Shuxuening Injection Inhibits Apoptosis and Reduces Myocardial Ischemia-Reperfusion Injury in Rats through PI3K/AKT Pathway

Objective: To investigate the main components and potential mechanism of Shuxuening Injection(SXNI) in the treatment of myocardial ischemia-reperfusion injury(MIRI) through network pharmacology and in vivo research. Methods: The Traditional Chinese Medicine Systems Pharmacology(TCMSP) and Pharm Mapper databases were used to extract and evaluate the effective components of Ginkgo biloba leaves, the main component of SXNI. The Online Mendelian Inheritance in Man(OMIM) and Gene Cards databases were searched for disease targets and obtain the drug target and disease target intersections. The active ingredient-target network was built using Cytoscape 3.9.1 software. The STRING database, Metascape online platform, and R language were used to obtain the key targets and signaling pathways of the anti-MIRI effects of SXNI. In order to verify the therapeutic effect of different concentrations of SXNI on MIRI in rats, 60 rats were first divided into 5 groups according to random number table method: the sham operation group, the model group, SXNI low-dose(3.68 mg/kg), medium-dose(7.35 mg/kg), and high-dose(14.7 mg/kg) groups, with 12 rats in each group. Then, another 60 rats were randomly divided into 5 groups: the sham operation group, the model group, SXNI group(14.7 mg/kg), SXNI+LY294002 group,and LY294002 group, with 12 rats in each group. The drug was then administered intraperitoneally at body weight for 14 days. The main biological processes were validated using in vivo testing. Evans blue/triphenyltetrazolium chloride(TTC) double staining, hematoxylin-eosin(HE) staining, terminal deoxynucleotidyl transferase d UTP nick end labeling(TUNEL) assay, enzyme-linked immunosorbent assay(EliSA), and Western blot analysis were used to investigate the efficacy and mechanism of SXNI in MIRI rats. 院前急救是指对各种患者(创伤、中毒、危急生命的急症、灾难性事故等)进行的医院前紧急治疗,包括在发病现场以及转运入院的途中对患者开展有效、安全的医疗救治以及护理,尽最大可能地稳定患者生命体征,为患者院内救治争取时间,降低患者伤残率及死亡率。院前急救是急诊医学的重要组成部分,院前对患者病情做出初步的判断及处置,能为院内进一步治疗打下基础,对挽救患者生命具有重要意义。需要接受院前急救的患者多为危急重症患者,面临生命危险,现场需通过多种治疗手段对此类患者进行生命支持,以维持其生命体征,争取院内治疗时机。气管插管是高级生命支持中气道管理最为常用的治疗措施。该措施不仅能为患者提供足够氧气,保证患者呼吸畅通,也可避免治疗过程中误吸的发生。但是,并非所有需要建立高级气道的患者均适用气管插管,部分患者存在气管插管禁忌证,故而在进行气管插管前需了解患者的基本情况,方可确定是否使用此种气道管理方式对患者进行救治。气管插管越来越多的被用于院前气道管理中,其应用效果较好。本文对气管插管在院前急救中的应用适应证、置入方法及效果研究进展进行综述,以期指导临床应用。

鲁西南地区13家医院213株耐碳青霉烯类肠杆菌目细菌流行病学及耐药基因特征

目的了解鲁西南地区耐碳青霉烯类肠杆菌目细菌(CRE)耐药基因及流行病学特点。方法收集鲁西南地区13家医院2020年1月—2022年7月住院患者标本中分离到的CRE菌株,采用VITEK-MS质谱鉴定仪、VITEK-2 Compact全自动药敏仪,改良碳青霉烯灭活试验(mCIM)及碳青霉烯酶免疫层析法,最后筛选出213株菌株,进行聚合酶链反应(PCR)法检测耐药基因,并对10株携带双基因型菌株进行分析,对医院83株CRE菌株进行方法学之间的比较。结果 213株CRE主要分布科室在重症医学科、急诊监护室等;标本主要来源痰液、尿液、血液等;耐药率统计显示肺炎克雷伯菌耐药率高于大肠埃希菌。耐药基因分布特点:主要以bla_(KPC-2)、bla_(NDM-5)基因为主,其次为bla_(NDM-1)基因,10株双基因型CRE菌进行凝胶电泳分析,结果显示6株大肠埃希菌产不同bla_(NDM)基因同时携带mcr-1耐药基因,1株阴沟肠杆菌产bla_(NDM-1)同时携带mcr-9基因。对方法学的比较显示:mCIM+eCIM、免疫层析方法检测耐药基因型,KPC基因型两种方法无差异,对NDM基因型前者敏感度更高。结论鲁西南地区CRE菌分离率最高的是产bla_(KPC-2)基因的肺炎克雷伯菌,其次是产bla_(NDM-5)基因的大肠埃希菌及产bla_(NDM-1)型阴沟肠杆菌。值得关注的是双基因型及少见基因型的检出,应引起感控部门的注意,以防耐药菌的传播和流行。

