One-year follow-up study of Helicobacter pylori eradication rate with ^(13)C-urea breath test after 3-d and 7-d rabeprazole-based triple therapy

AIM: To investigate the long-term role of a 3-d rabeprazole-based triple therapy in patients with Helicobacter pylori (H pylori)-infected active peptic ulcers. METHODS: We prospectively studied 115 consecutive patients with H pylori-infected active peptic ulcers. H pylori infection was confirmed if any two of H pylori DNA, histology, and rapid urease test were positive. Patients were assigned to either an open-labeled 3-d course of oral amoxicillin 1 000 mg b.i.d., clarithromycin 500 mg b.i.d., and rabeprazole 20 mg b.i.d., or 7-d course of oral amoxicillin 1 000 mg b.i.d., clarithromycin 500 mg b.i.d., and rabeprazole 20 mg b.i.d. Subsequently, all patients received oral rabeprazole 20 mg once daily until the 8th wk. Three months after therapy, all patients were followed-up endoscopically for the peptic ulcer, H pylori DNA, histology, and rapid urease test. One year after therapy, H pylori infection was tested using the 13C-urea breath test. RESULTS: The ulcer healing rates 3 mo after therapy were 81.0% vs 75.4% for the 3-d and 7-d groups [intention-to-treat (ITT) analysis, P=0.47] respectively, and 90.4% vs 89.6% for the 3-d and 7-d groups [per-protocol (PP) analysis, P=0.89] respectively. The eradication rates 3 mo after therapy were 75.9% vs 73.7% for the 3-d and 7-d groups (ITT, P=0.79) respectively, and 84.6% vs 87.5% for the 3-d and 7-d groups (PP, P=0.68) respectively. One year after therapy, seventy-five patients returned to receive the 13C-urea breath test, and the eradication rates were 78.4% vs 81.6% in 3-d and 7-d groups (PP, P=0.73) respectively. CONCLUSION: Our study showed the eradication rates against H pylori infection 3 and 12 mo after triple therapy were not different between the 3-d and 7-d rabeprazole-based groups. Therefore, the 3-d rabeprazole-based triple therapy may be an alternative treatment for peptic ulcers with H pylori infection.

Hepatocellular carcinoma(HCC)is the fifth most common malignancy and the second leading cause of cancer mortality *** cornerstone to improving the prognosis of HCC patients has been the control of loco-regional disease progression and the lesser toxicities of local *** radiotherapy has not been considered a preferred treatment modality for HCC,charged particle therapy(CPT),including proton beam therapy(PBT)and carbon ion radiotherapy(CIRT),possesses advantages(for example,it allows ablative radiation doses to be applied to tumors but simultaneously spares the normal liver parenchyma from radiation)and has emerged as an alternative treatment option for *** the technological advancements in CPT,various radiation dosages of CPT have been used for HCC treatment via ***,the efficacy and safety of the evolving dosages remain *** assess the association between locoregional control of HCC and the dose and regimen of CPT,we provide a brief overview of selected literature on dose regimens from conventional to hypofractionated short-course CPT in the treatment of HCC and the subsequent determinants of clinical ***,CPT provides a better local control rate compared with photon beam therapy,ranging from 80%to 96%,and a 3-year overall survival ranging from 50%to 75%,and it results in rare grade 3 toxicities of the late gastrointestinal tract(including radiation-induced liver disease).Regarding CPT for the treatment of locoregional HCC,conventional CPT is preferred to treat central tumors of HCC to avoid late toxicities of the biliary *** contrast,the hypo-fractionation regimen of CPT is suggested for treatment of larger-sized tumors of HCC to overcome potential radio-resistance.

为研究香港地区中学文凭考试(简称文凭试)等级预测结果,以过往文凭试结果作为依据,通过标准化程序,对2020—2023年共计146180名考生根据其校内评核成绩进行文凭试等级预测;并采用问卷调查法,基于各参与学校组织架构、教师的工作经验、公开考试阅卷经验及专业发展活动等,运用多层线性模型探讨学校属性与校内评核的关系。结果显示:中文、英文、数学、通识教育4门核心科目等级预测结果具有较高的准确度,其中英文预测结果最佳,其余3科预测结果类似;选修科目则因每年选考人数少导致文凭试成绩变化大,进而影响预测结果;多层线性模型分析显示,不同学校属性对于不同科目的校内评核具有不同的预测效果。

Porous active core-shell carbon material with excellent synergistic effect has been regarded as a prospective material for ***,we report an integrated method for the facile synthesis of carbide-derived carbon(CDC)encapsulated with porous N-doped carbon(CDC@NC)towards highperformance ***(PDA)as nitrogen and carbon sources was simply coated on SiC nanospheres to form SiC@PDA,which was then directly transformed into CDC@NC via a onestep molten salt electro-etching/in-situ doping *** synthesized CDC@NC with hierarchically porous structure has a high specific surface area of 1191 m^(2) g^(-1).The CDC core and NC shell are typical amorphous carbon and more ordered N-doped carbon,*** from its unique dual porous structures,the CDC@NC demonstrates high specific capacitances of 255 and 193 F g^(-1) at 0.5 and20 A g^(-1),*** reaction mechanism of the electro-etching/in-situ doping process has also been investigated through experimental characterizations and theoretical density functional theory *** is suggested that the molten salt electro-etching/in-situ doping strategy is promising for the synthesis of active core-shell porous carbon materials with synergistic properties for supercapacitors without the need for additional doping/activation processes.

