Objective: The great saphenous vein (GSV) is commonly used as a conduit for grafting during CABG surgery, and open GSV harvesting (OVH), commonly used with long incision to expose the vein. However, endoscopic vein harvesting (EVH) is an alternative approach, utilizing specialized instruments and small incisions to harvest the vein. Methods: A retrospective analysis was conducted on a cohort of patients who underwent Coronary artery bypass graft (CABG) requiring great saphenous vein (GSV) harvesting which was done by EVH or OVH procedures. Demographic variables, including age and gender, were assessed for both groups. Intraoperative variables such as the number of grafts, cardiopulmonary bypass time, X clamp time, and type of procedure were analyzed. Postoperative variables, including infection and bleeding rates, were also evaluated. Results: The study included 30 patients each undergoing Coronary artery bypass graft (CABG) with need of great saphenous vein harvesting which was done by EVH and OVH. Demographic variables were well-matched between the two groups in terms of age, while a significant difference in gender distribution was observed. Obesity and smoking were more prevalent in the OVH group, and EVH was associated with a higher mean number of grafts compared to OVH. Conversion to an open technique occurred in a portion of the EVH cases, and infection rates did not significantly differ between the EVH and OVH groups. However, the incidence of postoperative bleeding was significantly higher in the EVH group. Conclusion: This study provides valuable insights into the demographic, intraoperative, and postoperative variables associated with EVH and OVH techniques. EVH demonstrated advantages in terms of reduced infection rates compared to OVH. However, the higher incidence of postoperative bleeding associated with EVH raises concerns about potential risks.

Background: A major cause of mortality and disability on a global scale is myocardial infarction (MI). These days, the most reliable way to detect and measure MI is via cardiovascular magnetic resonance imaging (CMR). Aims and Objectives: To evaluate the effectiveness of the Electrocardiogram DETERMINE Score in predicting the severity of coronary artery disease (CAD) in patients who have experienced an Acute Myocardial Infarction (AMI) & to assess improvements in left ventricular function at 6 months following coronary artery bypass grafting (CABG). Subjects and Methods: This Observational cohort study was done at the Cardiology and Radiology department and cardiac surgery department, Al-Azhar university hospitals and Helwan University hospital. The study involved 700 cases who patients diagnosed with Acute Myocardial Infarction and fulfilled specific criteria for selection. Result: There was highly statistically significant relation between Myocardial infarction size and ECG Marker Score as mean infarct size elevated when the number of ECG markers increased. There was a highly statistically significant relation between myocardial infarct segments, myocardial infarction size and improvement of cardiac function 6 months post-CABG. Conclusion: The study found that larger myocardial infarctions corresponded with higher DETERMINE Scores. It concluded that an ECG-based score better estimates infarct size than LVEF alone. Additionally, there was a significant statistical correlation between the size and segmentation of myocardial infarction and better cardiac function six months after CABG.

One of the leading causes of mortality worldwide is liver *** earlier the detection of hepatic tumors,the lower the mortality *** paper introduces a computer-aided diagnosis system to extract hepatic tumors from computed tomography scans and classify them into malignant or benign *** hepatic tumors from computed tomography scans is considered a challenging task due to the fuzziness in the liver pixel range,intensity values overlap between the liver and neighboring organs,high noise from computed tomography scanner,and large variance in tumors *** proposed method consists of three main stages;liver segmentation using Fast Generalized Fuzzy C-Means,tumor segmentation using dynamic thresholding,and the tumor’s classification into malignant/benign using support vector machines *** performance of the proposed system was evaluated using three liver benchmark datasets,which are MICCAI-Sliver07,LiTS17,and *** proposed computer adided diagnosis system achieved an average accuracy of 96.75%,sensetivity of 96.38%,specificity of 95.20%and Dice similarity coefficient of 95.13%.

