Effect of drug therapies on self-reported chemosensory outcomes after COVID-19

Objective:The aim of this study was to assess the relative efficacy of medications used following severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)infection on self-reported alterations in taste and/or smell function.Methods:Seven hundred and fourteen persons with self-reported postcoronavirus disease 2019(post-COVID-19)chemosensory disorders were personally interviewed regarding specific medications they were administered following the acute phase of the disease.The dependent measure—self-reported total recovery of chemosensory symptoms—was subjected to stepwise logistic regression.Independent predictors included demographic and clinical variables,in addition to specific medications used to mitigate disease symptoms(i.e.,systemic corticosteroids,oseltamivir,vitamin C,ibuprofen,hydroxychloroquine,azithromycin,ivermectin,nitazoxanide,anticoagulants,and zinc).Results:The median time between COVID-19 symptom onset and the interviews was 81 days(interquartile range:60-104).Of the 714 subjects,249(34.9%)reported total recovery of their chemosensory function;437(61.2%)had at least one treatment since the beginning of the disease.Women and those with more comorbidities had undergone more treatments.The recovery rates of the treated and nontreated groups did not differ significantly.Nonetheless,respondents who had used nitazoxanide tended to have a higher rate of self-reported taste or smell recovery.Those who took oral zinc were less likely to improve.Conclusions:No medication employed during the first months after SARS-CoV-2 infection had a clear positive effect on returning self-reported smell or taste function to normal,although nitrazoxide trended in a positive direction.Oral zinc had a negative effect on the reported recovery of these senses.

Percutaneous nephrostomy versus ureteral stent in hydronephrosis secondary to obstructive urolithiasis:A systematic review and meta-analysis

Objective: To assess if there is a preferable intervention between retrograde ureteral stent (RUS) and percutaneous nephrostomy (PCN) tube, in cases of upper urinary tract stone obstruction with complications requiring urgent drainage, by evaluating outcomes regarding urinary symptoms, quality of life (QoL), spontaneous stone passage, and length of hospital stays, since there is no literature stating the superiority of one modality over the other.Methods: We searched MEDLINE and other sources for relevant articles in June 2019 without any date restrictions or filters applied. The selection was done first by the title and abstract screening and then by full-text assessment for eligibility. Only randomized controlled trials or cohort studies in patients with hydronephrosis secondary to obstructive urolithiasis that presented comparative data between PCN and RUS placement concerning at least one of the defined outcome measures were included. Lastly, MEDLINE database and PubMed platform were screened again using the same terms, from June 2019 until November 2022.Results: Of 556 initial articles, seven were included in this review. Most works were considered of moderate-to-high quality. Three studies regarding QoL showed a tendency against stenting, even though only one demonstrated statistically significant negative impact on overall health state. Two works reported significantly more post-intervention urinary symptoms in stenting patients. One article found that PCN is a significant predictor of spontaneous stone passage, when adjusted for stone size and location. Findings on length of hospital stays were not consistent among articles.Conclusion: PCN appears to be the intervention better tolerated, with less impact on the patient’s perceived QoL and less post-operative urinary symptoms, in comparison with RUS. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.

We identified that oncological treatments in general (chemotherapies, immunotherapies and radiotherapies) frequently cause peripheral neuropathy, including cramps, characterized by excess protons due to metabolic and neuronal factors, such as sudden changes in pH, uremia and aspects that affect neuromotor functions. Such situations and others like them are often neglected in treatment, which naturally concerns itself with the main problem: Cancer. Sometimes toxic solutions are implemented that have comorbid side effects, such as duloxetine (standard treatment). Based on monitoring of cancer patients who used the non-toxic product, called “Magicramp® Electrostatic Charge Reduction Cushion” (MECRC), approved in Europe more than 10 years ago, we carried out a controlled test in Brazil. In this clinical trial, we hypothesized that reducing excessive ionic charges (electrostatic charge), which is one of the side effects often described in the literature as “Chemotherapy-Induced Peripheral Neuropathy” (CIPN), would decrease or eliminate cramping, under the hypothesis that such elimination would prevent or attenuate muscular vulnerability to action impulses, and increase the power of relaxation through the same mechanism. In this double-blind and randomized clinical trial, 40 (forty) adult patients with muscle cramps caused by oncological treatments were tested, evaluating the degree of efficiency of the product that aims to reduce muscle cramps, by eliminating and/or reducing excess loads electrostatic ionic. Introduction: In the last thirty years, brain neuromodulation techniques have been used as an alternative to pharmacological treatment of neurological disorders. Parkinson’s disease (PD) is a neurodegenerative disorder leading to bradykinesia, rest tremor, postural changes, and non-motor symptoms such as depression, anxiety, sleep disorders, pain, and cognitive decline that compromises executive functions (EFs), responsible for the orderly execution of behaviors and tasks of daily life and intentional and directed actions. To this date, a few studies with transcranial direct current stimulation (tDCS) have shown beneficial effects in PD patients concerning specific motor and non-motor symptoms, targeting the motor cortex and/or prefrontal regions. Objective: The main objective of this study was to evaluate the effects of left prefrontal tDCS across a broad spectrum of motor and non-motor symptoms of PD using established validated scales. Method: Single-blind randomized clinical trial with 18 volunteers with PD, aged between 45 and 80 years (66.1 ± 9.65), who met inclusion and exclusion criteria. Participants were submitted to assessments of motor and non-motor functions employing psychometric scales and tests to evaluate EFs and were randomly divided into two groups: control (sham stimulation) and experimental (active stimulation). All participants were involved in three separate tDCS sessions. The anode was positioned over the left dorsolateral prefrontal cortex and the cathode over the right supraorbital region, with a direct current intensity of 2 mA, lasting 20 minutes. At the end of the three sessions, all participants were reassessed. Results: Significant effects of tDCS on non-motor functions were observed for cognition (verbal fluency of actions, clock copy test, appointment by visual confrontation, and verbal memory with immediate free recall) and subjective assessment of sleep quality (overall restlessness and discomfort in the arms and legs at night, leg and arm cramps at night and distressing dreams).

