of antibiotics. Background:Clinical outcomes of undifferentiated arthritis(UA)are diverse,and only 40%of patients with UA develop rheumatoid arthritis(RA)after 3 *** predictive markers at disease onset for further intervention is ***,our objective was to analyze the clinical outcomes of UA and ascertain the predictors for RA ***:We performed a prospective,multi-center study from January 2013 to October 2016 among Chinese patients diagnosed with UA in 22 tertiary-care *** and serological parameters were obtained at ***-up was undertaken in all patients every 12 weeks for 2 *** factors of disease progression were identified using multivariate Cox proportional hazards ***:A total of 234 patients were recruited in this study,and 17(7.3%)patients failed to follow up during the *** the 217 patients who completed the study,83(38.2%)patients went into *** patients who developed RA had a higher rheumatoid factor(RF)-positivity(42.9%vs.16.8%,χ^2=8.228,P=0.008),anti-cyclic citrullinated peptide(CCP)antibodypositivity(66.7%vs.10.7%,χ^2=43.897,P<0.001),and double-positivity rate of RF and anti-CCP antibody(38.1%vs.4.1%,χ^2=32.131,P<0.001)than those who did ***-CCP antibody but not RF was an independent predictor for RA development(hazard ratio 18.017,95%confidence interval:5.803–55.938;P<0.001).Conclusion:As an independent predictor of RA,anti-CCP antibody should be tested at disease onset in all patients with UA.

目的评价虎地肠溶胶囊联合美沙拉秦肠溶片对轻、中度活动期溃疡性结肠炎的临床疗效。方法按照区组随机、双盲双模拟对照临床试验研究设计,将18个临床中心的355例溃疡性结肠炎患者随机分为试验组(118例)、阳性对照组(118例)、联合用药组(119例),试验组予以虎地肠溶胶囊口服,阳性对照组口服美沙拉嗪肠溶片口服,联合用药组予以虎地肠溶胶囊+美沙拉秦肠溶片口服,疗程6周。记录治疗前及治疗后2、4和6周的症状积分,比较两组患者症状积分变化及临床疗效。结果355例患者中,符合方案有效病例318例,试验组101例,阳性对照组104例,联合用药组113例。治疗结束时,试验组中医证候疗效为91.09%(92/101),阳性对照组为84.62%(88/104),联合用药组为95.58%(108/113),联合用药组优于阳性药对照组(P<0.05);用药6周后,试验组和联合用药组在改善脓血便方面优于阳性药对照组(P<0.05);联合用药组对于里急后重、肛门灼热、小便短赤的改善优于阳性药对照组(P<0.05)。结论虎地肠溶胶囊能够对轻、中度活动期溃疡性结肠炎患者的症状有改善作用,和美沙拉嗪联用效果更佳。

The Circular Electron Positron Collider(CEPC)is a large scientific project initiated and hosted by China,fostered through extensive collaboration with international *** complex comprises four accelerators:a 30 GeV Linac,a 1.1 GeV Damping Ring,a Booster capable of achieving energies up to 180 GeV,and a Collider operating at varying energy modes(Z,W,H,and tt).The Linac and Damping Ring are situated on the surface,while the subterranean Booster and Collider are housed in a 100 km circumference underground tunnel,strategically accommodating future expansion with provisions for a potential Super Proton Proton Collider(SPPC).The CEPC primarily serves as a Higgs *** its baseline design with synchrotron radiation(SR)power of 30 MW per beam,it can achieve a luminosity of 5×10^(34)cm^(-2)s^(-1)per interaction point(IP),resulting in an integrated luminosity of 13 ab^(-1)for two IPs over a decade,producing 2.6 million Higgs *** the SR power to 50 MW per beam expands the CEPC's capability to generate 4.3 million Higgs bosons,facilitating precise measurements of Higgs coupling at sub-percent levels,exceeding the precision expected from the HL-LHC by an order of *** Technical Design Report(TDR)follows the Preliminary Conceptual Design Report(Pre-CDR,2015)and the Conceptual Design Report(CDR,2018),comprehensively detailing the machine's layout,performance metrics,physical design and analysis,technical systems design,R&D and prototyping efforts,and associated civil engineering ***,it includes a cost estimate and a preliminary construction timeline,establishing a framework for forthcoming engineering design phase and site selection *** is anticipated to begin around 2027-2028,pending government approval,with an estimated duration of 8 *** commencement of experiments and data collection could potentially be initiated in the mid-2030s.
