Background The optimal time to commence anticoagulation in patients with atrial fibrillation(AF)after ischaemic stroke or transient ischaemic attack(TIA)is unclear,with guidelines differing in recommendations.A limitation of previous studies is the focus on clinically overt stroke,rather than radiologically obvious diffusion-weighted imaging ischaemic *** aimed to quantify silent ischaemic lesions and haemorrhages on MRI at 1month in patients commenced on early(x.event(17%vs 8%,p=0.04),but no difference in haemorrhage rates(22%vs 32%,p=0.10).Baseline ischaemic lesion volume of≤5mL was less likely to have a new haemorrhage at 1month(p=0.02).There was no difference in haemorrhage rates in patients with an initial ischaemic lesion volume of>5mL,regardless of anticoagulation *** Commencing anticoagulation<4days after stroke or TIA is associated with fewer ischaemic lesions at 1month in AF *** is no increased rate of haemorrhage with early *** results suggest that early anticoagulation after mild-to-moderate acute ischaemic stroke associated w

AIM: To investigate rates of re-establishing gastroenterology care, colonoscopy, and/or initiating me-dical therapy after Crohn's disease(CD) surgery at a tertiary care referral ***: CD patients having small bowel or ileocolonic resections with a primary anastomosis between 2009-2012 were identified from a tertiary academic referral center. CD-specific features, medications, and surgical outcomes were abstracted from the medical record. The primary outcome measure was compliance rates with medical follow-up within 4 wk of hospital discharge and surveillance colonoscopy within 12 mo of surgery. RESULTS: Eighty-eight patients met study inclusion criteria with 92%(n = 81) of patients returning for surgical follow-up compared to only 41%(n = 36) of patients with documented gastroenterology follow-up within four-weeks of hospital discharge, P < 0.05. Factors associated with more timely postoperative medical follow-up included younger age, longer length of hospitalization, postoperative biologic use and academic center patients. In the study cohort, 75.0% of patients resumed medical therapy within 12 mo, whereas only 53.4% of patients underwent a colonoscopy within 12 mo of ***: Our study highlights the need for coordinated CD multidisciplinary clinics and structured handoffs among providers to improve of quality of care in the postoperative setting.

Emerging infectious diseases(EIDs)with pandemic potential are a major threat to global health security and the global ***,preparedness,and response to EIDs within hotspots for EIDs is considered a global public *** December 2019,coronavirus disease 2019 came under the spotlight;caused by a novel single-stranded RNA coronavirus,severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).The rapidity at which the virus spread worldwide has been remarkable and the global response to the pandemic *** rate of emergence of infectious diseases of zoonotic origin is *** new pathogens have emerged or reemerged over the past few decades,many of viral origin,of which 75%are considered to have originated in animals.1 Before SARSCoV-2,globally,around 1 billion zoonotic infections occurred in humans each year of which around 0.1%resulted in death.1 Prominent ex.mples of EIDs include acquired immunodeficiency syndrome,Ebola and Marburg hemorrhagic fevers,Escherichia coli O157 infection,Middle East respiratory syndrome(MERS),and severe acute respiratory syndrome(SARS).

Hyaluronan(HA)production by dendritic cells(DCs)is known to promote antigen presentation and to augment T-cell activation and *** hypothesized that pericellular HA can function as intercellular‘glue’directly mediating T cell–DC *** primary human cells,we observed HA-dependent binding between T cells and DCs,which was abrogated upon pre-treatment of the DCs with 4-methylumbelliferone(4-MU),an agent which blocks HA ***,T cells regulate HA production by DCs via T cell-derived cytokines in a T helper(Th)subset-specific manner,as demonstrated by the observation that cell-culture supernatants from Th1 but not Th2 clones promote HA *** effects were seen upon the addition of ex.genous Th1 cytokines,IL-2,interferon c(IFN-c)and tumor necrosis factor a(TNF-a).The critical factors which determined the ex.ent of DC–T cell binding in this system were the nature of the pre-treatment the DCs received and their capacity to synthesize HA,as T-cell clones which were pre-treated with monensin,added to block cytokine secretion,bound equivalently irrespective of their Th *** data support the ex.stence of a feedforward loop wherein T-cell cytokines influence DC production of HA,which in turn affects the ex.ent of DC–T cell *** also document the presence of focal deposits of HA at the immune synapse between T-cells and APC and on dendritic processes thought to be important in antigen *** data point to a pivotal role for HA in DC–T cell interactions at the IS.

The epididymal β-defensins have evolved by repeated gene duplication and divergence to encode a family of proteins that provide direct protection against pathogens and also support the male reproductive tract in its primary function. Male tract defensins also facilitate recovery from pathogen attack. The β-defensins possess ancient conserved sequence and structural features widespread in multi-cellular organisms, suggesting fundamental roles in species survival. Primate SPAG11, the functional fusion of two ancestrally independent β-defensin genes, produces a large family of alternatively spliced transcripts that are ex.ressed according to tissue-specific and species-specific constraints. The complex.ty of SPAG11 varies in different branches of mammalian evolution. Interactions of human SPAG11D with host proteins indicate involvement in multiple signaling pathways. （Asian J Andro12007 July; 9： 453- 462）

Adolescents and young adults(AYA)with a cancer diagnosis or those at risk for cancer due to hereditary cancer syndromes may benefit from genetic counseling and testing not only to manage personal risk but also to address reproductive concerns,especially *** opportunity for genetic counselors to provide important risk information is relevant to both the newly diagnosed as well as to unaffected carriers and ***,genetic counselors may need additional training in reproductive options related to AYA cancer to provide this valuable *** commentary uses hereditary breast and ovarian cancer syndrome as a model to highlight important considerations when discussing preimplanatation genetic diagnosis and prenatal diagnosis,particularly in the contex. of ex.anded testing for hereditary cancer risk including multigene panels or whole ex.me or whole genome *** hereditary cancers are also addressed;however,less is known about the psychosocial and fertility concerns in these AYA ***,we provide an overview of the concept of"oncofertility"-the linkage between cancer care and reproductive medicine that aims to ex.and the reproductive opportunities of cancer patients-and offer support for the ex.ansion of guidelines to include genetic counselors in AYA cancer patients’treatment planning related to reproductive health and fertility.
