This prospective multicenter phase II study evaluated the clinical efficacy of neoadjuvant nivolumab-exclusive(N)and nivolumab–chemotherapy(N/C)combinations based on PD-L1 *** patients exhibited resectable clinical stage IIA–IIIB(AJCC 8th edition)NSCLC without EGFR/ALK *** received either mono-nivolumab(N)or nivolumab+nabpaclitaxel+carboplatin(N/C)for three cycles based on PD-L1 *** primary endpoint was the major pathological response(MPR).Key secondary endpoints included the pathologic complete response(pCR),objective response rate(ORR),and event-free survival(EFS).Baseline PD-L1 expression and perioperative circulating tumor DNA(ctDNA)status were correlated with pCR and ***-two patients were enrolled,with 46 undergoing *** MPR was 50.0%(26/52),with 25.0%(13/52)achieving pCR,and 16.7%and 66.7%for patients with PD-L1≥50%in N and N/C groups,***(25.0%)patients experienced grade 3 or higher immune-related adverse events during neoadjuvant *** with post-neoadjuvant ctDNA negativity was more likely to have pCR(39.1%)compared with those remained positive(6.7%,odds ratio=6.14,95%CI 0.84-Inf,p=0.077).With a median follow-up of 25.1 months,the 18-month EFS rate was 64.8%(95%CI 51.9–81.0%).For patients with ctDNA–***+,the 18m-EFS rate was 93.8%vs 47.3%(HR,0.15;95%CI 0.04,0.94;p=0.005).Immunochemotherapy may serve as an optimal neoadjuvant treatment even for patients with PD-L1 expression≥50%.ctDNA negativity following neoadjuvant treatment and surgery could help identify superior pathological and survival benefits,which requires further confirmation in a prospective clinical trial(NCT04015778).

Lung cancer mortality has decreased over the past decade and can be partly attributed to advances in targeted therapy and *** checkpoint inhibitors(ICIs)have rapidly evolved from investigational drugs to standard of care for the treatment ofmetastatic non-small cell lung cancer(NSCLC).In particular,antibodies that block inhibitory immune checkpoints,such as programmed cell death protein 1(PD-1)and programmed cell death 1 ligand 1(PD-L1),have revolutionized the treatment of advanced NSCLC,when administered alone or in combination with *** is associated with higher response rates,improved overall survival(OS),and increased tolerability compared with conventional cytotoxic *** benefits may increase the utility of immunotherapy and its combinational use with chemotherapy in the neoadjuvant treatment of patients with *** findings from various ongoing clinical trials suggest that neoadjuvant ICIs alone or combined with chemotherapy may significantly reduce systemic recurrence and improve long-term OS or cure rates in resectable *** we further summarize the safety and efficacy of various neoadjuvant treatment regimens including immunotherapy from ongoing clinical trials and elaborate the role of neoadjuvant immunotherapy in patients with resectable *** addition,we discuss several unresolved challenges,including the evaluations to assess neoadjuvant immunotherapy response,the role of adjuvant treatment after neoadjuvant immunotherapy,the efficacy of treatment for oncogenic-addicted tumors,and predictive *** also provide our perspective on ways to overcome current obstacles and establish neoadjuvant immunotherapy as a standard of care.

A lobe-specific lymphadenectomy protocol for solitary pulmonary nodules in non-small cell lung cancer

Background： We want to establish a lobe-specific mediastinal lymphadenectomy protocol for solitary pulmonary nodules （SPNs） in non-small cell lung cancer （NSCLC）. Methods： We retrospectively analyzed 401 patients with pathological diagnoses of NSCLC who underwent lobectomy, bilobectomy, or pneumonectomy with systematic lymphadenectomy from March 2004 to June 2011 in our hospital. All of the patients enrolled had a SPN preoperatively. Information about the primary tumor location, lymph node metastasis, and other baseline data were collected. Stepwise logistic regression was used to identify the key factors indicating non-regional mediastinal lymph node metastases （NRM）. Results： Of the primary tumors, 117, 39, 74, 104, and 67 were in the right upper lung （RUL）, right middle lung （RML）, right lower lung （RLL）, left upper lung （LUL）, and left lower lung （LLL）, respectively. Stepwise regression showed that #2,4, #10,11, and #10,11 as well as #7 was the key lymph node station for RUL, LUL, and lower lobes： #2,4 [odds ratio （OR）=28.000, 95% confidence interval （CI）： 2.917-268.790, P=0.004] for RUL, #10,11 （OR=31.667, 95% CI： 2.502-400.833, P=0.008） for LUL, #10,11 （OR=19.540, 95% CI： 4.217-90.541, P〈0.001） and #7 （OR=7.395, 95% CI： 1.586-34.484, P=0.011） for lower lobes, respectively. Patients with tumors 〉2 cm rarely had NRM without primary regional mediastinal involvement. Conclusions： With rigid consideration, a lobe-specific lymphadenectomy is feasible in practice. This protocol can be used when the lobe-specific key nodes are negative in intraoperative frozen sections, especially for NSCLC diagnosed as SPN 〈2 cm preoperatively.

