We conducted a randomized,open-label,parallel-controlled,multicenter trial on the use of Shuanghuanglian(SHL),a traditional Chinese patent medicine,in treating cases of COVID-19.A total of 176 patients received SHL by three doses(56 in low dose,61 in middle dose,and 59 in high dose)in addition to standard *** control group was composed of 59 patients who received standard therapy *** with SHL was not associated with a difference from standard care in the time to disease *** with 14-day SHL treatment had significantly higher rate in negative conversion of SARS-CoV-2 in nucleic acid swab tests than the patients from the control group(93.4%vs.73.9%,P=0.006).Analysis of chest computed tomography images showed that treatment with high-dose SHL significantly promoted absorption of inflammatory focus of pneumonia,which was evaluated by density reduction of inflammatory focus from baseline,at day 7(mean difference(95%CI),−46.39(−86.83 to−5.94)HU;P=0.025)and day 14(mean difference(95%CI),−74.21(−133.35 to−15.08)HU;P=0.014).No serious adverse events occurred in the SHL *** study illustrated that SHL in combination with standard care was safe and partially effective for the treatment of COVID-19.

Background:Until January 18,2021,coronavirus disease-2019(COVID-19)has infected more than 93 million individuals and has caused a certain degree of *** pneumonia caused by common viruses such as respiratory syncytial virus,rhinovirus,human metapneumovirus,human bocavirus,and parainfluenza viruses have been more common in ***,the incidence of COVID-19 in children was significantly lower than that in *** purpose of this study was to describe the clinical manifestations,treatment and outcomes of COVID-19 in children compared with those of other sources of viral pneumonia diagnosed during the COVID-19 ***:Children with COVID-19 and viral pneumonia admitted to 20 hospitals were enrolled in this retrospective multi-center cohort study.A total of 64 children with COVID-19 were defined as the COVID-19 cohort,of which 40 children who developed pneumonia were defined as the COVID-19 pneumonia *** 284 children with pneumonia caused by other viruses were defined as the viral pneumonia *** epidemiologic,clinical,and laboratory findings were compared by Kolmogorov-Smirnov test,t-test,Mann-Whitney U test and Contingency table *** usage,immunotherapy,blood transfusion,and need for oxygen support were collected as the treatment ***,intensive care needs and symptomatic duration were collected as the outcome ***:Compared with the viral pneumonia cohort,children in the COVID-19 cohort were mostly exposed to family members confirmed to have COVID-19(53/64 vs.23/284),were of older median age(6.3 years vs.3.2 years),and had a higher proportion of ground-glass opacity(GGO)on computed tomography(18/40 vs.0/38,P<0.001).Children in the COVID-19 pneumonia cohort had a lower proportion of severe cases(1/40 vs.38/284,P=0.048),and lower cases with high fever(3/40 vs.167/284,P<0.001),requiring intensive care(1/40 vs.32/284,P<0.047)and with shorter symptomatic duration(median 5d vs.8d,P<0.001).The propor

肺炎克雷伯菌是一种典型的肠杆菌科条件致病菌,也是六大超级细菌“ESKAPE”中的一员,通常定植在人类呼吸道和肠道,可引起肺炎、脑膜炎和血流感染等多种感染。外膜囊泡(OMV)是由革兰阴性菌自发分泌的具有生物学活性的囊泡状结构,含有多糖、外膜蛋白和核酸等成分。现有研究证实肺炎克雷伯菌OMV在病原菌致病机制、传递效应因子以及与宿主相互作用等方面发挥重要作用。文章主要围绕肺炎克雷伯菌OMV的基本信息、生物学功能以及生物医学应用等方面进行讨论,对肺炎克雷伯菌OMV的最新研究进展进行综述。

Objective The relationship between serum uric acid(SUA)levels and glycemic indices,including plasma glucose(FPG),2-hour postload glucose(2 h-PG),and glycated hemoglobin(HbA1 c),remains *** aimed to explore the associations between glycemic indices and SUA levels in the general Chinese *** The current study was a cross-sectional analysis using the first follow-up survey data from The china Cardiometabolic Disease and Cancer Cohort Study.A total of 105,922 community-dwelling adults aged≥40 years underwent the oral glucose tolerance test and uric acid *** nonlinear relationships between glycemic indices and SUA levels were explored using generalized additive *** A total of 30,941 men and 62,361 women were eligible for the current *** additive models verified the inverted U-shaped association between glycemic indices and SUA levels,but with different inflection points in men and *** thresholds for FPG,2 h-PG,and HbA1 c for men and women were 6.5/8.0 mmol/L,11.0/14.0 mmol/L,and 6.1/6.5,respectively(SUA levels increased with increasing glycemic indices before the inflection points and then eventually decreased with further increases in the glycemic indices).Conclusion An inverted U-shaped association was observed between major glycemic indices and uric acid levels in both sexes,while the inflection points were reached earlier in men than in women.

