Corneal sensitivity,ocular surface health and tear film stability after punctal plug therapy of aqueous deficient dry eye

AIM： To evaluate the effect of punctal occlusion using thermosensitive（smart plug） versus silicone plug for management of aqueous deficient dry eye on cornea sensitivity, ocular surface health and tear film ***： A comparative prospective interventiona case study included 45 patients with bilateral severe form of aqueous deficient dry eye. In each patient, the smar plug was inserted in the lower punctum of the right eye which was considered as study group 1 and silicone plug was inserted in the lower punctum of the left eye o the same patient which was considered as study group2. All patients were subjected to careful history taking and questionnaire for subjective assessment of severity of symptoms. Corneal sensitivity, corneal fluorescein rose bengal staining, Schirmer＇s I test, tear film break up time and conjunctival impression cytology were performed pre and 1, 3 and 6mo post plug ***： A statistically significant improvement in subjective and objective manifestations occurred following treatment with both types of plugs（P 〈0.01）The thermosensitive plug caused significant overal improvement, decrease in frequency of application o tear substitutes and improvement of conjunctiva impression cytology parameters in the inserted side（P 〈0.01）. Canaliculitis was reported in two eyes（4.4% following punctal occlusion using thermosensitive plug（study group 1）. Spontaneous plug loss occurred in 21eyes（46.6%） in the silicone plug group（study group 2）.CONCLUSION： Improvement of subjective and objective manifestations of aqueous deficient dry eye occurs following punctal plug occlusion. Thermosensitive plug has good patient＇s compliance with fewer complications and lower rates of loss compared to the silicone plug.

The second Egyptian research reactor ET-RR-2 went critical on the 27th of November *** National Center of Nuclear Safety and Radiation Control (NCNSRC) has the responsibility of the evaluation and assessment of the safety of this *** purpose of this paper is to present an approach to optimization of the fuel element plate. For an efficient search through the solution space we use a multi objective genetic algorithm which allows us to identify a set of Pareto optimal solutions providing the decision maker with the complete spectrum of optimal solutions with respect to the various *** aim of this paper is to propose a new approach for optimizing the fuel element plate in the *** fuel element plate is designed with a view to improve reliability and lifetime and it is one of the most important elements during the shut *** this present paper,we present a conceptual design approach for fuel element plate,in conjunction with a genetic algorithm to obtain a fuel plate that maximizes a fitness value to optimize the safety design of the fuel plate.

Empirical Asset Pricing-- Saudi Stylized Facts and Evidence

This paper estimates proxy specifications of a five-factor asset pricing model to produce stylized facts of the Saudi capital market and test an arbitrage pricing theory （APT） model. The data set is the panel of 20 most actively traded firms, excluding firms with negative book value of equity. The contribution to the extant literature is three-fold：（l） organizing Saudi market data based on beta and firm-specific fundamentals, namely, growth, value, accounting earnings, and equity investments; （2） conducting a parsimony analysis within the theoretical framework of APT; and （3） quantifying the information risk facing the marginal investor by decomposing earnings into cash flows and accruals and investigating respective loadings in an unrestricted version of the parsimonious specification. Proxy asset pricing specifications, though intuitively appealing, are scant due to lack of theoretical frameworks and misguided significance tests of factor loadings. Throughout, this issue is addressed by keeping the empirical analysis under describing market facts and testing an APT model. The study concludes with a significant empirical explanation that specifies average returns in terms of the covariance risk and accounting accruals.

Reversibility of minimal hepatic encephalopathy following liver transplantation in Egyptian cirrhotic patients

AIMTo evaluate the reversibility of minimal hepatic encephalopathy (MHE) following liver transplantation (LT) in Egyptian cirrhotic patients. METHODSThis prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty age- and sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A (TMT A), TMT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score (PHES) was calculated for patients to diagnose MHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTSBefore LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls (P P P P = 0.005). More patients in whom reversal of MHE was observed had a pre-transplant MELD score CONCLUSIONReversal of MHE in cirrhotic patients could be achieved by LT, especially in those with a MELD score

Synthesis and Antimicrobial Activity of Heterocyclic Compounds Containing Chromene Ring

Synthesis of some new compounds containing chromene ring are described in this work. They were synthesized in moderate to good yield, started with Condensation of 7-hydroxy-4-methylcoumarin （1） with ethyl bromoacetate furnishes ethyl-2-（4-methyl-2-oxo-2H-chromen-7-yloxy） acetate （2）. The structures of all the new compounds were confirmed using IR, 1H-NMR spectra and micro analysis, selected members of the synthesized compound were screened for antimicrobial activity. They have shown a wide range of activity from one completely inactive compound to medium active ones.

Helicobacter Pylori Infection Is Associated with Dyslipidemia and Increased Levels of Oxidized LDL in Type-2 Diabetes Mellitus

Emerging data now indicate and address the strong relationship between H. pylori infection and the incidence of Type 2 DM, a growing body of evidence suggests that the infection with H. pylori may be associated with insulin resistance, chronic inflammation, long-term diabetes complications, and cardiovascular risk factors. The present study was conducted to evaluate the relationship between the infection with Helicobacter pylori and disturbance in Lipid profile in Type 2 Diabetic patients. One hundred and five participants were enrolled, categorized into two groups of H. pylori positive cases and negative controls according to their results of H. pylori IgG antibodies. Subjects in both groups fill the structured questionnaire. Blood samples were drawn for measuring the FBS, 2hr-PP blood sugar, HbA1c, Lipid profile and oxidized LDL. The obtained results were statistically analyzed. The study methodology was approved by the Biomedical Ethics Committee in the Faculty of Medicine, Umm Al-Qura University, Makkah, KSA. 48 cases (45.7%) were diagnosed as H. pylori seropositive and 57 (54.3%) were negative. There is no significant difference in the mean age or mean BMI between the H. pylori negative and positive cases. Glycemic control was similar in the two groups. Total Cholesterol was higher in cases of positive H. pylori compared to negative controls (P < 0.001) and Triglycerides was significantly elevated too (P < 0.005). No significant difference in the levels of HDL-Cholesterol between the two groups, while the mean LDL-Cholesterol was found to be significantly increased in cases of H. pylori positive compared to negative controls (P < 0.001). Oxidized LDL levels in the positive cases was found to be increased significantly (P = 0.001) compared to negative controls. Infection with H. pylori is associated with increased levels of Total Cholesterol, Triglycerides, LDL-C and oxidized LDL in Type 2 Diabetic patients.

