mycophenolic Acid Absorption Profiles in Patients with Kidney or Combined Pancreas-Kidney Transplantation

Background: mycophenolate mofetil (mmF) is widely used in organ transplant patients to avoid calcineurine inhibittor-associated side effects. Therapeutic monitoring of mmF is up to now perform by using trough level measurements (measurements before drug administration). The present study was designed to characterize potential differences in mmF absorption kinetics between patients with allogenic kidney transplantation [kidney Tx] and simultaneous pancreas kidney transplantation [PK Tx], which might for example occur due to diabetic gastrointestinal atony. methods: A total of 64 pharmacokinetic profiles were prospectively studied in 44 adult kidney Tx and 20 PK Tx patients. To calculate AUC by the trapezoidal rule, mycophenolic acid (mPA) levels were measured in EDTA-plasma by an EmIT assay at 0, 0.5, 1, 2, 3, 4 6, and 12h after oral mmF administration between postoperative day 14 to 28 instable patients. Results: Substantial differences between kidney Tx and PK Tx patients were evident concerning: donor age, recipient age, number of mismatches, and kidney function (serum creatinine). Despite these dissimilarities pharmacokinetic absorption profiles did not significantly differ between patient groups as measured by AUC, C2, maximum mPA concentration (Cmax), and time until maximum absorption (Tmax). Astonishingly, concomitant cyclosporine and tacrolimus medication did not influence adsorption profiles. Only mPA concentrations 6h post administration correlated closely with AUC in both patient groups, whereas trough levels failed to be predictive for AUC. Conclusions: In our study population, mmF absorption kinetics did not differ between kidney and PK Tx patients and did not seem influenced by concomitant immunosuppressive medication. Therefore, mPA measurements during the absorption phase could be useful to better estimate AUC in patients with kidney Tx and PK Tx.

Assessment of Artificial Recharge Test in Jeftlik – Faria Area, West Bank

An artificial recharge test site was selected in the Jeftlik area of the foot prints of the Faria basin, in the Upper Jordan Rift Valley. The artificial pond infiltration type of 26 meters diameter and 2.85 meters depth was used. It is a hyperbolic form of 500 cubic meters volume. The integration parameters of hydrogeological settings, hydrochemical characteristics of allocated water bodies, geophysical investigations and the potentiality of the aquifer system in the area were used for the test area selection. The test site area lies within the shallow Pleistocene aquifer of 30-50 meters saturated thickness. One groundwater well of 19 m distance and two groundwater wells of 1 km far distance were used for the system monitoring. The filled water in the test system comes from the wadi water floods brought by 1.5 km of open canal. The sub-catchment is of 3 km2. The recharge rate within the Pleistocene aquifer was determined. This experiment is tested for the water quality type fillings of the first flushing water floods, the water flood in the middle of the winter season and the pumped water from other wells far away from the experiment.

Endothelial adhesion of synchronized gastric tumor cells changes during cell cycle transit and correlates with the expression level of CD44 splice variants

AIm: To study adhesion capacity and CD44 expression of human gastric adenocarcinoma mKN45 cells at different stages of a first cell ***: mKN45 cells were synchronized by aphidicolin and assayed for adhesion to an endothelial cell (HUVEC)monolayer. Surface expression of CD44 and CD44 splice variants on mKN45 cells was evaluated by flow *** relevance of CD44 adhesion receptors was investigated by blocking studies using anti CD44 monoclonal antibodies or by hyaluronan ***: Adhesion of mKN45 to HUVEC was increased during G2/m transit, after which adhesion returned to baseline levels with cell cycle completion. In parallel, CD44splice variants CD44v4, CD44v5, and CD44v7 were all upregulated on mKN45 during cell cycle progression with a maximum effect in G2/m. The function of CD44 surface receptors was assessed with specific receptor blocking monodonal antibodies or removal of hyaluronan by digestion with hyaluronidase. Both strategies inhibited tumor cell adhesion to HUVEC by nearly 50%, which indicates that mKN45-HUVEC-interaction is CD44 ***: CD44 expression level is linked to the cell cycle in gastrointestinal tumor cells, which in turn leads to cell cyde dependent alterations of their adhesion behaviour to endothelium.

Bone disorders in experimentally induced liver disease in growing rats

AIm: To investigate the change of bone parameters in a new model of experimentally induced liver cirrhosis and hepatocellular carcinoma (HCC) in growing ***: Fischer-344 rats (n = 55) were used. Carbon tetrachloride (CCl4), phenobarbital (PB), and a single diethylnitrosamine (DEN) injection were used. Animals were killed at wk 8 and 16. Bone mineral content, femoral length, cortical index (quotient of cortical thickness and whole diameter) and ultimate bending load (Fmax)of the femora were determined. The results in animals treated with DEN+PB+CCl4 (DPC, n = 21) were compared to those in untreated animals (UNT,n = 14) and in control group treated only with DEN+PB (DP, n = 20).RESULTS: Fatty liver and cirrhosis developed in each DPC-treated rat at wk 8 and HCC was presented at wk 16. No skeletal changes were found in this group at wk 8,but each parameter was lower (P＜0.05 for each) at wk 16 in comparison to the control group. Neither fatty liver nor cirrhosis was observed in DP-treated animals at any time point. Femoral length and Fmax values were higher (P＜0.05 for both) in DP-treated animals at wk 8 compared to the UNT controls. However, no difference was found at wk ***: Experimental liver cirrhosis and HCC are accompanied with inhibited skeletal growth, reduced bone mass, and decreased mechanical resistance in growing rats. Our results are in concordance with the data of other studies using different animal models. A novel finding is the transiently accelerated skeletal growth and bone strength after a 8-wk long phenobarbital treatment following diethylnitrosamine injection.
