Ectopic pregnancy and failed oocyte retrieval during in vitro fertilization stimulation:Two case reports

BACKGROUND Due to a slight rise in beta-human chorionic(β-h CG)levels that are undetectable,and vaginal bleeding that is similar to regular menstruation,ectopic pregnancy(EP)that occurs during the expected menstrual cycle prior to ovulation induction as part of in vitro fertilization(IVF)is likely to be undiagnosed.We present two cases of unexpected EP and emphasize the importance of theβ-h CG assay when an unexplained increase in progesterone is present prior to the triggering of ovulation during controlled ovarian stimulation(COS).CASE SUMMARY A 26-year-old woman with primary infertility and a 31-year-old woman with secondary infertility.Both patients sought IVF treatment due to fallopian tube obstruction and underwent COS using the gonadotropin-releasing-hormone(Gn RH)-antagonist protocol.In the late stage of COS,progesterone levels in both patients significantly increased,and luteinizing hormone levels decreased,followed by oocyte retrieval failure.A right salpingectomy was performed and tubal ectopic pregnancy was diagnosed by pathology in the first patient,and the second patients was diagnosed with a suspected EP abortion because herβ-h CG levels declined to 12.5 m IU/m L.After full recovery for 2 mo,the first patient entered a new IVF treatment cycle with a Gn RH-antagonist regimen and successfully achieved eight oocytes and three viable embryos.After 6 mo,the second patient received another COS treatment with a progestin-primed ovarian stimulation protocol and successfully achieved nine oocytes and five viable embryos.CONCLUSIONβ-h CG levels in the initial and midterm phases of COS must be considered in patients with unusual hormone dynamics.

Trophoblast-derived CXCL16 induces M2 macrophage polarization that in turn inactivates NK cells at the maternal–fetal interface

Decidual macrophages (dMΦ) are distinct from the conventional macrophages present in other tissues and express M2macrophage markers, but the molecular mechanisms of formation and the roles of M2 MΦ during pregnancy have not beencompletely elucidated. The crosstalk between decidual natural killer cells (dNK) and dMΦ plays an important role in themaintenance of maternal–fetal immune tolerance. Here, CXCL16 derived from first-trimester trophoblast cells induces thepolarization of human M2 macrophages. The M2 MΦ polarized by CXCL16 exhibit decreased interleukin-15 production, whichfacilitates the inactivation of NK cells. The cytotoxicity of NK cells is attenuated by the CXCL16-polarized M2 MΦ. The data shown inthe present study provide evidence to support the hypothesis that CXCL16 secreted by trophoblast cells is a key molecule involvedin decidual M2 MΦ polarization, which in turn regulates the killing ability of NK cells, thereby contributing to the homeostatic andimmune-tolerant milieu required for successful fetal development.

Role of saliva proteinase 3 in dental caries

Salivary analysis can be used to assess the severity of caries. Of the known salivary proteins, a paucity of information exists concerning the role of proteinase 3（PR3）, a serine protease of the chymotrypsin family, in dental caries. Whole, unstimulated saliva was collected from children with varying degrees of active caries and tested using a Human Protease Array Kit and an enzyme-linked immunosorbent assay.A significantly decreased concentration of salivary PR3 was noted with increasing severity of dental caries（P,0.01）; a positive correlation（r50.87; P,0.01; Pearson’s correlation analysis） was also observed between salivary p H and PR3 concentration. In an antibacterial test,a PR3 concentration of 250 ng？m L21 or higher significantly inhibited Streptococcus mutans UA159 growth after 12 h of incubation（P,0.05）. These studies indicate that PR3 is a salivary factor associated with the severity of dental caries, as suggested by the negative relationship between salivary PR3 concentration and the severity of caries as well as the susceptibility of S. mutans to PR3.

