Comparison of linac-based fractionated stereotactic radiotherapy and tomotherapy treatment plans for intra-cranial tumors

This study compares and analyzes stereotactic radiotherapy using tomotherapy and linac-based fractionated stereotactic radiotherapy in the treatment of intra-cranial tumors,according to some cases.In this study,linac-based fractionated stereotactic radiotherapy and tomotherapy treatment were administered to five patients diagnosed with intra-cranial cancer in which the dose of 18–20 Gy was applied on 3–5 separate occasions.The tumor dosing was decided by evaluating the inhomogeneous index （II） and conformity index （CI）.Also,the radiation-sensitive tissue was evaluated using low dose factors V1,V2,V3,V4,V5,and V10,as well as the non-irradiation ratio volume （NIV）.The values of the II for each prescription dose in the linacbased non-coplanar radiotherapy plan and tomotherapy treatment plan were （0.125±0.113） and （0.090±0.180）,respectively,and the values of the CI were （0.899±0.149） and （0.917±0.114）,respectively.The low dose areas,V1,V2,V3,V4,V5,and V10,in radiation-sensitive tissues in the linac-based non-coplanar radiotherapy plan fell into the ranges 0.3%–95.6%,0.1%–87.6%,0.1%–78.8%,38.8%–69.9%,26.6%–65.2%,and 4.2%–39.7%,respectively,and the tomotherapy treatment plan had ranges of 13.6%–100%,3.5%–100%,0.4%–94.9%,0.2%– 82.2%,0.1%–78.5%,and 0.3%–46.3%,respectively.Regarding the NIV for each organ,it is possible to obtain similar values except for the irradiation area of the brain stem.The percentages of NIV10%,NIV20%,and NIV30%for the brain stem in each patient were 15%–99.8%,33.4%–100%,and 39.8%–100%,respectively,in the fractionated stereotactic treatment plan and 44.2%–96.5%,77.7%–99.8%,and 87.8%–100%,respectively,in the tomotherapy treatment plan.In order to achieve higher-quality treatment of intra-cranial tumors,treatment plans should be tailored according to the isodose target volume,inhomogeneous index,conformity index,position of the tumor upon fractionated stereotactic radiosurgery,and radiation dosage for radiation-sensitive tissues.

Reevaluation of the expanded indications in undifferentiated early gastric cancer for endoscopic submucosal dissection

BACKGROUND Although the criteria for the indication of endoscopic submucosal dissection(ESD)for undifferentiated early gastric cancer(UD-EGC)have been recently proposed,accumulating reports on the non-negligible rate of lymph node metastasis(LNM)after indicated ESD raise questions on the reliability of the current criteria.AIM To investigate the prevalence and risk factors of LNM in UD-EGC cases meeting the expanded indication for ESD.METHODS We retrospectively reviewed 4780 UD-EGC cases that underwent surgical resection between January 2008 and February 2019 at Asan Medical Center,a tertiary university hospital in Korea.To identify the risk factors of LNM of UDEGC meeting the expanded criteria for ESD,we performed a case-control study by matching the cases with LNM to those without at a ratio of 1:4.We reviewed the clinical,endoscopic,and histologic features of the cases to identify features with a significant difference according to the presence of LNM.Univariate and multivariate logistic regression analyses were performed to estimate the odds ratios(ORs).RESULTS Of the 4780 UD-EGC cases,1240(25.9%)were identified to meet the expanded indication for ESD.Of the 1240 cases,14(1.1%)cases had LNM.Among the various clinical,endoscopic,and histopathological features that were evaluated,mixed histology(tumors consisting of 10%-90%of signet ring cells)had a marginally significant association(P=0.059)with the risk of LNM.Moreover,diffuse blurring of the muscularis mucosae(MM)underneath the tumorous epithelium,a previously unrecognized histologic feature,had a significant association with the absence of LNM(P=0.028).Multivariate logistic regression analysis showed that the blurring of MM was the only explanatory variable significantly associated with a reduced risk of LNM(OR:0.12,95%CI:0.02-0.95;P=0.045).CONCLUSION The risk of LNM is higher than expected when using the current expanded indication for UDEGC.Histological evaluation could provide useful clues for reducing the risk of LNM.

Determination of efficient superfinishing conditions for mirror surface finishing of titanium

To determine mirror surface finishing conditions and efficient and economical superfinishing conditions for pure titanium and titanium alloys,an abrasive film is used when superfinishing is performed under varying conditions.These conditions include the workpiece rotation speed,the oscillation speed,the contact pressure of the roller,the hardness of the roller,and the type of abrasive film.The superfinishing device is applied to polishing a thin and long cylindrical bar.A micro-finishing film and a lapping film were used as abrasive films.Al2O3 grains or SiC grains were used as abrasives.The surface roughness of a polished workpiece was measured using a stylus-type surface-roughness measuring instrument.As a result,the conditions to improve the polishing surface efficiently include high values for the workpiece rotation speed,oscillation speed and contact pressure.The roller hardness has no effect on the efficient polishing conditions.The mirror finish of a surface can be created using lapping film of 3 μm with Al2O3 grains after polishing to a steady surface roughness under the efficient polishing conditions.

