目的實行由護士主導的經腹股溝血管行心導管手術病人術後傷口院外跟進計劃,縮短病人因傷口問題延長住院時間。方法於2018年9月~2019年4月實行術後傷口院外跟進計劃(跟進組n=27),内容包括:出院前病人及家屬傷口護理指導、派發健教單張、安排兩次出院後回病區傷口跟進服務(常規出院後第3天及第7天)或兩次傷口外展服務(行動不便病人),對比2018年1月~2018年8月該類病人住院時間(對照組n=25),觀察計劃效果。結果兩組於年齡、性別、術式均沒有顯著性差異(P>0.05),術後平均住院時間:跟進組為4.4±1.8天、對照組為6.4±2.2天,具有顯著性差異(P<0.01),跟進組病人均未有發生手術部位感染,住院組發生1例手術部位感染。結論經護士主導的傷口院外跟進計劃能有效減少病人術後住院時間。

A Qualitative Study of the Influence of Grooved Mold Surface Topography on the Formation of Surface Marks on As-Cast Ingots of Aluminum Alloy 3003

The effects of the wavelength and orientation of machined grooves on a mold surface, casting speed, and melt superheat on the formation of surface marks on as-cast ingots were studied with an immersion casting tester and copper mold chill blocks. The mold surface topographies included a polished smooth surface, and those with machined unidirectional parallel contoured grooves oriented either parallel (vertical) or perpendicular (horizontal) to the casting direction. The unidirectional grooves were 0.232 mm deep with wavelength or spacing between 1 and 15 mm. The casting speed and melt superheat were between 1 and 200 mm/s, and 10 and 50 K, respectively. Two primary types of surface marks were observed on ingots cast with the copper mold with smooth surface topography, namely the finer and closely spaced ripples (Type I), and the widely spaced but coarser laps (Type II). The latter were more prevalent at the higher casting speeds and melt superheats. Qualitatively, formation of both types of surface marks on the as-cast ingots of the aluminum alloy 3003 appeared to be alleviated by increase in casting speed and melt superheat, and by the use of molds with grooved surface topography. In fact, casting with a mold surface with 1 mm spaced grooves that are perpendicular to the casting direction eliminated the formation of surface marks at casting speeds greater than 1 mm/s. It also improved the uniformity of the ingot subsurface microstructure and eliminated the associated subsurface segregation.

Galvanic corrosion of AZ31B joined to dual-phase steel with and without Zn layer by ultrasonic and friction stir welding

Galvanic corrosion of AZ31B joined with bare or Zn-coated DP590 steel by ultrasonic spot welding or linear friction stir welding was quantitatively studied by pre-defining anode and cathode in the lap joint samples. Corrosion volume and depth from Mg anode surfaces exposed to 0.1 M sodium chloride solution was analyzed as functions of cathode surface type and welding method. Characterization of as-welded joints was performed to identify any microstructural feature of the bonding zone that could impact galvanic corrosion *** modeling with modified user subroutine was conducted to simulate the progression of Mg corrosion in the same joint and electrode configurations used for the corrosion experiments. The experimental results indicated that Zn-coated cathode surface can reduce Mg galvanic corrosion significantly as galvanic polarization and cathodic current on Zn-coated surface remained relatively low for Mg in the weld *** modeling described the growth of Mg galvanic corrosion in a reasonable manner but showed limitation by underestimating the corrosion volume as it did not capture self-corrosion.

Programmed cell death （apoptosis） is a key host response to virus infection. Viruses that can modulate host apoptotic responses are likely to gain important opportunities for transmission. Here we review recent studies that demonstrate that particles of Invertebrate iridescent virus 6 （IIV-6）（Iridoviridae, genus Iridovirus）, or an IIV-6 virion protein extract, are capable of inducing apoptosis in lepidopteran and coleopteran cells, at concentrations 1000-fold lower than that required to shut-off host macromolecular synthesis. Induction of apoptosis depends on endocytosis of one or more heat-sensitive virion component（s）. Studies with a JnK inhibitor （SP600125） indicated that the JnK signaling pathway is significantly involved in apoptosis in IIV-6 infections of Choristoneurafumiferana cells. The genome of IIV-6 codes for an inhibitor of apoptosis iap gene （193R） that encodes a protein of 208 aa with 15% identity and 28% similarity in its amino acid sequence to IAP-3 from Cydia pomonella ganulovirus （CpGV）. Transcription of IIV-6 iap did not require prior DnA or protein synthesis, indicating that it is an immediate-early class gene. Transient expression and gene knockdown studies have confirmed the functional nature of the IIV-6 lap gene. We present a tentative model for IIV-6 induction and inhibition of apoptosis in insect cells and discuss the potential applications of these findings in insect pest control.