Ultrasound, Fluoroscopic-Guided Caudal, Lumbar Epidural Steroid Injections and Blinding Paraspinal Lumbosacral Steroid Injections in Patients with Low Back Pain with Radiculopathy

Background and Aims: Low back pain (LBP) is considered one of the most common health conditions in the world right now, and it affects many individuals throughout different stages of their lives. Chronic LBP (CLBP) was estimated to be between 5% and 10%, defined as LBP that lasts for 12 weeks. The most common causes of CLBP with radiculopathy are lumbar disc prolapse (LDP) and degenerative facet osteoarthropathy (DFO);the aim of this study is to investigate the efficacy of ultrasound (US) guided, fluoroscopy (FL) guided, Caudal Epidural Steroid Injection (CESI), lumbar epidural steroid injections (LESI), and blinding lumbosacral steroid injections (LSPSI) in patients with CLBP with radiculopathy. Patients and Methods: This is a randomized prospective study that was conducted at the department of rheumatology at Al Azhar University Hospital in Egypt between November 2020 and August 2021. A total of 100 patients with refractory CLBP with radiculopathy were enrolled in the study. Consequently, they were divided into 2 groups: the first consisted of fifty patients with CLBP and radiculopathy caused by LDP, as determined by lumbosacral magnetic resonance imaging (MRI), and the second group consisted of fifty patients with refractory low back pain and radiculopathy caused by DFO, as determined by lumbosacral plain x-rays and lumbosacral MRI. The following procedures were performed: US-guided CESI, FL-guided CESI, FL-guided LESI, US-guided LESI, and blinding LSPSI. Results: In the LDP group, there is a statistically significant difference between considered spinal nerve roots as regards Visual Analogue Scale (VAS) (at 2 months). Likewise, a statistically significant difference was found between blinding LSPSI and US-Guided LESI with respect to VAS (baseline) and VAS (2 months) (P-value = 0.018 and 0.003, respectively). BACKGROUND Despite significant advancements in liver transplantation(LT)surgical procedures and perioperative care,post-LT biliary complications(BCs)remain a significant source of morbidity,mortality,and graft *** addition,data are conflicting regarding the health-related quality of life(HRQoL)of LT ***,the success of LT should be considered in terms of both the survival and recovery of *** To assess the impact of BCs on the HRQoL of live-donor LT recipients(LDLT-Rs).METHODS We retrospectively analysed data for 25 LDLT-Rs who developed BCs post-LT between January 2011 and December 2016 at our *** Short Form 12 version 2(SF 12v2)health survey was used to assess their *** also included 25 LDLT-Rs without any post-LT complications as a control *** The scores for HRQoL of LDLT-Rs who developed BCs were significantly higher than the norm-based scores in the domains of physical functioning(P=0.003),role-physical(P<0.001),bodily pain(P=0.003),general health(P=0.004),social functioning(P=0.005),role-emotional(P<0.001),and mental health(P<0.001).No significant difference between the two groups regarding vitality was detected(P=1.000).The LDLT-Rs with BCs had significantly lower scores than LDLT-Rs without BCs in all HRQoL domains(P<0.001)and the mental(P<0.001)and physical(P=0.0002)component summary *** The development of BCs in LDLT-Rs causes a lower range of improvement in HRQoL.

N-butyl-2-cyanoacrylate, iso-amyl-2-cyanoacrylate and hypertonic glucose with 72% chromated glycerin in gastric varices

cyanoacrylate and a mixture of 72% chromated glycerinwith hypertonic glucose solution in management ofgastric ***： Ninety patients with gastric varicespresented to Endoscopy Unit of Ain Shams UniversityHospital were included. They were randomly allocatedinto three groups; each group included 30 patients treatedwith intravariceal sclerosant injections in biweeklysessions till complete obturation of gastric varices;Group I （n-butyl-2-cyanoacrylate; Histoacryl？）, GroupII （iso-amyl-2-cyanoacrylate; Amcrylate？） and GroupIII （mixture of 72% chromated glycerin; Scleremo？with glucose solution 25%）. All the procedures wereperformed electively without active bleeding. Recruitedpatients were followed up for 3 ***： 26% of Scleremo group had bleeding duringpuncture vs 3.3% in each of the other two groups withsignificant difference, （P 〈 0.05）. None of Scleremogroup had needle obstruction vs 13.3% in each of theother two groups with no significant difference, （P 〉0.05）. Rebleeding occurred in 13.3% of Histoacryl andAmcrylate groups vs 0% in Scleremo group with nosignificant difference. The in hospital mortality was 6.6%in both Histoacryl and Amcrylate groups, while it was0% in Scleremo group with no significant difference. Inthe first and second sessions, the amount of Scleremoneeded for obturation was significantly high, while the amount of Histoacryl was significantly low. Scleremo was the less costly of the two treatments. CONCLUSION： All used sclerosant substances showed efficacy and success in management of gastric varices with no significant differences except in total amount,cost and bleeding during puncture.
