BACKGROUND There is currently a shortage of accurate,efficient,and precise predictive instruments for rectal neuroendocrine neoplasms(NENs).AIM To develop a predictive model for individuals with rectal NENs(R-NENs)using data from a large *** Data from patients with primary R-NENs were retrospectively collected from 17 large-scale referral medical centers in *** forest and Cox proportional hazard models were used to identify the risk factors for overall survival and progression-free survival,and two nomograms were *** A total of 1408 patients with R-NENs were *** grade,T stage,tumor size,age,and a prognostic nutritional index were important risk factors for *** GATIS score was calculated based on these five *** overall survival prediction,the respective C-indexes in the training set were 0.915(95%confidence interval:0.866-0.964)for overall survival prediction and 0.908(95%confidence interval:0.872-0.944)for progression-free survival *** to decision curve analysis,net benefit of the GATIS score was higher than that of a single *** time-dependent area under the receiver operating characteristic curve showed that the predictive power of the GATIS score was higher than that of the TNM stage and pathological grade at all time *** The GATIS score had a good predictive effect on the prognosis of patients with R-NENs,with efficacy superior to that of the World Health Organization grade and TNM stage.

For the applications of the frequency comb in microresonators,it is essential to obtain a fully frequency-stabilized microcomb laser *** this study,we present a system for generating a fully atom-referenced stabilized soliton *** pump light around 1560.48 nm is locked to an ultra-low-expansion(ULE)*** pump light is then frequency-doubled and referenced to the atomic transition *** repetition rate of the soliton microcomb is injection-locked to an atomic-clockstabilized radio frequency(RF)source,leading to mHz stabilization at 1 *** a result,all comb lines have been frequencystabilized based on the atomic reference and the ULE cavity,achieving a very high precision of approximately 18 Hz at 1 s,corresponding to the frequency stability of 9.5×10^(-14).Our approach provides a fully stabilized microcomb experiment scheme with no requirement of f-2f technique,which could be easily implemented and generalized to various photonic platforms,thus paving the way towards the ultraprecise optical sources for high precision spectroscopy.

Early insulin therapy is capable to achieve glycemic control and restoreβ-cell function in newly diagnosed type 2 diabetes(T2D),but its effect on cardiovascular outcomes in these patients remains *** this nationwide real-world study,we analyzed electronic health record data from 19 medical centers across China between 1 January 2000,and 26 May *** included 5424 eligible patients(mean age 56 years,2176 women/3248 men)who were diagnosed T2D within six months and did not have prior cardiovascular *** Cox regression models were used to estimate the associations of early insulin therapy(defined as the first-line therapy for at least two weeks in newly diagnosed T2D patients)with the incidence of major cardiovascular events including coronary heart disease(CHD),stroke,and hospitalization for heart failure(HF).During 17,158 persons years of observation,we documented 834 incident CHD cases,719 stroke cases,and 230 hospitalized cases for *** diagnosed T2D patients who received early insulin therapy,compared with those who did not receive such treatment,had 31%lower risk of incident stroke,and 28%lower risk of hospitalization for *** significant difference in the risk of CHD was *** found similar results when repeating the aforesaid analysis in a propensity-score matched population of 4578 patients and with inverse probability of treatment weighting *** findings suggest that early insulin therapy in newly diagnosed T2D may have cardiovascular benefits by reducing the risk of incident stroke and hospitalization for HF.