Carbon efficiency modeling and optimization of solar-powered cellular networks

As wireless communication traffic experiences rapid growth, the carbon emissions caused by the communication industry are also on the rise. To achieve “carbon neutrality”, researchers are considering the use of renewable energy sources to power cellular networks, thereby reducing carbon emissions. However, a challenge arises when using renewable energy, specifically owing to the unpredictable nature of both the energy consumption of the cellular network and the power generation from renewable sources. This inconsistency results in low renewable energy utilization and reduced carbon efficiency. Herein, we construct a carbon efficiency model of solar-powered cellular networks using practical data from solar radiation. We propose a mechanism that alternately optimizes the performance of the renewable energy network and the cellular network. This approach is based on convex optimization theory and the Dinkelback algorithm, and it leads to the design of a carbon efficiency optimization algorithm. This algorithm aims to improve the carbon efficiency of cellular networks and reduce their carbon emissions. Simulation results demonstrate that our optimization scheme yields a maximum improvement of 2.56 × 108bps/g in the carbon efficiency of the cellular network as compared to conventional power allocation schemes such as the traditional water filling method and heuristic energy sharing and charge/discharge algorithms.

Study of deep transportation and plugging performance of deformable gel particles in porous media

Deformable gel particles(DGPs) possess the capability of deep profile control and flooding. However, the deep migration behavior and plugging mechanism along their path remain unclear. Breakage, an inevitable phenomenon during particle migration, significantly impacts the deep plugging effect. Due to the complexity of the process, few studies have been conducted on this subject. In this paper, we conducted DGP flow experiments using a physical model of a multi-point sandpack under various injection rates and particle sizes. Particle size and concentration tests were performed at each measurement point to investigate the transportation behavior of particles in the deep part of the reservoir. The residual resistance coefficient and concentration changes along the porous media were combined to analyze the plugging performance of DGPs. Furthermore, the particle breakage along their path was revealed by analyzing the changes in particle size along the way. A mathematical model of breakage and concentration changes along the path was established. The results showed that the passage after breakage is a significant migration behavior of particles in porous media. The particles were reduced to less than half of their initial size at the front of the porous media. Breakage is an essential reason for the continuous decreases in particle concentration, size, and residual resistance coefficient. However, the particles can remain in porous media after breakage and play a significant role in deep plugging. Higher injection rates or larger particle sizes resulted in faster breakage along the injection direction, higher degrees of breakage, and faster decreases in residual resistance coefficient along the path. These conditions also led to a weaker deep plugging ability. Smaller particles were more evenly retained along the path, but more particles flowed out of the porous media, resulting in a poor deep plugging effect. Topology optimization of chiral metamaterials with application to underwater sound insulation

Chiral metamaterials have been proven to possess many appealing mechanical phenomena,such as negative Poisson's ratio,high-impact resistance,and energy *** work extends the applications of chiral metamaterials to underwater sound *** chiral metamaterials with low acoustic impedance and proper stiffness are inversely designed using the topology optimization *** acoustic impedance enables the metamaterials to have a high and broadband sound transmission loss(STL),while proper stiffness guarantees its robust acoustic performance under a hydrostatic *** proof-of-concept demonstrations,two specimens are fabricated and tested in a water-filled impedance *** results show that,on average,over 95%incident sound energy can be isolated by the specimens in a broad frequency range from 1 k Hz to 5 k Hz,while the sound insulation performance keeps stable under a certain hydrostatic *** work may provide new insights for chiral metamaterials into the underwater applications with sound insulation.

Dear Editor,Severe fever with thrombocytopenia syndrome(SFTS),caused by SFTS virus(SFTSV)in the genus Banyangvirus of the family Phenuiviridae,is an emerging tick-borne viral zoonosis(Liu et al.2014).Typical clinical symptoms of SFTS include fever,headache,thrombocytopenia,and leukocytopenia(Yu et al.2011).Since SFTSV was identified in 2009,it has been found in more than 20 provinces in China and is closely related to strains from Japan and South Korea(Kim et al.2013;Takahashi et al.2014).

Artificial intelligence-assisted repair of peripheral nerve injury: a new research hotspot and associated challenges

Artificial intelligence can be indirectly applied to the repair of peripheral nerve ***,it can be used to analyze and process data regarding peripheral nerve injury and repair,while study findings on peripheral nerve injury and repair can provide valuable data to enrich artificial intelligence *** investigate advances in the use of artificial intelligence in the diagnosis,rehabilitation,and scientific examination of peripheral nerve injury,we used CiteSpace and VOSviewer software to analyze the relevant literature included in the Web of Science from 1994–*** identified the following research hotspots in peripheral nerve injury and repair:(1)diagnosis,classification,and prognostic assessment of peripheral nerve injury using neuroimaging and artificial intelligence techniques,such as corneal confocal microscopy and coherent anti-Stokes Raman spectroscopy;(2)motion control and rehabilitation following peripheral nerve injury using artificial neural networks and machine learning algorithms,such as wearable devices and assisted wheelchair systems;(3)improving the accuracy and effectiveness of peripheral nerve electrical stimulation therapy using artificial intelligence techniques combined with deep learning,such as implantable peripheral nerve interfaces;(4)the application of artificial intelligence technology to brain-machine interfaces for disabled patients and those with reduced mobility,enabling them to control devices such as networked hand prostheses;(5)artificial intelligence robots that can replace doctors in certain procedures during surgery or rehabilitation,thereby reducing surgical risk and complications,and facilitating postoperative *** artificial intelligence has shown many benefits and potential applications in peripheral nerve injury and repair,there are some limitations to this technology,such as the consequences of missing or imbalanced data,low data accuracy and reproducibility,and ethical issues(e.g.,privac
