The“terminal line”:a novel sign for the identification of distal mesorectum end during TME for rectal cancer

Background:although the clinical importance of complete,intact total mesorectal excision(TME)is the widely accepted standard for decreasing local recurrence of rectal cancer,the residual mesorectum still represents a significant component of resection margin *** study aimed to use a visible intraoperative sign to detect the distal mesorectal end to ensure complete inclusion of the mesorectum and avoid unnecessary ***:The distal mesorectum end was investigated retrospectively through a review of 124 operative videos at the Union Hospital of Fujian Medical University(Fujian,China)and Cleveland Clinic(Ohio,USA)by two independent surgeons who were blinded to each ***,28 cadavers and 44 post-operative specimens were prospectively examined by hematoxylin and eosin(H&E)staining and Masson’s staining to validate and confirm the findings of the retrospective *** and multivariate analyses were carried out to detect the independent factors that can affect the visualization of the distal mesorectal ***:The terminal line(TL)is the distal mesorectal end of the transabdominal and transanal TME(taTME)and appears as a remarkable pearly white fascial structure extending posteriorly from 2 to 10 o’*** examination revealed that the fascia propria of the rectum merges with the presacral fascia at the TL,beyond which the mesorectum ends,with no further downward *** the retrospective observation,the TL was seen in 56.6%of transabdominal TME and 56.0%of taTME *** approach and tumor distance from the anal verge were the independent variables that directly influenced the detection of the TL(P=0.03 and P=0.01).Conclusion: The TL is a visible sign where the transabdominal TME should end and the taTME should begin. Recognitionof the mesorectal end may impact the certainty of complete mesorectum inclusion. Further clinical trials are needed toconfirm the preliminary findings.

Optimization Method to Determine Gross alpha-Beta in Water Samples Using Liquid Scintillation Counter

Liquid scintillation counting (LSC) is an adequate nuclear technique to determine radioactivity levels, as verified by this study for the determination of the gross alpha and beta activities of aqueous samples because of its simplicity and low associated cost comparison with other techniques. This paper discusses a new approach to the monitoring of gross alpha and gross beta activities in water. The method consists of using pulse decay discrimination (PDD) liquid scintillation counting LSC of 2 ml of the sample after conditioning with 12 ml of AB-Ultima Gold LSC cocktail, and PDD 139 condition. Different factors that affected on the counting efficiency were studied such as quench reaction, volume of sample, and type of vial. The gross alpha and gross beta particle activity measurement using 40K and 243Am with different activities concentration for efficiency determination. The calculated detection limits were 0.07 Bq/l and 0.12 Bq/l for gross alpha and gross beta respectively for 500 min measurements of samples produced by concentration of one liter of water.

The Ball and beam system(BBS)is an attractive laboratory experimental tool because of its inherent nonlinear and open-loop unstable *** an effective ball and beam system controller is a real challenge for researchers and *** this paper,the control design technique is investigated by using Intelligent Dynamic Inversion(IDI)method for this nonlinear and unstable *** proposed control law is an enhanced version of conventional Dynamic Inversion control incorporating an intelligent control element in *** Moore-PenroseGeneralized Inverse(MPGI)is used to invert the prescribed constraint dynamics to realize the baseline control law.A sliding mode-based intelligent control element is further augmented with the baseline control to enhance the robustness against uncertainties,nonlinearities,and external *** semi-global asymptotic stability of IDI control is guaranteed in the sense of *** simulations and laboratory experiments are carried out on this ball and beam physical system to analyze the effectiveness of the *** addition to that,comparative analysis of RGDI control with classical Linear Quadratic Regulator and Fractional Order Controller are also presented on the experimental test bench.

