Intestinal endometriosis:Diagnostic ambiguities and surgical outcomes

BACKGROUND Endometriosis is a common disease for women of reproductive age. However,when it involves intestines, it is difficult to diagnose preoperatively because its symptoms overlap with other diseases and the results of evaluations can be unspecific. Thus it is important to know the clinical characteristics of intestinal endometriosis and how to exactly *** To analyze patients in whom intestinal endometriosis was diagnosed after surgical treatments, and to evaluate the clinical characteristics of preoperatively misdiagnosed *** We retrospectively reviewed the pathologic reports of 30 patients diagnosed as having intestinal endometriosis based on surgical specimens between January2000 and December 2017. We reviewed their clinical characteristics and surgical *** Twenty-three(76.6%) patients showed symptoms associated with endometriosis,with dysmenorrhea being the most common(n = 9, 30.0%). Thirteen patients(43.3%) had a history of pelvic surgeries. Ten patients(33.3%) had a history of treatment for endometriosis. Only 4 patients(13.3%) had a diagnosis of endometriosis based on endoscopic biopsy findings. According to preoperative evaluations, 13 patients(43.3%) had an initial diagnosis of pelvic endometriosis and 17 patients(56.6%) were misdiagnosed as having other diseases. The most common misdiagnosis was submucosal tumor in the large intestine(n = 8, 26.7%),followed by malignancies of the colon/rectum(n = 3, 10.0%) and ovary(n = 3,10.0%). According to the Clavien-Dindo classification, 5 complications were grade I or II and 2 complications were grade IIIa. A Long Short-Term Memory(LSTM) Recurrent Neural Network(RNN) has driven tremendous improvements on an acoustic model based on Gaussian Mixture Model(GMM). However, these models based on a hybrid method require a forced aligned Hidden Markov Model(HMM) state sequence obtained from the GMM-based acoustic model. Therefore, it requires a long computation time for training both the GMM-based acoustic model and a deep learning-based acoustic model. In order to solve this problem, an acoustic model using CTC algorithm is proposed. CTC algorithm does not require the GMM-based acoustic model because it does not use the forced aligned HMM state sequence. However, previous works on a LSTM RNN-based acoustic model using CTC used a small-scale training corpus. In this paper, the LSTM RNN-based acoustic model using CTC is trained on a large-scale training corpus and its performance is evaluated. The implemented acoustic model has a performance of 6.18% and 15.01% in terms of Word Error Rate(WER) for clean speech and noisy speech, respectively. This is similar to a performance of the acoustic model based on the hybrid method.

As video compression is one of the core technologies required to enable seamless medical data streaming in mobile healthcare applications,there is a need to develop powerful media codecs that can achieve minimum bitrates while maintaining high perceptual *** Video Coding(VVC)is the latest video coding standard that can provide powerful coding performance with a similar visual quality compared to the previously developed method that is High Efficiency Video Coding(HEVC).In order to achieve this improved coding performance,VVC adopted various advanced coding tools,such as flexible Multi-type Tree(MTT)block structure which uses Binary Tree(BT)split and Ternary Tree(TT)***,VVC encoder requires heavy computational complexity due to the excessive Ratedistortion Optimization(RDO)processes used to determine the optimalMTT block *** this paper,we propose a fast MTT decision method with two Lightweight Neural Networks(LNNs)using Multi-layer Perceptron(MLP),which are applied to determine the early termination of the TT split within the encoding *** results show that the proposed method significantly reduced the encoding complexity up to 26%with unnoticeable coding loss compared to the VVC TestModel(VTM).

In recent years,pure iron(Fe)has attracted significant attention as a promising biodegradable orthopedic implant material due to its excellent mechanical and biological ***,in physiological conditions,Fe has an extremely slow degradation rate with localized and irregular degradation,which is problematic for practical *** this study,we developed a novel combination of a nanostructured surface topography and galvanic reaction to achieve uniform and accelerated degradation of an Fe *** target-ion induced plasma sputtering(TIPS)technique was applied on the Fe implant to introduce biologically compatible and electrochemically noble tantalum(Ta)onto its surface and develop surface nano-galvanic *** tests revealed that the uniformly distributed nano-galvanic corrosion cells of the TIPS-treated sample(nano Ta-Fe)led to relatively uniform and accelerated surface degradation compared to that of bare ***,the mechanical properties of nano Ta-Fe remained almost constant during a long-term in vitro immersion test(~40 weeks).Biocompatibility was also assessed on surfaces of bare Fe and nano Ta-Fe using in vitro osteoblast responses through direct and indirect contact assays and an in vivo rabbit femur medullary cavity implantation *** results revealed that nano Ta-Fe not only enhanced cell adhesion and spreading on its surface,but also exhibited no signs of cellular or tissue *** results demonstrate the immense potential of Ta-implanted surface nanostructures as an effective solution for the practical application of Fe-based orthopedic implants,ensuring long-term biosafety and clinical efficacy.
