Complex liver retransplantation to treat graft loss due to long-term biliary tract complication after liver transplantation: A case report

BACKGROUND Loss of graft function after liver transplantation(LT) inevitably requires liver retransplant. Retransplantation of the liver(Re LT) remains controversial because of inferior outcomes compared with the primary orthotopic LT(OLT).Meanwhile, if accompanied by vascular complications such as arterial and portal vein(PV) stenosis or thrombosis, it will increase difficulties of surgery. We hereby introduce our center’s experience in Re LT through a complicated case of *** SUMmaRY We report a patient who suffered from hepatitis B-associated cirrhosis and underwent LT in December 2012. Early postoperative recovery was *** months after LT, the patient’s bilirubin increased significantly and he was diagnosed with an ischemic-type biliary lesion caused by hepatic artery occlusion. The patient underwent percutaneous transhepatic cholangial drainage and repeatedly replaced intrahepatic biliary drainage tube regularly for 5 *** patient developed progressive deterioration of liver function and underwent liver re-transplant in January 2019. The operation was performed in a classic OLT manner without venous bypass. Both the hepatic artery and PV were occluded and could not be used for anastomosis. The donor PV was anastomosed with the recipient’s left renal vein. The donor hepatic artery was connected to the recipient’s abdominal aorta. The bile duct reconstruction was performed in an end-to-end manner. The postoperative process was very uneventful and the patient was discharged 1 mo after *** With the development of surgical techniques, portal thrombosis and arterial occlusion are no longer contraindications for ReLT.

Prognostic Significance of MiR-34a Expression in Patients with Gastric Cancer after Radical Gastrectomy

Background： MiR-34a dysregulation has been implicated in tumorigenesis and progression of gastric cancer, but its role in prognosis of patients with gastric cancer remains unknown. The aim of this study was to investigate the expression and prognostic significance of miR-34a in gastric cancer patients after radical gastrectomy. Methods： Quantitative real-time polymerase chain reaction was performed to detect the expression of miR-34a in human gastric cancer cell lines and tissues in 76 patients with gastric adenocarcinoma from China. Results are assessed for association with clinical features and overall survival （OS） using Kaplan-Meier analysis. Prognostic values of miR-34a expression and clinical outcomes were evaluated by Cox regression analysis. A molecular prognostic stratification scheme incorporating miR-34a expression was determined using receiver operating characteristic analysis. Results： The results show that the expression level of miR-34a was decreased in human gastric cancer cell lines and tissues, and down-regulated expression of miR-34a was associated with Lauren classification （P = 0.034）. Decreased miR-34a expression in gastric cancer tissues was positively correlated with poor OS of gastric cancer patients （P = 0.0 l 3）. Further multivariate Cox regression analysis suggested that miR-34a expression was an independent prognostic indicator for gastric cancer （P = 0.027）. Applying the prognostic value of miR-34a expression to tumor node metastasis （TNM） stage system showed a better prognostic value in patients with gastric cancer than miR-34a expression （P = 0.0435） or TNM stage （P = 0.0249） alone. Conclusion： The results reinforce the critical role for the down-regulated miR-34a expression in gastric cancer and suggest that miR-34a could be a prognostic indicator for this disease.

针对水下观测图像的颜色失真和散射模糊问题,提出一种基于改进循环一致性生成对抗网络(Cycle-consistent generative adversarial networks,CycleGAN)的水下图像颜色校正与增强算法.为了利用CycleGAN学习水下降质图像到空气中图像的映射关系,对传统CycleGAN的损失函数进行了改进,提出了基于图像强边缘结构相似度(Strong edge and structure similarity,SESS)损失函数的SESS-CycleGAN,SESS-CycleGAN可以在保留原水下图像的边缘结构信息的前提下实现水下降质图像的颜色校正和对比度增强.为了确保增强后图像和真实脱水图像颜色的一致性,建立了SESSCycleGAN和正向生成网络G相结合的网络结构;并提出了两阶段学习策略,即先利用非成对训练集以弱监督方式进行SESS-CycleGAN学习,然后再利用少量成对训练集以强监督方式进行正向生成网络G的监督式学习.实验结果表明:本文算法在校正水下图像颜色失真的同时还增强了图像对比度,且较好地实现了增强后图像和真实脱水图像视觉颜色的一致性.