目的观察5-氨基酮戊酸光动力疗法（alA-PDT）在肿瘤性皮肤病及炎症性皮肤病治疗中的临床疗效与不良反应。从疗效、不良反应、复发率、创伤性、美容、病人满意度多方面评价alA-PDT在皮肤病治疗的利弊,从而指导皮肤科医师选择和实施最佳治疗方案。方法1、本研究所有患者均来自南昌大学第一附属医院皮肤科门诊部,于2020年4月至2021年1月临床诊断或病理诊断为皮肤肿瘤、尖锐湿疣、中重度痤疮及秃发性毛囊炎等患者50名。2、所有患者均接受2～6次alA-PDT治疗,并记录皮疹改变,同时评价临床疗效、副作用、复发率、美容效果、皮肤损伤情况和患者满意度。结果1、疗效观察:（1）非黑色素性皮肤肿瘤8例,其中术后鳞状细胞癌4例,治愈2例,显效2例,总有效率100%;基底细胞癌1例,治愈;多发性日光性角化1例,治愈;Bowen病1例,显效,乳房外Paget病1例,有效。（2）尖锐湿疣38例,治愈29例,显效6例,总有效率92%。（3）重度痤疮、毛囊炎4例,其中重度痤疮3例,显效,有效率100%;秃发性毛囊炎1例,治愈。不同疾病类型总有效率进行比较,差异无统计学意义。2、不良反应:13例患者在治疗期间出现不良反应,总发生率为26%。主要表现为轻度烧灼感、红斑、发红、糜烂、恶心呕吐、局部肿胀、外周色素改变。其中轻度烧灼感6例,发生率12%;烧灼痛伴糜烂3例,发生率6%;局部及外周色素改变3例,发生率6%;局部红斑3例,发生率6%;恶心呕吐1例,发生率2%。3、创伤评估与处理:（1）疼痛评估与处理:50例患者中有2例疼痛评分为2分（当注意到疼痛时会有轻微的疼痛感）,均无需处理。其余48名患者的评分为0（无痛）。（2）功能评估:50例患者均为Ⅲ级（满意,功能不受限,不影响日常生活活动,无局部异常）。4、美容效果评估:50例患者中,1级2例,占4%,在治疗部位的表面和边缘有局限性红斑或显著的棕色色素斑;2级2例,占4%,有很薄的疤痕或无疤痕;其余为3级,占92%,无疤痕和色素改变。5、复发率（RR）:本研究共有9例复发。皮肤肿瘤组,其中Bowen病1例,复发率为100%,Paget病1例,复发率为100%;日光性角化、基底细胞癌、鳞状细胞癌无复发。生殖器疣组38例中复发6例为15.79%,其中尿道口尖锐湿疣3例,复发率为7.9%,肛管尖锐湿疣1例,复发率为2.6%,宫颈尖锐湿疣1例复发率,2.6%,阴茎尖锐湿疣1例,复发率为2.6%;外阴尖锐湿疣无复发;痤疮和毛囊炎组仅1例炎性囊性痤疮复发;斑秃性毛囊炎无复发。6、患者满意度调查:32例为5分（非常满意）,占64%;15例为4分（满意）,占30%;3例为3分（一般）,占6%。患者总满意率为94%。结论1、alA-PDT是一种新型的非侵入性的光化学疗法,可以广泛运用于皮肤疾病的治疗。2、对于肿瘤性和炎症性皮损,尤其是大面积皮损,联合治疗可取得更好的疗效,并减少复发率;因此,建议首先应用一种或多种其他方式（手术、冷冻治疗、电灼、局部化疗药物）治疗效果不好,然后进行alA-PDT治疗。3、alA-PDT治疗的副作用发生率低,仅有轻度疼痛、烧灼感、糜烂、外周色素改变等不良反应。4、alA-PDT治疗是一种无创性疗法,美容效果好,患者满意度高,不良反应少。PDT治疗也适用于特殊患者群体,例如器官移植受者、免疫功能低下的人、特殊部位（头皮、面部、生殖器）的肿瘤和炎症病变以及老年患者,尤其是患有多种慢性疾病的患者,不能忍受其他创伤治疗。

Vitamin D deficiency: Correlation to interleukin-17, interleukin-23 and PⅢNP in hepatitis C virus genotype 4

AIM: To assess vitamin D (Vit D) abnormalities in hepatitis C infected patients and their relationship with interleukin (IL)-17, IL-23 and N-terminal propeptide of type Ⅲ pro-collagen (PⅢNP) as immune response mediators. METHODS: The study was conducted on 50 Egyptian patients (36 male, 14 female) with hepatitis C virus (HCV) infection, who visited the Hepatology Outpatient Clinic in the Endemic Disease Hospital at Cairo University. Patients were compared with 25 ageand sexmatched healthy individuals. Inclusion criteria were based on a history of liver disease with HCV genotype 4 (HCV-4) infection (as new patients or under followup). Based on ultrasonography, patients were classified into four subgroups; 14 with bright hepatomegaly; 11 with perihepatic fibrosis; 11 with hepatic cirrhosis; and 14 with cirrhosis and hepatocellular carcinoma (HCC).Total Vit D (i.e., 25-OH-Vit D) and active Vit D [i.e., 1,25-(OH) 2 -Vit D] assays were carried out using commercial kits. IL-17, IL-23 and PⅢNP levels were assayed using enzyme linked immunosorbent assay kits, while HCV virus was measured by quantitative and qualitative polymerase chain reaction. RESULTS: Levels of Vit D and its active form were significantly lower in advanced liver disease (hepatic cirrhosis and/or carcinoma) patients, compared to those with bright hepatomegaly and perihepatic fibrosis. IL-17, IL-23 and PⅢNP levels were markedly increased in HCV patients and correlated with the progression of hepatic damage. The decrease in Vit D and active Vit D was concomitant with an increase in viral load, as well as levels of IL-17, IL-23 and PⅢNP among all subgroups of HCV-infected patients, compared to normal healthy controls. Incidence of orbital,conjunctival and lacrimal gland malignant tumors in USA from Surveillance,Epidemiology and End Results,1973-2009

AIM： To determine the types and incidence of tumors affecting the orbit, conjunctiva and lacrimal glands and to study the trend line of these tumors in the United States from 1973 to ***： We used the publicly available Surveillance, Epidemiology and End Results （SEER） database registries to determine the incidence rates. Age was adjusted to the 2000 US Standard Population. Patients were stratified according to age group, gender, race and histological grouping of tumor lesions. Three age groups were defined： 0-19, 20-49 and ≥50y. Annual percentage changes were calculated to examine ***： The overall age adjusted incidence rate was 3.39 （95%CI： 3.27-3.52） per million person-years. The tumors were more prevalent in age group ≥50 counting 9.51 （95%CI： 9.11-9.92） per million person-years. Most of the soft tissue sarcomas occurred in the young age with incidence rate of 0.35 （95%CI： 0.28-0.42） per million person-years. Lymphomas were the dominant subtype in the adult population with incidence rate of 5.74 （95%CI： 5.43-6.06） per million person-years. Incidence rates were higher in males than females with an overall rate ratio of 1.31 （95%CI： 1.21-1.41） mainly caused by the increase in carcinoma subtypes. White race had a higher tumor incidence with a rate ratio of 1.47（95%CI： 1.25-1.73） driven by the higher incidence of most histological subtypes. Orbital tumors showed a higher incidence rate followed by conjunctival and lacrimal gland tumors with incidence rates of 1.59, 1.37 and 0.43 per million person-years respectively. The trend line of overall incidence of tumors showed a significant increase （APC=3.11, 95%CI： 2.61-3.61） mainly due to increase of lymphomas. Dielectric Properties and a.c. Conductivity of Epoxy/alumina Silicate NGK Composites

alumina silicate powder which is extracted from the obsolete spark plug NGK (insulator part as a filler) has been used to produce epoxy/alumina silicate composite. The dielectric behavior of the composite materials (epoxy resin-alumina silicate NGK) is analyzed as a function of the filler content, temperature and frequency. AC conductivity and impedance are also studied. The results show that the permittivity, dielectric loss and loss tangent for all composites increase with increasing alumina silicate NGK filler content.

Real time endoscopic ultrasound elastography and strain ratio in the diagnosis of solid pancreatic lesions

AIM To evaluate the accuracy of the elastography score combined to the strain ratio in the diagnosis of solid pancreatic lesions(SPL). METHODS A total of 172 patients with SPL identified by endoscopic ultrasound were enrolled in the study to evaluate the efficacy of elastography and strain ratio in differentiating malignant from benign lesions. The semi quantitative score of elastography was represented by the strain ratio method. Two areas were selected, area(A) representing the region of interest and area(B) representing the normal area. Area(B) was then divided by area(A). Sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), and accuracy were calculated by comparing diagnoses made by elastography, strain ratio and final *** SPL were shown to be benign in 49 patients and malignant in 123 patients. Elastography alone had a sensitivity of 99%, a specificity of 63%, and an accuracy of 88%, a PPV of 87% and an NPV of 96%. The best cut-off level of strain ratio to obtain the maximal area under the curve was 7.8 with a sensitivity of 92%, specificity of 77%, PPV of 91%, NPV of 80% and an accuracy of 88%. Another estimated cut off strain ratio level of 3.8 had a higher sensitivity of 99% and NPV of 96%, but with less specificity, PPV and accuracy 53%, 84% and 86%, respectively. Adding both elastography to strain ratio resulted in a sensitivity of 98%, specificity of 77%, PPV of 91%, NPV of 95% and accuracy of 92% for the diagnosis of SPL. CONCLUSION Combining elastography to strain ratio increases the accuracy of the differentiation of benign from malignant SPL.

Trabeculectomy with Ologen implant versus perfluoropropane gas bubble for open angle glaucoma in pseudophakic eyes

AIM:To evaluate the safety and efficacy of augmented trabeculotomy with Ologen versus perfluoropropane in management of pseudophakic ***:This is a comparative randomized study included 57 pseudophakic eyes of 57 patients with medically uncontrolled open angle glaucoma(OAG).Twentynine patients were allocated in group I(trabeculectomy with Ologen;trab-ologen group),while 28 patients were assigned in group II(trabeculectomy with perfluoropropane gas bubble;trab-C3 F8 gas bubble group).RESULTS:The intraocular pressure(IOP)was significantly reduced in both study groups at all postoperative follow up intervals(1 wk,3,6,12,18,24,30 and 36 mo,Palues of both groups remained statistically insignificance during the early 12 months of follow ***,the trab-ologen group achieved a statistically significant reduction over the trab-C3 F8 gas bubble group during the last 24 months of follow ***:Augmentation of trabeculectomy with either Ologen implant or perfluoropropane gas bubble are associated with strict long term IOP control and evident safety in medically-uncontrolled pseudophakic eyes with OAG.