目的研究旨在开发一个新的可视化列线图模型来预测术后总生存率。方法研究为多中心、回顾性、观察性队列研究,纳入2015年8月1日至2018年6月30日在全国11家医疗中心接受根治性手术的胃癌和结直肠癌患者。收集基线临床、病理数据和营养状况评估,包括营养风险筛查2002(nutritional risk screening 2002,NRS 2002)评分和患者主观整体评估评分。采用最小绝对收缩和选择算子回归模型和Cox回归模型进行变量筛选,并进行内部和外部验证。结果训练组和验证组分别有681例和127例患者。中位随访时间为59(58~60)个月,共观察到188例患者死亡。研究最终筛选出两个独立的预测因子(NRS 2002评分和肿瘤TNM病理分期),将其与年龄共同纳入预测模型并绘制列线图。该模型1年、3年和5年总生存期的一致性指数在训练组分别为0.696、0.724和0.738,在验证组分别为0.801、0.812和0.793。结论研究开发并验证了一个基于NRS 2002评分、新的用于预测胃结直肠癌患者术后总生存期的列线图预测模型,该模型在预测胃肠癌患者根治术后长期生存率方面具有良好的区分度、校准度和临床实用性。

Because china is becoming an aging society,the incidence of diabetes and diabetic foot have been *** foot has become one of the main health-related killers due to its high disability and mortality *** pressure wound therapy(NPWT)is one of the most effective techniques for the treatment of diabetic foot wounds and great progress,both in terms of research and its clinical application,has been made in the last 20 years of its ***,due to the complex pathogenesis and management of diabetic foot,irregular application of NPWT often leads to complications,such as infection,bleeding and necrosis,that seriously affect its treatment *** 2020,under the leadership of Burns,Trauma and Tissue Repair Committee of the Cross-Straits Medicine Exchange Association,the writing group for‘Consensus on the application of negative pressure wound therapy of diabetic foot wounds’was established with the participation of scholars from the specialized areas of burns,endocrinology,vascular surgery,orthopedics and wound *** on evidence-based practice suggested by the latest clinical research,this consensus proposes the best clinical practice guidelines for the application and prognostic evaluation of NPWT for diabetic *** consensus aims to support the formation of standardized treatment schemes that clinicians can refer to when treating cases of diabetic foot.

Paroxysmal dyskinesias are a group of neurological diseases characterized by intermittent episodes of involuntary movements with different *** kinesigenic dyskinesia(PKD)is the most common type of paroxysmal dyskinesia and can be divided into primary and secondary types based on the ***,PKD is characterized by recurrent and transient attacks of involuntary movements precipitated by a sudden voluntary *** major cause of primary PKD is genetic abnormalities,and the inheritance pattern of PKD is mainly autosomal-dominant with incomplete *** proline-rich transmembrane protein 2(PRRT2)was the first identified causative gene of PKD,accounting for the majority of PKD cases *** increasing number of studies has revealed the clinical and genetic characteristics,as well as the underlying mechanisms of *** seeking the views of domestic experts,we propose an expert consensus regarding the diagnosis and treatment of PKD to help establish standardized clinical evaluation and therapies for *** this consensus,we review the clinical manifestations,etiology,clinical diagnostic criteria and therapeutic recommendations for PKD,and results of genetic analyses in PKD patients performed in domestic hospitals.

Emerged evidence has indicated that immunosuppression is involved in the occurrence and development of *** provide clinical practice recommendations on the immune function in sepsis,an expert consensus focusing on the monitoring and treatment of sepsis-induced immunosuppression was *** related to the immune monitoring and treatment of sepsis were retrieved from PubMed,Web of Science,and Chinese National Knowledge Infrastructure to design items and expert opinions were collected through an online ***,the Delphi method was used to form consensus opinions,and RAND appropriateness method was developed to provide consistency evaluation and recommendation levels for consensus *** consensus achieved satisfactory results through two rounds of questionnaire survey,with 2 statements rated as perfect consistency,13 as very good consistency,and 9 as good *** summarizing the results,a total of 14 strong recommended opinions,8 weak recommended opinions and 2 non-recommended opinions were ***,a face-to-face discussion of the consensus opinions was performed through an online meeting,and all judges unanimously agreed on the content of this *** summary,this expert consensus provides a preliminary guidance for the monitoring and treatment of immunosuppression in patients with sepsis.
