Diagnostic accuracy of ePOS score in predicting DNR labeling after ICU admission:A prospective observational study(ePOS-DNR)

Background Resuscitation can sometimes be futile and making a do-not-resuscitate(DNR)decision is in the best interest of the *** electronic poor outcome screening(ePOS)score was developed to predict 6-month poor outcomes of critically ill *** explored the diagnostic accuracy of the ePOS score in predicting DNR decisions in the intensive care unit(ICU).Methods This study was conducted at the ICU of a tertiary referral hospital in Saudi Arabia between March and May ***,we calculated ePOS scores for all eligible consecutive admissions after 48 h in the ICU and recorded the DNR *** ability of the score to predict DNR was explored using logistic ***'s ideal cut-off value was calculated using the DeLong method,and different diagnostic accuracy measures were generated with corresponding 95%confidence intervals(CIs).Results We enrolled 857 patients,125 received a DNR order and 732 did *** average ePOS score of DNR and non-DNR patients was 28.2±10.7 and 15.2±9.7,*** score,as a predictor of DNR order,had an area under receiver operator characteristic(AUROC)curve of 81.8%(95%CI:79.0 to 84.3,Pal cut-off value>17 was associated with a sensitivity of 87.2(95%CI:80.0 to 92.5,Palue of 29.2(95%CI:24.6 to 33.8,Palue of 96.7(95%CI:95.1 to 98.3,P17 may help guide clinical decisions to withhold or commence resuscitative measures.

A discoid meniscus is a morphological abnormality wherein the meniscus loses its normal‘C’*** most patients are asymptomatic,patients might still present with symptoms such as locking,pain,swelling,or giving *** resonance imaging is usually needed for confirmation of *** on a constellation of factors,including clinical and radiological,different approaches are chosen for the management of discoid *** purpose of this review is to outline the treatment of discoid meniscus,starting from conservative approach,to the different surgical options for this *** PubMed and Google Scholar databases were used for this *** discussing the treatment of discoid meniscus from 2018 to 2023 were *** there were 369 studies retrieved,and after removal of studies using the exclusion criteria,26 studies were included in this *** such as stability,presence of tear,and morphology can help with surgical *** approaches have been used to treat discoid meniscus,where the choice is tailored for each patient ***,factors that may positively impact patient outcomes include male sex,body mass indexal approach is partial meniscectomy with or without repair;however,recently,there has been an increased emphasis on discoid-preserving techniques such as meniscoplasty,meniscopexy,and meniscal allograft transplantation.

BACKGROUND Hepatitis C virus(HCV),hepatitis B virus(HBV),and human immunodeficiency virus 1(HIV-1)are the most epidemic blood-borne viruses,posing threats to human health and causing economic losses to nations for combating the infection *** diagnostic methodologies that depend on the detection of viral nucleic acids are much more expensive,but they are more accurate than sero-logical *** To develop a rapid,cost-effective,and accurate diagnostic multiplex polymerase chain reaction(PCR)assay for simultaneous detection of HCV,HBV,and *** The design of the proposed PCR assay targets the amplification of a short conserved region featured with a distinguishable melting profile and electro-phoretic molecular weight inside each viral ***,this diagnostic method will be appropriate for application in both conventional(combined with electrophoresis)and real-time PCR *** in silico investigations were conducted to prove the capability of the approached PCR assay to detect variants of each ***,Egyptian isolates of each virus were subjected to the wet lab examination using the given diagnostic *** The in silico investigations confirmed that the PCR primers can match many viral variants in a multiplex PCR *** wet lab experiment proved the efficiency of the assay in distinguishing each viral type through high-resolution melting *** to related published assays,the proposed assay in the current study is more sensitive and competitive with many expensive PCR *** This study provides a simple,cost-effective,and sensitive diagnostic PCR assay facilitating the detection of the most epidemic blood-borne viruses;this makes the proposed assay promising to be substitutive for the mistakable and cheap serological-based assays.

