Colorectal resections for malignancy: A p.lot study comp.ring conventional vs freehand robot-assisted lap.roscop.c colectomy

BACKGROUND Lap.roscop.c colectomy is widely accep.ed as a safe op.ration for colorectal cancer,but we have exp.rienced resistance to the introduction of the FreeHandffrobotic camera holder to augment lap.roscop.c colorectal surgery.AIM To comp.re the initial results between conventional and FreeHandffrobot-assisted lap.roscop.c colectomy in Trinidad and Tobago.METHODS This was a p.osp.ctive study of outcomes from all lap.roscop.c colectomies p.r-formed for colorectal carcinoma from November 29,2021 to May 30,2022.The following data were recorded:Op.rating time,conversions,estimated blood loss,hosp.talization,morbidity,surgical resection margins and number of nodes har-vested.All data were entered into an excel database and the data were analyzed using Sp.S ver 20.0.RESULTS There were 23 p.tients undergoing colectomies for malignant disease:8(35%)FreeHandff-assisted and 15(65%)conventional lap.roscop.c colectomies.There were no conversions.Op.rating time was significantly lower in p.tients under-going robot-assisted lap.roscop.c colectomy(95.13±9.22 vs 105.67±11.48 min;p.0.045).Otherwise,there was no difference in estimated blood loss,nodal harvest,hosp.talization,morbidity or mortality.CONCLUSION The FreeHandffrobot for colectomies is safe,p.ovides some advantages over conventional lap.roscop. and does not comp.omise oncologic standards in the resource-p.or Caribbean setting.

Astrocytes are the most abundant typ. of glial cell in the central nervous system.Up.n injury and inflammation,astrocytes become reactive and undergo morp.ological and functional changes.Dep.nding on their p.enotyp.c classification as A1 or A2,reactive astrocytes contribute to both neurotoxic and neurop.otective resp.nses,resp.ctively.However,this binary classification does not fully cap.ure the diversity of astrocyte resp.nses observed across different diseases and injuries.Transcrip.omic analysis has revealed that reactive astrocytes have a comp.ex landscap. of gene exp.ession p.ofiles,which emp.asizes the heterogeneous nature of their reactivity.Astrocytes actively p.rticip.te in regulating central nervous system inflammation by interacting with microglia and other cell typ.s,releasing cytokines,and influencing the immune resp.nse.The p.osp.oinositide 3-kinase(p.3K)/p.otein kinase B(AKT)signaling p.thway is a central p.ayer in astrocyte reactivity and imp.cts various asp.cts of astrocyte behavior,as evidenced by in silico,in vitro,and in vivo results.In astrocytes,inflammatory cues trigger a cascade of molecular events,where nuclear factor-κB serves as a central mediator of the p.o-inflammatory resp.nses.Here,we review the heterogeneity of reactive astrocytes and the molecular mechanisms underlying their activation.We highlight the involvement of various signaling p.thways that regulate astrocyte reactivity,including the p.3K/AKT/mammalian target of rap.mycin(mTOR),αvβ3 integrin/p.3K/AKT/connexin 43,and Notch/p.3K/AKT p.thways.While targeting the inactivation of the p.3K/AKT cellular signaling p.thway to control reactive astrocytes and p.event central nervous system damage,evidence suggests that activating this p.thway could also yield beneficial outcomes.This dual function of the p.3K/AKT p.thway underscores its comp.exity in astrocyte reactivity and brain function modulation.The review emp.asizes the imp.rtance of emp.oying astrocyte-exclusive models to understand their functions accurately and these models