From Pingcheng to Luoyang—Substantiation of the climatic cause for capital relocation of the Beiwei Dynasty

Emperor Tuoba Hong 1) of Beiwei relocated the capital from Pingcheng to Luoyang approximately 1500 years ago. Most historians speculate that his motivation was based on considerations of politics, economics, military, history, and Chinese culture. However, Sima Guang, a famous historian during the Song Dynasty, claimed 1000 years ago that cold climate was the cause of this historical event. In order to assess the credibility of Sima Guang’s claim, this research examines related ancient documents and the recent frost data for a comparative study. It is found that the mean minimum temperature in Pingcheng during 479AD～509AD is estimated to be lower than present minimum temperature by 2.48 ℃ .

By expanding the Gibbs free energy of a ternary system into a Taylor series with respect to the composition,a thermodynamic criterion of spinodal decomposition is conducted as G_(xx)(δx)2+2G_(xx)δxδy+G_(yy)(δy)~20,or G_(xx)>0, G_(xy)~2-G_(xx)G_(yy)>0,as well as G_(xx)=0,are all considered as unstable decomposition restricted by the direction of composition fluctuation δx and δ*** phenomenon of turning round in composition during spinodal decomposition is attempted to be explained from above point of *** computer calculated ranges of unstable decomposition according to the criterion of spinodal decomposition on Cu-Ni-Fe ternary system is consistent with the experimental re- sults obtained from literatures.

本文基于星间加速度法开展了插值公式、相关系数和采样间隔对GRACE Follow-On星间加速度精度影响的研究.模拟结果表明:1)适当增加数值微分公式的插值点数可有效提高插值精度.基于9点Newton插值公式,星间加速度的插值误差为4.401×10-13 m.s-2,分别基于7点、5点和3点插值公式,插值误差增加了1.192倍、6.912倍和274.029倍.2)适当增大相关系数可有效降低星间加速度的误差.基于相关系数0.99,星间加速度方差为3.777×10-24 m2.s-4,分别基于相关系数0.90、0.70、0.50和0.00,方差增加了9.780倍、22.404倍、26.217倍和26.820倍.3)随着采样间隔增大,星间加速度方差逐渐降低,但卫星观测值的空间分辨率也同时降低,因此合理选取采样间隔有利于地球重力场精度的提高.4)基于9点Newton插值公式、相关系数(K波段测量系统星间距离和星间速度0.85、GPS轨道位置和轨道速度0.95、星载加速度计非保守力0.90)和采样间隔10s,利用预处理共轭梯度迭代法,精确和快速反演了120阶GRACE Follow-On地球重力场,在120阶处累计大地水准面精度为4.602×10-4 m.

Hypothesis of human penile anatomy,erection hemodynamics and their clinical applications

Aim： To summarize recent advances in human penile anatomy, hemodynamics and their clinical applications. Methods： Using dissecting, light, scanning and transmission electron microscopy the fibroskeleton structure, penile venous vasculature, the relationship of the architecture between the skeletal and smooth muscles, and erection hemodynamics were studied on human cadaveric penises and clinical patients over a period of 10 years. Results： The tunica albuginea of the corpora cavernosa is a bi-layered structure with inner circular and outer longitudinal collagen bundles. Although there is no bone in the human glans, a strong equivalent distal ligament acts as a trunk of the glans penis. A guaranteed method of local anesthesia for penile surgeries and a tunical surgery was developed accordingly. On the venous vasculature it is elucidated that a deep dorsal vein, a couple of cavernosal veins and two pairs of para-arterial veins are located between the Buck＇s fascia and the tunica albuginea. Furthermore, a hemodynamic study suggests that a fully rigid erection may depend upon the drainage veins as well, rather than just the intracavemosal smooth muscle. It is believed that penile venous surgery deserves another look, and that it may be meaningful if thoroughly and carefully performed. Accordingly, a penile venous surgery was developed. Conclusion： Using this new insight into penile anatomy and physiology, exact penile curvature correction, refined penile implants and promising penile venous surgery, as well as a venous patch, for treating Peyronie＇s deformity might be performed under pure local anesthesia on an outpatient basis. （Asian J Androl 2006 Mar; 8： 225-234）