Decidual immune cells(DICs),including T-cells,regulatory T-cells,macrophages/dendritic cells,natural killer cells,and neutrophils,are resident at the maternal-fetal interface,and play vital roles in regulating trophoblast migration,decidual angiogenesis,immune tolerance,placentation,and decidualization during the early pregnancy.Extensive researches have revealed that these maternal DICs cooperated with each other,or with maternal decidual stromal cells,or with fetal-derived trophoblasts,and further formed a special maternal-fetal cross talk at the maternal-fetal interface,which was essential for the construction and maintenance of physiological pregnancy.Once aberrant cross talk and immune regulation arise,many pregnancy complications will inevitably occur,such as spontaneous abortion,intrauterine growth restriction(IUGR),preeclampsia(PE),and preterm birth.Here,we reviewed how critical immune cells are either enriched or excluded from the decidua,how their function is regulated within the decidua,and how they variously contribute to pregnancy success or failure.

Objective::The maternal-fetal interface undergoes dynamic changes to allow the fetus to grow and develop in the uterus.The interaction between decidualγδT cells and trophoblasts plays a pivotal role during successful pregnancy;however,their physiological functions in early-term human pregnancy are still not completely illustrated.This study was undertaken to illustrate the functional roles of CXCL16/CXCR6 to prevent pregnancy loss via the crosstalk between decidualγδT cells and HTR8/SVneo trophoblast cells.Methods::The percentile of CXCR6+γδT cells in the peripheral blood from normal female and recurrent spontaneous abortion(RSA)patients was analyzed by flow cytometry.The expression of CXCR6 was detected in decidual immune cells via flow cytometry,and the expression of CXCL16 was analyzed in HTR8/SVneo trophoblast cells and lentivirus(LV)-HTR8/SVneo trophoblast cells via enzyme-linked immunosorbent assay.Reverse transcriptase-polymerase chain reaction was used to verify the expression of the CXCL16 gene in LV-HTR8/SVneo trophoblast cells.Expression of granzyme B and cytokines and proliferation of decidualγδT cocultured with HTR8/SVneo trophoblast cells were analyzed by flow cytometry.Invasion of HTR8/SVneo trophoblast cells was assessed via Matrigel transwell assay.Adoptive transfer was induced in vivo further to illustrate that the normal expression of CXCL16/CXCR6 could prevent pregnancy loss.Results::The percentile of CXCR6+γδT cells in the peripheral blood from RSA patients was lower than normal pregnancies.The expression of CXCR6 was highest in the decidualγδT cells among decidual immune cells,and the expression of CXCL16 increased as the amount of HTR8/SVneo trophoblast cells increased.Expression of granzyme B in the decidualγδT cells was downregulated by cocultured with HTR8/SVneo cells dependent of CXCL16,and HTR8/SVneo trophoblast cells induced the Th2 cytokines production in the decidualγδT cells.Both the expression of CXCR6 in the decidualγδT cells and proliferation of the decidualγδT cells were promot

研究目的:求解双圆盘转子-密封系统的非线性振动特性和运动响应创新要点:采用有限元法(FEM)和拉格朗日方程求解双圆盘转子-密封系统,进而为研究多级转子系统的非线性振动问题提供有效方法。研究方法:基于有限元法(FEM)和拉格朗日方程,得到包含Muszynska非线性密封流体力和圆盘重力作用下的双圆盘转子-密封系统的运动方程。同时利用四阶龙格-库塔法求解系统动特性运动响应情况,利用分岔图、时间历程图、轴心轨迹图、庞加莱映射和幅值谱等分析图研究双圆盘转子-密封系统的非线性振动特性。重要结论:随着转速的增大,双圆盘转子-密封系统呈现丰富的非线性运动形式,包括周期性运动、多周期运动、准周期运动以及混沌运动。在右端圆盘不平衡质量小于34 kg、密封间隙范围为0.376 mm–0.54 mm、密封长度大于0.13 m或者密封压差高于0.104 MPa的情况下均有利于提高双圆盘转子-密封系统的稳定性。

Protection of tight junction between RPE cells with tissue factor targeting peptide

