The Relationship with Previous Use of Community-Based Services and Informal Caregiver Support and Elderly and Informal Caregivers’ Views of Living Arrangement: Analysis of Official Statistics from 2004 to 2014

Background and Objectives: Views on living arrangement from elderly and informal caregivers are crucial to “ageing in place”. They might be related to the experience in the use of elderly care services, which remains inconclusive in previous literature. This study aimed to explore the association of previous experience in formal and informal long-term care services with views of both elderly and their informal caregivers on living arrangement. Research Design and Methods: This study adopted a cross-sectional design. Assessment records of Minimum Data Set-Home Care for community-dwelling elderly who were eligible for subsidized long-term care services in Hong Kong from 2004 to 2014 were made available. Multivariate logistic regression was applied to examine associations between both views on elder’s living arrangement from the elderly, caregivers and their previous informal caregiver support, and experience in formal care services. Results: 82,306 dyads of elderly and informal caregivers were included in the analysis. The elderly with previous use of home and community-based services (OR = 0.84, 95% CI 0.80 - 0.88) and informal caregivers (OR = 0.78, 95% CI 0.76 - 0.81) believed that the elderly should live away from home. Temporal trends of fewer elderly and caregivers supporting the idea of living away from home were also observed. Discussion and Implications: The results highlighted the importance of informal caregiver’s support and previous utilization of formal home and community-based services. It was concluded that resources and information of community-based care have a significant association with views on living at home also proper support services and training of care for the elderly should be made available to informal caregivers to reduce their burden.

Tunable ultraviolet to deep blue light emission from sulfur nanodots fabricated by a controllable fission-aggregation strategy

Bright tunable light emission in the short wavelength range from sulfur nanodots was demonstrated with a photoluminescence quantum yield(PLQY)of up to 59.4%.A fission-aggregation mechanism was proposed for the formation of sulfur nanodots with desired *** synthetic strategy allowed for simultaneous size control from 3.2 to 5.6 nm,thus tuning the emission color from ultraviolet(UV)to deep blue(342±430 nm),and for the suppression of unwanted nonradiative recombination centers and deep level *** luminescence mechanism and quantum confinement effect of the synthesized sulfur nanodots were investigated by optical spectroscopy and theoretical *** results show promise toward the application of sulfur nanodots in UV optoelectronics,biomedical treatments,and sterilization.

Mesenchymal stem/stromal cells(MSCs)have demonstrated therapeutic efficacy for bone regeneration in animal and clinical *** MSCs were initially thought to differentiate to various cell types to replace the injured/diseased tissue,it is now accepted that these cells secrete factors to promote tissue repair.1 Among these factors,small extracellular vesicles(sEVs)of size 50–200 nm,which include the exosomes,have been identified as the principal agent mediating the wide-ranging therapeutic efficacy of MSCs.2 Several studies have also reported the therapeutic effects of MSC-sEVs to enhance bone repair in animal models,as recently reviewed.3 However,the cellular processes and mechanisms mediated by MSC-sEVs in bone regeneration remain to be fully elucidated.

Genomic Organization and Transcriptional Regulation of Two Homologous Fucolectin Genes AjFTL-1 and AjFTL-2 in Apostichopus japonicus

F-type lectins(Fucolectins)are carbohydrate-binding proteins and play important roles in innate immune responses against pathogenic microbial *** our previous research,we found that two homologous Fucolectin genes,AjFTL-1 and AjFTL-2,exhibited different expression profiles after lipopolysaccharides(LPS)challenge in Apostichopus ***,the transcriptional regulation mechanism of these two genes remains largely *** this study,the 5'flanking regions of Aj FTL-1 and Aj FTL-2 genes were cloned and the promoter activities were studied in epithelioma papulosum cyprinid(EPC)cell ***,in silico analysis indicated that these two promoters both contain numerous putative transcription factor binding sites including NF-κB,CREB,and CREBP1,and both contain a TATA ***,luciferase assay and progressive 5'truncation analysis revealed that AjFTL-1 and AjFTL-2 both possess high promoter activities in EPC ***,the luciferase activity of AjFTL-1 promoter was significantly regulated by peptidoglycan(PGN)and mannan(MAN),while AjFTL-2 promoter was prominently regulated by LPS and MAN,indicating Aj FTL-1 and Aj FTL-2 genes showed different transcriptional regulation pattern under different immune *** importantly,analyses of the functional promoter regions revealed the presence of two potential NF-κB binding sites(-769 bp to-761 bp,-185 bp to-172 bp)in Aj FTL-1 and one potential binding site(-530 to-517 bp)in Aj *** truncated reporter vectors and expression vector co-transfection revealed that transcription factor NF-κB/Rel could significantly increase the AjFTL-2 promoter activity,but not AjFTL-1 promoter *** findings indicated that in marine invertebrates,different Fucolectin members differ in transcription regulations and expression patterns,and might play different roles in immune defenses during pathogen infection.