A regularization algorithm of dynamic light scattering for estimating the particle size distribution of dual-substance mixture in water

Dynamic light scattering(DLS)is a nondestructive,well-established technique for the size characterization of proteins,nanoparticles,polymers,and colloidal ***,current DLS techniques are only applied to particle groups of single composition due to the limitation of their inversion *** this study,we propose a particle size distribution inversion algorithm based on the Tikhnonov regularization method that can be applied to the dual-substance particle *** algorithm retrieves the particle size distributions of two substances,respectively,by taking advantage of their refractive index *** simulation results reveal that the algorithm has excellent accuracy and stability when the scattering angle is 30°.Instead of the original identity matrix,the first-order difference matrix and second-order difference matrix are used as the regular matrix when utilizing the Tikhnonov algorithm,which obviously improves the anti-interference,accuracy,and stability of the ***,the inversion of particle size distribution is carried out at a 0.01%–1%noise level,which shows that the algorithm has an available antinoise ***,experimental particle size measurements for a mixture of polystyrene beads and toner particles demonstrate that the proposed algorithm is superior to the traditional Tikhnonov algorithm in applicability and accuracy.

BACKGROUND Ankylosing spondylitis(AS)is a chronic rheumatic disease that primarily affects the spine and the sacroiliac and peripheral ***-onset AS(JoAS)patients will likely present with peripheral joint *** flexion contracture(KFC)and hip flexion contracture(HFC)are common in these patients due to subchondral bone *** Ilizarov technique is the most commonly used technique for treating ***,its use to treat JoAS-associated KFC has not been *** SUMMARY This report presents a case study of a 31-year-old male patient with a squatting gait due to severe bilateral KFC and *** patient had a normal walking pattern until the age of eight,after which he experienced knee and hip pain,leading to the gradual development of KFC and *** patient’s primary complaint was an inability to walk *** patient was diagnosed with JoAS and under-went hip dissection and release,limited soft tissue release of the hamstring,and gradual traction using the Ilizarov ***,the patient was able to walk *** The incidence of squatting gait due to KFC in individuals diagnosed with JoAS was *** the Ilizarov technique has proven to be a secure and effective method for managing KFC in JoAS *** the Ilizarov technique cannot substitute for total knee arthroplasty(TKA),its application can delay the need for primary TKA in JoAS patients and alleviate the intricacy and potential complications associated with the procedure.

本研究基于代谢组学结合生物信息学的策略分析热毒宁注射液与青霉素注射液联用引发类过敏反应(pseudo-allergic reactions,PARs)的潜在致敏原及作用机制。所有动物实验操作和福利均遵循河南中医药大学第一附属实验动物伦理与动物福利委员会要求(批号:YFYDW2020002)。基于UPLC-Q-TOF/MS技术结合UNIFI软件在热毒宁与青霉素混合注射液中共鉴定得到21种化合物。进一步基于分子对接技术筛选出其10种与类过敏原受体MrgprX2激动剂位点具有强烈结合活性的潜在致敏原。采用UPLC-Q-TOF/MS技术进行代谢组学分析发现花生四烯酸、磷脂酰胆碱、磷脂酰丝氨酸、前列腺素类以及白三烯类等34个内源性代谢物为热毒宁注射液与青霉素注射液联用引发PARs的差异代谢物。通过对“潜在致敏原-靶标-差异代谢物”交互网络分析获得二者联用时药液中的绿原酸类(绿原酸、新绿原酸、隐绿原酸、异绿原酸A等)以及β-内酰胺类致敏原可能主要通过调控PLD1、PLA2G12A和CYP1A1这三种上游信号靶标,主要激活花生四烯酸代谢通路,促使肥大细胞的脱颗粒,释放下游的内源性炎性介质,继而诱发PARs。

An improved deep dilated convolutional neural network for seismic facies interpretation

With the successful application and breakthrough of deep learning technology in image segmentation,there has been continuous development in the field of seismic facies interpretation using convolutional neural networks. These intelligent and automated methods significantly reduce manual labor, particularly in the laborious task of manually labeling seismic facies. However, the extensive demand for training data imposes limitations on their wider application. To overcome this challenge, we adopt the UNet architecture as the foundational network structure for seismic facies classification, which has demonstrated effective segmentation results even with small-sample training data. Additionally, we integrate spatial pyramid pooling and dilated convolution modules into the network architecture to enhance the perception of spatial information across a broader range. The seismic facies classification test on the public data from the F3 block verifies the superior performance of our proposed improved network structure in delineating seismic facies boundaries. Comparative analysis against the traditional UNet model reveals that our method achieves more accurate predictive classification results, as evidenced by various evaluation metrics for image segmentation. Obviously, the classification accuracy reaches an impressive 96%. Furthermore, the results of seismic facies classification in the seismic slice dimension provide further confirmation of the superior performance of our proposed method, which accurately defines the range of different seismic facies. This approach holds significant potential for analyzing geological patterns and extracting valuable depositional information.