Development of twin wheel creep-feed grinding machine using continuous dressing for machining of aircraft rotary wing

The purpose of this study is to develop a twin wheel creep-feed grinding machine using continuous dressing to machine precise axisymmetric turbine blades that have been difficult to machine using a conventional creep-feed machine.In order to develop such a machine,3D-modeling and machine simulations were performed and a twin wheel creep-feed grinding machine was manufactured.Furthermore,the axisymmetric precision of the machined workpieces through practical machining was evaluated and the quality of the continuous dressing effect of the developed machine was established.In addition,experimental considerations for a proper dresser-to-wheel speed ratio and proper feed rate of the dresser were carried out.As a result,a twin wheel creep-feed grinding machine with continuous dressing is developed through machine simulation,manufacturing and performance evaluation.Optimum condition for the dresser feed rate is 0.3μm/rev.In cases of large dressor-to-wheel speed ratio,grinding efficiency can be enhanced,but the surface roughness shows a conflicting trend.Developed twin wheel creep-feed grinding machine has satisfactory appraisal with regard to surface roughness,flatness,and parallelism.Satisfactory surface roughness below 0.1μm can be obtained for the blade of aircraft.However,in order to perform precise machining,it is necessary to improve the structure of the twin wheel creep-feed grinding machine.

Assessment criteria for nonverbal interaction contents in r-learning

r-learning,which is based on e-learning and u-learning,is defined as a learning support system that intelligent robots serve verbal and nonverbal interactions on ubiquitous computing environment.In order to guarantee the advantages of r-learning contents with no limits of time and place and with nonverbal interaction which are not in e-learning contents,in recent years,assessment criteria for r-learning contents are urgently required.Therefore,the reliable and valid assessment criteria were developed for nonverbal interaction contents in r-learning,and its detailed research content is as follows.First,assessment criteria for nonverbal interaction in r-learning contents will be specified into gesture,facial expression,semi-verbal message,distance,physical contact and time.Second,the validity of the developed assessment criteria will be proved by statistics.Consequently,the assessment criteria for nonverbal interaction contents will be helpful when choosing the better r-learning content and producing the better r-learning content,and the reliability of school education is improved ultimately.

Map-based control method for vehicle stability enhancement

This work proposes a map-based control method to improve a vehicle's lateral stability, and the performance of the proposed method is compared with that of the conventional model-referenced control method. Model-referenced control uses the sliding mode method to determine the compensated yaw moment; in contrast, the proposed map-based control uses the compensated yaw moment map acquired by vehicle stability analysis. The vehicle stability region is calculated by a topological method based on the trajectory reversal method. A 2-DOF vehicle model and Pacejka's tire model are used to evaluate the proposed map-based control method. The properties of model-referenced control and map-based control are compared under various road conditions and driving inputs. Model-referenced control uses a control input to satisfy the linear reference model, and it generates unnecessary tire lateral forces that may lead to worse performance than an uncontrolled vehicle with step steering input on a road with a low friction coefficient. However, map-based control determines a compensated yaw moment to maintain the vehicle within the stability region,so the typical responses of vehicle enable to converge rapidly. The simulation results with sine and step steering show that map-based control provides better the tracking responsibility and control performance than model-referenced control.

Ultra-Fast Next Generation Human Genome Sequencing Data Processing Using DRAGEN<sup>TM</sup>Bio-IT Processor for Precision Medicine

Slow speed of the Next-Generation sequencing data analysis, compared to the latest high throughput sequencers such as HiSeq X system, using the current industry standard genome analysis pipeline, has been the major factor of data backlog which limits the real-time use of genomic data for precision medicine. This study demonstrates the DRAGEN Bio-IT Processor as a potential candidate to remove the “Big Data Bottleneck”. DRAGENTM accomplished the variant calling, for ~40× coverage WGS data in as low as ~30 minutes using a single command, achieving the over 50-fold data analysis speed while maintaining the similar or better variant calling accuracy than the standard GATK Best Practices workflow. This systematic comparison provides the faster and efficient NGS data analysis alternative to NGS-based healthcare industries and research institutes to meet the requirement for precision medicine based healthcare.

本文研究了重金属和石油烃复合污染土壤中,腐植酸对草本植物吸收重金属的影响。结果显示:腐植酸大幅度地降低了污染土壤中可溶性和交换态重金属含量,但提高了植物可利用态重金属含量。土壤添加腐植酸后,除Ni外,重金属潜在生物有效性和可淋出性因子大于1,这表明大多数重金属对植物具有潜在的有效性。此外,腐植酸增加了铅(Pb)、铜(Cu)、镉(Cd)和镍(Ni)在供试植物幼苗和根部的积累,最高的是高羊茅幼苗,达264.7%,生物富集系数从0.30提高到1.10。芸苔根中Ni和Pb的生物富集系数也有所提高。这些研究结果表明,石油烃与重金属同时污染的情况下,加入腐植酸可以促进植物吸收重金属,但降低重金属淋失,防止地下污染。

Multiple z-Score Based Method for Noninvasive Prenatal Test Using Cell-Free DNA in Maternal Plasma

Objective: To improve the detecting accuracy of chromosomal aneuploidy of fetus by non-invasive prenatal testing (NIPT) using next generation sequencing data of pregnant women’s cell-free DNA. Methods: We proposed the multi-Z method which uses 21 z-scores for each autosomal chromosome to detect aneuploidy of the chromosome, while the conventional NIPT method uses only one z-score. To do this, mapped read numbers of a certain chromosome were normalized by those of the other 21 chromosomes. Average and standard deviation (SD), which are used for calculating z-score of each sample, were obtained with normalized values between all autosomal chromosomes of control samples. In this way, multiple z-scores can be calculated for 21 autosomal chromosomes except oneself. Results: Multi-Z method showed 100% sensitivity and specificity for 187 samples sequenced to 3 M reads while the conventional NIPT method showed 95.1% specificity. Similarly, for 216 samples sequenced to 1 M reads, Multi-Z method showed 100% sensitivity and 95.6% specificity and the conventional NIPT method showed a result of 75.1% specificity. Conclusion: Multi-Z method showed higher accuracy and robust results than the conventional method even at low coverage reads.