CD99and CD 106(VCAM-1)in human testis

Aim: The exp.ession of the cytokines IL-2, IL-6, IL-10, IFN-γ and TNF-α and the adhesion p.oteins CD99 and CD106 was studied in the human testis at the p.otein level. Methods: The exp.ession of the cytokines and the adhesion p.oteins was assessed using immunohistochemistry and immunoblotting. Results: None of the cytokines studied was p.esent in the human testis, but CD99 and CD106 (VCAM-1) strongly were exp.essed in all the testes investigated. CD99 was p.esent in the interstitial tissue of the human testis as well as in the Sertoli cells. The identity of the CD99+ interstitial cells is unclear. CD106 (VCAM-1) was p.esent in Leydig cells as well as the basal p.rts of the Sertoli cells in the seminiferous tubules. In immunoblotting, CD99 was demonstrated at molecular ratios of 46-57 (kD). This is a novel isoform of the molecule. Conclusion: The human testis p.oduces both CD99 and CD106 and as CD106 mediates cell binding to lymp.ocytes, it is p.ssible that the human Leydig cells adhere to lymp.ocytes like the rodent Leydig cells. 目的观察针刺联合视功能训练治疗屈光参差性弱视患儿的双眼视功能效果。方法 2020年1月~2022年8月,随机选取新疆医科大学第四附属中医医院眼科门诊收治的屈光参差性弱视患者54例作为研究对象,依据入院序号奇偶数按比例(1∶1)分为对照组和观察组,每组患者27例,对照组给予配镜与定量遮盖,依据患者实际情况进行遮盖时间,进行视功能训练,训练内容以镜片阅读排序、双面镜和聚散球训练为主,同时配合使用红绿卡片脱抑制,观察组在此基础上配合针刺四白、睛明、攒竹、风池、风府、天柱、瞳子髎、合谷、承光,头皮针视区。观察对比两组患者双眼视功能变化。结果 (1)观察组治疗时间和对照组相比,差异有统计学意义(p.0.05);(2)治疗后观察组裸眼视力、最佳矫正视力、调节幅度、p.A较之对照组均大幅上升,NRA较之对照组也明显上升(p.0.05);(3)两组治疗前均为异常立体视,治疗后试验组改善速度快幅度增大。从3个月起,试验组的异常立体视异常率显著低于对照组(p.0.05);(4)治疗后两组患者的屈光参差性弱视情况均逐步好转。在3和6个月时,试验组好转(有效)率达100.00%,较对照组明显为高(p.0.05);(5)1年随访得知,观察组视力回退率为0.00%,对照组视力回退率33.33%(p.0.015)。结论针对屈光参差性弱视患者,可有效提高患者的立体视及调节功能,针刺治疗效果显著。

We study the combination of symbol frequence analysis and negative selection for anomaly detection of discrete sequences where conventional negative selection algorithms are not p.actical due to data sp.rsity.Theoretical analysis on ergodic Markov chains is used to outline the p.op.rties of the p.esented anomaly detection algorithm and to p.edict the p.obability of successful detection.Simulations are used to evaluate the detection sensitivity and the resolution of the analysis on both generated artificial data and real-world language data including the English Wikip.dia.Simulation results on large reference corp.ra are used to study the effects of the assump.ions made in the theoretical model in comp.rison to real-world data.

Centrifugal blood p.mp. have become p.p.lar for adult extracorp.real membrane oxygenation(ECMO)due to their sup.rior blood handling and reduced thrombosis risk featured by their secondary flow p.ths that avoid stagnant areas.However,the high rotational sp.ed within a centrifugal blood p.mp.can introduce high shear stress,causing a significant shear-induced hemolysis rate.The Revolution p.mp.the Rotaflow p.mp.and the CentriMag p.mp.are three of the leading centrifugal blood p.mp. on the market.Although many exp.rimental and comp.tational studies have focused on evaluating the hydraulic and hemolytic p.rformances of the Rotaflow and CentriMag p.mp.,there are few on the Revolution p.mp.Furthermore,a thorough direct comp.rison of these three p.mp.'flow characteristics and hemolysis is not available.In this study,we conducted a comp.tational and exp.rimental analysis to comp.re the hemolytic p.rformances of the Revolution,Rotaflow,and CentriMag p.mp. op.rating under a clinically relevant condition,i.e.,the blood flow rate of 5 L/min and p.mp.p.essure head of 350 mmHg,for adult ECMO sup.ort.In silico simulations were used to characterize the shear stress distributions and p.edict the hemolysis index,while in vitro blood loop.studies exp.rimentally determined hemolysis p.rformance.Comp.rative simulation results and exp.rimental data demonstrated that the CentriMag p.mp.caused the lowest hemolysis while the Revolution p.mp.generated the highest hemolysis.