Background Pediatric antineutrophil cytoplasmic antibody-associated vasculitis(AAV)is a life-threatening systemic vasculitis featured by liability to renal ***,there are few studies on the risk factors and predictive models for renal outcomes of AAV in *** Data from 179 AAV children in multiple centers between January 2012 and March 2020 were collected *** risk factors and predictive model of end-stage renal disease(ESRD)in AAV were *** Renal involvement was the most typical manifestation(95.5%),and the crescent was the predominant pathological lesion(84.9%).The estimated glomerular filtration rate(eGFR)was evaluated in 114 patients,of whom 59.6%developed ESRD,and the median time to ESRD was 3.20 *** eGFR[P=0.006,odds ratio(OR)=0.955,95%confidence interval(CI)=0.924–0.987]and the percentages of global glomerulosclerosis(pGGS;P=0.018,OR=1.060,95%CI=1.010–1.112)were independent risk factors for ESRD of renal *** on the pGGS and eGFR at renal biopsy,we developed three risk grades of ESRD and one predictive *** Kaplan‒Meier curve indicated that renal outcomes were significantly different in different risk grades(Pline,the predictive model had higher accuracy(0.86 versus 0.58,P<0.001)and a lower coefficient of variation(0.07 versus 0.92)in external *** Renal involvement is the most common manifestation of pediatric AAV in China,of which more than half deteriorates into *** predictive model based on eGFR at renal biopsy and the pGGS may be stable and accurate in speculating the risk of ESRD in AAV children.

Objective:This cross-sectional study aimed to determine the epidemiology of olfactory and gustatory dysfunctions related to COVID-19 in ***:This study was conducted by 45 tertiary Grade-A hospitals in *** and offline questionnaire data were obtained from patients infected with COVID-19 between December 28,2022,and February 21,*** collected information included basic demographics,medical history,smoking and drinking history,vaccination history,changes in olfactory and gustatory functions before and after infection,and other postinfection symptoms,as well as the duration and improvement status of olfactory and gustatory ***:Complete questionnaires were obtained from 35,566 *** overall incidence of olfactory and taste dysfunction was 67.75%.Being female or being a cigarette smoker increased the likelihood of developing olfactory and taste *** received four doses of the vaccine or having good oral health or being a alcohol drinker decreased the risk of such *** infection,the average olfactory and taste VAS scores were 8.41 and 8.51,respectively;after infection,they decreased to 3.69 and 4.29 and recovered to 5.83 and 6.55 by the time of the *** median duration of dysosmia and dysgeusia was 15 and 12 days,respectively,with 0.5%of patients having symptoms lasting for more than 28 *** overall self-reported improvement rate was 59.16%.Recovery was higher in males,never smokers,those who received two or three vaccine doses,and those that had never experienced dental health issues,or chronic accompanying ***:The incidence of dysosmia and dysgeusia following infection with the SARS-CoV-2 virus is high in *** and prognosis are influenced by several factors,including sex,SARS-CoV-2 vaccination,history of head-facial trauma,nasal and oral health status,smoking and drinking history,and the persistence of accompanying symptoms.

目的了解我国农村老年人成功老龄化现状，分析其影响因素。方法选取2018年中国健康与养老追踪调查(CHARLS)数据中60岁及以上农村老年人作为研究对象，判定同时满足“无重大慢性疾病”、“无抑郁”、“认知和躯体功能良好”、“积极的社会参与”这四个维度为成功老龄化，继而基于社会生态系统理论，采用多因素logistic回归分析农村老年人成功老龄化的影响因素。结果我国农村老年人成功老龄化率为6.79%。多因素logistic回归结果显示，个人特征层次中的自评健康状况(OR=0.426,95%CI:0.294～0.615)、听力(OR=0.605,95%CI:0.447～0.819)、儿童期健康状况(OR=0.635,95%CI:0.450～0.897)、文化程度(OR=7.571,95%CI:4.091～14.011)、个人年收入(OR=3.422,95%CI:1.952～6.001),个人行为层次中的体育锻炼(OR=0.525,95%CI:0.291～0.948)、每日睡眠时长(OR=0.615,95%CI:0.449～0.843),人际层次中的婚姻状态(OR=0.565,95%CI:0.349～0.917)、子女经济支持(OR=1.427,95%CI:1.053～1.934)、互联网使用(OR=0.469,95%CI:0.271～0.814),社区层次中的生活满意度(OR=0.352,95%CI:0.161～0.769),政策层次中的养老保险(OR=0.491,95%CI:0.245～0.983)是农村老年人实现成功老龄化的影响因素。结论我国农村老年人成功老龄化实现程度偏低，认知和躯体功能良好、积极的社会参与维度是农村老年人实现成功老龄化的主要短板。

