Esophageal adenocarcinoma is the most rapidly growing cancer in America.Although the prognosis after diagnosis is unfavorable,the chance of a successful outcome increases tremendously if detected early while the lesion is still dysplastic.Unfortunately,the present standard-of-care,endoscopic surveillance,has major limitations,since dysplasia is invisible,often focal,and systematic biopsies typically sample less than one percent of the esophageal lining and therefore easily miss malignancies.To solve this problem we developed a multispectral light scattering endoscopic imaging system.It surveys the entire esophageal lining and accurately detects subcellular dysplastic changes.The system combines light scattering spectroscopy,which detects and identifies invisible dysplastic sites by analyzing light scattered from epithelial cells,with rapid scanning of the entire esophageal lining using a collimated broadband light beam delivered by an endoscopically compatible fiber optic probe.Here we report the results of the first comprehensive multispectral imaging study,conducted as part of routine endoscopic procedures performed on patients with suspected dysplasia.In a double-blind study that characterized the system’s ability to serve as a screening tool,55 out of 57 patients were diagnosed correctly.In addition,a smaller double-blind comparison of the multispectral data in 24 patients with subsequent pathology at locations where 411 biopsies were collected yielded an accuracy of 90%in detecting individual locations of dysplasia,demonstrating the capability of this method to serve as a guide for biopsy.

Probiotic effects on intestinal fermentation patterns in patients with irritable bowel syndrome

AIM: To determine whether Lactobacillus casei strain Shirota (Yakult ) can alter small intestinal bacterial overgrowth (SIBO), as tested by the lactulose breath test, and whether this is associated with changes in symptoms in irritable bowel syndrome (IBS). METHODS: 18 patients with IBS (Rome Ⅱ criteria), who showed an early rise in breath hydrogen with lactulose (ERBHAL), consumed 65 mL of Yakult daily for 6 wk. Lactulose breath test was repeated at the end of the treatment period. Symptoms were recorded daily using a 10 cm visual analogue scale. RESULTS: 14 patients completed the study, 9 (64%) had reversal of ERBHAL, with the median time of f irst rise in breath hydrogen increasing from 45 to 75 min (P = 0.03). There was no signifi cant improvement in the symptom score with probiotic therapy, except for wind (P=0.04). Patients commencing with at least moderate symptoms and who no longer had ERBHAL at the end of treatment, showed improvement in the overall symptoms scores [median fi nal score 5.3 (IQR3.9-5.9), 55% reduction; n=6] to a greater extent than those who had had persisting ERBHAL [final score 6.9 (5.0-7.0), 12% reduction; n = 5; P = 0.18]. COnCLUSIOn: Yakult is effective in altering fermentation patterns in the small bowel, consistent with reducing SIBO. The loss of ERBHAL was associated with reduced symptoms. The true interpretation of these fi ndings awaits a randomised, controlled trial.

“病态科学”一文发表在1989年美国物理学会的刊物《今日物理》第10期上.本刊特将此文翻译摘登,以飨国内的读者.该文自发表后,已引起不少反响,不少人认为朗缪提出了科学研究中一个重要的社会现象,而且还联系了当前科学研究中的实际,但也有人担心这会影响科学上不同学派之间的正常争论.我们认为,必须将科学研究过程中不可避免的错误和病态科学区分开来.不能将一切错误都归结为病态科学.但另一方面,从大量科学史中的例子,包括我国科学发展中出现的事例来看,病态科学提出的确是一类真实的并且产生影响的社会现象.正视它,研究它,并从中吸取必要的教训,正是为了繁荣我国的科学事业.而引起国内读者对这一问题的注意,正是编者发表此文的目的.

Dear Editor,The activating receptor DnAX accessory molecule-1(DnAM-1)plays an important role in T and natural killer(nK)cell-mediated cytotoxicity via the interaction with its ligands poliovirus receptor(PVR,CD155)and nectin-2(CD112).Compared with peripheral blood nK cells,tumor-infiltrating nK cells show reduced expression of DnAM-1 across several solid tumors,including ovarian and breast cancer resulting in impaired nK cell function.Early studies reported an inverse correlation between DnAM-1 expression and its ligands in leukemic blasts and ovarian cancer[1].

