Concurrent removal of intravitreal lens fragments after phacoemulsification with pars plana vitrectomy prevents development of retinal detachment

AIM: To evaluate the outcomes of "concurrent vitrectomy" to retrieve dislocated lens fragment during ***: In a retrospective, observational case series, data of patients who underwent "concurrent" pars plana vitrectomy(PPV) for dislocated lens fragments between the period 2000 and 2008 were reviewed. Data collected included patient demographics, pre-operative visual acuity, intra-operative occurrence of retinal breaks, duration of follow up, post-operative intraocular pressure, final best-corrected visual acuity(BCVA),presence of cystoid macular edema(CME) and occurrence of rhegmatogenous retinal detachment(RRD).· RESULTS: A total of 58 eyes of 58 patients were included in the study. At 12 mo the mean postoperative BCVA was log mar 0.17(20/30) with a range of log mar0 to 0.69(20/20 to 20/100), with 96.6%(56/58) of patients showing post- operative improvement in visual acuity(P =0.005). None of the patients developed postoperative retinal detachment, endophthalmitis or non-resolving uveitis at 12 ***: Our study results suggest concurrent PPV for retained lens fragments after cataract surgery is beneficial and may decrease the risk of glaucoma and prevent development of RRD.

Pediatric autoimmune neuropsychiatric disorders associated with or without streptococcal and other bacterial infections (PANDAS/CANS) are emerging as a featured pediatric disorder. Although there is some controversy regarding treatment approaches, especially related to the behavioral sequelae, we have hypothesized in other published work that it is characterized by the rapid onset of Reward Deficiency Syndrome (RDS) in children. We propose utilizing a multi-systems biological approach involving the coupling of genetic addiction risk testing and pro-dopamine regulation (kB220/POLYGEN ® ) to help induce “dopamine homeostasis” in patients with PANDAS, especially those with known DNA-induced hypodopaminergia. This case study examines a 12-year-old Caucasian male with no prior psychiatric issues who presented with a sudden onset of severe anxiety, depression, emotional liability, and suicidal ideation. The patient underwent genotyping and the genetic addiction risk score (GARS) testing, which revealed risk polymorphisms in the dopamine D2 (-DRD2/ANkk (Taq1A), OPRM1 (A/G), DRD3 (C/T), and MAOA (4R) genes. These polymorphisms have been linked to hypodopaminergia. The patient was subsequently placed on research ID-kB220ZPBMPOLY (POLYGEN ® ), and albeit the possibility of bias, based upon self and parental assessment, a marked rapid improvement in psychiatric symptoms was observed. In the second phase of treatment (102 days utilizing kB220), the patient received standard antibody testing, which was positive for Lyme. Antibacterial therapy started immediately, and kB220z was discontinued to provide a wash-out period. A monotonic trend analysis was performed on each outcome measure, and a consistently decreasing trend was observed utilizing antibacterial therapy. AIM：To report the change in visual acuity and central macular thickness（CMT）following treatment with intravitreal aflibercept injections in patients with neovascular agerelated macular degeneration（n AMD）with suboptimum response to ***：This was a retrospective study. The inclusion criteria were patients with n AMD who responded poorly to ranibizumab. Patients then received either 3 consecutive aflibercept injections followed by pro re nata（PRN）treatment or PRN alone. Primary endpoints were mean change in bestcorrected visual acuity（BCVA）and CMT at 12 mo. Secondary endpoints were number of injections and adverse ***：Forty-nine eyes from 49 patients met the inclusion criteria and completed 12-month follow up on aflibercept. Thirty-eight eyes received 3 consecutive aflibercept injections followed by PRN treatment and 11 eyes received PRN injections alone. At 12 mo,mean BCVA improved by one letters（log mar 0.56±0.31 to 0.54±0.34）and mean CMT decreased from 303.9±82.1 to 259.2±108.3 μ*** percent of eyes gained 15 letters or more,6% lost more than 15 letters and the remaining 90% had stable BCVA. The mean number of aflibercept injections was *** was one case of infectious ***：Intravitreal aflibercept in patients with n AMD with a previous suboptimal response to ranibizumab resulted in an anatomical improvement in macular appearance at 12 mo without a corresponding improvement in visual acuity.

To the Editor:We sincerely applaud the work done by gupta et al.,in which they analyzed a single-center experience regarding gallbladder per-foration(GBP)management[1].Currently,there is a lack of cohort studies to adequately describe the approach strategies and man-agement for this pathology,with international guidelines making ambiguous recommendations[2,3].Their study stratified GBP types and provided details regarding clinical presentation,comorbidities,preoperative diagnosis,etiology,imaging techniques,and surgical *** study reported a high conversion rate when a la-paroscopic approach was attempted for cholecystectomy.

Lichen biota of Pathanamthitta district of kerala state was thoroughly studied which resulted in 24 lichen species as new to kerala and one species,Phaeographis nylanderi(Vain.)*** new to *** the species identified are either crustose or foliose forms belonging to families Graphidaceae and Caliciaceae indicating the dominance of these groups in the study *** of all the 25 species are provided along with their distribution.