AIM:To investigate the role of p53 antibodies (p53Abs),metallothioneins (MTs) and oxidative stress markers in the early detection of dysplasia in chronic ulcerative colitis (UC).METHODS:The study included 30 UC patients,15 without dysplasia (group Ⅱ) and 15 with dysplasia (group Ⅲ),in addition to 15 healthy volunteers (group Ⅰ,control subjects).The enzyme-linked immunosorbent assay technique was used to measure serum p53Abs and MTs,while advanced oxidation protein products (AOPPs),and reduced glutathione (GSH) levels were measured by spectrophotometric method in all ***:In group Ⅱ and group Ⅲ compared to group Ⅰ,there were significant increases in serum levels of AOPPs (145.94 ± 29.86 μmol/L and 192.21 ± 46.71 μmol/L vs 128.95 ± 3.06 μmol/L,P were significantly higher levels of AOPPs (P with dysplasia compared to those without dysplasia,while MTs showed no significant difference between the 2 groups (P > 0.096).In contrast,GSH levels showed a significant decrease in both patients' groups (1.87 ± 0.02 μmol/mL and 1.37 ± 0.09 μmol/mL vs 2.49 ± 0.10 μmol/mL,P with group Ⅰ,and the levels were significantly lower in group Ⅲ than group Ⅱ (P was a positive correlation between AOPPs and both MTs (r=0.678,P ween p53Abs and MTs (r=0.739,P was a negative correlation between AOPPs and GSH (r =-0.385,P ween GSH and both MTs (r=-0.662,P < 0.001) and p53Abs (r=-0.923,P < 0.001).CONCLUSION:Oxidative stress and oxidative cellular damage play an important role in the pathogenesis of chronic UC and the associated carci

Aim:The aim of this study is to prove or disprove the superiority of surgical resections over radiofrequency ablation(RFA)with respect to efficacy and ***:The study was conducted in Zagazig University Hospitals,which included 40 patients with hepatocellular carcinoma(HCC)during the period from November 2011 to December 2014,using either liver resection or ***:Hepatic resection was done in 20 patients(13 males,7 females).Interventional RFA was done in 20 patients(12 males,8 females).There was no in-hospital mortality after ***-and two-year survival rates were 85% and 70% *** was no in-hospital mortality after ***-and two-year survival rates were 80% and 65% ***:Surgical resection is preferred over RFA in HCC-liver cirrhosis Child A patients with tumor sizes≥3 ***-liver cirrhosis Child A patients with masseswith both surgery and *** in special cases such as central position lesions,RFA is preferred over *** the decision for management may be changed according to patients *** resection 1-and 2-year survival rates were better than those treated with RFA.

Clinical profile and angiographic findings among patients with atrial fibrillation presenting for selective coronary angiography

Background: Coronary artery disease is the most common form of cardiovascular disease while Atrial fibrillation is the most common sustained arrhythmia. we set out to investigate the prevalence and clinical profile of patients with atrial fibrillation who present for selective coronary angiography and whether this has an effect on the angiogram findings and the treatment options offered to them. Methods and Results: This was a retrospectively collected database of 494 patients presenting for SCAG from 01.11.2010 to 30.11.2010 and 01.04.2010 to 30.04.2010. we collected and analyzed clinical characteristics of patients, their SCAG finding and modes of treatment offered up to discharge. 24.6% of patients had AF. They tended to be older, more commonly women, current or ex-smokers;presented with symptoms of atypical chest pain and were more likely to have a history of congestive heart failure or valvular heart disease. They had a longer hospital stay. Using CHADS-Vasc score, 83.6% were moderate to high risk for CVA. At SCAG, they were more likely to have non-significant coronary artery disease, and hence were more likely to be treated conservatively. Conclusion: AF patients presenting for SCAG constitute a unique subset of patients who despite having a higher likelihood of non-significant coronary stenosis are still prone to suffering from poorly-understood and under-appreciated myocardial ischemia.

语言教学领域有一个广为人知的说法,即“语言教学只不过是教学文化而已”。人们普遍认为,外语教育中的文化教学是获得文化差异意识的重要因素。文化意识是沟通的基础,它包括超然于我们自身的文化,既了解我们的社会规范、价值观,又了解其他文化的能力。因此,近一个世纪以来,外语教育中的文化教学一直被认为是重要的。在今天这个沟通便利和技术进步的世界里,任何一个国家或社会,若与其他语言和文化隔离开来都是无法想象的。然而,这些文化差异确实存在,每个国家都竭力保护自己的规范和价值观。这两个方向的对立需要通过对本国和外国文化的认识来加以协调。本研究采用定量与定性相结合的方法,运用皮尔森相关分析、回归分析、描述性统计等方法对问卷进行分析,采用定性方法对访谈进行分析。本研究的主要目的是探讨学习英语对也门本科生文化认同的影响。此外,调查研究还表明了由于学习英语语言和文化,受试者的文化认同是如何受到影响的,以及他们的生活方式如何得以改变。研究结果表明这种影响有正面和负面之分。此外,本研究旨在为专家和研究者提供系统和实际的理由,在保证英语教育的同时,限制使用英语语言和文化的自由,以保护当地文化认同不受损害。这种影响被分为三个层次:对个人的巨大影响、文化中能看到影响、以及没有明显证据表明文化认同发生了显著变化。最后,文化意识对于维持学生学习英语的需要以及了解两种文化(当地文化和英语文化)之间的差异非常重要。