The complexity of calcareous deposits processes in a marine environment results in simultaneous effects of the following parameters： temperature, polarization potential, interfacial pH, chemical composition, etc.. The comprehension of these processes implies studies in artificial seawater and a follow-up of the parameters by voltarnperometry and chronoamperometry. Calcareous deposits electrochemically are very often used to follow up the evolution of scale deposition in desalination circuits. Again, the scale formation is brought about by electrochemical reduction of dissolved oxygen. The hydroxyl ions formed on the metallic surface engender a rise of interfacial pH which causes calcareous deposition （CaCOs and Mg（OH）2）. This reaction goes with reaction of hydrogen evolution which could disrupt the formed deposit, A study is carried out in a solution of seawater ASTM without calcium and without magnesium （so that the deposit will not be formed） on a titanium rotating disk electrode monitored between 300 and 1000 rpm. Study shows that Levich criterion is checked for the four values of selected potential on the diffusion plateau and a very cathodic polarization potential and a high temperature favors hydrogen current rate.

Transformation of low-volatility gaseous precursors to new particles affects aerosol number concentration,cloud formation and hence the *** clustering of acid and base molecules is a major mechanism driving fast nucleation and initial growth of new particles in the ***,the acid-base cluster composition,measured using state-of-the-art mass spectrometers,cannot explain the measured high formation rate of new *** we present strong evidence for the existence of base molecules such as amines in the smallest atmospheric sulfuric acid clusters prior to their detection by mass *** demonstrate that forming(H2SO4)1(amine)1is the rate-limiting step in atmospheric H2SO4-amine nucleation and the uptake of(H2SO4)1(amine)1is a major pathway for the initial growth of *** proposed mechanism is very consistent with measured new particle formation in urban Beijing,in which dimethylamine is the key base for H2SO4nucleation while other bases such as ammonia may contribute to the growth of larger *** findings further underline the fact that strong amines,even at low concentrations and when undetected in the smallest clusters,can be crucial to particle formation in the planetary boundary layer.

AIM： To investigate intestinal alkaline phosphatase （iAP） in the intestinal mucosa of children with inflammatory bowel disease （IBD）. METHODS： Colonic biopsy samples were taken from 15 newly diagnosed IBD patients and from 10 healthy controls. In IBD patients, specimens were obtainedboth from inflamed and non-inflamed areas. The lap mRnA and protein expression was determined by reverse transcription-polymerase chain reaction and Western blotting analysis, respectively. Tissue localiza- tion of lap and Toll-like receptor （TLR） 4 was investi- gated by immunofluorescent staining. RESULTS： The lap protein level in the inflamed muco- sa of children with Crohn＇s disease （CD） and ulcerative colitis （UC） was significantly decreased when compared with controls （both P 〈 0.05）. Similarly, we found a significantly decreased level of lap protein in the in- flamed mucosa in CD compared with non-inflamed mucosa in CD （P 〈 0.05）. In addition, the iAP protein level in inflamed colonic mucosa in patients with UC was decreased compared with non-inflamed mucosa in patients with CD （P 〈 0.05）. lap protein levels in the non-inflamed mucosa of patients with CD were similar to controls, lap mRnA expression in inflamed colonic mucosa of children with CD and UC was not significant- ly different from that in non-inflamed colonic mucosa with CD. Expression of lap mRnA in patients with non- inflamed mucosa and in controls were similar. Co-local- ization of lap with TLR4 showed intense staining with a dotted-like pattern, lap was present in the inflamed and non-inflamed mucosa of patients with CD, UC, and in control biopsy specimens, irrespective of whether it was present in the terminal ileum or in the colon. However, the fluorescent signal of TLR4 was more pro- nounced in the colon compared with the terminal ileum in all groups studied.