Antioxidative Effect of Luteolin Pretreatment on Simulated Ischemia/Reperfusion Injury in Cardiomyocyte and Perfused Rat Heart

Objective：To investigate the antioxidative effect and mechanism of luteolin on rat cardiomyocytes and isolated hearts fol owed by simulated ischemia/reperfusion（SI/R） injury. Methods：The left ventricular cardiomyocytes and the isolated hearts from adult rats were subjected to SI/R injury. The experiment groups included control, SI/R, luteolin ＋ SI/R（Lut ＋ SI/R）, vitamin E（Vit E）＋ SI/R, and LY294002 ＋ luteolin ＋ SI/R（LY ＋ Lut ＋ SI/R） groups. Cell viability, shortening amplitude, lactate dehydrogenase（LDH） release, superoxide dismutase（SOD） activity, the production of reactive oxygen species（ROS） and malondialdehyde（MDA）, expression levels of Akt, phosphorylated Akt, NOX2（gp91phox）, NOX2 m RNA, mitogen-activated protein kinase（p38 MAPK） and phosphorylated p38 MAPK were al measured after 3-h simulated ischemia and 2-h simulated reperfusion procedure in cardiomyocytes. Vit E was used as a standard control. The contractile function of isolated hearts was further observed after they were subjected to 30-min global ischemia and 120-min reperfusion. Results：Pretreatment with 8-μmol/L luteolin substantially increased cel viability and shortening amplitude, while reducing evidence of oxidative stress-induced damage in the cel s. In addition, the expression of NOX2, NOX2 m RNA and phosphorylation of p38 MAPK were al downregulated. Furthermore, pretreatment with 40-μmol/L luteolin improved the recovery of myocardial contractile function fol owing SI/R-induced injury, and luteolin markedly increased phosphorylation of Akt. However, all of the above effects were partially inhibited by the phosphatidylinositol 3-kinase（PI3K） inhibitor, LY294002. Conclusions：Luteolin prevents SI/R-induced myocardial damage by reducing oxidative stress-induced injury in isolated rat hearts and cardiomyocytes, and the cardioprotection induced by luteolin was partial y mediated by the PI3K/Akt pathway.

Effect of cigarette smoking on clinical outcomes of hospitalized Chinese male smokers with acute myocardial infarction

Background Smoking is known to be a strong risk factor for premature atherosclerosis, acute myocardial infarction （AMI） and sudden cardiac death. According to a cross-sectional survey conducted in 2000-2001 in China, the prevalence of smoking among the Chinese men was 60.2%, the highest prevalence in the world. Up to date, the relationship between smoking and AMI in Chinese male smokers is still unclear. This study analyzed the baseline characteristics for male smokers hospitalized with AMI and investigated the effect of cigarette smoking on their clinical *** A total of 890 men aged 18 years or over with AMI were prospectively recruited from 1 January 2007 to 31 December 2009 from Shanxi Provincial People＇s Hospital. Patients were grouped into smokers and nonsmokers. The relationships between baseline characteristics and clinical outcomes were tested using either the chi-square test for trend for discrete variables or analysis of variance for continuous *** Smokers accounted for 66.7% （594）, more than twice of nonsmokers （296 （33.3%））, and were averaged 7 years younger （（56.61±11.44） vs. （63.61±11.62） years, P 〈0.001）. Smokers had the higher rate of TIMI flow grade 2 or 3 after thrombolytic therapy （42.4 % vs. 24.5%, P=0.002）, 1 vessel disease （25.5% vs. 14.5%, P=0.003） than *** had better in-hospital outcome with lower in-hospital mortality rate than nonsmokers （6.2% vs. 10.8%,P=0.023）.Conclusions Male smokers suffered from AMI in this study presented an average of 7 years earlier than nonsmokers and were more than twice as likely to have AMI as nonsmokers in China. Smoking appeared to result in earlier infarction,especially ST elevated myocardial infarction in otherwise healthier patients who are likely to survive.