A beta regression analysis of COVID-19 mortality in Brazil

Brazil was one of the countries most impacted by the COVID-19 pandemic,with a cumulative total of nearly 700,000 deaths by early 2023.The country's federative units were unevenly affected by the pandemic and adopted mitigation measures of different scopes and intensity.There was intense conflict between the federal government and state governments over the relevance and extent of such measures.We build a simple regression model with good predictive power on state COVID-19 mortality rates in Brazil.Our results reveal that the federative units'urbanization rate and per capita income are important for determining their mean mortality rate and that the number of physicians per 100,000 inhabitants is important for modeling the mortality rate precision.Based on the fitted model,we obtain approximations for the levels of administrative efficiency of local governments in dealing with the pandemic.

Background:Hemodialysis(HD)per se is a risk factor for thrombosis.Considering the growing body of evidence on blood-flow restriction(BFR)exercise in HD patients,identification of possible risk factors related to the prothrombotic agent D-dimer is required for the safety and feasibility of this training model.The aim of the present study was to identify risk factors associated with higher D-dimer levels and to determine the acute effect of resistance exercise(RE)with BFR on this molecule.Methods:Two hundred and six HD patients volunteered for this study(all with a glomerular filtration rate of70 years old),and HD vintage.Furthermore,RE+BFR significantly increases D-dimer after 4 h.Patients with borderline baseline D-dimer levels(400-490 ng/mL)displayed increased risk of elevating D-dimer over the normal range(-500 ng/mL).Conclusion:These results identified factors associated with a heightened prothrombotic state and may assist in the screening process for HD patients who wish to undergo RE+BFR.D-dimer and/or other fibrinolysis factors should be assessed at baseline and throughout the protocol as a precautionary measure to maximize safety during RE+BFR.

Since its first description in 2013,robust evidence supporting the efficacy and safety of the endoscopic sleeve gastroplasty(ESG)has been on the rise.A large case series and meta-analysis report supported results up to 24 months,while some other studies already described 5-year data.If associated with pharmacotherapy,the ESG may help one to achieve weight loss similar to that of surgical sleeve gastrectomy.Though the results of the ongoing randomized trials on ESG are awaited,currently available data support the clinical use of the ESG,especially for patients who are refusing or unfit for bariatric surgery.

The Sergipano Orogenic System(SOS)in southern Borborema Province(NE Brazil)hosts voluminous Neoproterozoic plutonism related to the Brasiliano/Pan-African Orogeny.This paper presents comprehensive whole-rock geochemical data,titanite U-Pb ages,and the first combined zircon U-Pb and Lu-Hf isotope results for plutonic rocks from the MacururéDomain to constrain their sources,petrogenesis,and tectonic setting.Three magmatic episodes are recognized and record the evolutionary stages of the orogen.(i)Early-collisional magmatism(643–628 Ma)comprises gabbros and diorites with minor tonalites characterized by well-developed tectonic foliation and evidence of solid-state deformation.These rocks are magnesian,high-K calc-alkaline,LILE-and LREE-enriched and provide subchondriticε_(Hf)(t)values(6.5 to4.7)and Orosirian Hf-T^(C)_(DM)model ages(1.83–1.94 Ga).Such features indicate derivation from a lithospheric mantle source metasomatized by incorporating crustal components through subduction processes prior to magma generation,possibly related to the Rhyacian Orogeny(2.20–1.96 Ga).Extensive mixing/mingling between basaltic and crust-derived magmas took place at lower crustal depths,producing coeval hybrid diorites and quartz-diorites.(ii)Syn-collisional magmatism(630–624 Ma)encompasses biotite-and muscovite-bearing granodiorites and monzogranites,preserving their structures parallel to the schistosity of the country rocks.These rocks are leucocratic,weakly metaluminous to peraluminous,and contain abundant surmicaceous enclaves.Petrographic features and geochemical composition suggest an origin related to the partial melting of graywacke protoliths with a subordinate igneous component.(iii)Late-collisional magmatism(625–600 Ma)includes undeformed and isotropic monzonites and granodiorites,which truncate the regional foliation.These rocks are consistently metaluminous and magnesian,showing affinities with the high-K calc-alkaline to shoshonite series.Trace element modeling with subchondriticε_(Hf)(t)values(8.3 to4.1)and Paleo