AIM：To investigate the effect of tissue factor targeting peptide（TF-TP）on retinal pigment epithelium（RPE）cells tight junctions.METHODS：Cell counting kit-8（CCK-8）was used to measure the proliferation of ARPE-19 cells.Expression of tight junction,ZO-1 in ARPE-19 cells was measured by Western blot and immunofluorescent staining.Western blot was also used to detect the expression of tissue factor（TF）.CEC Transmigration Assay was used to measure the migration of ARPE-19 cells.The transport of fluorescent markers [fluorescein isothiocyanate dextrans of 4,10,20（FD4,FD10,FD20） ]and the transepithelial electrical resistance（TEER）were used to measure in ARPE-19 cell RESULTS：CCK-8 assay showed that 5μmol/L TF-TP can inhibit ARPE-19 cells abnormally proliferation stimulated by lipopolysaccharide（LPS;P〈0.05）.LPS increased the transport of fluorescent markers（FD4,FD10,FD20）and decreased TEER levels in ARPE-19 cells,respectively,which were prevented by 5μmol/L TF-TP pretreatment（P〈0.05）. Furthermore,LPS significantly up-regulated the expression of TF and downregulated the expression of ZO-1（P〈0.05）in ARPE-19 cell which was inhibited by the TF-TP（P〈0.05）.In addition,TF-TP inhibited the abnormal migration induced by LPS in ARPE-19 cell（P〈0.05）.CONCLUSION：Our findings suggest that TF-TP suppressed proliferation and migration of ARPE-19 cells induced by LPS,and maintained the RPE tight junctions through inhibition of TF expression and increased expression of ZO-1.

AIM: To evaluate the protective effects of lipoic acid-niacin(N2 L) dimers against blue light(BL)-induced oxidative damage to human retinal pigment epithelium(hRPE) cells in vitro.METHODS: h RPE cells were divided into a control group(CG), a BL group, an N2 L plus BL irradiation group, an α-lipoic acid(ALA) plus BL group, an ALA-only group, and an N2 L-only group. hRPE cellular viability was detected by performing 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium(MTT) bromide assays, and apoptosis was evaluated by annexin-V-PE/7-AAD staining followed by flow cytometry. Ultrastructural changes in subcellular organelles were observed by transmission electron microscopy. Reactive oxygen species formation was assayed by flow cytometry. The expression levels of the apoptosis-related proteins BCL-2 associated X protein(BAX), B-cell leukmia/lymphoma 2(BCL-2), and caspase-3 were quantified by Western blot analysis.RESULTS: BL exposure with a light density of 4±0.5 mW/cm2 exceeding 6 h caused hRPE toxicity, whereas treatment with a high dose of N2 L(100 mol/L) or ALA(150 mol/L) maintained cell viability at control levels. BL exposure caused vacuole-like degeneration, mitochondrial swelling, and reduced microvillus formation;however, a high dose of N2 L or ALA maintained the ultrastructure of hRPE cells and their organelles. High doses of N2 L and ALA also protected hRPE cells from BL-induced apoptosis, which was confirmed by Western blot analysis: BCL-2 expression significantly increased, while BAX and caspase-3 expression slightly decreased compared to the CG.CONCLUSION: High-dose N2 L treatment(>100 mol/L) can reduce oxidative damage in degenerating hRPE cells exposed to BL with an efficacy similar to ALA.

Botulinum toxin type A in treating early-stage patients with small-angle acute acquired comitant esotropia

AIM:To investigate botulinum toxin A(BTXA)efficacy on small-angle(≤25Δ)acute acquired concomitant esotropia(AACE)in early-stage patients.METHODS:The electronic medical record data of AACE patients during March 2019 and June 2023 were collected in this retrospective and hospital-based cohort study.A total of 72 small-angle AACE patients received BTXA extraocular muscle injection.Patients were grouped by onset-to-treatment time(Group A:≤6mo,Group B:>6mo).Deviation of esotropia,eye alignment and stereopsis were analyzed at the period of pre/post-injection(1wk,1,3,and 6mo).Orthophoria rate at 6mo(horizontal deviation0.05)between two groups except onset-to-treatment time(2mo vs 11mo,P0.05);while in 3 and 6mo Group A was significantly smaller than group B(all P6mo)may reduce BTXA efficacy.Early BTXA intervention benefits long-term eye alignment and stereopsis recovery.