在自然语言处理(NLP)领域中,文本到SQL转换上取得的显著进步,使得用自然语言与数据库交互成为可能。然而,它在低资源语言如乌尔都语上的表现不足。本研究提出USpider,一个乌尔都语文本到SQL转换数据集,旨在解决文本到SQL任务在乌尔都语上的资源稀缺问题,为构建该领域在乌尔都语上的发展奠定基础。USpider包含9,693个乌尔都语自然语言问题及其对应的SQL查询,涉及133个不同领域的166个不同的数据库。数据集的创建采用严谨方法,结合机器翻译、人工改进和迭代审查,确保语言的准确性、连贯性和文化相关性。由此产生的高质量数据集展现了乌尔都语在数据库查询中的丰富性和复杂性。为了验证USpider的科研和使用价值,实验部分我们使用了大型语言模型(LLM),具体是GPT-3.5,应用DINSQL方法进行少样本学习。实验通过执行准确性和匹配准确性等指标进行评估,展示了模型在简单和中等难度查询上表现出色。对于简单查询,模型在不同测试批次中达到69%至72%的执行准确性和54%至58%的精确匹配准确性。对于中等难度查询,执行准确性和精确匹配准确性分别在59%至63%和42%至48%之间。这些发现证实了USpider在提高准确SQL查询方面的作用。然而,实验亦揭示了处理复杂查询和语言变化时的挑战。随着查询难度的增加,困难级别查询的执行准确性下降到40%-59%,非常困难级别的查询下降到38%-42%。精确匹配准确性在这些类别中分别在10%-34%和0%-23%之间。USpider数据集对乌尔都语社区将会具有重大意义,将能开启医疗保健、教育、金融和电子政务等领域数据库的自然语言接口。通过让乌尔都语使用者使用母语与技术互动,研究有助于缩小数字鸿沟,推动NLP的包容性。此外,本研究的发现将对更广泛的NLP领域,尤其是对低资源语言有重要启示。DINSQL方法成功应用于乌尔都语,,强调了为低资源语言专门构建数据集的重要性。USpider的创建方法将为其他语言中的类似挑战提供参考。未来的研究方向包括改进USpider以适应较弱的LLM,探索LLM的局限性,并适应Spider数据集的演变。随着Spider 2.0的预期发布,我将开发USpider 2.0,与它的查询复杂性和多样性的提高保持同步,同时利用LLM架构和训练技术的最新进展。总的来说,USpider为乌尔都语上的NLP研究作出了贡献,提供全面翻译的数据集,并证明了它在乌尔都语文本到SQL转换中的效用。实验结果展示了准确生成SQL查询的潜力,同时指出了需要改进的方面。这项研究为开发兼容性强和可用性高的数据库自然语言接口奠定了基础,赋能乌尔都语社区,推动低资源语言NLP领域的发展。

Air pollution from shipping is becoming a critical issue,particularly in dense hub port *** proposed solution to minimize ship-based emissions at ports is the implementation of an Onshore Power Supply(OPS)*** allows ships to shut off their auxiliary engines and instead connect to the port *** there have been numerous studies conducted on ports in Europe and the United States,little research has been done on Egyptian ***,this paper aims to investigate the feasibility of implementing OPS at Port Said West Port in Egypt,aligning with Egypt Vision 2030’s goals for addressing climate *** research primarily focuses on analyzing data collected from calling ships to generate socio-economic and cost-effectiveness analyses of *** further enhance the environmental benefits of OPS,the paper proposes the use of solar energy as the OPS electricity *** findings of the study revealed that by relying on the national grid,emissions can be reduced by 28%.Moreover,it is predicted that this reduction could reach 100%if electricity generation is solely based on solar ***,the economic analysis demonstrates promising profitability,with a payback period of approximately two years.

The Impact of Blood Transfusion on the Efficiency of Stem Cell Transplants

Background: While blood product transfusion is essential for managing hematologic deficits in allogenic Hematopoietic stem cell transplant (AHSCT) recipients, it has risks including infectious disease transmission, alloimmunization, and transfusion reactions. These risks have sparked an ongoing debate regarding the overall impact of transfusions on patient outcomes. Thus, this study aimed to evaluate the impact of Red Blood Cells (RBCs) and/or platelet transfusion on the infection incidence and overall survival in AHSCT patients. Methods: We performed a retrospective analysis of clinical and laboratory data of sixty adult patients with primary malignant hematological disorder who had undergone AHSCT. Participants’ data were categorized into two groups;Group 1 (low transfusion group) consisted of patients receiving 10 units. Quantitative data were expressed as mean ± SD. The t-test of significance and Chi-square (χ2) test were used, with p ≤ 0.05 considered significant. Result: A total of 60 patients’ data was included. In Group 1, out of 30 patients, 13 (43.33%) developed infections. In contrast, Group 2 had 21 (70%) out of 30 patients develop infections. Group 1 had a higher survival rate (57.8%) than Group 2 (transfusion > 10 units) (46.2%) with a chi-square value = 23.56, and p-value Conclusion: The volume of blood product transfusions has a considerable impact on patient outcomes, particularly infection and survival rates. Additional long-term prospective studies and larger randomized controlled trials are needed to strengthen the evidence for determining transfusion protocols for these patients.

Moringa oleifera is well known as a highly nutritious plant and a water purifier in fish *** fish has many impressive qualities making them very suitable for ***,this study aimed to evaluate the effect of supplementation of Moringa oleifera aqueous extract(MOAE)at varying concentrations(0,100,200,and 400 mg/kg diet for 90 days)on growth performance,hematological,and some biochemical parameters of Nile ***,histological changes and immunohistochemical expression of caspase 3 in gills and livers were *** enhancements(Pal weight,weight gain,specific growth rate,red blood cells,hemoglobin,packed cell volume,white blood cells,and total serum protein were observed in fish fed 200 mg MOAE/kg diet compared with other ***,a significant reduction(Paled disturbance in the corresponding *** histological degenerative changes were detected in the gills and liver of fish fed 400 mg MOAE/kg *** comparison,fish fed 200 mg MOAE/kg diet showed the best histo-architectures of these organs compared with other *** immune expressions of caspase 3 were determined where gills and liver of fish fed 400 mg MOAE/kg diet revealed extensive number of positive caspase 3 cells indicating severe apoptotic ***,a dietary MOAE at 200 mg/kg diet is recommended to improve the growth performance,hematological parameters,liver and kidney functions in Nile ***,caution must be considered when using MOAE at a concentration over 200 mg/kg diet.