本研究以阶层身份认同为探讨精英大学农村籍学生就读经历的理论工具。首先探索性地在社会身份转换中融入布尔迪厄阶层模型理论,即资本和生存心态支撑个体抽象意义上的结构空间位置,遵循文化建构的阶层身份分析路径,将情感回应纳入分析框架之中,以此搭建弱势阶层大学生阶层身份认同的理论框架。然后结合实证研究,考察高等教育场域结构与行动者主体能动性交互作用下,个体所秉承资本的增长和生存心态的转换如何成为他们重新确认阶层身份的锚点,而对不同阶层群体的评价和情感则在阶层身份认同过程中发挥重要的调节作用,基于此提出以阶层身份认同作为理解弱势阶层大学生就读经验的新视角。

We operated a p-type point contact high purity germanium(PPCGe)detector(CDEX-1B,1.008 kg)in the China Jinping Underground Laboratory(CJPL)for 500.3 days to search for neutrinoless double beta(0νββ)decay of^(76)Ge.A total of 504.3 kg⋅day effective exposure data was *** anti-coincidence and the multi/single-site event(MSE/SSE)discrimination methods were used to suppress the background in the energy region of interest(ROI,1989–2089 keV for this work)with a factor of 23.A background level of 0.33 counts/(keV⋅kg⋅yr)was *** lower limit on the half life of^(76)Ge 0νββdecay was constrained as T_(1/2)^(0ν)>1.0×10^(23)yr(90%C.L.),corresponding to the upper limits on the effective Majorana neutrino mass:<3.2–7.5 eV.

Cystic meconium peritonitis with jejunoileal atresia:Is it associated with unfavorable outcome?

AIM To compare the outcome between patients with jejunoileal atresia(JIA) associated with cystic meconium peritonitis(CMP) and patients with isolated JIA(JIA without CMP).METHODS A retrospective study was conducted for all neonates with JIA operated in our institute from January 2005 to January *** including the gestation age,sex,birth weight,age at operation,the presence of associated syndrome was *** outcome including the type of operation performed,operative time,the need for reoperation and mortality were *** demographics and the outcome between the 2 groups were *** During the study period,53 neonates had JIA underwent operation in our *** neonates(32%) were associated with *** was no statistical difference on the demographics in the two *** with CMP had earlier operation than patients with isolated JIA(mean 1.4 d vs 3 d,P = 0.038).Primaryanastomosis was performed in 16 patients(94%) with CMP and 30 patients(83%) with isolated JIA(P = 0.269).Patients with CMP had longer operation(mean 190 min vs 154 min,P = 0.004).There were no statistical difference the need for reoperation(3 vs 6,P = 0.606) and mortality(2 vs 1,P = 0.269) between the two *** Primary intestinal anastomosis can be performed in94% of patients with JIA associated with *** patients with CMP had longer operative time,the mortality and reoperation rates were low and were comparable to patients with isolated JIA.

为了得到PPT点火可靠性表征参量的内在联系及放电能量对其影响规律,开展了传统PTFE与掺碳PTFE两种工质的可靠性试验研究,通过记录点火故障率和分析放电延迟时间,得到了不同工质PPT的点火可靠性,为PPT点火可靠性分析提供了新的研究思路。研究结果表明:在不同放电电压下,PTFE工质PPT放电延迟时间随点火次数增加的变化规律明显不同。放电电压为1kV时,放电延迟时间在1μs^15μs内变化,在初始阶段时明显增加,然后在波动中上升,并伴随有点火故障发生;放电电压为1250V和1500V时,放电延迟时间在1μs^5μs之内波动,且没有点火故障发生。两种工质的放电延迟时间均随点火故障率增加呈非线性规律变化,掺碳PTFE的放电延迟阈值与放电电压之间呈指数函数规律变化。

Splitting white light into its constituent spectral components has been of interest ever since Newton first discovered the phenomenon of color *** devices have since been conceived to achieve efficient wavelength separation,yet a large number of applications,e.g.,in display technology,still use simple color absorption or rejection filters that absorb or reflect unwanted wavelengths,thus wasting luminous ***,we demonstrate a novel microsized device concept that enables efficient color *** device operation is based on differential material dispersion in a waveguide array,which causes different wavelength signals to couple selectively into appropriate waveguides.A theoretical power delivery of greater than 50%for a tricolor wavelength router is obtained,compared to 33%expected from geometry *** principle of operation is demonstrated experimentally for a dual-color light field,where we achieve a higher than 70%routing efficiency(compared to 50%from geometry),thus highlighting the feasibility of this novel and promising approach.