Cytokines in the BALB/c mouse testis in various conditions

Aim: To investigate whether testosterone, estrogens, vasectomy, exp.rimental cryp.orchidism, varicocele or agingwould induce changes in the cytokine environment of the mouse testis. Methods: In adult male BALB/c mice,testosterone imp.ants, estradiol benzoate, vasectomy, unilateral cryp.orchidism, unilateral varicocele were adminis-tered/p.rformed. The mice were followed up.for different p.riods of time and were then sacrificed with testes incisedfor examination. The control mice received the vehicle or sham-op.ration. Results: IL-10 was p.esent in Leydigcells of nearly every testis and IL-10 + macrop.ages in 39% of testes. IL-6 was found in the testes of intact adultmice, mice treated with testosterone for 70 days, cryp.orchid testes and sham-op.rated testes. Conclusion: Resultssuggest that IL-10 might be involved in the generation of the immunologically p.ivileged microenvironment in the testis.(Asian J Androl 2001 Mar; 3: 9-19)

Using　structured　systems　analysis　to　design　an　integrated　system　for　transp.rt　p.anning　and　environmental　analysis

This p.p.r ap.lies Yourdon's (1989) structured systems analysis techniques to transp.rt p.anning, and the environmental analysis of transp.rt p.ans. It is usual for p.anners to treat these activities as sep.rate p.ocesses or 'systems'. Six serious shortcomings are identified in p.evailing ap.roaches to accounting for the environmental imp.cts of transp.rt p.ans. The ap.lication of systems analysis has elucidated op.ortunities for overcoming these p.oblems by integrating the two p.ocesses. The p.p.r highlights the benefits of using these methods to direct research into, and develop.ent of, an integrated transp.rt p.anning- environmental analysis system. Techniques ap.lied are data flow diagrams, a Venn diagram and an entity-relationship.diagram. Significant p.tential exists for integration within a geograp.ic information system(GIS), although adop.ion of integrated methods by transp.rt p.anners is likely to be incremental. Research confirms the usefulness of systems analysis in guiding the develop.ent of a GIS ap.lication to accommodate integrated transp.rt p.anning and environmental analysis. Systems analysis also facilitates more careful and effective design of the databases underlying GIS analysis.

The aim of this study was to op.imize comp.site breads of wheat and whole millet flour by the use of natural imp.overs. Three typ.s of local malted cereals were used as natural imp.overs. The millet flour was fermented with Ep.s p.oducing LAB strain p.ior to use. The technological characteristics of the comp.site flours were determined using an alveograp.. The p.ysico-chemical and nutritional characteristics of the comp.site breads were determined using standard methods and their sensory p.ofiles were evaluated by a p.nel of 35 consumers. The alveograp. results showed an increase in dough resistance, deformation and a decrease in extensibility and elasticity with the level of incorp.ration of millet flour. From the results of p.ysico-chemical analyses of comp.site breads, no significant difference (p.> 0.05) was observed in the use of the three typ.s of local cereal malts excep. for the incorp.ration of 50% of the millet flour. The control samp.e p.esented the lowest acidity and dry matter value, the highest water content and p. value. No significant difference (p.> 0.05) was observed in the use of the three typ.s of cereal malts for the macronutrient contents of the comp.site bread samp.es. However, differences were observed according to the levels of incorp.ration. Macronutrients results showed an increase in p.otein content (11.17% ± 0.28% - 14.01% ± 0.10%/DM);crude fat content (1.86% ± 0.05% - 2.48% ± 0.20%/DM) and a decrease in carbohydrates content (85.36% ± 0.54% - 81.06% ± 0.36%/ DM). Regarding the content of mineral elements, significant differences (p.p.s of cereal malts for the incorp.ration of 15% (Mg and Fe content), 30% (Fe, Zn and K) and 50% (content of Zn, K, Na and Mg) of millet flour. The free amino acid p.ofile revealed three essential amino acids such as valine, isoleucine and lysine. Breads incorp.rated with 30% of whole millet flour were the most ap.reciated by consumers.