Modii cation ef ect of calcium-magnesia phosphate fertilizer on microstructure and mechanical properties of Mg_2Si/Mg-4Si composite

In order to modify in-situ synthesized Mg_2Si particles in Mg_2Si/Mg-4Si composite, the modif ication effect of calcium-magnesia phosphate fertilizer on primary Mg_2Si phase in Mg_2Si/Mg-4Si composite was investigated by means of X-ray diffraction(XRD), optical microscopy(OM), scanning electron microscopy(SEM) and energy dispersive spectroscopy(EDS) analysis. The results indicate that the morphology of the primary Mg_2Si phase apparently changes from coarse dendrites to f ine dispersive polygonal particles, and the mean size is decreased from 277 μm to 17 μm. With the addition of 4.0wt.% calcium-magnesia phosphate fertilizer as a modif ier, the ultimate tensile strength and elongation of the Mg_2Si/Mg-4Si composite are increased from 78.7 MPa and 2.1% to 105.2 MPa and 2.6%, as compared to those of the base composite, which is probably attributed to the formation of the phosphorous compound and the cluster of Ca compounds that acted as the heterogeneous nucleation substrates of the primary Mg_2Si particles, resulting in a ref ined distribution of these precipitates.

NMR STUDIES ON THE WEAK INTERACTION OF FOUR-MONONUCLEOTIDES WITH TRYPTAMIDE AND LEUCINAMIDE

NMR spectroscopy analysis was introduced to investigate the weak interaction of four 5'—mononucleotides with tryptamide and leucinamide. Assuming a 1∶1 complex formation, the association constants were determined by nonlinear regression analysis. The association affinity of 4 mononucleotides with tryptamide decreased in the order: G>A>C>U, indicating that purine bases stack more strongly with tryptamide than pyrimidine bases. But with leucinamide the affinity decreases in the order: G>C>A=U, suggesting an amino acid—anticodonic preferential interaction. The implication of the present data to the origin of genetic code was briefly discussed.

In this review,the recent developments in microelectronics,spintronics,and magnonics have been summarized and ***,the history of the spintronics has been briefly ***,the recent development of magnonics such as magnon-mediated current drag effect(MCDE),magnon valve effect(MVE),magnon junction effect(MJE),magnon blocking effect(MBE),magnon-mediated nonlocal spin Hall magnetoresistance(MNSMR),magnon-transfer torque(MTT)effect,and magnon resonant tunneling(MRT)effect,magnon skin effect(MSE),etc.,existing in magnon junctions or magnon heterojunctions,have been summarized and their potential applications in memory and logic devices,etc.,are prospected,from which we can see a promising future for spintronics and magnonics beyond micro-electronics.

Crebanine inhibits voltage-dependent Na～+ current in guinea-pig ventricular myocytes

AIM: To study the effects of crebanine on voltage-gated Na+ channels in cardiac tissues. METHODS: Single ventricular myocytes were enzymatically dissociated from adult guinea-pig heart. Voltage-dependent Na+ current was recorded using the whole cell voltage-clamp technique. RESULTS: Crebanine reversibly inhibited Na+ current with an IC50 value of 0.283 mmol?L-1(95% confidence range: 0.248-0.318 mmol?L-1). Crebanine at 0.262 mmol?L-1 caused a negative shift(about 12 mV) in the voltage-dependence of steady-state inactivation of Na+ current, and retarded its recovery from inactivation, but did not affect its activation curve. CONCLUSION: In addition to blocking other voltage-gated ion channels, crebanine blocked Na+ channels in guinea-pig ventricular myocytes. Crebanine acted as an inactivation stabilizer of Na+ channels in cardiac tissues.

Objective To investigate the distribution and antimicrobial susceptibility of causative microorganisms recovered from patients with intra-abdominal infections(IAIs).Methods A total of 2,926 bacterial and fungal strains were identified in samples collected from 1,679 patients with IAIs at the Peking Union Medical College Hospital between 2011 and *** bacteria and fungi were identified using matrix-assisted laser desorption/ionization time-of-flight mass *** susceptibility testing(AST)was performed using the VITEK 2 compact system and the Kirby–Bauer *** results were interpreted based on the M100-Ed31 clinical breakpoints of the Clinical and Laboratory Standards *** Of the 2,926 strains identified,49.2%,40.8%,and 9.5%were gram-negative bacteria,gram-positive bacteria,and fungi,*** coli was the most prevalent pathogen in intensive care unit(ICU)and non-ICU patients;however,a significant decrease was observed in the isolation of *** between 2011 and ***,significant decreases were observed between 2011 and 2021 in the levels of extended-spectrumβ-lactamase(ESBL)-producing ***(from 76.9%to 14.3%)and Klebsiella pneumoniae(from 45.8%to 4.8%).Polymicrobial infections,particularly those involving co-infection with gram-positive and gram-negative bacteria,were commonly observed in IAI ***,Candida albicans was more commonly isolated from hospital-associated IAI samples,while Staphylococcus epidermidis had a higher ratio in community-associated ***,AST results revealed that most antimicrobial agents performed better in non-ESBL-producers than in ESBL-producers,while the overall resistance rates(56.9%–76.8%)of Acinetobacter baumanmii were higher against all antimicrobial agents than those of other common gram-negative ***,Enterococcus faecium,Enterococcus faecalis,***,and *** were consistently found to be susceptible to vancomycin