Ionophores consist of molecules which surround and carry positive metal ions and other ions through biological membranes. One classe of ionophores which we have been developing contains dipeptides which are encouraged to become part of a ring because of possible hydrogen bond formation between the 2-hydroxy on the phenyl group and carboxyl group (COOH) of the final amide proline. Formation of a ring should increase the complexation ability of the ionophore. We report that the synthesis of n-(2-hydroxyl-1-phenoxyacetyl) prolyproline(1), a novel ionophore is prepared from activated 2-acetoxy phenoxyacetic acid (3a) and the appropriate dipeptide ester using coupling methods such as dicyclohexyl carbodiimide with hydroxyben-ztriazole or carbonyl diimidazole.

目的构建大鼠二硫化碳染毒学习损伤的动物模型,观察其海马组织的病理学改变,并探讨其中枢神经损伤的原因与机制。方法 80只雄性Wistar大鼠随机分为正常对照组,低、中、高剂量二硫化碳组,每组20只,将二硫化碳溶于玉米油中,经口给予大鼠,染毒剂量分别为200、400和600 mg/kg,正常对照组给予等体积的溶剂,连续20 d。染毒第16天起,采用Morris水迷宫检测动物的学习能力。实验结束后,每组随机选取10只经心脏灌注固定,制作大脑冰冻切片,采用nissl染色及免疫组织化学法分别观察大脑海马神经元的形态学及神经元数量变化。其余10只海马进行一氧化氮(nO)含量和一氧化氮合酶(nOS)活性检测。结果二硫化碳染毒导致中枢神经系统损伤,水迷宫实验中的游泳总路程和逃避潜伏期呈现剂量依赖性延长(PnO含量和nOS活性明显增高。结论二硫化碳暴露可导致神经系统功能障碍和海马神经元损伤,并可能与其影响nOS活性,使脑内nO代谢紊乱,发生氧化应激有关。

【目的】建立二硫化碳学习记忆损伤的大鼠模型,研究海马区神经元的改变及细胞凋亡在其中的作用。【材料和方法】80只雄性Wistar大鼠随机分为正常对照组、低、中、高剂量二硫化碳组（n=20）,剂量分别为200 mg/kg、400 mg/kg和600 mg/kg。经口染毒,连续20d,染毒第16天起,采用Morris水迷宫检测动物的学习记忆能力。实验结束后,每组随机选取10只大鼠经心脏灌注固定,制作大脑冰冻切片,免疫组织化学方法观察大脑海马神经元数量变化,TUnEL检测海马区细胞凋亡情况。其余10只大鼠海马匀浆后Western Blot检测凋亡相关蛋白Caspase-3、Bax含量的改变。【结果】二硫化碳染毒导致动物中枢神经系统损伤,水迷宫实验中的游泳总路程和逃避潜伏期呈现剂量依赖性延长（p＜0.05）,穿越平台次数减少（p＜0.05）;海马CA3和DG区神经元特异标志核蛋白（neun）抗体免疫染色结果显示,与正常对照组相比,中、高剂量二硫化碳组海马CA3区神经元细胞数目明显减少（p＜0.01）;TUnEL染色结果显示低、中、高剂量二硫化碳组CA1、CA3和DG区均可见阳性细胞且随剂量增加而逐渐增多;中、高剂量二硫化碳组海马凋亡相关蛋白Caspase-3、Bax较对照组升高（p＜0.05）。【结论】二硫化碳暴露可引起学习记忆的损伤,其机制可能与细胞凋亡增加,导致海马神经元减少有关。

We present a case of a patient with progressive unilateral sensorineural hearing loss and tinnitus with internal auditory canal enhancement on magnetic resonance imaging (MRI) secondary to isolated cochlear neuritis from varicella reactivation. MRI following antiviral treatment showed resolution of enhancement. Varicella reactivation is commonly seen in the form of Ramsay Hunt syndrome, which is known to produce abnormal MRI enhancement from facial and vestibulocochlear neuritis; however, its characteristic clinical signs aid the diagnosis. This case is unique in that the only manifestation of varicella infection was unilateral hearing loss. This case outlines the importance of maintaining a broad differential diagnosis in the evaluation of unilateral hearing loss as well as recognizing the limited specificity of MRI.
