Background: the mechanism responsible for optic disc pit maculopathy is unclea r, but abnormal vitreous structures, including the anomalous Cloquet’s canal at the optic disc pit, have been suggested as important factors. Case: We report t he intraoperative and ultrastructual findings of an unusual posterior vitreous s trand in the eye of an 8-year-old girl with optic disc pit maculopathy. Observ ations: the patient presented with decreased vision in the left eye. Examination of the left eye revealed a best-corrected visual acuity (VA) of 0.08 and a mac ular detachment associated with an optic disc pit. Vitrectomy was performed with the adjunctive use of triamcinolone acetonide intraoperatively. the presence of an unusual posterior hyaloid strand tightly attached to the margin of the optic disc pit was noted. An unusual movement of this strand was observed during the surgery. the strand was excised, and fluid-gas exchange was performed using gas tamponade with 20%SF6. After 12 months, a complete macular reattachment was ob tained, with the VA improving to 1.2. Electron microscopic examination of the re moved strand revealed abundant thick collagen fibrils with a frame of fine fibri ls. Conclusion: the unusual posterior vitreous strand connected to the optic dis c pit may have contributed to the pathogenesis of maculopathy in this young chil d.

Objectives: Glioblastoma(GBM) is one of the worst cancers in terms of *** therapy consists of resection with concomitant *** to nimustine hydrochloride(ACNU),an alkylating agent,has been linked to methylguanine DNA methyltransferase(MGMt).Daily administration of procarbazine(PCZ) has been reported to decrease MGMt *** study investigated the efficacy of ACNU + PCZ compared to ACNU alone for GBM and anaplastic astrocytoma(AA).Methods: Patients(20-69 years) who had newly diagnosed AA and GBM were randomly assigned to receive radiotherapy with ACNU alone or with ACNU + *** primary endpoint was overall survival(OS).this was designed as a phase Ⅱ/Ⅲ trial with a total sample size of 310 patients and was registered as UMIN-CtR ***: After 111 patients from 19 centers in Japan were enrolled,this study was terminated early because temozolomide was newly approved in *** median OS and median progression-free survival(PFS) with ACNU alone(n = 55) or ACNU + PCZ(n = 56) in the intention-to-treat population were 27.4 and 22.4 months(P = 0.75),and 8.6 and 6.9 months,*** median OS and median PFS of the GBM subgroup treated with ACNU alone(n = 40) or ACNU + PCZ(n = 41) were 19.0 and 19.5 months,and 6.2 and 6.3 months,*** 3/4 hematologic adverse events occurred in more than 40% of patients in both arms,and 27% of patients discontinued treatment because of adverse ***: the addition of PCZ to ACNU was not beneficial,in comparison with ACNU alone,for patients with newly diagnosed AA and GBM.

Effect of Wearing a Maxillofacial Prosthesis on Masticatory Rhythm Improvement in Mandibulectomy Patients

Background: Recording jaw movement rhythms in patients with jaw defects is challenging using conventional tracking devices. However, masticatory rhythm is crucial for masticatory performance and nutritional intake. this study aimed to explore the impact of using a maxillofacial prosthesis to restore masticatory rhythm in mandibulectomy patients with a novel wearable device. Methods: twelve patients who underwent mandibulectomy and were rehabilitated with maxillofacial prosthesis were recruited. Chewing rate, cycle durations, coefficient of variation of cycle durations (CV), changing times of skin morphology on the back of the ear pinna area (SM), and masticatory performance were measured during gum chewing without and with a denture using a wearable jaw movement rhythm tracking device. A paired t-test and the Pearson correlation coefficient were used for statistical analysis. Results: the chewing rate increased by 8.6 cycles/min with dentures (P P = 0.01). the CV and SM decreased with dentures (P = 0.004 and P = 0.01, respectively). Significant correlations were found between the CV and SM without dentures (P = 0.004). Conclusion: 1. Wearing maxillofacial prostheses can improve masticatory rhythm in patients who undergo mandibulectomy with Eichner B2 occlusal supports. 2. the wearable device is available for measuring masticatory rhythm in patients with jaw defects in clinics. 3. Changing times of the skin morphology on the back of the ear pinna indicate jaw movement stability through a simpler process than the coefficient of variation of cycle durations.

Intraductal growth of nonfunctioning endocrine tumors of the pancreas may be very rare, and our survey of literature shows only two cases have been described. We report a case of a 43-year-old man with a nonfunctioning endocrine tumor of the pancreas that uniquely grew within the lumen of the main pancreatic duct (MPD) without ductal involvement and completely obstructed the MPD. Endoscopic ultrasonography (EUS) and endoscopic retrograde cholangio pancreatography(ERCP) were very helpful to delineate the intraductal growth of the tumor and to determine the resection line of the pancreas. A nonfunctioning pancreatic endocrine tumor is important to consider on differential diagnoses when complete obstruction of the MPD is demonstrated on ERCP. It is speculated that the tumor originated from precursor cells of the pancreatic duct or islet cells adjacent to the MPD and slowly proliferated within the lumen of the MPD.

A 59-year-old woman was admitted to our hospital because of recurrent follicular lymphoma(FL).Colonoscopic examination revealed a rectal submucosal tumor(SMt)without any erosions and *** this patient,it was difficult to distinguish non-Hodgkin's lymphoma(NHL)invasion from other disorders of the colon including carcinoid tumor merely based on endoscopic *** and immunohistochemical studies on biopsy specimens showed an infiltration of atypical lymphocytes that were positive for CD20 and BCL2 but negative for *** in situ hybridization on paraffin-embedded tissue sections (t-FISH)identified a translocation of BCL2 with IGHgene. Based on these findings,the tumor was defined as an invasion of FL.t-FISH method is useful for the detection of a monoclonality of atypical lymphocytes in an SMt of the gastrointestinal tract,and particularly for the detection of chromosomal translocations specific to lymphoma subtypes.

在X射线暴高温高密度的环境中,18Ne(α,p)21Na很可能是由热CNO循环突破到rp过程的一个重要反应。到目前为止,人们测得的反应率还存在很大的不确定性。实验中用CRIB(CNS Radioactive Ion Beam separator)提供的21Na放射性束轰击8.8 mg/cm2的聚乙烯厚靶,利用放置在θlab=-14°,0°,14°处的3套硅条探测器望远镜阵列对反冲轻粒子进行鉴别和测量,测得在一段能区(Ex=5.5～9.2MeV)内21Na(p,p)21Na的激发函数。通过对21Na(p,p)共振弹性散射截面进行R矩阵拟合,得到了22Mg共振能级的自旋宇称以及质子宽度等信息,从而为计算18Ne(α,p)21Na反应率提供了实验参数。

the paper includes discussions on the important role of neutron and alpha configurations in proton-rich nuclei in nuclear astrophysics in terms of nucleosynthesis under extremely high-temperature hydrogenburning conditions. the νp-process, which is supposed to take place at the very early epoch of type II supernovae, has considerable neutrons and alphas together with protons. the alpha-induced reactions on proton-rich unstable nuclei in the light mass regions is expected to play a crucial role, but very few of them were investigated well yet because of the experimental difficulties. Specifically, I report our recent experimental effort for the breakout process from the pp-chain region,~7Be(α, γ)^(11)C(α,p)^(14)N under the νp-process. the neutron-induced reactions on proton-rich nuclei, which is even more a challenging subject, were investigated previously for very few nuclei. One possible experimental method is the trojan Horse Method(tHM). We successfully have applied tHM to the ^(18)F(n,α)^(14)N reaction study with an unstable beam of ^(18)F.

A nuclear astrophysics experiment was performed at CRIB (CNS low-energy Radioactive-Ion Beam separator)on Mar. 2011. the goal of this experiment is to study the reaction rate of 18Ne(,p)21Na reaction, which mightbe a key breakout reaction from the hot CNO cycle to rp-process in X-ray burst and nova. Yet its reaction rate ispoorly *** hydrogen burning is thought to be the main source of energy generation and a source of nucleosynthesisin X-ray burst and nova[1;2]. For example, XRBs are characterized by a sudden increase of X-ray emission withinonly a few seconds to a total energy output of about 1040 ergs, which is observed to repeat with some *** recurrence time for single bursts can range from hours to days at the typical temperature of 0.4～2 GK. thebursts have been interpreted as being generated by thermonuclear runaway on the surface of a neutron star thataccretes H- and He-rich material from a less evolved companion star in a close binary system. As shown in Fig. 1,under its typical temperature, the hydrogen burning in X-ray burst occurs from the hot CNO cycle:
